A Study on Korean Medical Clinical Management of Female Overactive Bladder (여성 과민성 방광의 한방 임상에 관한 고찰)

  • Objectives: This study aimed to review Korean medical clinical management of female overactive bladder (OAB). Methods: We reviewed domestic and foreign papers related to Korean Medicine Treatment for OAB and literatures on Korean Medicine added clinical views of authors. Results: OAB is usually diagnosed to Deficiency pattern (Kidney qi deficiency-cold, Spleen-Lung qi deficiency et al.). The primary treatment goals of OAB is improvement of urinary symptom. But a comprehensive treatment approach including improvement of accompanied symptoms such as depression, insomnia, sexual dysfunction and improvement of quality of life is needed. Warming and tonifying herbal medicine, electro-acupuncture and moxibustion using acupoints specially acting bladder function in lower abdomen and lumbar-sacral area and behavioral therapies such as lifestyle modification, bladder training are usually primary treatments. Treatment period is recommended about 3~6 months to recover and stabilize bladder function. Conclusions: OAB is a clinical area that Korean Medicine tend to be more effective. but additional research about Korean Medical Clinical Management of OAB is needed.

  • Biogenic monoamines are divided into three categories; catecholamines(dopamine, norepinephrine, and epinephrine), indoleamine(serotonin and melatonin) and histamine. Among them, serotonin has been intensively studied by many researchers with a broad spectrum of biomedical interests. A concise overview of serotonin-related topics such as biosynthetic pathway, receptor subtypes, and roles in reproduction will be provided. In particular, serotonergic efffect on the regulation of hypothalamus-pituitary-gonad hormonal axis and sexual behaviors will be emphasized. Though our Knowledge on the biological roles and its clinical applications are still limited, these topics are quite promising subjects which will be helpful for improving our 'quality of life' in near future.

The Study on the Sexual Behavior of Unmarried Female Workers in the Small and Medium Scale Industries (중소규모 산업장 미혼 근로여성의 성행태에 관한 연구)

  • The purpose of this study was to find the distribution of the variables on the quality of life and the determinants of the sexual attitude and behavior of the unmarried female workers. This study was surveyed to the 306 unmarried women who worked in the small and medium scale industries in Kyungin area and analyzed the respondent's knowledge of sex, sexual behavior, health behavior, health status, satisfaction of working condition and recognition of working environment. The result of this study could be summarized as follows: The respondent's age are mostly early of twenties and their education level are high school and more. They recognize that their health condition is not so good but they hardly try to improve health condition. They think that their working condition are mostly unsatisfied and they also believe that they expose themselves to the toxic working environment. Although their knowledge of sex are low they have few chances for the education of sex and family planning. Their attitude of premarital sex are conservertive but the rate of approval of living together before marriage are high and the rate of premarital sex is around 15 percent. The premarital sex behavior are positively related with family size, living condition, knowledge of sex and working period but the sex experiences are negatively related with working period and knowledge of sex. As a result we suggest that the sex education and consultative program are necessary for improving the quality of life of the unmarried working women in small and medium scale industries.

Adolescent's Risk Behavior and the Quality of Life: the Role of Protective Factors on Risk Behavior (청소년의 위험행동과 삶의 질: 위험행동에 대한 보호요인의 역할)

  • This paper discuss adolescent's a quality of life related with risk behavior. The purpose of this study investigate to influence on risk behavior(runaway, smoking, sexual behavior) of the protective factors that moderate adolescent's problem behavior(delinquency). The assumption of this study that the protective factors counterbalance the negative influence of risk factors and finally, diminish a the problem behavior including a delinquent. A total of 1,020 students of a vocational high schook and a 216 adolescents of a special groups(the public institution that consisted with a delinquent young man) completed the questionnaires(risk behavior, 5 protective factors) of compiled by this researcher. The protective factors have selected based on the various prior studies analyzed with adolescent's risk behavior a family functioning, a father(a mother) each and child communication, a self efficacy, and a social support. Statistics appled for the data analysis are Chisqure analysis, two-way ANOVA, and Standard Discrimination analysis. The results of this study are as follows. First, the special group is higher than the general group in the rate of runaway, smoking, and sexual deviant behavior. Second, the protective factors are not action in the special group have experienced delinquency, but are only action in the general group consisted with the students of a vocational high schools. This means that the protective factors discriminating the participation of the risk behaviors, and blocking out the intervention of a problem behavior in the general adolescents. Although each protective factor influence to different according to each risk behavior, a role of a parent-child communication, a family functioning, and self-efficacy high orderly. Finally, discussed based on the previous studies that the protective factors moderate the negative influence of risk factors, offset the connection between a risk behavior and a. problem behavior, and improve and a resilience and the quality of life of the adolescents.

