High Resolution HC$_3$N Observations toward the Central Region of Sagittarius B2

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500,550,570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500,550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100,300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. The results showed that the functional characteristics of the polymer valve compared favorably with those of the mechanical valve at higher heart rate. 30621 T00401030621 ^x Explosive evaporative removal process of biological tissue by absorption of a CW laser has been simulated by using gelatin and a multimode Nd:YAG laser. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. We propose a new image reconstruction method using internal current density data measured by NMR. We obtained a two-dimensional current density distribution within a phantom by processing the real and imaginary MR images from a 4.77 NMR machine. We implemented a resistivity mage reconstruction algorithm using the finite element method and sensitivity matrix. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. Third, best-fit boundary is detected by the cost function which is based on the best-fit elliptic model. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. EMG signals are acquired by using a amputation simulator and 2 dimensional joystick motion. The bedside monitor consists of a main unit and module cases with various parameter modules. The main unit includes a 12.1" TFT color LCD, a main CPU board, and peripherals such as a module controller, Ethernet LAN card, video card, rotate/push button controller, etc. The main unit can connect at maximum three module cases each of which can accommodate up to 7 parameter modules. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.

새로운 결제서비스의 성공요인: 다중사례연구 (Critical Success Factor of Noble Payment System: Multiple Case Studies)

  • 결제서비스에 대한 기존의 연구는 결제서비스의 채택요인 또는 지속적인 사용에 영향을 미치는 요인 등 행동이론을 중심으로 진행되어 왔다. 이러한 요인들이 미치는 영향에 대한 결과는 결제서비스의 종류에 따라 또는 연구 지역에 따라 상이하게 나타나고 있다. 본 연구는 결제 서비스의 종류나 문화등의 변수에 관계없이 새로운 결제 서비스가 성공할 수 있는 일반적인 요인이 무엇인지에 대한 의문에서 시작하게 되었다. 기존 연구에서 중요한 영향을 미친다고 제시한 채택요인들은 실제 결제사례의 결과에 비추어 보면 기존 연구에서 주장한 바와 일치하지 않는 경우를 볼 수 있다. 이러한 이론과 현실사이의 괴리를 발견하고 새로운 결제서비스가 성공하기 위한 근본적이고 결정적인 요인이 무엇인지에 대해 제시하고 사례연구를 통해 가설을 입증하고자 하는 것이 본 연구의 목적이다. 따라서 본 연구는 새로운 결제서비스가 성공하기 위해서는 기존 결제서비스의 비고객에게 이들이 결제할 수 있는 수단을 제공함으로써 새로운 결제 시장을 창출해야 함을 주장한다. 이를 위해 성공한 결제사례인 신용카드, 휴대폰 소액결제, PayPal, Square을 채택하였으며, 기존 결제서비스의 비고객을 3개의 계층으로 분류하여 분석하였다. 그리고 새로운 결제서비스가 어떠한 계층을 타겟으로 하였으며 이들에게 어떠한 결제수단을 제공하여 새로운 시장을 창출하였는지 제시한다. 사례 분석 결과, 성공 사례 모두 본 연구의 가설을 지지하는 것으로 나타났다. 따라서 새로운 결제서비스는 결국 기존의 결제수단으로 거래를 할 수 없었던 이들이 결제를 할 수 있도록 함으로써 성공할 수 있다는 가설을 입증하였다. 모바일 결제서비스가 아직 대중화되지 못한 원인을 본 가설에 비추어 분석해 보면 보면, 기존의 결제 인프라를 이용할 수 있는 바코드, QR코드 기반의 모바일 결제 서비스뿐만 아니라 NFC, BLE, 음파 등의 새로운 기술이 적용된 모바일 결제 서비스가 출시되는 등 새로운 시도가 계속되고 있다. 또한 모바일 월렛은 사용자들이 소지하고 있는 카드정보를 스마트폰에 저장하여 지갑 없이도 결제가 가능하며, 쿠폰 제공, 적립카드 관리, 신분증을 저장하는 등의 다양한 부가적인 기능을 제공하고 있어 성공할 것이라는 전망이 대두되고 있다. 하지만 이러한 서비스들은 본 연구 관점에서 보자면 기존 결제서비스의 비고객이(기존 결제수단을 이용할 수 없었던 사용자) 거래할 수 있는 새로운 결제 수단을 제공해 주지 못하고 있기 때문에 결국 초기사용자에게만 채택될 뿐 대중화되는데 한계가 있을 것으로 예상된다. 반면, 새로운 모바일 결제서비스의 성공사례 중 하나인 PaybyPhone은 기존 코인주차 결제서비스의 비고객인 현금 미소지 고객에게 스마트폰을 이용한 새로운 결제수단을 제공함으로써 새로운 주차 결제 시장을 창출하였으며 현재 미국뿐만 아니라 유럽시장까지 진출하는 등 급성장하고 있다. 결론적으로, 많은 이해관계자들이 모바일 결제시장을 선점하기 위해 다양한 형태의 모바일 결제 서비스를 출시하고 있지만 캐즘을 뛰어넘어 주류 시장에 성공적으로 정착할 수 있느냐는 결국 기존 결제서비스의 비고객군에게 그들이 필요로 하는 새로운 결제수단을 제공하는지의 여부에 달려있다고 볼 수 있다. 따라서 모바일 결제 서비스의 기획자나 매니저들은 서비스 기획 시 기존 결제서비스의 비고객군은 누구인가? 그들은 어떠한 결제수단을 원하는가?를 먼저 고려해야 한다. 본 연구는 새로운 결제서비스가 성공하는데 미치는 요인에 대한 가설을 검증하기 위해 4개의 성공사례를 선택하였으며 각 사례에 동일한 가설을 검증하는 '반복연구논리'를 적용하였다. 본 가설을 더욱 공고히 하기 위해 사례연구방법론에서 제시하고 있는 경쟁가설을 포함한 후속 사례연구가 진행되어야 할 것이다.

