방사선치료실 내의 광중성자 에너지 분포 평가 (Evaluation of Photoneutron Energy Distribution in the Radiotherapy Room)

  • 의료용의 선형가속장치는 방사선치료에 있어서 그 활용도가 가장 높으며, 최근에는 10MV 이상의 고에너지 광자선을 이용한 치료가 보편화되고 있다. 그러나 광핵반응에 의한 광중성자가 생성됨으로써 방사선 방호측면에서 많은 문제를 야기 시키고 있다. 이에 본 연구는 MCNPX 프로그램을 이용하여 치료실 내의 위치별 중성자의 특성을 분석하였다. 그 결과, 광중성자의 생성 개수도 선원 중심점을 기준으로 거리가 멀어질수록 감소됨을 알 수 있었다. 그리고 10MV에서 20MV로 에너지가 높아짐에 따른 열중성자와 속중성자의 비율은 큰 차이가 없었지만, 선원 중심점으로부터 거리가 이격될수록 열중성자의 비율이 높아지는 현상을 확인할 수 있었다.

  • Purpose: This study was performed to analyze the results of radiologic parameters between weight bearing affected single ankle anteroposterior (AP) view and both ankle AP view in ankle osteoarthritis (OA). Materials and Methods: Between January 2009 and August 2010, 41 patients (50 ankles) who visited our institution to treat ankle OA were reviewed retrospectively. In radiographic assessment, weight bearing affected single ankle AP view and both ankle AP view were checked, and measured tibial anterior surface angle (TAS), tibial medial malleolar angle (TMM), talar tilting angle (TT), maximum and minimun joint space width (JSW) of ankle, width between articular surface of medial malleolar and medial articular surface of talus as radiologic parameters. Results: On weight bearing both ankle AP view, TAS was $85.4{\pm}4.1^{\circ}$, TMM was $33.1{\pm}9^{\circ}$, TT was $5.4{\pm}6.1^{\circ}$, maximum JSW was $3.2{\pm}2.7$ mm, minimum JSW was $1.1{\pm}1.3$ mm, width between articular surface of medial malleolar and medial articular surface of talus was $1.8{\pm}1.8$ mm and on weight bearing affected ankle AP view, TAS was $85.3{\pm}3.9^{\circ}$, TMM was $34.3{\pm}10.9^{\circ}$, TT was $5.4{\pm}6.5^{\circ}$, maximum JSW was $3.2{\pm}2.7$ mm, minimum JSW was $1.1{\pm}1.3$ mm, width between articular surface of medial malleolar and medial articular surface of talus was $1.6{\pm}1.7$ mm. Conclusion: There is no statistical significance in radiologic parameters between weight-bearing affected single ankle AP view and both ankle AP view in ankle OA.

성인의 유연성 편평족에 대한 종골 연장술 (Calcaneal Lengthening Osteotomy for the Symptomatic Flexible Flatfoot in Adults)

  • Purpose: Calcaneal lengthening osteotomy is one option for the treatment of symptomatic flexible flatfoot in adults. The aim of the study was to evaluate the short term clinical and radiologic results and analyze its complications. Materials and Methods: Twelve feet who had undergone calcaneal lengthening osteotomy without flexor digitorum longus transfer between December 2009 and July 2011 were included. The mean age was 40.6 years (23~75 years). The mean followup was 17.3 months (13~25 months). Clinical outcome were assessed using American Orthopadics Foot ans Ankle Society (AOFAS) score and visual analogue scale (VAS) for pain. Four radiologic parameters were measured from weightbearing radiographs to evaluate the difference between preoperatively and postoperatively measures. To analyze the complications, calcaneocuboid joint subluxation and degenerative change were measured postoperatively. Results: The mean AOFAS score improved from 55.3 points preoperatively to 82.2 points at lastest follow-up (p=0.000). The mean VAS improved from 6.3 points preoperatively to 3.2 points postoperatively (p=0.002).All radiologic parameters, the mean talonavicular coverage angle on AP view, the mean talo-1st metatarsal angle on AP and lateral view and the mean calcaneal pitch angle, significantly improved after calcaneal lengthening osteotomy. Nine feets (75%) were shown the degenerative change in the calcaneocuboid joint at latest follow-up radiographs. Conclusion: Calcaneal lengthening osteotomy for the symptomatic flexible flatfoot in adults produced significant improvement in clinical and radiologic parameters, but calcaneocuboid joint osteoarthritis occurred postoperatively remained a major problem.

