Radiopharmaceuticals for Imaging of Cellular Proliferation (세포 증식 영상용 방사성의약품)

  • By considering the biological properties of a tumor, it should be possible to realize better results in cancer therapy. PET imaging offers the opportunity to measure tumor growth non-invasively and repeatedly as an early assessment of response to cancer therapy. Measuring cellular growth instead of energy metabolism showed offer significant advantages in evaluating therapy. Thymidine and its derivative nucleoside compounds can be changed to mono, di- and tri- phosphate compounds by thymidine kinase and then be incorporated into DNA. Their bindings are increased in highly proliferating cells due to the high DNA synthesis rate. To evaluate cell proliferation, many kinds of thymidine and uridine derivatives have been labeled with positron emitter and radioactive iodine. Compared to radiopharmaceuticals which have radioisotope labeled base ring such as pyirmidine, the radiopharmacuticals which have radioisotope labeled sugar ring are more stable in vivo and have metabolic resistance. The biological properties such as DNA incorporation ratios are highly dependent on their chemical structures and metabolic processes. This overview describes synthesis of radiopharmaceuticals and their biological properties for imaging of tumor cell proliferation.

  • The differences in the change of the uptake rate of radioactive iodine ($^{131}I$) in the thyroid gland after $Tapazole^{(R)}$ administration before $^{131}I$ treatment were analysed in 137 patients who were diagnosed as diffuse toxic goiter in the Radioisotope Clinic and Laboratory, Seoul National University Hospital since Jan. 1967 to July, 1969. The uptake rate of the therapeutic dose of $^{131}I$ was changed diffusely compared with that of the trace dose in the patients who had no $Tapazole^{(R)}$ administration before $^{131}I$ treatment. In those patients who had $Tapazole^{(R)}$ more than 20 mg/day for more than one week before $^{131}I$ treatment, the uptake rate was decreased significantly. When the patients discontinued the administration of $Tapazole^{(R)}$ 7 days prior to $^{131}I$ treatment, the uptake rate was increased in all cases.

  • Objectives: Hepatoma is a very serious disease in Korea and vvorldwiclc. Hepatitis B vims (HBV) has proved the most significant cause of hepatoma. We canied out this study to investigate the effect of Acanthopanax sessilifloms on inhibiting cell proliferation and DNA synthesis in HepG2.2.15 cell line and on inhibiting phosphorilation of oncogene (MAP kinase) in NIT/3T3-HBx ceIl. Methods: To investigate the anti-cancer effect of Acanthopanax sessiliflorus, we did the CellTiter 96 Aqueous Non-radioactive Cell Proliferation assay (Promega); MTS/PMS assay, [$^3H$]-thymicline incorporation assay, and we measured the gene expression through westem blotting. Results: Acanthopanax sessiliflorus showed an inhibiting effect on the increase of HepG2.2.15 in the NTS/PMS assay. It also showed an inhibiting effect on DNA synthesis of HepG2.2.15 in the [$^3H$]-thymidine incorporation assay. Acanthopanax sessiliflorus showed an inhibiting effect of phosphorilation of MAP kinase in HBV - X genes. too. Conclusions: The results suggested that this herb had an anti cancer effect. We may discover an effective anti-cancer herb medicine through further studies on this herb medicine.

A Study on the Condition Analysis and Improvement of Domestic Medical 99Mo/99mTc Generators Self-disposal (국내 의료용 99Mo/99mTc Generator 자체 처분 지침 현황 분석 및 개선 방향에 대한 연구)

  • The nuclear medicine department of a domestic medical institution uses $^{99m}TcI$, a radionuclide, from $^{99}Mo/^{99m}TcI$ Generator, to inject radioactive drugs into patients. Among the expired generators, imported from foreign countries, the medical institution implements its own disposal. Each medical institution shall satisfy the permitted in-house disposal concentration of radioactive wastes. The guidelines for self-disposal presented in Korea suggested that self-disposal can be performed 80 days after the generator is used. The purpose of these guidelines is to analyze them by comparing them with the data measured directly with the generator and to study if they are feasible. As a result, the generator with a capacity of 1,000 mCi has the longest half-life, and when tested with a high-radiation Mo(molybdenum) column, the number of days that are below the permitted concentration of body disposal with radioactive waste was 72 days and 71 days that were derived from direct column measurement. The results of the direct study confirmed that the guidelines for in-house disposal in Korea were reasonable, as there were 8 to 9 days of storage compared to the number of in-house disposal days provided in the guidelines.