  • 최근까지도 중소기업의 지속성장 및 경쟁력 확보에 대한 중요함을 인식함에 따라, 정부 차원에서의 유형 자원(R&D 인력, 자금 등)에 대한 지원이 주로 투입되어 왔다. 그러나 사업지원의 적절성이나 효과성, 효율성 면에서 서로 상충되는 정책부분이 존재하여 과소 지원이나 중복 지원 등 지원체계의 비효율성 문제가 제기되어온 것도 사실이다. 정부나 기업 관점에서는 중소기업의 한정된 자원으로 인해, 외부와의 협력을 통한 기술개발 및 역량강화가 기업의 경쟁우위를 창출하는 근간이라 보고 있으며, 이를 위한 가치창출 활동을 강조하고 있다. 기업 레벨에서의 지식생태계 구축을 통해 일련의 가치사슬로부터 기업거래 관계를 분석하고 결과를 가시화할 수 있는 밸류체인 네트워크 분석이 필요한 것도 이 때문이다. 특허/제품/기업명 검색을 통해 관련 제품의 정보나 특허 보유 기업의 기술(제품) 현황 정보를 제공하는 기술기회발굴시스템(Technology Opportunity Discovery system), 기업(재무)정보와 신용정보을 열람하게 해주는 CRETOP이나 KISLINE 등은 존재하고 있으나 밸류체인 네트워크 분석기반으로 유사(경쟁)기업의 리스트나 향후 거래 가능한 잠재 거래처 정보를 제공해주는 시스템은 부재한 실정이다. 따라서, 본 고에서는 KISTI에서 개발 운영중인 기업 비즈니스 전략수립 지원 파트너인 '밸류체인 네트워크 시스템(Value Chain Network System : VCNS)'을 중심으로, 탑재된 네트워크 기반 분석모듈의 유형, 이를 지원하는 참조정보 및 데이터베이스(D/B)의 구성 로직과 시스템 활용방안을 고찰하며, 산업구조를 이해하고 기업의 신제품 개발을 위한 핵심정보가 되고 있는 지능형 밸류체인 분석 시스템의 네트워크 가시화 기능을 살펴보기로 한다. 한 기업이 다른 기업 대비 경쟁우위를 확보하기 위해서는 보유 특허 또는 현재 생산하고 있는 제품에 대한 경쟁자 식별이 필요하며, 세부 업종별 유사(경쟁)기업을 탐색하는 일은 대상기업의 사업화 경쟁력 확보에 핵심이 된다. 또한 기업간 비즈니스 활동인 거래정보는 유사 분야로 진출할 경우 잠재 거래처 정보를 제공하는 중요한 역할을 수행한다. 이러한 기업간 판매정보를 기반으로 구축된 네트워크 맵을 활용하여 기업 또는 업종 수준의 경쟁자를 식별하는 일은 밸류체인 분석의 핵심모듈로 탑재될 수 있다. 밸류체인 네트워크 시스템(VCNS)은 단순 수집된 종래의 기업정보에 밸류체인(value chain) 및 산업구조 분석개념을 접목하여 개별 기업의 시장경쟁 상황은 물론 특정 산업의 가치사슬 관계를 파악할 수 있다. 특히 업종구조 파악, 경쟁사 동향 파악, 경쟁사 분석, 판매처 및 구매처 발굴, 품목별 산업동향, 유망 품목 발굴, 신규 진입기업 발굴, VC별 핵심기업 및 품목 도출, 해당 기업별 보유 특허 파악 등 기업 레벨에서의 유용한 정보분석 툴로 활용 가능하다. 또한, 거래처 정보 및 재무데이터로부터 분석된 결과의 객관성 및 신뢰성을 기반으로, 현재 국내에서 이용 중인 15,000여개 회원기업과 연구개발서비스업 종사자, 출연(연) 및 공공기관 등에서 사업평가 정보지원, R&D 의사결정 지원 및 중 단기 수요예측 전망 등 다양한 목적(용도)에 밸류체인 네트워크 시스템을 활용할 수 있을 것으로 기대된다. 