The Meaning of Menopause Experienced by Women (여성이 경험한 폐경의 의미)

  • The purpose of this study was attempted to understand the substance and meaning of menopause experienced by women through informal interviews with oral consent. The informants were 6 perimenopausal women of 50-55 years old who are executing menopause. Colaizz's analytical method, a type of phenomenological analysis, was used to analyze data recorded by audiotape. One professor and a master's degree student who understand phenomenology, and the one who has a master of arts examined the validity between the meanings composed of the clusters of themes. Findings were turned out to be valid through validation process as the last step. The meaning of menopause implied both 'concept about menopause' and 'menopause as a time of change'. Menopause was mostly considered as cessation of menstruation as a physiological, natural, and normal process by aging. However, some people regarded menopause as a loss of youth and womanhood and lessening of every function of the body. Menopause as the time of changes means 'the period of' 'hormonal changes' such as change of menstruation, hot flushes, perspiration, and palpitation ; 'body function changes' of visual acuity, physical strength, sleeping, digestion, thoughts, bone and joints, skin sensibility, sexual pattern and intelligence ; 'emotional changes' such as anxiety, loneliness, gloominess, and nervousness. Menopause is a turning point on the women's life cycle accompanying various kind of changes and health problems. Therefore it is inevitable to develop strategy helping menopausal women pass through the critical successfully by adapting and coping with their critical period toward the healthy and better quality of life individually rather than putting them all into the standardized hormonal replacement protocol.

Development and Validation of Self-Efficacy Scale for Self-Management of Breast Cancer (SESSM-B) (유방암 환자의 자가관리에 대한 자기효능감 측정도구 개발)

  • Purpose: This purpose of this study was to develop and validate a Self-Efficacy Scale for Self-Management of Breast Cancer (SESSM-B). Methods: The SESSM-B was developed and validated as follows: Item generation, pilot study, and tests of validity and reliability. Twenty-one items were developed through evaluation by 10 experts and 13 items were finally confirmed through item analysis and factor analysis. Psychometric testing was performed with a convenience sample of 303 women with breast cancer. Data were analyzed using factor analysis, Pearson correlation coefficients, and Cronbach's alpha. Results: Five factors evolved from the factor analysis, which explained 69.8% of the total variance. The first factor 'coping with psycho-informational demand' explained 17.2%, 2nd factor 'maintenance of healthy lifestyle' 14.5%. 3rd factor 'management of side-effects' 13.3%, 4th factor 'therapeutic compliance' 12.8%, and 5th factor 'sexual life' 11.9%. SESSM-B also demonstrated a concurrent validity with health-related quality of life scale, EORTC QLQ-C30 & BR23. The internal consistency, Cronbach's alpha, was .78, and reliability of the subscales ranged from .61 to .79. Conclusion: The results of this study suggest that the SESSM-B is an easy, reliable, and valid instrument to measure self-efficacy for self-management of breast cancer.

Development of a comprehensive health promotion program for North Korean young adult defectors in South Korea (북한이탈 청소년을 위한 통합적 건강증진 프로그램 개발)

  • Purpose: The purpose of this study was to develop a comprehensive health promotion program for North Korean young adult defectors in South Korea. Methods: The comprehensive health promotion program consisted of nutrition, mental healthcare, physical activity and sexual behavior was developed on the basis of need assessment results. For the evaluation of the program, 70 North Korean young adults who were attending two alternative schools for North Korean defectors were recruited. The program had taken place once a week for 13 or 19 weeks. Effectiveness of the health promotion program was evaluated using anthropometric measurement, 3-day food records and a questionnaires that comprised the Hospital Anxiety and Depression Scale (HADS), the Short Form with 36 questions (SF-36) and health behaviors. The surveys were proceeded at the beginning and after the program. Results: After health promotion program, participants' height was significantly increased (p=0.004) and body fat mass (0=0.004) and percentage of body fat mass (p=0.003) were significantly decreased. The number of subjects who ate breakfast alone was decreased whereas the number of subjects who ate breakfast with friends was increased (p<0.001). There were no significant changes in dietary intakes, mental health status and quality of life. North Korean young adult defectors' willingness to participate and interests in the health promotion program were high, however the practice rate was low. Conclusion: The health promotion program could induce interests and willingness to participate, but bring about limited effects on the health behaviors. These results imply that a health promotion program for North Korean young adult defectors should have a long-term strategy as well as short-term plan. Furthermore, it should be based on their health problems, health related behaviors, academic performance and daily life matters.

