Diagnosis in Canine Tricuspid Valve Dysplasia (개에서 삼첨판 이형성의 증례)

  • Tricuspid valve dysplasia was diagnosed in 2 month-old german shepherd dog. Radiographic and echocardiographic examination was performed and they revealed severe dilatation of the right atrium, dilatation of the right ventricle, and septal leaflet tethered to the right side of the septum. On Doppler echocardiography, tricuspid valve incompetence was detected, flow velocity ranging from 3-4 m/sec. Color Doppler examination showed large turbulent jet in the right atrium.

  • Statement of problem. There were several studies on the effects of irradiation to peri-implant bone tissue. However, no clear biological effect of irradiation on peri-implant bone tissue was reported yet. Purpose. This study compared the effect of irradiation on the surrounding tissue of a HA-coated implant fixture with controls. Material and methods. 6 Steri-Oss implants were implanted into the femur of 6 mongrels. The implanted dogs were divided into three groups and received irradiation. After 1 month, 2months and 4 months healing period, the histologic examination and mobility test and digital radiographic imaging analyses were performed to compare the control and experimental group respectively. Results. The irradiated group showed slower healing than control group in light microscopic observations. The mobility test demonstrated significant less number (Periotest) in control group than that of irradiated groups. The digital radiographic imaging analysis showed that the bone density of irradiated group was higher than control group. Conclusion. Generally, control group showed favorable biological response and less mobility than irradiated group. The conflict result of bone density value were measured by the digital radiographic imaging analysis. The digital radiographic imaging analysis needs more research in future.

Evaluation of canalis basilaris medianus using cone-beam computed tomography

  • The aim of this report is to present two cases of canalis basilaris medianus as identified on cone-beam computed tomography (CBCT) in the base of the skull. The CBCT data sets were sent for radiographic consultation. In both cases, multi-planar views revealed an osseous defect in the base of the skull in the clivus region, the sagittal view showed a unilateral, well-defined, non-corticated, track-like low-attenuation osseous defect in the clivus. The appearance of the defect was highly reminiscent of a fracture of the clivus. The borders of osseous defect were smooth, and no other radiographic signs suggestive of osteolytic destructive processes were noted. Based on the overall radiographic examination, a radiographic impression of canalis basilaris medianus was made. Canalis basilaris medianus is a rare anatomical variant and is generally observed on the clivus. Due to its potential association with meningitis, it should be recognized and reported to avoid potential complications.

Myocardial degeneration in Russian rat snake (Elaphe schrenckii)

  • A female Russian rat snake, Elaphe schrenckii, was presented for loss of movement. Physical examination showed the swelling in the area of heart. Radiographic examination revealed cardiomegaly, pericardial effusion, and a soft opacity in the area of swelling. Although pericardiocentesis to remove fluid out from the heart as well as vigorous treatments were given to the Russian rat snake, it died during treatments. Postmortem examination confirmed pericardial effusion of pale yellow, translucent fluid with mild dilation of the right atrium and ventricle. Formalin -fixed paraffin embedded tissue sections were stained with routine H&E and the classical von Kossa's method for histopathological demonstration. Histopathological examination revealed multifocal calcification in myocardium and consists of the displacement of muscular fiber by limy deposits. Congestive heart failure was suspicious for the snake when it was alive. In wild reptiles, muscle degeneration has been reported with nutrition disorders but the present case is the first report of myocardial degeneration in a Russian rat snake and contributes to the rare reports of cardiac disease in snakes.

Screening for Early Detection of Lung Cancer: Results from Seoul National University Hospital (폐암의 조기진단 방법에 관한 연구)

  • This study was designed to observe the problems in performing the screening for early detection of lung cancer, and the degree to which regular radiographic and cytologic screening contributes to the early detection of lung cancer in asymptomatic volunteers. Through mass media campaign, 346 male volunteers had registered to receive radiographic and sputum cytologic screening every four months. Initial chest x-ray examination showed 83 cases of lesions suggesting tuberculosis. Among them, two cases were proved to be active tuberculosis. The rate of long-term follw-up over two years was about 15%. The screening tests detected two cases of lung cancer, one prevalent lung cancer by sputum cytologic examination, and the other by sputum cytologic examination during follow-up. So the prevalence rate of lung cancer was 0.28% and the incidence rate was 3.1/1,000 person·years. Both were localized lesions; ie, American Joint Committee on Cancer (AJCC) stage I and occult lung cancer, respectively. With these results, we suggest that the maintenance of long-term follow-up seems to be the most important problem to evaluate the effect of early detection of lung cancer. It would require thorough explanation of the risk of smoking in lung cancer and the wide public education on the government's base. It should be done at several hospitals simultaneously to include a large population in the study. Although we couldn't determine the effect of screening for the early detection of lung cancer in this report, new diagnostic procedure other than chest x-ray and sputum cytologic examination would be required, according to the literature, to reduce the mortality of lung cancer by the screening program for the early detection of lung cancer.