기업의 사업경쟁력 강화를 위해 정부기관 및 민간 연구개발서비스 기업을 중심으로 기술(특허) 및 시장정보가 제공되어 왔으며, 이는 특허분석(등급, 계량분석 위주) 또는 시장분석(시장보고서 기반 시장규모 및 수요예측 위주)의 형태로 지원되어 왔다. 그러나 기업이 사업화진출 단계에서 겪게 되는 애로요인의 하나인 정보부족을 해결하는데 한계가 있었으며, 특히 경쟁기업 및 거래가능 기업 후보군에 대한 탐색정보는 입수하기 어려웠다. 본 연구를 통해 제안된 네트워크맵 및 보유 데이터 기반의 실시간 밸류체인 가시화 서비스모듈이 중견 중소기업이 당면한 신규시장 진출시 경쟁기업 대비 예상점유율, (예상)매출액 수준, 어느 기업을 컨택하여 유통망(원자재/부품에 대한 공급처, 완제품/모듈에 대한 수요처)을 확보할 지에 대한 핵심정보를 제공할 수 있을 것으로 기대된다. 향후 연구에서는 대체기업(또는 대체품목) 경쟁지표의 개발과 연구주체의 참여를 통한 경쟁요인별 지표의 고도화 연구, VCNS의 성능향상을 위한 데이터마이닝 기술 및 알고리즘을 추가 반영하도록 수행하고자 한다.

특발성 폐섬유화증에 동반된 폐암 환자의 임상적 특정 (The Clinical Characteristics of Lung Cancer in Patients with Idiopathic Pulmonary Fibrosis)

  • 연구배경 : 특발생 폐섬유화증에서 폐암의 발생이 높다고 알려져 왔으나 실제로는 보고자마다 차이가 많고 가장 많은 조직형에 대해서도 논란이 많다. 또한 동반된 폐섬유증에 의해 폐암의 발견시기나 치료방침, 예후 등이 다를 것이 예상되나 아직 이에 대한 보고는 없는 실정이다. 본 연구는 특발성 폐섬유화증이 동반된 폐암 환자에서 위험인자, 폐암의 발생부위 및 조직학적 분포, 특히 폐암과 폐섬유화 부위의 일치도 등을 분석하여 특발성 폐섬유화증이 폐암 발생에 미친 영향을 알아보고, 특발성 폐섬유화증이 폐암의 치료에 미친 영향을 알아보고자 시행되었다. 방 법 : 서울중앙병원을 방문하였던 특발성 폐섬유화증 환자중 폐암이 동반되었던 63명($66.8{\pm}:7.8$세, 남 : 녀= 61 : 2)을 대상으로 폐암의 위치, 조직형, 폐암 진단시 폐기능 검사소견, 폐암의 치료형태, 폐암 진단 후 생존기간 등을 후향적으로 조사하였다. 또한 대조군으로 본원을 내원한 2,660명의 대조폐암환자와 조직형의 차이를 분석하였다. 결 과 : 특발성 폐섬유화증환자들 중 22.4%(63/281명)에서 폐암이 동반되었다(IPF-CA). 45명(71.4%)의 환자에서는 폐암이 동시에 발견되었고 18명(28.6%)에서는 특발성 폐섬유화증 진단 후 25.2(3.7-42.9)개월이 지나 폐암이 발견되어, 폐섬유증의 경과 도중에 폐암의 발생율은 6.7%이었다. 폐 말초부에 위치한 폐암은 35명(55.5%) 뿐 이었고, 28명(44.4%)에서는 중심부 폐암이었으며, 폐하엽 보다는 상엽에 33명(51.6%)으로 더 많았다. 전체 특발성 폐섬유화증군에 비하여 특발성 폐섬유화증에 동반된 폐암군에서 연령 ($66.8{\pm}:7.84$ vs. $63.4{\pm}:11.1$세, p=0.007), 남성 비율 (96.8 vs. 67.6%, p<0.001), 흡연력 (88.9 vs. 67.2%, p<0.001)이 의미있게 높았다. 폐섬유화 병변과 일치한 부위에 폐암이 발생한 경우는 23명(35.9%) 뿐 이었고, 40명(62.5%)은 폐섬유화 병변과 다른 부위에서 폐암이 발생하였다. 