In highly developed society, information and communication technologies are widely used for better medical services. These information and communication technologies should be more and more acceptable in all hospitals for exchange medical records. EMR becomes more convenient than the previously used paper charts. It will be able to record medical institutions every time and dual treatment. Each is different specifications for each medical institution to use the program or document to exchange it. The personal clinic records still does not exchange well. To solve this gap between medical alienation, this paper describes the concepts of HL7-CDA and proposes types of telemedicine system. To resolve time and space constraints, new form of treatment methods presents in future directions after described about related systems. CDA enables electronic medical records to the each medical center and gradually expanded by exchanging the patient's medical records. This paper is using XML-based CDA documents as a hierarchical for medical information exchange standards compliant HL7-CDA documents. It could be possible currently used structural variety of multimedia data. Thus It is able to send and receive HL7-CDA-based medical information and clinical information to identify the medical institutions of medical information with interchange system design and building standards, and through mutual exchange of clinical information.
The purpose of this study is to describe the current practises and procedure of building the authority file for corporate names and conference names for Korea Institute of Science and Technology Information Authority Database, and the characteristics of OCLC WorldCat and Library of Congress Authorities Database as the sources for importing authority records. Workflow of creating authorities records is explained and the structures and search strategies for OCLC and LC databases are compared and analyzed. The actual procedures of importing records from outside sources and revising their contents to represent KISTI's needs are described step by step, and records are illustrated.
The specific characteristics of near-field earthquake records can lead to different dynamic responses of bridges compared to far-field records. However, the effect of near-field strong ground motion has often been neglected in the seismic performance assessment of the bridges. Furthermore, damage to horizontally curved multi-frame RC box-girder bridges in the past earthquakes has intensified the potential of seismic vulnerability of these structures due to their distinctive dynamic behavior. Based on the nonlinear time history analyses in OpenSEES, this article, assesses the effects of near-field versus far-field earthquakes on the seismic performance of horizontally curved multi-frame RC box-girder bridges by accounting the vertical component of the earthquake records. Analytical seismic fragility curves have been derived thru considering uncertainties in the earthquake records, material and geometric properties of bridges. The findings indicate that near-field effects reasonably increase the seismic vulnerability in this bridge sub-class. The results pave the way for future regional risk assessments regarding the importance of either including or excluding near-field effects on the seismic performance of horizontally curved bridges.
Objectives The Korean Medicine (KM) PHR platform is a personalized healthcare service which allows individuals to keep and manage their own health records. When parents are reporting for their children from their memories, there is high possibility of recall errors. In these cases, it could be useful for doctors to collect the patient's symptoms through PHR platform. In this study, we aimed to investigate the clinical significance of the PHR by using the KM PHR platform in the pediatric clinic. Methods The PHR platform was used to collect child health information from parents and child care teachers. The collected data were analyzed in comparison with the results of screening by pediatrician. Results A total of 58 children were recruited, 44 of which health information were collected from their parents and their child care teachers. The remaining 14 children's health information were collected from their parents only. As a result the parents tended to perceive their children weaker than the child care teacher. Compared to other organs, there was a only significant difference in the heart weakness score and spleen weakness score in the comparison of the weak and healthy children. Conclusions Although the study was conducted on a small group of subjects, and used PHR platform developed specifically for adults to indirectly input child's symptoms, and analyzed their health information, there was a difference in health records between information providers. Development of PHR platform for children is needed to collect more reliable information.
Considering the vast usage of time-history dynamic analyses to calculate structural responses and lack of sufficient and suitable earthquake records, generation of artificial accelerograms is very necessary. The main target of this paper is to present a novel method based on nonstationary Kanai-Tajimi model and wavelet transform to generate more artificial earthquake records, which are compatible with target spectrum. In this regard, the generalized nonstationary Kanai-Tajimi model to include the nonstationary evaluation of amplitude and dominant frequency of ground motion and properties of wavelet transform is used to generate ground acceleration time history. Application of the method for El Centro 1940 earthquake and two Iranian earthquakes (Tabas 1978 and Manjil 1990) is presented. It is shown that the model and identification algorithms are able to accurately capture the nonstationary features of these earthquake accelerograms. The statistical characteristics of the spectral response of the generated accelerograms are compared with those for the actual records to demonstrate the effectiveness of the method. Also, for comparison of the presented method with other methods, the response spectra of the synthetic accelerograms compared with the models of Fan and Ahmadi (1990) and Rofooei et al. (2001) and it is shown that the response spectra of the synthetic accelerograms with the method of this paper are close to those of actual earthquakes.

