Comparative Study of Contrast-Enhanced Ultrasound Qualitative and Quantitative Analysis for Identifying Benign and Malignant Breast Tumor Lumps

  • Background: To compare the value of contrast-enhanced ultrasound (CEUS) qualitative and quantitative analysis in the identification of breast tumor lumps. Materials and Methods: Qualitative and quantitative indicators of CEUS for 73 cases of breast tumor lumps were retrospectively analyzed by univariate and multivariate approaches. Logistic regression was applied and ROC curves were drawn for evaluation and comparison. Results: The CEUS qualitative indicator-generated regression equation contained three indicators, namely enhanced homogeneity, diameter line expansion and peak intensity grading, which demonstrated prediction accuracy for benign and malignant breast tumor lumps of 91.8%; the quantitative indicator-generated regression equation only contained one indicator, namely the relative peak intensity, and its prediction accuracy was 61.5%. The corresponding areas under the ROC curve for qualitative and quantitative analyses were 91.3% and 75.7%, respectively, which exhibited a statistically significant difference by the Z test (P<0.05). Conclusions: The ability of CEUS qualitative analysis to identify breast tumor lumps is better than with quantitative analysis.

Age Related Analysis of Ultrasound Images of Normal Skeletal Muscle (연령에 따른 정상 골격근의 초음파 영상 분석)

  • Since ultrasound has different reflections depending on components of organization, analysis of ultrasound images of skeletal muscle can offer both quantitative and qualitative reports as concerns skeletal muscle structure. This study is focused on the ultrasound method for evaluating the structural characteristics of skeletal muscle and also conducted to examine its practicality. After obtaining images of the elbow flexors from an ultrasound image device with 88 normal subjects whose ages were between twenty and seventy years old (44 men and 44 women), muscular density and white area index (WAI) which indicated structural characteristics of skeletal muscle were analyzed with gray scale analysis. The study examined correlations between subject's age and items which obtained from measuring ultrasound images and the differences in relations to sex and age. Muscular density and WAI had a high correlation with age and were significantly increased in men and women with greater age. The quantitative evaluation method of skeletal muscle structure which analyzed the ultrasound images has high practicality because it is a non-invasive method which complements physical therapy diagnosis and research methods and promotes functionality evaluation.

Fatty Liver Analysis through Quantitative Measurement Study of Ultrasonography Images (초음파 검사 영상의 정량적인 측정 연구를 통한 지방간 분석)

  • This study attempted to find out the degree of agreement between ultrasound image findings along with analysis of attenuation index and scatter distribution index values within tissues through quantitative measurement analysis using liver ultrasound images. From August 2022 to October 2022, liver ultrasound was performed on 45 patients who were suspected of having fatty liver and who received a prescription for liver ultrasound. As a result of the study, as a result of analyzing the agreement between the ultrasound image findings and the tissue attenuation index, the Kappa value was 0.82 (p<0.05), showing a very high agreement between the two examination methods. In addition, as a result of the agreement analysis between the ultrasound image findings and the scatter distribution index in the tissue, the Kappa value was 0.642 (p<0.05), showing high agreement between the two examination methods. At the time of fat liver prediction, the use of liver ultrasound findings and quantitative ultrasonography techniques, such as intra-tissue attenuation index and intra-tissue scatter distribution index, may be helpful in determining the degree of progression of fatty liver patients.

Analysis of sEMG Median frequency and Ultrasound Image Echodensity of Normal Skeletal Muscle (정상 골격근의 근전도 중앙주파수 및 초음파 영상 밀도 분석)

  • Purpose: This study conducts quantitative evaluation or structural and functional characteristics or normal skeletal muscle with ultrasound image and surface electromyography, and is to provide basic materials for utilizing ultrasound image analysis in physical therapy diagnosis and assessment of skeletal muscle. Methods: Measurement of three stages was conducted with 88 normal adults between their twenties and seventies, correlations and differences using collected data according to age and gender were compared and correlations among measured items were analyzed and then the following conclusions were obtained. Results: Analysis of ultrasound image of normal skeletal muscle showed that density, median frequency had the closest relations with age. In addition, it was found that there were high correlations between density explaining structural characteristics of skeletal muscle and median frequency explaining functional characteristics. Conclusion: Analysis of ultrasound image makes complex evaluation of structure and function of skeletal muscle possible when it is connected with functional evaluation method using physical measurement surface electromyography as well as quantitative evaluation of structural changes of skeletal muscle and is effective in complementing physical therapy diagnosis centering around functionality evaluation.

  • Quantitative ultrasound analysis provides fundamental information of various ultrasound parameters using spectral information of the short-gated radiofrequency(RF) data. Therefore, accurate extraction of spectral information from backscattered RF signal is crucial for further analysis of medical ultrasound parameters. In this paper, we propose two techniques for calculating a more accurate power spectrum which are based on the phase-compensation using the normalized cross-correlation to minimize estimation errors due to phase variations, and the weighted averaging technique to maximize the signal-to-noise ratio(SNR). The simulation results demonstrate that the proposed method estimates better results with 10% smaller estimation variances compared to the conventional methods.