특발성 폐섬유화증에 동반된 폐암의 조직형은 편평상피암 22명(34.9%), 선암 19명(30.2%), 소세포암 12명(19.0%), 대세포암 4명(6.3%), 기타가 6명(9.5%)이었다. 2,660명의 대조폐암환자군의 조직형은 편평상피암 44.6%, 선암 34.2%, 소세포암 14.7%, 대세포암 1.5%, 기타 5.0%으로 조직형의 분포에 있어 두 군간에 유의한 차이는 없었다. 특발성 폐섬유화증의 경과 도중에 폐암이 발생한 군과 특발성 폐섬유화증과 폐암이 동시에 발견된 군간에 인적사항이나, 폐암의 위치, 조직형 및 병기 등에 유의한 차이를 관찰 할 수 없었으나 중증 섬유화증(방사선학적 소견상 폐침범 정도가 50% 이상)은 특발성 폐섬유화증 경과 도중에 폐암이 발견된 군에서 특발성 폐섬유화증과 폐암 동시 발견군에 비하여 심한 양상을 나타내었다(33.3 % vs. 61.1%, p=0.043). 폐섬유화와 일치한 부위의 폐암군은 방사선학적으로 폐섬유화가 더 광범위하게 진행된 경향을 보였고 중앙값생존기간이 폐섬유화와 다른 부위의 폐암군에 비하여 낮았다(4개월 vs. 7개월, p=0.04). 비소세포암군에서 병기 IIIa 이하인 19명중 33.3%(폐엽절제술 6명, 전폐절제술 1명)에서만 수술이 시행되었다. 10명은 수술 후 잔여 폐기량과 활동능력을 고려하여 수술을 시행하지 못하였고 2명은 수술을 거부하였다. 결 론 : 특발성 폐섬유화증에서 폐암 발생은 폐섬유화에 의한 영향보다는 연령, 흡연 및 성별(남성) 등의 위험인자와 더 연관되는 것으로 사료된다.

소비자의 제품 지각 위험에 대한 기업연상과 효과: 지식과 관여의 조절적 역활을 중심으로 (The Effect of Corporate Association on the Perceived Risk of the Product)

  • 기업연상(corporate association)이 제품 평가(product responses)에 어떻게 영향을 미치는 가에 대한 연구가 부진하다는 Brown and Dacin(1997)의 문제 제기가 있은 후, 기업연상이 제품 판단에 미치는 영향과 과정에 대한 조절변수와 매개변수들을 파악하려는 연구가 진행되어 왔다. 본 연구에서는 기업연상의 두가지 유형인 CA(corporate ability) 연상과 CSR(corporate social responsibility) 연상이 성능과 재무위험에 미치는 영향력과 그 영향력을 조절하는 변수들을 조사하였다. 분석 결과에 의하면, 주효과(main effects)에 있어서는, 가설에서 기대한 바와 같이 CA 연상이 성능위험과 재무위험에 유의한 영향력을 갖는 것으로 나타난 반면, CSR 연상은 성능위험과 재무위험에 대해 유의한 영향력을 갖지 않는 것으로 나타났다. 조절변수로 인한 상호작용효과와 관련해서는, CA 연상이 성능위험과 재무위험에 미치는 주효과에 대해 제품범주 지식과 관여는 각각 유의한 조절효과를 나타내었다. 하지만, CSR 연상이 성능위험과 재무위험에 미치는 주효과에 대해서는 제품범주 지식과 관여의 조절효과는 나타나지 않았다. 이러한 연구 결과를 통하여 제품의 기능적인 속성에 대한 정보가 부족한 제품에 대해 소비자가 지각하는 위험을 감소시키기 위하여, 기업은 CSR 연상보다는 CA 연상에 대해 강조할 필요가 있다는 결론을 내리게 되었다.

