A Study on Infection Control Practices among Dental Hygienists (치과위생사의 감염관리 실태에 관한 연구)

  • Background: Dental hygienists are at risk of acquiring infections through exposure to infectious agents. This study explores the frequency of exposure to infectious agents and infection control practices among dental hygienists. Methods: We undertook a cross-sectional survey to examine the exposure to infectious agents and infection control practices among dental hygienists through questionnaire. Data were obtained from 124 dental hygienists attending educational conferences. A multiple logistic regression analysis was conducted to determine the factors that effect infection control practices in relation to age, completion of infection control education, recognition of their own serum hepatitis Ag/Ab status, the number of one day patients, location of clinics, type of clinics, and career years. Result: Of the 124 dental hygienists, 91.7% were exposed to at least one accident such as needle stick injuries during their practices periods. The health screening coverage rate in dental hygienists was 16.5%. The number of one day patients and career years were important in relation to infection control practices among dental hygienists. The adjusted odds ratio estimates for career years were 5.049 times greater(95% Cl 1.238-20.597) for groups with over 4 career years than under 2 career years. That for the number of one day patients were 0.261(95% Cl 0.071-0.955) lower for through 20 up to 30 patients, 0.531(95% Cl 0.102-2.78) lower for through 30 up to 40 patients and 0.498(95% Cl 0.123-2.017) lower for more than 40 patients than under 20 patients. Conclusion: From these results, prevalence of infection control practices among dental hygienists is related to the number of one day patients and career years. Prevention of cross infection and reduction of future transmission should be a priority to dental hygienists for promotion of infection control and further efforts to educate newcomers on infection prevention should be made.

Relationship Survey Study between Diabetic Control Education and Diabetic Retinopathy: data from the Korea National Health and Nutrition Examination Survey V (당뇨병 환자에서 당뇨관리 교육과 당뇨병성 망막병증 발생간 관련성 조사연구: 제5기 국민건강영양조사를 중심으로)

  • Background: Diabetes is accompanied by complications. One of the chronic complications, diabetic retinopathy is the most common cause of the loss of eyesight and thus has enormous impacts on the quality of life to the patients. It has been reported that thorough glucose regulation can prevent or postpone the outset of diabetic retinopathy in diabetic patients and that the patients who received anti-diabetic manage & care education would be capable of more thorough glucose-level regulation than those who did not. Method: This study set out to investigate the current state of education on anti-diabetic manage & care in South Korea and connections between anti-diabetic manage & care education and occurrence of diabetic retinopathy in diabetic patients based on the Korea National Health and Nutrition Examination Survey of 2011. Results: Of the 410 diabetes patients, 74 received anti-diabetic manage & care education, which means that only 15% of diabetic patients benefited from the education in the nation. The occurrence rate of diabetic retinopathy was 28% in the education group and 24% in the non-education group with no significant differences between them. The anti-diabetic manage & care education group recorded a higher occurrence rate of diabetic retinopathy, one of the chronic diabetic complications, than the non-education group contrary to the hypothesis. One of the reasons was that the educated group had a significantly longer duration of diabetes and significantly higher HbA1c than the noneducated group, which indicates that anti-diabetic manage & care education is provided to those who have progressed farther along the course of diabetes instead of the early stage and cannot regulate their glucose-level well in the nation. Conclusion: Those findings raise a need for active educational policies in order to provide anti-diabetic manage & care education under the goals of preventing complications through anti-diabetic education for many patients in early stages of diabetes.

