Case Study of Cancer Patients Treated with Herbal Acupuncture Therapy (약침치료를 이용한 각종 암환자의 임상사례)

  • Objective : This study was aimed to validate the Herbal Acupuncture Therapies(HAT) for cancer patients. Patients and methods : This retrospective study was performed on 8 patients who were diagnosed as cancer in Korea and treated with HAT in the oriental hospital of Daejeon University, from January 2003 to January 2004. We retrospectively analyzed the medical records of 8 patients for improvement of symptoms, toxic effects of liver and kidney, myelosupression and changes of Quality of Life(QOL). Results : Analysis of change of chief complaints showed that 75% patients replied moderate relief and 25% replied complete relief in Likert scale. Analysis of Liver Function Test(LFT), Renal Function Test(RFT) level showed that HAT does not have toxic effects on liver and kidney. Analysis of Complete Blood Count(CBC) level showed that HAT does not have myelosuppression effects on bone marrow. Analysis of QOL showed that 100% patients replied improvement in Eastern Cooperative Oncology Group status(ECOG) status. Conclusion : Our findings suggest that HAT offer potential benefits for cancer patients.

A Case Report of Symptomatic Epilepsy (수면중 간질발작을 주소로 하는 증후성 간질 환아 증례 보고)

  • Objectives The purpose of this study is to report a case which gave various remarkable epilepsy treatment results by using oriental medicine. Methods We treated the patient with acupuncture as a main treatment and herbal medicine. We also recommended to the patient and patient's parents to continue oriental medical treatment for 2 years from now because symptomatic epilepsy is a type of disease which needs long term care. Results The epileptic seizure has been well controlled for 12 months so far, and the results was recorded by EEG. The EEG level was in normal range. Conclusions Even though this is only one case of the study but this case has an important meaning because the epilepsy was treated by oriental medicine, especially acupuncture treatment. The control of the symptomatic epileptic seizure was effective by means of increasing patient's quality of life. Further studies about effective treatment of symptomatic epilepsy would be needed with more various cases.

The Research of Pain and Functional Disability Assessment Scales for Knee Joint Disease (슬관절의 통증과 기능장애의 평가도구에 관한 연구)

  • Objectives : The knee pain is the main reason of getting acupuncture treatment. In order to have confidence in the results of a study, it is necessary to establish that questionnaire is both valid and reliable in questionnaire related study. The aim of this study was to review the instruments that are currently in sue for assessing the knee joint. Methods : A literature study was performed to choose appropriate scales for assessment for pain and the function of the knee. Theoretically based scales were selected for review. Therefore, 18 scales for knee and 9 scale for pain were reviewed. the status of scales involved in knee treatment of acupuncture throughout several countries was evaluated. Results & Conclusion : Lysholm Knee Scoring Scale, Cincinnati knee rating system(CKRS) are adequate for ligament injury in knee. International Knee Documentation Committee scoring system(IKDC) may become a publication requirement for journals in view of the international standing of the committee. The available outcome measure for use in osteoarthritis are Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC), Lequesne Functional Severity Index(LFI) and Knee Injury and Osteoarthritis Outcome Score(KOOS), and in rheumatoid arthritis are McMaster-Toronto Arthritis patient function preference questionnaire(MACTAR), Quality of Life-Rheumatoid Arthritis Scale(QoL-RA Scale). Visual analog scale(VAS), verbal rating scale(VRS) are commonly used for the standard pain scale. For long term follow-up study The Medical Outcomes Study Short Form-36(SF-36), Arthritis Impact Measurement Scales(AIMS), Health Assessment Questionnaire(HAQ) should be included. Each measurement has its own composition and characteristics. Their validity, reliability, responsiveness and practical characteristics were already evaluated. We found 20 domestic and 28 overseas papers about knee treatment using acupuncture assessed with knee scales.