병원 간호행정 개선을 위한 연구 (A Study for Improvement of Nursing Service Administration)

  • Much has teed changed in the field of hospital administration in the It wake of the rapid development of sciences, techniques ana systematic hospital management. However, we still have a long way to go in organization, in the quality of hospital employees and hospital equipment and facilities, and in financial support in order to achieve proper hospital management. The above factors greatly effect the ability of hospitals to fulfill their obligation in patient care and nursing services. The purpose of this study is to determine the optimal methods of standardization and quality nursing so as to improve present nursing services through investigations and analyses of various problems concerning nursing administration. This study has been undertaken during the six month period from October 1971 to March 1972. The 41 comprehensive hospitals have been selected iron amongst the 139 in the whole country. These have been categorized according-to the specific purposes of their establishment, such as 7 university hospitals, 18 national or public hospitals, 12 religious hospitals and 4 enterprise ones. The following conclusions have been acquired thus far from information obtained through interviews with nursing directors who are in charge of the nursing administration in each hospital, and further investigations concerning the purposes of establishment, the organization, personnel arrangements, working conditions, practices of service, and budgets of the nursing service department. 1. The nursing administration along with its activities in this country has been uncritical1y adopted from that of the developed countries. It is necessary for us to re-establish a new medical and nursing system which is adequate for our social environments through continuous study and research. 2. The survey shows that the 7 university hospitals were chiefly concerned with education, medical care and research; the 18 national or public hospitals with medical care, public health and charity work; the 2 religious hospitals with medical care, charity and missionary works; and the 4 enterprise hospitals with public health, medical care and charity works. In general, the main purposes of the hospitals were those of charity organizations in the pursuit of medical care, education and public benefits. 3. The survey shows that in general hospital facilities rate 64 per cent and medical care 60 per-cent against a 100 per cent optimum basis in accordance with the medical treatment law and approved criteria for training hospitals. In these respects, university hospitals have achieved the highest standards, followed by religious ones, enterprise ones, and national or public ones in that order. 4. The ages of nursing directors range from 30 to 50. The level of education achieved by most of the directors is that of graduation from a nursing technical high school and a three year nursing junior college; a very few have graduated from college or have taken graduate courses. 5. As for the career tenure of nurses in the hospitals: one-third of the nurses, or 38 per cent, have worked less than one year; those in the category of one year to two represent 24 pet cent. This means that a total of 62 per cent of the career nurses have been practicing their profession for less than two years. Career nurses with over 5 years experience number only 16 per cent: therefore the efficiency of nursing services has been rated very low. 6. As for the standard of education of the nurses: 62 per cent of them have taken a three year course of nursing in junior colleges, and 22 per cent in nursing technical high schools. College graduate nurses come up to only 15 per cent; and those with graduate course only 0.4 per cent. This indicates that most of the nurses are front nursing technical high schools and three year nursing junior colleges. Accordingly, it is advisable that nursing services be divided according to their functions, such as professional, technical nurses and nurse's aides. 7. The survey also shows that the purpose of nursing service administration in the hospitals has been regulated in writing in 74 per cent of the hospitals and not regulated in writing in 26 per cent of the hospitals. The general purposes of nursing are as follows: patient care, assistance in medical care and education. The main purpose of these nursing services is to establish proper operational and personnel management which focus on in-service education. 8. The nursing service departments belong to the medical departments in almost 60 per cent of the hospitals. Even though the nursing service department is formally separated, about 24 per cent of the hospitals regard it as a functional unit in the medical department. Only 5 per cent of the hospitals keep the department as a separate one. To the contrary, approximately 12 per cent of the hospitals have not established a nursing service department at all but surbodinate it to the other department. In this respect, it is required that a new hospital organization be made to acknowledge the independent function of the nursing department. In 76 per cent of the hospitals they have advisory committees under the nursing department, such as a dormitory self·regulating committee, an in-service education committee and a nursing procedure and policy committee. 9. Personnel arrangement and working conditions of nurses 1) The ratio of nurses to patients is as follows: In university hospitals, 1 to 2.9 for hospitalized patients and 1 to 4.0 for out-patients; in religious hospitals, 1 to 2.3 for hospitalized patients and 1 to 5.4 for out-patients. Grouped together this indicates that one nurse covers 2.2 hospitalized patients and 4.3 out-patients on a daily basis. The current medical treatment law stipulates that one nurse should care for 2.5 hospitalized patients or 30.0 out-patients. Therefore the statistics indicate that nursing services are being peformed with an insufficient number of nurses to cover out-patients. The current law concerns the minimum number of nurses and disregards the required number of nurses for operation rooms, recovery rooms, delivery rooms, new-born baby rooms, central supply rooms and emergency rooms. Accordingly, tile medical treatment law has been requested to be amended. 