A study on research trends for pregnancy in adolescence: Focusing on text network analysis and topic modeling (청소년 임신에 대한 연구 동향 분석: 텍스트 네트워크 분석과 토픽 모델링)

  • Purpose: The aim of this study was to identify core keywords and topic groups in the "adolescent pregnancy" field of research for a better understanding of research trends in the past 10 years. Methods: Topics related to adolescent pregnancy were extracted from 3,819 articles that were published in journals between January 2013 and July 2023. Abstracts were retrieved from five databases (MEDLINE, CINAHL, Embase, RISS, and KISS). Keywords were extracted from the abstracts and cleaned using semantic morphemes. Text network analysis and topic modeling were performed using NetMiner 4.3.3. Results: The most important keywords were "health," "woman," "risk," "group," "girl," "school," "service," "family," "program," and "contraception." Five topic groups were identified through topic modeling. Through the topic modeling analysis, five themes were derived: "health service," "community program for school girls," "risks for adult women," "relationship risks," and "sexual contraceptive knowledge." Conclusion: This study utilized text network analysis and topic modeling to analyze keywords from abstracts of research conducted over the past decade on adolescent pregnancy. Given that adolescent pregnancy leads to physical, mental, social, and economic issues, it is imperative to provide integrated intervention programs, including prenatal/postnatal care, psychological services, proper contraception methods, and sex education, through school and community partnerships, as well as related research studies. Nurses can play a vital role by actively engaging in prevention efforts and directly supporting and educating socially disadvantaged adolescent mothers, which could significantly contribute to improving their quality of life.

  • Since the first radioimmunoassay (RIA) was developed in 1970s, many conventional RIAs and non-isotopic immunoassays (NIA) had been developed in which the degree of precision, accuracy, specificity and practicability have progressively increased. Recently ultrasensitive assay method at femtogram to determine testosterone in serum, saliva and feces is required for the study of sexual dysfunctions in male and female, monitoring the psychological stress and conditions, aging process such as menopause and partial androgen deficiency in aging male, the hormonal changes of small experimental animals etc. This review discussed the recentd evelopments of steroid assay methods, based upon the testosterone assay results of authors far 20 years, and the problems associated the assay set-up, the characterizations and applications of the established procedures, and desifls of assay, reliablity criteria, and the practical aspects of assay set-up and application, based upon the data of the authors. The present study demonstrates the general problems methods to be consider in order to set up the highly sensitive assay methods and to increase the assay quality and the necessity of assay quality control program. To improve the assay quality of each laboratory and to compare the assay results in homeland, the national QC programs should be organized.

Surgery Is the Last Resort for Huge Scrotal Lymphedema: A Series of Challenging Cases

  • We aim to provide our surgical techniques, and outcomes of functional scrotal reduction procedures with complete preservation of the genitourinary original anatomy in a simple way without using complicated skin grafting or skin advancement flaps in Patients with huge and long-standing scrotal lymphedema 18 patients ages ranged from 14-65 with a median of 30 years. Functional scrotal and penoscrotal reduction was attained in all cases, without distortion of the genitourinary anatomy and without the need for advancement, rotational or free flaps, maximal scrotal diameter was reduced from median of 61[48-92] cms to a median of 25[21-29] cms (P<0.0001) and remained almost unchanged at the end of the follow up period 26[22-34] cms (P<0.0001). Sexual performance and voiding capacity were improved in all patients, testicular vascularity was unaffected and the Glasgow Benefit Inventory (GBI) for the quality of life showed marked enhancement in the total 55.5[50-72], general 55.5[50-72], social 100[50-100] and physical 16.6[16-33] points subscales. According to our experience, surgery remains the gold standard treatment for management of huge scrotal lymphedema, successful preservation of the genitourinary functions can be attained despite the size in most cases with excellent cosmoses.