The Study on the Effectiveness of Acupuncture in Stroke Rehabilitation (중풍(中風) 재활(再活)의 침치료(鍼治療) 효과(效果)에 대한 고찰(考察) -최근 RCT(Randomized controlled trial) 논문을 중심으로-)

  • Objective : The aim of this study is to review clinical trials on the effectiveness of acupuncture in stroke rehabilitation. Methods : Computerized literature searches was carried out on three electronic databases, and hand-searching on some chinese medical journals in library of Kyung Hee Medical Center. Results : 1) Sixteen articles of clinical trials were collected and reviewed. Among these articles, randomized controlled trials were achieved in nine articles. 2) In three articles, statically significant results in improvement of mobile abilities, activities of daily life and Quality of life were reported after acupuncture treatment applied as a part of stroke rehabilitation. In three articles no statically significant changes were reported. 3) Among two articles about spasticity, One about the upper limbs and the results showed statically significant improvement of the spasticity after acupuncture treatment as stroke rehabilitation, and the other was about the lower limbs and the results showed no statically significant changes. 4) One article about acupuncture and postural control showed statically significant results suggesting that acupuncture promotes normalization of postural control after stroke.

Moxibustion Treatment for Knee Pain: A Systematic Review (슬통의 뜸치료에 대한 체계적 고찰)

  • Objectives : This study was designed to evaluate clinical evidence of moxibustion treatment for knee pain. Methods : All processes were independently carried out by three investigators. A literature search was performed in 3 databases from their inception to May 2015. Ten reports were found based on their title, abstract and text. Following this, data extraction and analysis were done using a risk of bias(ROB) and through an assessment of multiple systematic reviews(AMSTAR). Results : 10 studies(6RCT, 2SR, 2CR) were included. Generally, indirect moxibustion was used for knee pain, but only one study indicated the use of direct moxibustion. Moxibustion was shown to be effective in treating knee pain, and the number of required treatments was fourteen on average. In assessing risk of bias, indefinite and uncertain information made all included trials subject to a high risk of bias. On the other hand, SR showed all evaluation items in the assessment of multiple systematic reviews, with the exception of an included or excluded studies list. Conclusions : Because of deficient study design or limited research planning, there is not sufficient evidence to allow for any conclusion about the efficacy of moxibustion for knee pain. Therefore, well-designed high quality trials are needed from now on.

One Case Report of Internal Derangement of Knee with Patella Tendon and Posterior Cruciate Ligament Injury - with a Focus on Burning Acupuncture Therapy and Exercise Therapy - (슬개건 및 후방십자인대 손상을 동반한 슬내장증 환자에 대한 치험 1례 - 가열식 화침 치료 및 운동 치료를 중심으로 -)

  • Objectives: The purpose of this study is to evaluate the effect of the burning acupuncture therapy and exercise therapy on the internal derangement of knee with patella tendon and posterior cruciate ligament injury. Methods: Burning acupuncture therapy was administered three times and exercise therapy was administered during 4 weeks. The improvement of clinical symptom was evaluated by VAS(visual analogue scale) and KOOS(knee injury and osteoarthritis outcome score). Results: After treatment, the patient's VAS score was reduced to 0 pant. The KOOS score of pain, symptom, ADL, sport/rec, QOL(quality of life) improved from 28 to 75, from 36 to 71, from 22 to 65, from 0 to 15, from 25 to 38 respectively. Conclusions: Burning acupuncture therapy and exercise therapy fer internal derangement of knee with patella tendon and posterior cruciate ligament injury was effective. Although this case presented valuable result, further research is encouraged to confirm the effectiveness of this treatment with large number of patient.

Literature Review of Clinical Research Electro-Acupuncture Treatment on Post-stroke Dysphagia (뇌졸중 후 연하장애 전침 치료의 임상연구에 대한 고찰)

  • The aim of this study was to collect and analyze clinical studies on electro-acupuncture therapies of post-stroke dysphagia to suggest the methods of the study about electro-acupuncture therapy. In this study, electronic searches were performed with searching sites. Searched words such as '중풍', '뇌졸중', '연하장애', '전침', 'stroke', 'dysphagia', 'deglutition disorders', 'electro-acupuncture' were used as a single keyword or in combinations. Through searching, 27 studies were selected and analyzed. In comparison of results before and after electro-acupuncture treatment, there were statistically significant improvement in 26 studies by using assessment such as video fluoroscopy swallowing study, swallowing water function test. The study of electro-acupuncture treatment for post-stroke dysphagia needs to be standardized and high-quality study models should be introduced.

