Evaluation of Critical Notch radius using Notch/Crack Critical Average Stress Fracture Model (노치/균열 임계평균응력 파손모델을 이용한 임계노치반경 평가)

  • In this study, intrinsic static/dynamic fracture toughness of Al 7175-T74 are evaluated from the apparent static/dynamic toughness of notched specimen. The notch/crack critical average stress fracture model is suggested to establish the relationship to predict the intrinsic fracture toughness from the apparent fracture toughness of a notched specimen. The notch/crack critical average stress fracture model is established using the relation between the notch root radius and the effective distance calculated by finite element analysis. It is conclude that the true fracture toughness can be estimated from test results of apparent fracture toughness measured by using a notched specimen. Also, critical notch root radius can be predicted by notch/crack critical average stress fracture model.

Evaluation of Fracture Healing in Canine Radius by Bone Scan with $^ {99m}Tc-MDP$ ($^ {99m}Tc-MDP$를 이용한 개의 요골 골절 치유 과정의 평가)

  • Bone scintigraphy with $^{99m}Tc-methylene$ diphosphonate(MDP) performed for 20 weeks per two weeks interval respectively after fracture in seven(male 3 heads, female 4 heads) canine radius were analysed. All of bone scans performed 2 weeks to 20 weeks after fracture showed increase in generalized tracer uptake and showed localized increase in tracer uptake at the fracture site. Bone scans and ratio performed 6 weeks after fracture showed the most outstanding increase in generalized and localized tracer uptake. New bone formation had been observed from 2 weeks and they were incorporated completely on 18 weeks, they showed most activity during 6-10 weeks after fracture. It was recognized that the bone scan with $^{99m}Tc-methylene$ was quite sensitivity but low specificity on the fracture healing in canine radius.

  • Late rupture of the extensor pollicis longus (EPL) tendon is an occasional complication following a distal radius fracture. Early rupture of the EPL tendon as a complication of distal radius fracture is rare. We report one case of early rupture of the EPL tendon after distal radius fracture.

  • In this study, intrinsic dynamic fracture toughness of 17-4PH casting steel is evaluated from the apparent dynamic fracture toughness of notched specimen. Notch radius of notched specimen is manufactured from 0.1mm to 4mm. The results shows that dynamic fracture toughness decreases with decreasing of notch root radius above critical notch roof radius. The true dynamic fracture toughness can be predicted from test results of apparent dynamic fracture toughness measured by using notched specimen.

Microscopic fracture criterion of crack growth initiation (연성 균열성장 개시의 미시적 파괴조건)

  • For the prediction of the crack growth initiation from a blunt notch or a precrack in a prestrained material under plane strain tension and small-scale yielding conditions, a microscopic fracture criterion is proposed in terms of the crack tip opening displacement(COD) needed for the attainment of fracture strain at a microstructural distance. Smooth blunting of a crack tip with an initial root radius is assumed, and strain distributions on the crack-line axis are calculated at each deformation stage until the distributions against an original distance normalized to the COD are insensitive to an initial root radius. This case of no initial-root-radius effect is taken as for a sharp crack tip, on which the criterion is applied to determine the characteristic length of material from a critical COD for a fatigue-precracked specimen. The predicted COD at the fracture initiation from a crack with an initial root radius or a prestraining shows reasonable agreement with experimental values.

The Clinical Effect of Rehabilitation Protocol for Distal Radius Fracture in Korean Medicine: A Report of 3 Cases (원위 요골 골절에 대한 한방 재활치료 프로토콜 적용의 임상적 효과: 증례 보고)

  • The objective of this study was to propose a rehabilitation protocol for distal radius fracture in Korean Medicine and to report its effectiveness. Three patients who had undergone distal radius fracture were treated using the rehabilitation protocol in Korean Medicine. We estimated the outcome evaluating physical examination findings of the wrist joint, numeric rating scale and quick the Disabilities of the Arm, Shoulder and Hand (DASH) score. After the treatment, we observed that wrist movement, numeric rating scale and quick DASH score were improved. A rehabilitation in Korean Medicine can be effectively used for distal radius fracture patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean Medicine.