연골하 낭종을 동반한 거골의 골연골 병변의 관절경적 치료 후 임상적 결과 및 방사선학적 변화 (Clinical and Radiologic Results of Arthroscopic Treatment for Osteochondral Lesion with Subchondral Cyst on Talus)

  • Purpose: The purpose of this study is to review clinical and radiologic changes after arthroscopic operation without bone graft for osteochondral lesion with subchondral cyst on the talus. And we compared the results with those without cyst. Radiologic changes were also observed. Materials and Methods: Arthroscopic microfrature or abrasion arthroplasty was performed on 10 caeses of osteochondral lesions with subchondral cyst and 20 cases without cysts. Preoperative and postoperative symptoms were evaluated by Ankle-Hind foot scale of AOFAS and the score of two groups were compared(t-Test : Paired Two Test for Means). Radiologic evaluation was performed after operation on patients with osteochondral lesion with cyst. Results: On patients of osteochondral lesion with subchondral cyst on talus, there was clinical improvement compared to the preoperative status and compared to patients without cysts(P=0.01) after arthroscopic operation. We could get increase of density and decrease of size of cystic lesion on plain film with time passage. Conclusion: We report 10 cases with osteochondral lesion with subchondral cyst on talus which resulted in clinical and radiological improvement after arthroscopic microfracture or abrasion arthroplasty without bone graft.

법랑모세포종과 치성각화낭의 방사선학적 진단의 정확도 및 판독자간과 판독자내 일치 (ACCURACY AND INTEROBSERVER- INTRAOBSERVER AGREEMENT IN THE RADIOLOGIC DIAGNOSIS OF AMELOBLASTOMA AND ODONTOGENIC KERATOCYST)

  • Six observers with different amount of experience in the field of Oral and Maxillofacial Radiology interpreted the radiographs of 13 cases of ameloblastoma and 8 cases of odontogenic keratocyct which were confirmed histopathologically and showed pseudo-multilocular appearance and scalloped border radiographically. The authors examined the accuracy of radiologic diagnoses, interobserver agrement and intraobserver agreement. The obtained results were as follows; 1. The accuracy of radiologic diagnosis ranged from 0.48 to 0.81. The average value was 0.61. 2. The accuracy of radiologic diagnosis for ameloblastoma(0.55) was lower than that for odontogenic keratocyst(0.70) (P<0.05). 3. The overall agreement among the 6 observers was 14.3% at the first interpretation and 19.0% at the second interpretation. 4. The intraobserver agreement of each observer expressed in kappa index ranged from -0.14 to 0.64. The overall intraobserver agreement was 0.29. 5. The intraobserver agreement of each observer for ameloblastoma and odontogenic keratocyst ranged from -0.07 to 0.65 and from -0.25 to 1.00, respectively. The overall intraobserver agreement for ameloblatoma and odontogenic keratocyst were 0.27 and 0.26, respectively. 6. The diagnostic accuracy highly correlated to the intraobserver agreement(r=0.6370).

The Radiologic Comparison of Operative Treatment Using a Hook Plate versus a Distal Clavicle Locking Plate of Distal Clavicle Fracture

  • Background: The purpose of this study was to compare the radiologic results of patients who underwent surgery with a hook plate and a locking plate in distal clavicle fractures. Methods: Sixty patients underwent surgical treatment for Neer type IIa, IIb, III, and V distal clavicle fracture. Twenty-eight patients underwent fracture fixation with a hook plate and 32 with a locking plate. Coracoclavicular distance was measured on standard anteroposterior radiographs before and after the surgery, and union was confirmed by radiograph or computed tomography taken at 6 months postoperatively. Other radiologic complications like osteolysis was also checked. Results: Bony union was confirmed in 59 patients out of 60 patients, and 1 patient in the hook plate group showed delayed union. Coracoclavicular distance was decreased more in the hook plate group after surgery (p<0.01). After 6 weeks of the hook plate removal, the coracoclavicular distance was increased a little compared to before metal removal, but there was no difference compared to the contralateral shoulder. Eleven out of 28 patients (39.3%) showed osteolysis on the acromial undersurface in the hook plate group. Conclusions: Both the hook plate group and the locking plate group showed satisfactory radiologic results in distal clavicle fractures. Both hook plate and locking plate could be a good treatment option if it is used in proper indication in distal clavicle fracture with acromioclavicular subluxation or dislocation.

납과 텅스텐 기반 차폐체의 성능 비교 평가 (Evaluation of the Shield Performance of Lead and Tungsten Based Radiation Shields)

  • This study was intended to evaluate the shielding rate of radiation shields manufactured using 3D printers that have recently been used in various fields by comparing them with existing shields made of lead, and to find out their applicability through experiments. A 3D printer shield made of tungsten filament 1 mm, 2 mm, 4 mm shield, RNS-TX (nanotungsten) 1.1 mm, lead 0.2 mmPb, and 1mmPb were exposed to 99mTc, 18F, and 201TI for 15, 30, 45 minutes, and 60 minutes after measuring cumulative dose three times. Based on this, the shielding rate of each shield was calculated based on the dose in the absence of the shield. In addition, 99mTc, 18F, and 201TI were located 100 cm away from the phantom in which the OSLD nano Dot device was inserted, and if there was no shield for 60 minutes, the dose of thyroid was measured using 1.0 mm of lead shield, 1.1 mm of RNS-TX shield, and 2 mm of tungsten shield made by 3D printer. The use of shields during radiation shielding emitted from open radiation sources all resulted in a reduction in dose. The radiation dose emitted from the radionuclides under the experiment was all reduced when the shield was used. This study has been confirmed that tungsten is a material that can replace lead due to its excellent performance and efficiency as shield, and that it even shows the possibility of manufacturing a customized shield using 3D printer.