2) The ratio of doctors to nurses: In university hospitals, the ratio is 1 to 1.1; in national of public hospitals, 1 to 0.8; in religious hospitals 1 to 0.5; and in private hospitals 1 to 0.7. The average ratio is 1 to 0.8; generally the ideal ratio is 3 to 1. Since the number of doctors working in hospitals has been recently increasing, the nursing services have consequently teen overloaded, sacrificing the services to the patients. 3) The ratio of nurses to clerical staff is 1 to 0.4. However, the ideal ratio is 5 to 1, that is, 1 to 0.2. This means that clerical personnel far outnumber the nursing staff. 4) The ratio of nurses to nurse's-aides; The average 2.5 to 1 indicates that most of the nursing service are delegated to nurse's-aides owing to the shortage of registered nurses. This is the main cause of the deterioration in the quality of nursing services. It is a real problem in the guest for better nursing services that certain hospitals employ a disproportionate number of nurse's-aides in order to meet financial requirements. 5) As for the working conditions, most of hospitals employ a three-shift day with 8 hours of duty each. However, certain hospitals still use two shifts a day. 6) As for the working environment, most of the hospitals lack welfare and hygienic facilities. 7) The salary basis is the highest in the private university hospitals, with enterprise hospitals next and religious hospitals and national or public ones lowest. 8) Method of employment is made through paper screening, and further that the appointment of nurses is conditional upon the favorable opinion of the nursing directors. 9) The unemployment ratio for one year in 1971 averaged 29 per cent. The reasons for unemployment indicate that the highest is because of marriage up to 40 per cent, and next is because of overseas employment. This high unemployment ratio further causes the deterioration of efficiency in nursing services and supplementary activities. The hospital authorities concerned should take this matter into a jeep consideration in order to reduce unemployment. 10) The importance of in-service education is well recognized and established. 1% has been noted that on the-job nurses. training has been most active, with nursing directors taking charge of the orientation programs of newly employed nurses. However, it is most necessary that a comprehensive study be made of instructors, contents and methods of education with a separate section for in-service education. 10. Nursing services'activities 1) Division of services and job descriptions are urgently required. 81 per rent of the hospitals keep written regulations of services in accordance with nursing service manuals. 19 per cent of the hospitals do not keep written regulations. Most of hospitals delegate to the nursing directors or certain supervisors the power of stipulating service regulations. In 21 per cent of the total hospitals they have policy committees, standardization committees and advisory committees to proceed with the stipulation of regulations. 2) Approximately 81 per cent of the hospitals have service channels in which directors, supervisors, head nurses and staff nurses perform their appropriate services according to the service plans and make up the service reports. In approximately 19 per cent of the hospitals the staff perform their nursing services without utilizing the above channels. 3) In the performance of nursing services, a ward manual is considered the most important one to be utilized in about 32 percent of hospitals. 25 per cent of hospitals indicate they use a kardex; 17 per cent use ward-rounding, and others take advantage of work sheets or coordination with other departments through conferences. 4) In about 78 per cent of hospitals they have records which indicate the status of personnel, and in 22 per cent they have not. 5) It has been advised that morale among nurses may be increased, ensuring more efficient services, by their being able to exchange opinions and views with each other. 6) The satisfactory performance of nursing services rely on the following factors to the degree indicated: approximately 32 per cent to the systematic nursing activities and services; 27 per cent to the head nurses ability for nursing diagnosis; 22 per cent to an effective supervisory system; 16 per cent to the hospital facilities and proper supply, and 3 per cent to effective in·service education. This means that nurses, supervisors, head nurses and directors play the most important roles in the performance of nursing services. 11. About 87 per cent of the hospitals do not have separate budgets for their nursing departments, and only 13 per cent of the hospitals have separate budgets. It is recommended that the planning and execution of the nursing administration be delegated to the pertinent administrators in order to bring about improved proved performances and activities in nursing services.

가족계획과 모자보건 통합을 위한 조산원의 투입효과 분석 -서산지역의 개입연구 평가보고- (An Intervention Study on Integration of Family Planning and Maternal/Infant Care Services in Rural Korea)

