Stiff-Person Syndrome: Diagnostic Difficulty (근육 강직 증후군: 진단적 어려움)

  • Stiff-person syndrome is a rare disorder, and the natural history of the syndrome has not been completely ascertained. The symptoms range from mild to severe and can progress over time: the final result can be significant disability. However, this syndrome is often misdiagnosed due to a lack of understanding of the clinical manifestations. We report the case of a patient who presented with slowly progressing gait disturbance and lower extremity pain and was later diagnosed as suffering from stiff-person syndrome. The patient experienced symptomatic improvement after the administration of benzodiazepines. No recurrence of symptoms has been reported. If the characteristic clinical features and electromyography findings of the syndrome are accurately interpreted, diagnosis of patients with abnormal muscle tension can be effectively done.

A Case Study of a Patient with Breast Cancer Treated with a Combination of Traditional Korean Medicine and Chemotherapy (유방암에 대한 한양방 병용 치료 증례 보고 1례)

  • The prevalence of breast cancer is very high in Korea. Although the patients receive standard treatments, such as surgery, chemotherapy, or radiotherapy, they frequently experience recurrence or metastasis of their tumors. In addition, many patients with breast cancer also suffer from side effect symptoms induced by these standard treatments. Therefore, increasing numbers of patients now want to undergo treatment with traditional Korean medicine (TKM) in addition to conventional treatment. We present a case of 46-year-old female with recurred breast cancer. She first received two kinds of chemotherapy and then underwent surgery. She then also received 4 cycles of adjuvant chemotherapy. At a follow-up examination, she was informed of recurrent lesions on the right anterior chest wall. She started to receive TKM treatments together with a new chemotherapy. After about one month, the size of the recurred tumor in right chest wall had decreased. Her symptoms, such as shoulder pain, chest pain, and nausea induced by conventional therapy, were also improved. We suggest that a combination of TKM and chemotherapy is a promising treatment for breast cancer.

  • Internal deficiency of vital Qi is origination of cancer and root cause of its development from the viewpoint of traditional Chinese medicine(TCM). For about 40 years, China has developed a combined system of TCM and Western medical system in treatment of cancers, laying stress on the therapeutic principle of strengthening the body resistance and restoring normal functioning of the body to consolidate the constitution (Fu Zheng Pei Ben). According to the results of the latest clinical and experimental studies, Chinese medicine not only increases curative effectiveness, but also reduces the side effect caused by the toxic, radio and chemical treatment of cancer cases in the western medical system as well as various complications. Long-term taking Chinese herbal medicine prevents recurrence and metastasis of tumor, and heightens long-term curative effectiveness after western medical treatments. It also helps to relieve symptoms, improve the quality of life, and prolong life of terminal cancer patients who have already lost opportunities of western medical treatments. In addition, it can prevent and cure premonitory symptoms of cancer.

Clinical evaluation of thymic tumors: a report of 10 cases (흉선에 발생한 종양 10례에 대한 임상적 고찰)

  • Thymic tumor is the most common neoplasm originating in the anterior mediastinum. Histologically, thymic tumors and tumorlike conditions have been classified into thymic cyst, thymolipoma, true thymic hyperplasia, thymoma and carcinoid tumor of the thymus gland. We have experienced several tumors and a few tumorlike conditions of the thymus gland in 10 patients: thymoma 6, thymic hyperplasia 2, carcinoid tumor 1 and thymic cyst 1. The age distributions were ranging from 30 to 60 years except for one 3 year old child [malignant thymoma, lymphocytic type], and male to female ratio was 1:1.5. In 6 cases, thymic lesions were removed through lateral thoracic incision [right 2, left 4] because the mediastinal masses were growing far into the pleural space, meanwhile, medially located lesions [4 cases] through median sternotomy incision. Among these 10 patients, 3 were malignant thymomas, in which 2 were died of recurrence after tumor resection. Myasthenia gravis was all noted in 3 females [thymoma 2, thymic follicular hyperplasia 1], in which only two revealed remission in symptoms following thymectomy. The authors would like to recommend early radical thymectomy through median sternotomy incision whenever patients demonstrate suspicious thymic tumor lesions on the chest roentgenogram or generalized myasthenic symptoms.

Schwannoma of the tongue: a case report with review of literature

  • Background: Schwannomas (or neurilemmomas) of the tongue are benign, usually solitary, encapsulated masses derived from Schwann cells. Clinical evidence indicates that schwannoma is painless and slow growing. In general, schwannoma is treated by surgical excision. Here, we describe a case of schwannoma of the tongue, include a review of the literature from 1955 to 2016, and provide data on age, gender, location, presenting symptoms, size, and treatment methods. Case presentation: A 71-year-old female patient presented with a swelling at the base of the tongue of unknown duration. Magnetic resonance images (MRI) showed a large well-circumscribed solid mass and no significant lymph node enlargement. The mass was excised without removing overlying mucosa. Conclusions: The authors report a case of lingual schwannoma that was completely removed intraorally without preoperative biopsy. No sign or symptoms of recurrence were observed at 12 months postoperatively.