Purpose: In this study, we retrospectively investigated the medical records of patients with facial fractures and suspected cranial injuries in order to determine if there was any relationship between various facial fracture patterns and cranial injuries. Methods: Medical records were reviewed to identify patients diagnosed with facial fractures who underwent cranial computed tomography (CT) scans. Records were reviewed for gender, age, injury mechanism, facial fracture pattern, and presence or absence of cranial injuries. Facial fracture patterns were classified as isolated fractures (tripod, zygomatic arch, maxilla, orbit, and mandible), combined fractures, or total fractures. Cranial injuries included skull fractures, traumatic subarachnoid hemorrhages, subdural hemorrhages, epidural hemorrhages, and contusional hemorrhages. All cranial injuries were established by using cranial CT scans, and these kinds of cranial injuries were defined radiologically-proven cranial injuries (RPCIs). We evaluated the relationship between each pattern of facial fractures and the incidence of RPCIs. Results: Of 132 eligible patients with facial fractures who underwent cranial CT scans, a total of 27 (20.5%) patients had RPCIs associated with facial fractures. Falls and slips were the most common causes of the fractures (31.8%), followed by assaults and motor vehicle accidents (MVAs). One hundred one (76.5%) patients had isolated facial fractures, and 31 (23.5%) patients had combined facial fractures. Fractures were found most commonly in the orbital and maxillary bones. Patients with isolated maxillary fractures had a lower incidence of RPCIs than those with total mandibular fractures. RPCIs frequently accompanied combined facial fractures. Conclusion: Combined facial fractures had a significant positive correlation with RPCIs. This means that facial fractures caused by stronger or multidirectional external force are likely to be accompanied by cranial injuries.

Purpose: To explore the scope and method of applying standardized nursing terminologies to nursing diagnosis and problems used in nursing practice. Methods: A descriptive study was done with a retrospective analysis of the nursing records of 141,420 patients that were hospitalized in 4 tertiary hospitals. The nursing diagnosis and problems collected from the records were standardized, and the standardized nursing diagnosis and problems cross mapped with NANDA-I, confirmed in a nursing focus group. Results: 65 (67.7%) of the 96 standardized nursing diagnosis and problems were equal with NANDA-I and included in the 10 domains of NANDA-I. Among 86 nursing diagnosis and problems excluded from the cross mapping with NANDA-I, the 63 terms (73.3%) related to surgery/procedure were the most common. Conclusion: It is meaningful that multi-tertiary hospital nursing diagnosis and problems were standardized and cross mapping with standard nursing terminologies was performed. As for the method of applying standardized nursing terminologies in nursing practice, it is appropriate to use several standardized nursing terminologies complementarily.
This study was done in order to analyze the factors which influenced the performance level of urban family planning health personnels' task. Interview survey was done during the period May, 1989. Interviewee were 130 family planning health personnels among total of 140 family planning health personnels in Seoul metropolitan city area. The internal consistency reliability was tested by Cronbach’s Alpha and the construct validity of the survey tool was tested by Factor analysis. Multiple stepwise regression analysis was used to identify major factors influenced to perform family planning health personnels' task. The results of this study were summarized as follows: 1. The internal consistency reliability was high and very significant (Cronbach's Alphs=0.8445, p<0.0000). 2. The construct validity was high and very significant. Objectives : King Hyojong (1619-1659) was the 17th king of the Joseon Dynasty. He was on the throne from 1649 to 1659. The king was a center of state rule in ancient times. This meant that the physical and mental status of a king was very confidential information. In the royal court of Joseon Korea, the treatment process for the king had to be performed under the surveillance of close associates, and this whole process had to be recorded by the official secretary. This is the reason that I referred to the Daily Records of Royal Secretariat of Joseon Dynasty. Methods : Relevant articles were extracted from Daily Records of Royal Secretariat of Joseon Dynasty and analyzed according to the main arguments of this paper. Some other related sources were referred in terms of King Hyojong's ailments and treatments. Results & Conclusions : King Hyojong suffered from various diseases during his time on the throne. Fever was the most frequent ailment; he also often had diarrhea followed by poor appetite. I assume that he must have suffered from diabetes considering his ailments. Additionally, his final treatment records clearly show that his ailments were similar to diabetic complications.

Until recently lots of time-history seismic analyses were performed with the earthquake motions recorded at the soft soil surface without taking into account the effects of the soft soil amplification. However, it is important to utilize the bedrock seismic motions for the rational seismic analyses of a structure considering the site soil conditions. In this study, 26 bedrock earthquake records were selected from publicly available 1557 seismic records provided by the Pacific Earthquake Engineering Research Center (PEER) for the study, and the characteristics of them were investigated. Study results showed that it is not reasonable to estimate earthquake acceleration intensity from the magnitude of an earthquake without considering the site soil conditions and it is also hard to draw any general relationships between earthquake acceleration intensity, earthquake magnitude and epicenter distance with bedrock earthquake records in the PEER database. However, 26 bedrock earthquake records selected in this study can be utilized for the time-history seismic analyses of a structure-soil system as bedrock earthquake ones, and it is also confirmed that it is necessary to take into account acceleration intensity, magnitude, epicenter distance and site conditions simultaneously for the proper use of those selected earthquake records.