A Clinical Study for Promoting Quality Nusing Care in a University Hospital (질적 간호제공을 위한 간호단위 시범 운영 효과에 관한 임상적 연구)

  • The purpose of this study was to develop a new nursing unit which can meet changing health care needs, enhance patients' satisfaction and nurses' job satisfaction, and finally guarantee quality nursing care with present manpower. For this, one medical unit was selected as a unit for quality care. And one medical unit which is similar in staffing and patients' characteristics was selected as a control unit. To assess present problems and identify the remedies to the problems a hospital-wide survey and a workshop were performed. According to the survey results, educational programs and improvement of the facilities and equipment supply system, managereal support for interdepartmental cooperation and intensification of bed-side nursing care were adopted as main principles for operating model unit, This model unit was operated for 3 months from Sep. 1, 1992 to Nov. 30, 1992. To evaluate the effectiveness of the model unit, derect/indirect nursing care hours, patients' satisfaction to nursing care, nurses' job satisfaction, and quality care index were measured. Direct/indirect nursing care hours were compared with that of the control unit, and patients' and nurses' satisfaction and quality care index were measured before and after operating model unit and compared with each other. The results of the study were as follows; 1. In the model unit mean direct nursing care hours per cach shift was 146.88 minutes and indirect nursing care hours was 354.72 minutes. The ratio of the direct nursing care hour to indirect nursing hour was 29.6 ; 70.4 and that of the control unit was 26.9 : 73.1. Direct nursing care hour in model unit was longer than that of the control unit. But, the difference was not significant. In subcategories of direct nursing care, the time spent in mobility and exercise, conservation of body temperature, hygiene, and communication and health education were longer than that of the con" trol unit. 2. Indirect nursing care hour in model unit was shorter than that of the control unit. But, the difference was not significant. In subcategories of indirect nursing care, the time spent in drug management and ward arrangement was shorter than that of the control unit. 3. Patients' satisfaction to nursing care was increased significantly after operating the model unit (T=-3.48, P=-0.002) and satisfaction to subcategories of physical comfort measure, psychological cate, and unit management components were significantly higher than before. 4. In the model unit, nurses' total job satisfaction was increased significantly after operating the model unit(Z=2.1004, P=.0357) and satisfaction to subcategory of satisfaction to administration was significantly higher than before (Z=-2.0732, P=.0382). 5. After operating the model unit, quality care index was increased from 89 to 93. With this results, it can be summarized that all the measures tried for quality care, such as educational programs, managereal support for interdepartmental cooperation, and improvement of the equipment and facility provision resulted in partial increase in direct nursing care hours, nurses satisfaction to their job and patients' satisfaction to nursing care. In can be postulated that managereal support and motivation without proper staff supplementation is not enough for increasing direct nursing care hours. And for the enhancement of the level in clinical nursing, and staff supplement must be considered sincerely and the measures for reducing indirect nursing care hours, such as computerization of nursing care activities, improvement of facilities and equipment and facilities supply system, must be instituted in addition.

Dynamic changes and characterization of the protein and carbohydrate fractions of native grass grown in Inner Mongolia during ensiling and the aerobic stage

  • Objective: To improve the utility of native grass resources as feed in China, we investigated the dynamics of protein and carbohydrate fractions among Inner Mongolian native grasses, during ensiling and the aerobic stage, using the Cornell Net Carbohydrate and Protein System. Methods: Silages were prepared without or with lactic acid bacteria (LAB) inoculant. We analyzed the protein and carbohydrate fractions and fermentation quality of silages at 0, 5, 15, 20, 30, and 60 d of ensiling, and the stability at 0.5, 2, 5, and 10 d during the aerobic stage. Results: Inner Mongolian native grass contained 10.8% crude protein (CP) and 3.6% water-soluble carbohydrates (WSC) on a dry matter basis. During ensiling, pH and CP and WSC content decreased (p<0.05), whereas lactic acid and ammonia nitrogen (N) content increased (p<0.05). Non-protein N (PA) content increased significantly, whereas rapidly degraded true protein (PB1), intermediately degraded true protein (PB2), total carbohydrate (CHO), sugars (CA), starch (CB1), and degradable cell wall carbohydrate (CB2) content decreased during ensiling (p<0.05). At 30 d of ensiling, control and LAB-treated silages were well preserved and had lower pH (<4.2) and ammonia-N content (<0.4 g/kg of fresh matter [FM]) and higher lactic acid content (>1.0% of FM). During the aerobic stage, CP, extract ether, WSC, lactic acid, acetic acid, PB1, PB2, true protein degraded slowly (PB3), CHO, CA, CB1, and CB2 content decreased significantly in all silages, whereas pH, ammonia-N, PA, and bound true protein (PC) content increased significantly. Conclusion: Control and LAB-treated silages produced similar results in terms of fermentation quality, aerobic stability, and protein and carbohydrate fractions. Inner Mongolian native grass produced good silage, nutrients were preserved during ensiling and protein and carbohydrate losses largely occurred during the aerobic stage.

