Purpose: This study investigated correlations between findings on panoramic radiographs and cone-beam computed tomography (CBCT) to assess the relationship between the maxillary sinus floor and the roots of maxillary posterior teeth. In addition, radiographic signs indicating actual root protrusion into the maxillary sinus were evaluated on panoramic radiographs. Materials and Methods: Paired panoramic radiographs and CBCT images from 305 subjects were analyzed. This analysis classified 2,440 maxillary premolars and molars according to their relationship with the maxillary sinus floor on panoramic radiographs and CBCT images. In addition, interruption of the sinus floor was examined on panoramic radiographs. Results: Root protrusion into the maxillary sinus occurred most frequently in the mesiobuccal roots of the second molars. The classification according to panoramic radiographs and CBCT images was the same in more than 90% of cases when there was no contact between the root apex and the sinus floor. When the panoramic radiograph showed root protrusion into the sinus, the CBCT images showed the same classification in 67.5% of second molars, 48.8% of first molars, and 53.3% of second premolars. There was a statistically significant relationship between interruption of the sinus floor on panoramic radiographs and root protrusion into the sinus on CBCT images. Conclusion: The presence of root protrusion into the sinus on panoramic radiographs demonstrated a moderate ability to predict root protrusion into the maxillary sinus. Interruption of the maxillary sinus floor could be considered an indicator of actual root protrusion into the maxillary sinus.

The aim of this study was to investigate the thickness of alveolar lamina dura and alveolar bone height by the change of periodontal status. The periapical radiograms of the 853 cases were taken by paralleling technique. The conclusions from this study were as follows: 1. In the normal periodontal status, the alveolar lamina dura was the thickest at alveolar crest, followed by apex, midroot, and the average thickness of the posterior teeth was thicker than that of the anterior teeth. 2. In Russel's Periodontal Index 0 and 1, no significant differences of thickness were found to be 3 areas. In Russel's Periodontal Index 2 and 6, the thickness of alveolar lamina dura at alveolar crest was decreased, only. 3. In Russel's Periodontal Index 0 and 1, distance from the cemento-enamel junction to the alveolar crest was within 1.5㎜. In Russel's Periodontal Index 2 and 6, the distance was 3. increased over 1.5㎜.
Purpose: This study investigated the developmental stages of third molars in relation to chronological age and compared third molar development according to location and gender. Materials and Methods: A retrospective analysis of panoramic radiographs of 2490 patients aged between 6 and 24 years was conducted, and the developmental stages of the third molars were evaluated using the modified Demirjian's classification. The mean age, standard deviation, minimal and maximal age, and percentile distributions were recorded for each stage of development. A Mann-Whitney U test was performed to test the developmental differences in the third molars between the maxillary and mandibular arches and between genders. A linear regression analysis was used for assessing the correlation between the third molar development and chronological age. Results: The developmental stages of the third molars were more advanced in the maxillary arch than the mandibular arch. Males reached the developmental stages earlier than females. The average age of the initial mineralization of the third molars was 8.57 years, and the average age at apex closure was 21.96 years. The mean age of crown completion was 14.52 and 15.04 years for the maxillary and the mandibular third molars, respectively. Conclusion: The developmental stages of the third molars clearly showed a strong correlation with age. The third molars developed earlier in the upper arch than the lower arch; further, they developed earlier in males than in females.
Purpose: Root resection can be a valuable procedure when the tooth in question has a high strategic value. The prognosis of root resection has been well documented in previous studies, but the results focused on the palatal root resection have not been discussed in depth. I represent here the short term effectiveness of palatal root resection of maxillary first molars. Methods: Palatal root resection was performed on maxillary first molars of three patients. All the palatal roots were floating state on the radiographic finding and showed full probing depth and purulent exudation at initial examination. Reduction of palatal cusp and occlusal table was performed concomitantly. Endodontic therapy was completed after root resection. Results: Compromised maxillary first molars were treated successfully by palatal root resection in 3 cases. The mobility of resected tooth was decreased a little bit. The probing pocket depth of remaining buccal roots was not increased compared to initial depth. All the patients satisfied with comfort and cost effective results and the fact they could save their natural teeth. Conclusions: Within the above results, palatal root resection is an effective procedure treating compromised maxillary first molar showing advanced palatal bone loss to root apex with or without pulp involvement when proper case selection is performed.
Rapid crestal bone resorption following maxillary tooth loss is further accentuated in the posterior regions because of pneumatization and enlargement of the maxillary sinuses. A treatment rationale that allows preservation and augmentation of vertical available bone at the time of posterior maxil-lary tooth extraction may offer numerous therapeutic benefits which are more short courses of ther-apy and no needs of additional surgical augmentation. The present study comprised 3 patients who had 4 posterior maxillary teeth with no evident bone between the tooth apex and sinus floor, as estimated through preoperative radiographic analysis. Sinus floor augmentation at the time of tooth extraction was chosen for the ltreatment of these patients. After the tooth was carefully extracted, the empty alveolus was thoroughly debrided and a trephine approach was performed. Particulated autogenous bone was gently pushed beyond the empty alveolus to elevate the sinus membrane using an osteotome. The distance between bone crest and si-nus floor was radiographically estimated 4 months after the first procedure. Another procedure was then carried out to place the implants of 11 mm length without another augmentation procedure. All implant were clinically stable, with no sign of infection. The presented surgical procedure performed at the time of extraction of posterior maxillary teeth in close proximity to the sinus floor allowed placement of implants of proper length.