  • Microarray technology allows the simultaneous analysis of gene expression patterns of thousands of genes, in a systematic fashion, under a similar set of experimental conditions, thus making the data highly comparable. In some cases arrays are used simply as a primary screen loading to downstream molecular characterization of individual gene candidates. In other cases, the goal of expression profiling is to begin to identify complex regulatory networks underlying developmental processes and disease states. Microarrays were originally used with ceil lines or other simple model systems. More recently, microarrays have been used in the analysis of more complex biological tissues including neural systems and the brain. The application of cDNA arrays in neuropsychiatry has lagged behind other fields for a number of reasons. These include a requirement for a large amount of input probe RNA In fluorescent-glass based array systems and the cellular complexity introduced by multicellular brain and neural tissues. An additional factor that impacts the general use of microarrays in neuropsychiatry is the lack of availability of sequenced clone sets from model systems. While human cDNA clones have been widely available, high qualify rat, mouse, and drosophilae, among others are just becoming widely available. A final factor in the application of cDNA microarrays in neuropsychiatry is cost of commercial arrays. As academic microarray facilitates become more commonplace custom made arrays will become more widely available at a lower cost allowing more widespread applications. in summary, microarray technology is rapidly having an impact on many areas of biomedical research. Radioisotope-nylon based microarrays offer alternatives that may in some cases be more sensitive, flexible, inexpensive, and universal as compared to other array formats, such as fluorescent-glass arrays. In some situations of limited RNA or exotic species, radioactive membrane microarrays may be the most practical experimental approach in studying psychiatric and neurodegenerative disorders, and other complex questions in the brain.

  • Purpose Serum Thyroglobulin measurement is a major tool for the follow-up of differentiated thyroid cancer (DTC) patients. Thyroglobulin is Normal thyroid tissue, or thyroid cancer tissue produced only.   • The occurrence of a mixed tumor containing papillary thyroid carcinoma (PTC) and primary squamous-cell carcinoma (SCC) is rare because there is no squamous epithelium in the thyroid gland. Reported herein is a 30-year-old female with mixed PTC and primary sec of the thyroid presented as thyroid incidentaloma. Fine-needle aspiration biopsy of the thyroid nodule revealed the presence of malignant thyroid cells. The histopathological examination following total thyroidectomy yielded two mixed, morphologically distinct histotypes that included PTC and sec. After total thyroidectomy, the patient underwent radioactive iodine therapy. No recurrence or metastasis occurred during the 20-month follow-up period after the operation.

  • The folate receptor (FR) is a promising cell membrane-associated target for nuclear imaging of various cancers (via imaging $FR-{\alpha}$) and potentially also inflammatory diseases (via imaging $FR-{\beta}$), through the use of folic acid-based radioconjugates. However, there have been several drawbacks of previously reported radioconjugates, such as a short half-life of the radiolabel ($^{68}Ga\;t_{1/2}$ 68 min), a complex and time-consuming multistep radiosynthesis, and a high renal uptake of radiolabeled folate derivatives. The goal of this study was to develop an imaging probe by directly labeling folate with radioactive iodine without using an extra prosthetic group. The radiolabeling of folate was optimized using various labeling conditions and the labeled tracers were isolated by high-performance liquid chromatography. The in vitro stability of labeled folate was checked in phosphate-buffered saline and serum. The tumor-targeting efficacy of the probe was also evaluated by biodistribution studies using a murine 4T1 tumor model.

  • We experienced a case with meningioma showing false positive I-131 uptake. A 55-years old female patient underwent high dose (150 mCi) radioactive iodine therapy to ablate remnant tissue after total thyroidectomy for papillary thyroid cancer. in addition to intense tracer uptake in thyroid bed, there was mild but focal abnormal uptake in left frontal lobe of the brain on post-therapy I-131 whole body scan. Subsequent brain MR imaging showed single mass lesion in left frontal lobe and the mass was resected under the impression of brain metastasis of thyroid carcinoma. Pathologic report confirmed meningioma from the surgical specimen.

  • A patient diagnosed as cellulitis by bone scan was taken the therapeutic intervention of Oriental medicine for 35 days. Cellulitis is characterized by acute purulence inflammation deeply diffused throughout epidermal connective tissue to subcutaneous adipose tissue. This patient presented a finding of soft tissue radioactive isotope uptake increase on bone scan of both foot and ankle. Cellulitis belongs to the category of ong(癰) in Oriental medicine. During therapeutic intervention, 3 herbal prescriptions were applied. Yeonkyopedok-san was applied for high fever and severe chilling on acute stage. After the application of Yeonkyopedok-san and Sunbangwhalmyong-eum, clinical symptoms improved with changes of CRP($112{\rightarro}12),\;WBC(13{\rightarro}8.8),\;ESR(65{\rightarro}46$). After the application of Chungpesagan-tang, clinical symptoms improved with changes of $CRP(12{\rightarro}3),\;WBC(8.8{\rightarro}5),\;ESR(46{\rightarro}13$).