A Case Report of Acute Urticaria Treated by Korean Medicine Treatment (사물탕(四物湯) 등 한의 치료로 호전된 급성 두드러기에 대한 증례 보고)

  • Objectives : This study is to report effectiveness of Korean medicine monotherapy on an acute urticaria patient. Methods : A 26-year-old female acute urticaria patient visited Dong-Eui university Korean medical hospital at November 10th, 2020. The patient received Korean monotherapy including herbal medicine and aroma cream. The treatment continued for about 1 month until December 4th, 2020. Urticaria activity score(UAS), visual analogue scale(VAS), photographs were used to assess the symptoms. Results : After 20 days of medication, the symptoms were disappeared. UAS score was dropped 6 to 0 and VAS score dropped 9 to 0. About two years later, there was no recurrence. Conclusions : These results suggests that acute urticaria which accompanied by a moderate degree of pruritus can be sufficiently managed by Korean medicine therapy.

  • Drug rash with eosinophilia and systemic symptoms (DRESS) syndrome is characterized by a severe idiosyncratic reaction including rash and fever, often with associated hepatitis, arthralgias, lymph node enlargement, or hematologic abnormalities. The mortality rate is approximately 10%, primarily owing to liver failure with massive or multiple disseminated focal necrosis. Here, we report a case of a 14-year-old girl treated with vancomycin because of a wound infection by methicillin-resistant Staphylococcus aureus, who presented with non-specific symptoms, which progressed to acute liver failure, displaying the hallmarks of DRESS syndrome. With the presence of aggravated hepatic encephalopathy and azotemia, the patient was refractory to medical treatments, she received a living-donor liver transplantation, and a cure was achieved without any sign of recurrence. Vancomycin can be a cause of DRESS syndrome. A high index of suspicion and rapid diagnosis are necessary not to miss this potentially lethal disease.

  • Back pain has plagued humans for many thousands of years. The treatment of back pain is divided into operative treatment and conservative treatment. It is reported that cure rate of conservative treatment is 80~90 percent. Generally, the treatment of oriental medicine is mostly conservative treatment. But, surgery should not be used as a last resort in treatment; it is just one of many treatment options for various spinal conditions. In some instance, it can be to preferred choice; in other situations, alternative therapies may be superior. Selections of the operation in HIVD 1. Acute disc herniations with a protracted significant component af back pain. 2. Chronic disc degeneration with significant back pain and degeneration limited to one or two disc levels. 3. Sugical instability created during decompression. 4. The presence of neural arch defects coincident with disc disease. 5. Symptamatic and radiographically demonstrable segmental instability. Selections of the operation in stenosis 1. If it does not slowly progress in physical therapy and other nonoperative measures, many of these patients may ultimately need surgical decompression. 2. Absolute stenosis in an impression of CT, MRI.(under 10mm) 3. In patients with established symptoms of .neurogenic claudication. 4. In patients with bad influence of neurogenic derangement.(strength, sensory) Selections of the operation in spondylolisthesis 1. Persistence or recurrence of major symptoms for at least one year despite activity modification and physical therapy. 2. Tight hamstrings, persistently abnormal gait, or postural deformities unrelieved by physical therapy. 3. Sciatic scoliosis. 4. Progressive neurologic deficit. 5. Progressive slipping beyond 25 or 50 percent, even when asymptomatic. 6. A high slip angle (40 to 50 degrees) in a growing child, since it is likely to be associated with further progression and deformity. 7. Psychologic problems attributed to shortness of trunk, abnormal gait, and postural deformities characteristic of more severe slips.

A Clinical Study on Salivary Gland Tumors (타액선 종양의 임상적 고찰)

  • Because of the diversity of tumor types and the relative rarity of salivary gland neoplasm, exact diagnosis and treatment decision have been difficult. Seventy cases of salivary gland tumors which were diagnosed at our hospital from January 1981 to December 1992 were reviewed according to age, sex, site, presenting symptoms, staging, histology, and outcome, retrospectively. The following results were obtained ; 1) Of all salivary gland tumors, 35 cases(50%) arose in the parotid gland, 16 cases(23%) in the submandibular gland, and 19 cases(27%) in the minor salivary glands. 2) There were 55(79%) benign tumors, and 15(21 %) malignant tumors. The most frequent benign tumor was pleomorphic adenoma(89%), and malignant tumor was adenoid cystic carcinoma(40%). 3) The symptoms varied in duration from several weeks to 26 years. 29 cases(41 %) had symptoms for one to five years. 4) Among the parotid and submandibular glands, postoperative facial nerve paralysis was occurred 9 cases(18%). Recurrence was seen in 4 cases(6%).

A Case of Mucous Gland Adenoma of the Trachea (기관에서 발생한 점액선 선종 1예)

  • Mucous gland adenoma of the bronchus is a rare benign tumor arising from the bronchial mucous gland. It accounts for less than 0.5% of all lung tumors. In adults, tracheal tumors are most often malignant. Among benign tumors arising in the trachea, mucous gland adenoma of the trachea is extremely rare. First case was reported by Ferguson and Cleeland in 1988, as "Mucous gland adenoma of the trachea". Microscopic study shows it to arise from normal submucosal mucous glands forming glandular or tubular structures composed of mucous secreting cells. Common symptoms were cough, hemoptysis, recurrent and protracted pneumonia, shortness of breath, and wheeze. Duration of symptoms before diagnosis varied from a few weeks to 10 years with prolonged symptoms being usual. Management of these tumors should be complete excision, including pulmonary resection because two instances of recurrence after local excision have been reported.