Computed tomography[CT is an effective technique for the evaluation of the thorax following blunt trauma. To evaluate multiply injured 30 patients who were diagnosed as hemothorax in emergency room, computed tomography of thorax was done. The thickness of slice was one centimeter and the entire pleural cavity from the apex to the costophrenic angle was included in the evaluation. Integration and addition of the hemothorax area for each CT slice was made and amount of blood in the pleural cavity was estimated. The slice which showed largest area of hemothorax was selected and the height and width of the hemothorax area were measured. The number of slices which showed radiographic evidence of hemothorax was counted. Regression analysis was done and measured amount of hemothorax, the height and width of the hemothorax area for each slice and number of slices were put as variables. And following equation was derived. V=108.3A-0.8B-7.4C+84.7 [R2=0.74 [ V: amount of hemothorax, A: height, B: width, C: number of slices Total amount of blood from thoracic drainage was compared to the measured amount by computed tomography and the relation between the two values was statistically significant.[p=0.001 In conclusion, quantitative estimation of size of hemothorax was possible by the above equation and the process was very helpful for determination policy of treatment of individual patient.
In order to evaluate the relationship of age with the developmental stage on mandibular second molars in female, the author examined the radiographs in standard films taken by intraoral paralleling technic and analysed the development of 620 roots of 310 females ranged from 10 to 18 years. The development was divided into 7 stages : Crown complete (Cr.C.), Root lengthed 1/4(R.1/4), Root length 1/2 (R.1/2), Rooth length 3/4 (R.3/4), Rooth length complete (R.C), Apex 1.2 closed (A. 1/2), Apical closure complete (A.C) The obtained results were as follows : 1. The formation of roots in fulllength of mandibular second molars was completed at 14.21-year-age in mesial root and at 14.96-year-age in distal root. 2. The formation of apical foramen of mandibular second molars was completed at 17.11-year-age in mesial root and at 17.53-year-age in distal root. 3. The mesial root of mandibular second molar was generally developed earlier than distal root (p<0.01) 4. Coefficients of correlation and regression equations between for age(Y) and the developmental stage of mandibular second molar(X) were "Y=1.53X+6.54, r=0.91 (p<0.01)" in mesial root and "Y=1.52X+7.11, r=0.92(p<0.01)" in distal root respectively.l root respectively.
The purpose of this study was to examine the response of periradicular tissues to amalgam, IRM, Ketac-silver and MTA(Mineral trioxide aggregate) used as a root end fillings. The lower third and fourth premolars of 5 mongrel dogs were used. Each root was resected, followed by root end fillings with experimental materials. The animals were sacrificed after 16 weeks and radiographic and histologic results were evaluated. The results were as follows : 1. Severe inflammation around apex and disruption of cortical were noted in relation to the amalgam. 2. With IRM, there was severe infiltration of inflammatory cells around filling material, but healing of cortical bone was noted. 3. With Ketac-silver, mild inflammation and thick band of fibrous connetive tissue around filling material were seen, with a cortical bone healing. 4. In case of MTA, complete regeneration of cortical bone was seen, and free MTA was surrounded with newly formed bone tissue.
The purpose of this study was to evaluate the root canal transportation and remaining dentin / cementum thickness after using hand and ultrasonic instrumentation in the curved mesial root canals of extracted human mandibular molars. Fourty - six clear polyether blocks were made and randomly divided into two groups: hand instrumentation group with K - Flex files and ultrasonic instrumentation group with Suprasson SP unit. All root canals were instrumented to a size corresponding to a # 30 K - Flex file 1mm short from the radiographic apex. The roots were then sectioned perpendicular to the long axis so the apical and middle third could be evaluated with the Zoom stereomicroscope. The results were as follows : 1. In the total amount of removed dentin at middle third level, there was not significant difference between the hand instrumentation and ultrasonic instrumentation (P>0.05). 2. In the total amount of removed dentin at apical third level, there was more removed by the ultrasonic instrumentation than hand instrumentation(P<0.005). 3. In the transportation width, there was not significant difference between the two groups at both sectioned levels(P>0.05). 4. It was suggested that the canal was transported distally at middle third level and mesially at apical third level by booth techniques.

A-12year-old boy visited the clinic with chief complaint of anterior maxillary trauma. He was diagnosed with first degree mental retardation and cerebral disorders. By clinical and radiographic examination, intrusion of maxillary central incisors were found. The intrusion was not severe, teeth were luxated with a slight force and the prognosis was followed. After the first year, external root resorption was seen radiographically. Due to difficult behavior management, one visit root canal filling with OrthoMTA(BioMTA, Korea) which is known to generate of cementum and periodontal ligament was planned along with general anesthesia. OrthoMTA was filled from the apex to 1-2mm below cervical area and composite resin used for crown restoration. 6 months after, further resorption, discoloration and mobility was not found. This case is currently checked yearly and further research is needed for inflammatory root resorption and ankylosis.