Noncontrast Computed Tomography-Based Radiomics Analysis in Discriminating Early Hematoma Expansion after Spontaneous Intracerebral Hemorrhage

  • Objective: To determine whether noncontrast computed tomography (NCCT) models based on multivariable, radiomics features, and machine learning (ML) algorithms could further improve the discrimination of early hematoma expansion (HE) in patients with spontaneous intracerebral hemorrhage (sICH). Materials and Methods: We retrospectively reviewed 261 patients with sICH who underwent initial NCCT within 6 hours of ictus and follow-up CT within 24 hours after initial NCCT, between April 2011 and March 2019. The clinical characteristics, imaging signs and radiomics features extracted from the initial NCCT images were used to construct models to discriminate early HE. A clinical-radiologic model was constructed using a multivariate logistic regression (LR) analysis. Radiomics models, a radiomics-radiologic model, and a combined model were constructed in the training cohort (n = 182) and independently verified in the validation cohort (n = 79). Receiver operating characteristic analysis and the area under the curve (AUC) were used to evaluate the discriminative power. Results: The AUC of the clinical-radiologic model for discriminating early HE was 0.766. The AUCs of the radiomics model for discriminating early HE built using the LR algorithm in the training and validation cohorts were 0.926 and 0.850, respectively. The AUCs of the radiomics-radiologic model in the training and validation cohorts were 0.946 and 0.867, respectively. The AUCs of the combined model in the training and validation cohorts were 0.960 and 0.867, respectively. Conclusion: NCCT models based on multivariable, radiomics features and ML algorithm could improve the discrimination of early HE. The combined model was the best recommended model to identify sICH patients at risk of early HE.

방사선학과 교육과정 개선을 위한 현 교육과정 평가 (Evaluation of Present Curriculum for Devlopment of Dept. of Radiological Science Curriculum)

  • 시대와 사회적인 변화에 따라서 교육과정은 변화의 요구를 받게 된다. 교육과정의 변화를 요구받는 현시점에서 교과과정을 개선하기 위한 선행단계인 현재의 교과과정을 분석, 평가하고자 한다. 교육전문가 집단의 설문과 현재 방사선학과가 개설되어 있는 21개 대학 중 19개 대학 방사선학과의 교육과정 및 참고문헌을 중심으로 연구하였으며, 연구내용은 방사선사 업무범위와 교과과정, 방사선과학 교육과 교과과정, 디지털과 국제화시대의 교과과정, 방사선과학에 대한 학문적인 발전과 교과과정 등의 내용을 중심으로 연구하였다. 업무범위에서는 혈관조영학 및 중재적검사학은 6 ~ 8개교, 투시영상학은 4개교, 초음파영상학 및 실습은 6개교, 자기공명영상학은 2개교 등에서 미개설된 것으로 나타났다. 기초의학 교과목인 인체생리학, 인체해부학 및 실습, 의학용어 등은 대부분의 대학에서 개설되어 있었으나, 병리학 5개교, 영상해부학 6개교, 임상의학 11개교 등에서 미개설 되어 있는 것으로 나타났다. 기초이공학 교과목 중에서 일반생물학 및 실습은 11개교, 일반물리학 및 실습은 14개교, 일반화학 및 실습은 8개교 등 약 절반정도의 대학에서 개설된 것으로 나타났다. 전공기초에서 디지털화 교과목인 보건전산학, 컴퓨터프로그램밍, PACS 등이 개설된 대학은 4 ~ 5개교에 불과하였다. 방사선과학 교육은 확대된 업무범위를 포함하여, 방사선과학의 학문적 발전과 디지털화 교육 및 국제화 그리고 방사선과학 분야의 미래지향적인 교육의 토대를 마련하기 위해서는 현재 각 대학에서 개설, 운영되고 있는 교과과정은 일정부분 표준화의 필요성을 인식하였으며, 이에 방사선학과 교육과정은 인증제 도입의 필요성을 제시한다.

폐공동성 병변의 임상적 고찰 (A Clinical Study for the Cavitary Lesion of the Lung)

  • Pulmonary cavity is the result of necrosis of lung parenchyma with evacuation of the necrotic material via the tracheobronchial tree. A communication with the tracheobronchial tree permits air to enter the area of necrosis, so the radiologic result show the a lucent defect. The radiologic characteristics of the wall of a cavity are determined by the reaction of the lung parenchyma to the pathologic process. Therefore, the shadows of the chest films in cavitary lesion were variable in its nature. The author, in 42 cases which have a cavitary lesion in X-ray findings among 172 cases resected lung obtained in P.N.U.H. from 1979 to June, 1985, studied similarities and differences between the pathogenesis of these lesions and the radiologic findings. The author reviewed the 42 cavitary lesions and the following results were obtained. 1. The cavitary lesions were seen in 42 [24.4%] out of 172 cases of resected lung disease. 2. Histopathologically, pulmonary tuberculosis was 47.6% and primary lung cancer was 9.5%. 3. The most common site of the lesion was right upper lobe. 4. The most common size of the cavity was from 3 to 6 cm in diameter. 5. Lobectomy was the most common operated method.