  • This project was a service-cum-research effort with a quasi-experimental study design to examine the health benefits of an integrated Family Planning (FP)/Maternal & Child health (MCH) Service approach that provides crucial factors missing in the present on-going programs. The specific objectives were: 1) To test the effectiveness of trained nurse/midwives (MW) assigned as change agents in the Health Sub-Center (HSC) to bring about the changes in the eight FP/MCH indicators, namely; (i)FP/MCH contacts between field workers and their clients (ii) the use of effective FP methods, (iii) the inter-birth interval and/or open interval, (iv) prenatal care by medically qualified personnel, (v) medically supervised deliveries, (vi) the rate of induced abortion, (vii) maternal and infant morbidity, and (viii) preinatal & infant mortality. 2) To measure the integrative linkage (contacts) between MW & HSC workers and between HSC and clients. 3) To examine the organizational or administrative factors influencing integrative linkage between health workers. Study design; The above objectives called for quasi-experimental design setting up a study and control area with and without a midwife. An active intervention program (FP/MCH minimum 'package' program) was conducted for a 2 year period from June 1982-July 1984 in Seosan County and 'before and after' surveys were conducted to measure the change. Service input; This study was undertaken by the Soonchunhyang University in collaboration with WHO. After a baseline survery in 1981, trained nurses/midwives were introduced into two health sub-centers in a rural setting (Seosan county) for a 2 year period from 1982 to 1984. A major service input was the establishment of midwifery services in the existing health delivery system with emphasis on nurse/midwife's role as the link between health workers (nurse aids) and village health workers, and the referral of risk patients to the private physician (OBGY specialist). An evaluation survey was made in August 1984 to assess the effectiveness of this alternative integrated approach in the study areas in comparison with the control area which had normal government services. Method of evaluation; a. In this study, the primary objective was first to examine to what extent the FP/MCH package program brought about changes in the pre-determined eight indicators (outcome and impact measures) and the following relationship was first analyzed; b. Nevertheless, this project did not automatically accept the assumption that if two or more activities were integrated, the results would automatically be better than a non-integrated or categorical program. There is a need to assess the 'integration process' itself within the package program. The process of integration was measured in terms of interactive linkages, or the quantity & quality of contacts between workers & clients and among workers. Intergrative linkages were hypothesized to be influenced by organizational factors at the HSC clinic level including HSC goals, sltrurture, authority, leadership style, resources, and personal characteristics of HSC staff. The extent or degree of integration, as measured by the intensity of integrative linkages, was in turn presumed to influence programme performance. Thus as indicated diagrammatically below, organizational factors constituted the independent variables, integration as the intervening variable and programme performance with respect to family planning and health services as the dependent variable: Concerning organizational factors, however, due to the limited number of HSCs (2 in the study area and 3 in the control area), they were studied by participatory observation of an anthropologist who was independent of the project. In this observation, we examined whether the assumed integration process actually occurred or not. If not, what were the constraints in producing an effective integration process. Summary of Findings; A) Program effects and impact 1. Effects on FP use: During this 2 year action period, FP acceptance increased from 58% in 1981 to 78% in 1984 in both the study and control areas. This increase in both areas was mainly due to the new family planning campaign driven by the Government for the same study period. Therefore, there was no increment of FP acceptance rate due to additional input of MW to the on-going FP program. But in the study area, quality aspects of FP were somewhat improved, having a better continuation rate of IUDs & pills and more use of effective Contraceptive methods in comparison with the control area. 2. Effects of use of MCH services: Between the study and control areas, however, there was a significant difference in maternal and child health care. For example, the coverage of prenatal care was increased from 53% for 1981 birth cohort to 75% for 1984 birth cohort in the study area. In the control area, the same increased from 41% (1981) to 65% (1984). It is noteworthy that almost two thirds of the recent birth cohort received prenatal care even in the control area, indicating that there is a growing demand of MCH care as the size of family norm becomes smaller 3. There has been a substantive increase in delivery care by medical professions in the study area, with an annual increase rate of 10% due to midwives input in the study areas. The project had about two times greater effect on postnatal care (68% vs. 33%) at delivery care(45.2% vs. 26.1%). 4. The study area had better reproductive efficiency (wanted pregancies with FP practice & healthy live births survived by one year old) than the control area, especially among women under 30 (14.1% vs. 9.6%). The proportion of women who preferred the 1st trimester for their first prenatal care rose significantly in the study area as compared to the control area (24% vs 13%). B) Effects on Interactive Linkage 1. This project made a contribution in making several useful steps in the direction of service integration, namely; i) The health workers have become familiar with procedures on how to work together with each other (especially with a midwife) in carrying out their work in FP/MCH and, ii) The health workers have gotten a feeling of the usefulness of family health records (statistical integration) in identifying targets in their own work and their usefulness in caring for family health. 2. On the other hand, because of a lack of required organizational factors, complete linkage was not obtained as the project intended. i) In regards to the government health worker's activities in terms of home visiting there was not much difference between the study & control areas though the MW did more home visiting than Government health workers. ii) In assessing the service performance of MW & health workers, the midwives balanced their workload between 40% FP, 40% MCH & 20% other activities (mainly immunization). However, $85{\sim}90%$ of the services provided by the health workers were other than FP/MCH, mainly for immunizations such as the encephalitis campaign. In the control area, a similar pattern was observed. Over 75% of their service was other than FP/MCH. Therefore, the pattern shows the health workers are a long way from becoming multipurpose workers even though the government is pushing in this direction. 