PERINEURAL SPREAD IN ADENOID CYSTIC CARCINOMA (신경주위 전파를 수반한 유선낭종암)

  • This is a report of adenoid cystic carcinoma occurred in the palate in 30-year-old patient with a complaint of exophytic mass. The authors diagnosed it as adenoid cystic carcinoma by the clinical examination, radiographic findings and histopathological findings. The obtained results are as follows: 1. In clinical examination, asymptomatic exophytic mass of palate was observed. 2. In radiographic findings, soft tissue mass infiltrated the left maxillary sinus, nasal cavity, infraorbital fossa, hard palate, pterygopalatine fossa and pterygoid plate, and enhanced soft tissue mass was also observed in CT. 3. In histopathological findings, tubular and solid patterns of glandular structures were observed and the infiltration of tumor cells into the nerve fibers was also observed. 4. Two years after radical surgery, radiation therapy and chemotherapy, the perineural spread to orbital area was observed. 5. Much longer follow-up than 5 years is needed for early diagnosis of recurrence and distant metastasis.

Digital contrast subtraction radiography for proximal caries diagnosis (인접면 치아우식 진단을 위한 디지털 방사선 조영 공제술)

  • Purpose : To determine whether subtraction images utilizing contrast media can improve the diagnostic performance of proximal caries diagnosis compared to conventional periapical radiographic images. Materials and Methods: Thirty-six teeth with 57 proximal surfaces were radiographied using a size #2 RVG-ui sensor (Trophy Radiology, Marne-la-Vallee, France). The teeth immersed in water-soluble contrast media and subtraction images were taken. Each tooth was then sectioned for histologic examination. The digital radiographic images and subtraction images were examined and interpreted by three dentists for proximal caries. The results of the proximal caries diagnosis were then verified with the results of the histologic examination. Results: The proximal caries sensitivity using digital subtraction radiography was significantly higher than simply examining a single digital radiograph. The sensitivity of the proximal dentinal carious lesion when analyzed with the subtraction radiograph and the radiograph together was higher than with the subtraction radiograph or the radiograph alone. Conclusion: The use of subtraction radiography with contrast media may be useful for detecting proximal dentinal carious lesions.

Modified Robert-Jones Bandage Application to a Pointer Puppy with Carpal Laxity Syndrome

  • A 5-month-old, intact female pointer weighing 13 kg was referred for diagnosis and treatment of bilateral thoracic limb hyperflexion without trauma. After physical and radiographic examination, carpal laxity syndrome was diagnosed. A modified Robert-Jones bandage was applied and cage rest, including general puppy food, was prescribed for 3 weeks. The patient acquired normal gait with weight bearing, with non-remarkable findings on radiographic examination. The modified Robert-Jones bandage was sufficient to treat carpal laxity syndrome in this case.

A case of Sinusitis Patient Improved by Kami-Hyunggyeyungyotang (가미형개연교탕으로 치료한 부비동염 치험 1례)

  • Objectives : The purpose of this study is to evaluate the effect of oriental medical treatment on a sinusitis patient Methods : We treated a 56-years old man patient with sinusitis. His symptoms are Ozena, Postnasal drip, Cough, sputum, nasal obstruction, rhinorrhea and Headache. The patient was treated with herb-med(Kami-Hyunggyeyungyotang,加味荊芥連翹湯), acupuncture, aroma oil therapy. During about a month, the patient was treated only with oriental medical treatment and without any of western medicine including antibiotics. We confirmed the progress of treatment with subjective symptoms, physical examination and radiographic inspection(PNS image). Results & Conclusions : After one month, Combined application of the oriental treatment including Kami-Hyunggyeyungyotang showed remarkable effects on subjective symptoms, physical examination and radiographic inspection(PNS image). This study show that Kami-Hyunggyeyungyotang effects on sinusitis.

Nephrotomy for Removal of Renal Calculi in Two Dog (개에서 신 절개를 통한 신 결석 제거 2례)

  • 2 Cases of nephrotomy for removal of calculi in dog were referred to veterinary teaching hospital of Konkuk University. In case 1, a 5 year-old, castrated male Yorkshire Terrier dog was referred because of intermittent hematuria, pain in urination for one month. Hematologic and chemical examination showed mild increased BUN and CPK. Radiographic findings revealed radiopaque materials in the urinary bladder, urethra, and left kidney. Retrograde hydropropulsion was performed to move the calculi into the bladder, and cystotomy was done to remove calculi. Nephrotomy was performed to removal of the calculi from the left renal pelvis and calyx. After operation renal function were recovered and preserved. In case 2, a 5 year-old, neutral female Schnauzer dog was referred because of persistant vomiting, anorexia, and celialgia for 20 days. Hematologic and chemical examination showed stress leucogram, moderate azotemia, hypercalcemia, hyperphosphatemia, and increased ALP. Radiographic findings revealed enlargement of the left kidney and radiopaque materials in the both of the kidneys. On excretory urography, left kidney was no pyelogram. On ultrasonography, renal tissue was very thin and distended renal pelvis appeared. Nephrectomy of nonfunctional left kidney and nephrotomy for removal of calculi from the right renal pelvis and calyx were done. One week after operation, renal and hepatic functions were recovered. So, in cases of renal calculi, it is necessary that renal calculi are extracted actively as far as the patient's body condition endurable.