A Meta-analysis of the Effectiveness of Acupuncture for the Treatment of Essential Tremors (본태성 떨림의 침치료 효과에 대한 메타분석)

  • Objectives: This study provides an evaluation of the effectiveness of acupuncture for the treatment of essential tremors (ETs) using randomized controlled trials (RCTs). Methods: Nine databases (PubMed, CINAHL, EMBASE, OASIS, NDSL, CNKI, Wanfang, J-STAGE, and CiNii) and the keywords "Essential tremor", "Benign tremor", "Familial tremor", and "Acupuncture" were used to compile RCTs that used acupuncture to treat ETs. Studies that were not RCTs, such as case studies, reviews, and essays, were excluded. The methodological quality of each RCT was assessed using the Cochrane risk-of-bias tool, and where appropriate, meta-analyses were performed. Results: Initially, 819 studies were found, but 773 were excluded after title and abstract screening. After further screening of the resulting 46 papers, eight RCTs were selected that had been published between 2003 and 2016, and the total effective rate (TER) was used as the primary evaluation method for all eight studies. It was found that the TER was significantly higher in treatment groups (83.33-96.7%) than in control groups (56.5-80.0%), and seven studies included in the meta-analysis showed positive results for acupuncture for ETs (Risk Ratio: 1.43, 95% Confidence Interval: 1.21-1.69, P<0.0001) Confidence Interval. Conclusions: This meta-analysis of clinical trials suggests that acupuncture is effective for treating ET patients; however, further studies are needed or larger populations to prove its effectiveness.

The Effects of Acupuncture and Herbal Medicine for the Treatment of Whabyung: A Meta-analysis (화병치료에 대한 침과 한약의 효과에 대한 메타분석)

  • Background and Objective: Whabyung is a Korean cultural disorder characterized by a combination of emotional stress and physical symptoms. Acupuncture, herbal medicines and other complementary medicinal treatments are used to treat the symptoms of whabyung. The purpose of this study is to investigate the effects of acupuncture treatment and herbal medicine treatments on anger, anxiety and depression of whabyung. Methods: Using PubMed, CENTRAL, AMED, MEDLINE, EMBASE, MEDLINE, CINAHL, Cnii, CNKI, J-SATGE, PsycArticles, KISS, KoreaMed, kmbase, NDSL, RISS, OASIS and Korean Traditional Knowledge Portal, articles published by March 2017 for whabyung or whabyung-related syndrome were searched. The quality of selected articles was evaluated using RoB and RoBANS. The effects of acupuncture and herbal medicines on anger, anxiety, and depression levels of whabyung patients were considered for meta-analyses. Results: Of the total 2408 articles retrieved from the database, 11 were finally selected and 4 articles were used for meta-analyses. Acupuncture reduced whabyung patients' anger and anxiety. Especially, the state anxiety was significantly reduced (effect size= -0.676, p= .026). Herbal medicine had effects on reducing anxiety and state anger while trait anger was increased without statistical significance. Conclusion: The present study showed the effectiveness of acupuncture and herbal medicines for whabyung patients in general. However, considering the limited availability of articles and the heterogeneity between them included for the meta-analysis, the results should be generalized with caution. In the future, more studies within the structure of evidence-based medicine need to be conducted to generate objective and valid evidences of korean medicine.

A Study on recent tendency of anti-tumor herbal acupuncture (항암약침의 최신경향에 대한 고찰)

  • Objective: The purpose of this study is to develop and activate anti-tumor herbal acupuncture for cancer patients in South Korea. Methods: We investigated some literatures on anti-tumor herbal acupuncture which is used in South Korea and China, and made diagrams. Results: The results are summarized as follows. Anti-tumor herbal acupuncture is one of the traditional oriental medical method which is effective for cancer patients. In domestic studies, most of herb materials are belong to action of cooling&detoxification(25.0%) and strengthening body resistance(46.4%) which are proved to have effects of anti-tumor, immune activation and preventing tumor. In China, point injection therapy are used for improving symptoms of cancer patients and hea1ing tumor. Also herbal intravenous injection is used for combination of chinese traditional and western cancer therapy and treating cancer patients variously. Conclusions: From the above results. it is expected that anti-tumor herbal acupuncture is useful to improve clinical symptoms and quality of life(QOL) of cancer patients. Also we must develop new progressive methods of point injection and herbal intravenous injection for treating cancer patients, and advance clinical studies and trials.