3. Villagers were much more likely to visit the health sub-center clinic in the study area than in the control area (58% vs.31%) and for more combined care (45% vs.23%). C) Organization factors (admistrative integrative issues) 1. When MW (new workers with higher qualification) were introduced to HSC, it was noted that there were conflicts between the existing HSC workers (Nurse aids with less qualification than MW) and the MW for the beginning period of the project. The cause of the conflict was studied by an anthropologist and it was pointed out that these functional integration problems stemmed from the structural inadequacies of the health subcenter organization as indicated below; i) There is still no general consensus about the objectives and goals of the project between the project staff and the existing health workers. ii) There is no formal linkage between the responsibility of each member's job in the health sub-center. iii) There is still little chance for midwives to play a catalytic role or to establish communicative networks between workers in order to link various knowledge and skills to provide better FP/MCH services in the health sub-center. 2. Based on the above findings the project recommended to the County Chief (who has power to control the administrative staff and the technical staff in his county) the following ; i) In order to solve the conflicts between the individual roles and functions in performing health care activities, there must be goals agreed upon by both. ii) The health sub·center must function as an autonomous organization to undertake the integration health project. In order to do that, it is necessary to support administrative considerations, and to establish a communication system for supervision and to control of the health sub-centers. iii) The administrative organization, tentatively, must be organized to bind the health worker's midwive's and director's jobs by an organic relationship in order to achieve the integrative system under the leadership of health sub-center director. After submitting this observation report, there has been better understanding from frequent meetings & communication between HW/MW in FP/MCH work as the program developed. Lessons learned from the Seosan Project (on issues of FP/MCH integration in Korea); 1) A majority or about 80% of the couples are now practicing FP. As indicated by the study, there is a growing demand from clients for the health system to provide more MCH services than FP in order to maintain the achieved small size of family through FP practice. It is fortunate to see that the government is now formulating a MCH policy for the year 2,000 and revising MCH laws and regulations to emphasize more MCH care for achieving a small size family through family planning practice. 2) Goal consensus in FP/MCH shouBd be made among the health workers It administrators, especially to emphasize the need of care of 'wanted' child. But there is a long way to go to realize the 'real' integration of FP into MCH in Korea, unless there is a structural integration FP/MCH because a categorical FP is still first priority to reduce the rate of population growth for economic reasons but not yet for health/welfare reasons in practice. 3) There should be more financial allocation: (i) a midwife should be made available to help to promote the MCH program and coordinate services, (in) there should be a health sub·center director who can provide leadership training for managing the integrated program. There is a need for 'organizational support', if the decision of integration is made to obtain benefit from both FP & MCH. In other words, costs should be paid equally to both FP/MCH. The integration slogan itself, without the commitment of paying such costs, is powerless to advocate it. 4) Need of management training for middle level health personnel is more acute as the Government has already constructed 90 MCH centers attached to the County Health Center but without adequate manpower, facilities, and guidelines for integrating the work of both FP and MCH. 5) The local government still considers these MCH centers only as delivery centers to take care only of those visiting maternity cases. The MCH center should be a center for the managment of all pregnancies occurring in the community and the promotion of FP with a systematic and effective linkage of resources available in the county such as i.e. Village Health Worker, Community Health Practitioner, Health Sub-center Physicians & Health workers, Doctors and Midwives in MCH center, OBGY Specialists in clinics & hospitals as practiced by the Seosan project at primary health care level.

온라인 서비스 품질이 고객만족 및 충성의도에 미치는 영향 -항공권 예약.발권 웹사이트를 중심으로- (The Effects of Online Service Quality on Consumer Satisfaction and Loyalty Intention -About Booking and Issuing Air Tickets on Website-)

  • 본 연구에서는 항공권 예약 발권 웹사이트의 서비스 품질을 측정 뿐만 아니라 서비스 회복도 측정하고자 하였다. 또한 서비스 품질과 서비스 회복이 고객만족 및 충성의도에 미치는 영향관계를 실증하고자 하였다. 온라인 서비스 품질과 온라인 서비스 회복의 측정을 위해 Parasuraman, Zeithaml, & Malhotra(2005)가 개발한 E-S-QUAL과 E-RecS-QUAL을 사용했으며, 했다. E-S-QUAL은 온라인 서비스 품질을 측정하는 도구로써, 효율성, 시스템 이용가능성, 이행성, 프라이버시의 4개 차원 22개 항목으로 구성된다. E-RecS-QUAL은 온라인 서비스 회복을 측정하는 도구로써, 반응, 보상, 접촉의 3개 차원 11개 항목으로 구성된다. 실증분석을 위한 설문조사는 항공사나 여행사의 웹사이트를 통해 국내 외 항공권을 구입해 본 경험이 있는 소비자를 대상으로 실시하였는데, 총 400부가 회수되었고, 이 중 342부를 최종분석에 사용하였다. 실증분석을 위해 AMOS 7.0과 SPSS 15.0을 사용하였다. 먼저, SPSS 15.0을 사용하여, 요인점수를 이용한 회귀분석으로 가설검증을 한 결과, <가설 I-1, 2, 3, 4, II-1, 2, 3, III-1, IV-1>이 전부 채택되었다. 온라인 서비스 품질과 온라인 서비스 회복의 각 차원은 모두 전반적인 서비스 품질에 유의한 영향을 보였고, 전반적인 서비스 품질은 고객만족에 유의한 영향을 미쳤다. 마지막으로 고객만족 역시 충성의도에 유의한 영향을 미치는 것으로 확인되었다. 한편 AMOS 7.0을 사용하여 모형 분석을 하였는데, 모형의 적합도는 가설검증을 하기에 합당한 수치가 나왔다. 이를 토대로 가설검증을 한 결과, <가설 I-1, 3, II-1, 3, III-1, IV-1>은 채택되었고, <가설 I-2, 4, II-2>는 기각되었다. 이 결과는 Parasuraman et al.(2005)이 주장한 것처럼 E-S-QUAL을 나타내는 데는 요인점수를 이용한 회귀분석이 더 적합하다는 것을 보여주는 것이라고 판단된다. 이를 토대로 본 연구의 시사점을 정리하였다.