Treatment of Radius Fracture Using a Shuttle Pin in a Cinereous Vulture (Aegypius monachus): A Case Study

  • Cinereous vulture (Aegypius monachus) is a large raptor that belongs to the family Accipitridae in the order Falconiformes. It is a migratory bird that returns to Mongolia after wintering in the Korean Peninsula. In February 2015, a cinereous vulture was rescued from Asan-si, Chungcheongnam-do Province as it could not fly. It was transferred to the Chungnam Wild Animal Rescue Center in Korea. It was found to have a closed fracture on the right radius through radiographic examination, a basic procedure. Surgery was performed at the fracture site using a shuttle pin and wire. After successful physical therapy and rehabilitation, the cinereous vulture was released in October 2015. A metal ring (150-00713) and wing tags (G/BL 3) were attached to the bird before its release for subsequent identification. Two years and 4 months later, in March 2018, the cinereous vulture was rescued and brought back to the center. It was diagnosed with secondary pesticide poisoning. After receiving treatment, a radiography test revealed no specific findings related to the previous fracture surgery. Considering that migratory birds travel long distances, it was confirmed that the treatment for reducing the fracture in the radius using a shuttle pin and a cerclage wire was suitable.

Risk Factors in Stability after Immobilization of the Distal Radius in Unstable Fractures in Children (소아 요골 원위부 불안정 골절의 캐스트 후 안정성에 영향을 미치는 요소)

  • Purpose: Distal radius fractures in youth are treated conservatively in most cases, but there are some cases of redisplacement in the follow-up period after cast immobilization, even after complete reduction. This study examined the risk factors of redisplacement in reduced unstable distal radius fractures. Materials and Methods: From February 2011 to June 2018, 44 unstable distal radius fractures were managed with a closed reduction and cast immobilization. The patients were aged between 6 and 14 years. The cases of redisplacement were analyzed with the fracture characteristics (fracture obliquity, fracture level ratio, ulnar fracture combined), cast qualities (gap index, cast index, 3 point index, and radius-2nd metacarpal angle) and host factors (age, sex). Results: The mean angulation in the union was 9.2° (0°-32.8°). In the categorical grouping 29 cases were within 10° angulation, and 15 cases were more than 10°. No significant differences in the factors of the cast indices or host factors were noted. The meaningful factor was the fracture level calculated by the relative width of the fracture site divided by the sum of width of diaphysis and epiphysis (p=0.001) and combined ulnar fracture (p=0.019). Conclusion: Unstable distal radius fractures should be treated with more stubborn guidelines lest the fracture loses its anatomical alignment. In particular, in patients with less remodeling power, operative treatment would secure a better result if the fracture occurs in a more proximal location.

Delayed Rupture of Flexor Pollicis Longus by Plate for Fracture of the Distal Radius (요골골절에 사용된 금속판에 의한 장무지 굴곡건의 지연성 파열)

  • Purpose: Delayed rupture of flexor pollicis longus as a sequelae of the plate inserted for distal radius fracture is a very rare. This is the first case reported and published in Korea. Methods: A 48 years old female patient visited hospital, complaining flexor disturbance of interphalangeal joint of left thumb, which suddenly occurred without any external wound. We found that she had received operation of fixing plate for fracture of left distal radius 10 years ago. As operational opinion, we have checked that flexor pollicis longus tendon has been ruptured with oblique ways being stimulated by extended plate to palmar side over long period. Results: Authors performed tenorrhaphy of flexor pollicis longus without tendon graft and presented a successful active flexion of the left thumb interphalangeal joint 1 year after the operation. Conclusion: If the extruded part of the end plate is observed during the operation or follow-up, it is considered to be necessary to get rid of the plate as early as possible after the fracture healing.
