  • PURPOSE: This study aims to compare the effects of upper backbone joint mobilization and self-stretching exercise in the patients with chronic neck pain. METHODS: Thirty seven patients with chronic neck pain were divides into self stretching group(SSG, n=18) and mobilization group(MG, n=19). To assess the degree of neck pain, the visual analog scale (VAS) was utilized, and to measure the joint range of motion at the flexion-extension, it was compared and analyzed by using the cervical range of motion (CROM) device. RESULTS: The joint range of motion and visual analog scale of SSG and MG showed significant effects on both groups. In the comparison of groups, there was no significant difference, but it indicated effects on improving the pain and the range of joint motion in MG. CONCLUSION: According to the analysis, not only self-stretching exercise intervening for direct treatment but also upper backbone joint mobilization intervening for indirect treatment are effective to improve the pain and the range of motion.

  • Purpose : to investigate the effectiveness of joint mobilization and myofascial release on the neck pain and to provide the effective treatment. Methods : Twenty-two subjects with neck pain participated in the experiment. All subjects were randomly assigned to a joint mobilization group(n=11) or a myofascial release group(n=11). Both groups received treatment for 15 minutes four times during 2 weeks. Cervical range of motion(CROM) instrument was used to measure range of neck motion, and Algometer was used to measure tenderness. All measurements of the subjects were measured at pre-treatment and post-treatment. Results : 1. The range of neck right side-bending motion of the myofascial release group was significantly increased(P<0.05), and the range of neck extension, right side-bending, left side-bending, right rotation motion of the joint mobilization group was significantly increased(P<0.05). 2. There was no significant improvement of tenderness in both groups(p>0.05). Conclusion : These data suggest that joint mobilization is more effective against increasing the range of motion than myofascial release, but myofascial release is more beneficial to tenderness than joint mobilization although it does not have a significant difference in the tenderness because there was a little improvement.

  • Objective: We aimed to identify the effects of proprioceptive neuromuscular facilitation (PNF) stretching on pain, hip range of motion, and functional disability in patients with chronic low back pain. Design: Randomized controlled trial Methods: In total, 45 patients with chronic low back pain were randomly divided into a conventional stretching group (n=22) and a PNF stretching group (n=23). Both interventions were performed three times per week for 6 weeks. Assessments were made using the visual analog scale, Flexion-Abduction-External Rotation test, modified Thomas test, prone hip extension test, and Oswestry disability index before and after the 6-week intervention period. We conducted a paired t-test to compare the within-group findings before and after the intervention. An independent t-test was used to compare the between-group differences. The statistical significance level was set at α=0.05, for all variables. Results: Both groups showed significant improvements in pain, hip range of motion, and functional disability after the intervention (p<0.05). A significant difference was observed in pain, hip range of motion, and functional disability in patients belonging to the PNF stretching group (p<0.05). Conclusions: This study provides evidence that the application of PNF stretching can effectively reduce pain and improve hip range of motion and functional disability in patients with chronic low back pain.

  • Purpose: This study assessed the differences in the forward head posture (FHP), cervical range of motion, and headache clinical parameters in episodic tension-type headache (ETTH) subjects, chronic tension-type headache (CTTH) subjects, and healthy controls (university students). Methods: Fifteen CTTH subjects, 15 ETTH subjects and 15 controls without headache were examined. Side-view images of each group were taken in both the sitting and standing positions, in order to assess the FHP by measuring the craniovertebral angle. The CROM was used to measure the cervical range of motion. A headache diary was kept for 4 weeks to assess the headache intensity, frequency, and duration. Results: The craniovertebral angle was smaller, ie, there was a greater FHP, in the CTTH and ETTH subjects than in the healthy controls in both the sitting and standing positions (p<0.05). The CTTH and ETTH subjects showed a lower cervical range of motion than the healthy controls in the total range of motion as well as in the half-cycles (p<0.05). Conclusion: The increased FHP and decreased cervical range of motion might be a contributing factor in the initiation of tension-type headache.

  • Purpose: This study examines the correlations between neck pain, neck range of motion, static and dynamic balance abilities, and walking abilities in patients with chronic stroke. Methods: A total of 29 participants were selected from patients with chronic stroke experiencing moderate to severe neck pain. Their neck pain, neck range of motion, static and dynamic balance abilities, and walking abilities were assessed and measured. Pearson's correlation coefficient was used to analyze the measurements and determine the correlations between neck pain, neck range of motion, static and dynamic balance abilities, and walking abilities. Results: Neck pain significantly correlated with static and dynamic balance and walking abilities. Regarding neck ROM, neither neck flexion nor extension showed significant correlations with static balance and walking abilities. However, except for left-lateral flexion, neck lateral flexion showed significant correlations with static and dynamic balance and walking abilities. Left-lateral flexion did not show a significant correlation with dynamic balance ability. Except for left rotation, neck rotation showed significant correlations with static and dynamic balance and walking abilities. Left rotation did not show a significant correlation with dynamic balance ability. Conclusion: The results of this study demonstrate that there are correlations between neck pain, neck range of motion, balance abilities, and walking ability.

  • High Efficiency Video Coding (HEVC) suffers from high computational complexity due to its quad-tree structure in motion estimation (ME). This paper exposes an adaptive search range decision algorithm for accelerating HEVC integer-pel ME on GPU which estimates the optimal search range (SR) using a MAP (Maximum A Posteriori) estimator. There are three main contributions; First, we define the motion feature as the standard deviation of motion vector difference values in a CTU. Second, a MAP estimator is proposed, which theoretically estimates the motion feature of the current CTU using the motion feature of a temporally adjacent CTU and its SR without any data dependency. Thus, the SR for the current CTU is parallelly determined. Finally, the values of the prior distribution and the likelihood for each discretized motion feature are computed in advance and stored at a look-up table to further save the computational complexity. Experimental results show in conventional HEVC test sequences that the proposed algorithm can achieves high average time reductions without any subjective quality loss as well as with little BD-bitrate increase.

  • Purpose: Postoperative exercise for acute Achilles tendon rupture is important for a patient's return to daily life and sports. On the other hand, the protocol requires considerable effort to educate patients and continuous checking. This study evaluated the outcome of a new simple and delayed rehabilitation protocol after Achilles tendon rupture repair. Materials and Methods: From July 2014 to November 2020, one hundred eighty-three patients were operated on by one surgeon. The exercise protocol was classified into two methods. One group (immediate protocol, control group) started immediate full weight bearing with a 20° plantar flexion range of motion from two days postoperatively. Ankle dorsiflexion was restricted to 0°. The other group (delayed protocol, case group) started full weight bearing with a controlled ankle motion boot from two weeks postoperatively. No range of motion exercise was allowed until six weeks postoperatively. Age, sex, body mass index, ankle range of motion, muscle power, time to return to previous physical activity, functional score, and complication rate were evaluated. The results of the two groups were compared using a Mann-Whitney test. Statistical significance was set as p<0.05. Results: The range of motion, double heel rising, and one-leg standing were achieved faster in the control group (p<0.05). However, single-heel rising, repeated single-heel rising, return to previous activity (work, run, and sport), and functional scores showed no statistical difference (p>0.05). Conclusion: Simple and delayed postoperative rehabilitation of acute Achilles tendon rupture without active range of motion exercises showed satisfactory functional results and a low complication rate.