  • Purpose: The objective of this study was to offer primary clinical data examining whether change of imaging structure and quantitative evaluation of muscle activity on myofascial trigger points can lead to implementation of an analytical technique for evaluation of myofascial pain diagnoses. In addition, we examined the effect of a variety of mediation techniques, in order to examine neuromuscular physiological characteristics of myofascial trigger points muscle by comparing differences in pressure pain threshold and ultrasound imaging. Methods: Participants in the study included 30 adults in their twenties. The subjects were divided into the normal and myofascial trigger points groups. Clinical outcomes were evaluated by pressure pain threshold for pain and ultrasound imaging was performed for evaluation of the structural characteristics of muscle. Independent t-test was used for statistical analysis. Results: The two groups showed statistical significance in the change in pressure pain threshold (p<0.05). Findings of ultrasound imaging analysis showed no significant differences, increased muscle thickness was observed (p>0.05). Findings of ultrasound imaging analysis showed significant differences, increased muscle echodensity was observed (p<0.05). Findings on ultrasound imaging analysis showed significant differences, increased muscle white area index was observed (p<0.05). Conclusion: From these results, active myofascial trigger points muscle showed quality deterioration on ultrasound imaging. Thorough evaluation of imaging structure and physiological characteristics can be useful quantitative analytical techniques for diagnosis of myofascial pain syndrome and a primary factor reflected in physical therapy intervention.

  • The objective of this study was to investigate the relation of serum calcium level, body mass index(BMI) with bone status expressed as broadband ultrasound attenuation(BUA) measured by quantitative ultrasound (QUS) and the occurrence of osteopenia among adult men and women. Finite Element Analysis of Pressure Distribution by Ultrasound in Human Thigh Model (대퇴부 모델에서의 초음파 압력분포에 관한 유한요소 해석)

  • Quantitative analysis for distribution of penetrating ultrasound in vivo is very important to determine the treatment region and method. In this paper, we constructed a simplified 2-D femoral region model that consists of skin-fat-muscle-bone layered system, and simulated the pressure distribution in the model in case of applying ultrasound using Finite Element Method(FEM). The ultrasound used in the simulation was assumed to be pulse wave and the pressure distribution was analyzed during only one period of pulse wave. In order to find the penetration depth, amplitude of pressure and sphere that ultrasound reaches in the model, we performed the simulation with varying the applied frequency, transducer size and amplitude of transducer's output. The result showed that applied frequency is inversely proportional to the penetration depth and amplitude of pressure but the amplitude of transducer's output is proportional to the amplitude of pressure in the model. Also, the sphere that ultrasound reaches was widened and the amplitude of pressure became larger as the transducer size became larger. This results were similar to that obtained from the previous model consisting of fat-muscle-bone layered system, but we observed that the pressure of ultrasound is decreased due to the decrements of pressure by the absorption coefficient of skin and the interference that depends on the reflection of ultrasound caused by the difference of acoustic impedance of skin and fat. Finally, we can infer that the model proposed in this study is closer to the realistic model than the previous ones. It shows that the results obtained from this study can be useful in designing the ultrasound treatment instrument or in setting up the treatment plan.

  • In the recent years, studies using Computer-Aided Diagnostics(CAD) have been actively conducted, such as signal and image processing technology using breast ultrasound images, automatic image optimization technology, and automatic detection and classification of breast masses. As computer diagnostic technology is developed, it is expected that early detection of cancer will proceed accurately and quickly, reducing health insurance and test ice for patients, and eliminating anxiety about biopsy. In this paper, a quantitative analysis of tumors was conducted in ultrasound images using a gray level co-occurrence matrix(GLCM) to experiment with the possibility of use for computer assistance diagnosis.

Quantitative Ultrasound for Osteoporosis Screening in Postmenopausal Women (폐경 후 여성에서 골다공증의 조기검진도구로서 골초음파의 유용성)

  • Objectives : To evaluate the diagnostic value of quantitative ultrasound (QUS) in the prediction of osteoporosis as defined by dual energy x-ray absorptiometry (DEXA) in postmenopausal women. Methods : Questionnaires and height and weight measurements were used in the investigation of 176 postmenopausal women. QUS measurements were taken on the right calcaneus while bone mineral density (BMD) measurements of the lumbar spine and femoral neck were made with DEXA. The areas under the curves (AUC) of the speed of sound (SOS) for osteoporosis in the lumbar spine and femoral neck were obtained through receiver operating characteristic (ROC) analysis and evaluated. A comparison was made, for osteoporosis in the lumbar spine and femoral neck, between the AUCs of the logistic model with clinical risk factors and SOS. Results : Pearson's correlation coefficients of SOS and lumbar spine BMD, and of SOS and femoral neck BMD were 0.26 and 0.37. The AUC for the logistic model in its discrimination for lumbar spine osteoporosis was 0.764, and for SOS 0.605. The AUCs for the logistic model in its discrimination for femoral neck osteoporosis and for SOS were 0.890 and 0.892, respectively. Conclusions : These results suggest that the diagnostic value of QUS as a screening tool for osteoporosis is moderate for the femoral neck, but merely low for the lumbar spine and that the predictability provided by SOS is no better than that by the sole use of clinical risk factors in postmenopausal women.