  • The purpose of this study is to analyze some problems of general high school and provide some promotion ways of general high school in Gyeongsangbuk-do in long and short term perspective, considering necessity for consolidating educational capacity of general high school which is in disadvantage due to high school diversity and special high school promotion policy. For this, first, the study explores educational policy basis of Park Geun-hye's government, educational policy direction of Gyeongsangbuk-do, educational situation of general high school in Gyeongbuk area, and main contents of educational capacity consolidation way of general high school. Main tasks of Gyeongsangbuk-do general high school educational capacity reconsideration based on the educational capacity consolidation way of Ministry of Education are (1) enlargement of autonomy for formation and operation of focused curriculum by guidance, (2) enlargement of guidance and vocational education for general high school student, (3) consolidation of support program for student at risk, and (4) consolidation of support for general high school High-up program. For these main tasks are fixed and operated in school successfully, the fixation of school responsible management system based on geological environment and trait of each general high school, the application of whole quality control in school management, and the formation of cooperative school organizational culture should be constructed. And specific practice tasks and strategies for Gyeongsangbuk-do general high school educational capacity reconsideration (the cultivation of grobal democratic citizen, human being image in 2009 revised curriculum, and 'talented people with new thinking and genuine mind' of Gyeongsangbuk-do) are provided.

Study on Aged Men's Utilization Status and Satisfaction of Removable Dentures (일부 노인들의 가철성 틀니 사용 실태 및 만족도 조사)

  • Purpose: It is to survey old men's utilization status of dental clinic and temporary-binding dentures and then to provide basic data to make a program for manufacturing old men's dentures and their teeth health education. Methods: Random selection was made on old men living in Daejeon and Chungnam, and a survey was conducted from April 15 to May 20, 2011. 500 questionnaires were distributed and 420 questionnaires were collected. Using spss 18.0 program, final analysis was made on 363 questionnaires except for 57 questionnaires which had missing value or showed some doubts about the unreliability. Results: 1. With respect to number of natural teeth as per the age, the upper/lower jaws had meaningful differences (P<.001), which means that the older the person is, the smaller number of natural teeth is. In the meantime, there was no meaningful difference according to sex. 2. With respect to kind of prosthetics installed in a mouth as per the age, there was meaningful difference in the upper jaws (P<.05) but there was no meaningful difference in the lower jaws. It was found out that the older the persons are, the more they use removable dentures while the younger the persons are, the more they use fixed partial dentures. 3. With respect to mouth health condition recognized by oneself, it showed bad (44.4%) and good (10.3%),which means recognition as bad is greater, and the age when prosthetics were first used is dispersed variously from below 40 to over 70. 4. Dental care institutions which they mainly used are hospital and clinics (91.5%), and 66.4% of them visited dental clinics lately. Preferential treatment if they go to dentists is as follows in the order of prosthetics (33.9%), gum treatment (24.2%) and pain treatment (14.9%). 5. 68.4% of them revisited dentists after installation of dentures and the reason for revisit is as follows in the order of poorly fitted dentures (35.1%), painful gum 25.9%, regular check-up 25.4% and difficult chewing 9.6%. Conclusion: As our country enters into an aging society, mouth health problem of old men has come to a serious issue, and therefore, a survey was conducted on some old men with respect to their utilization status of dental clinic and removable dentures in order to provide basic data for improvement of quality of their life, maintenance and enhancement of their mouth health. The study result shows that the older they are, the higher the loss rate of teeth as well as needs for prosthetics are, and thus it is thought the study will help to develop mouth health education program which may improve quality of old men's life by recovery of mouth functions declined due to loss of teeth and by regular check-up, education for mouth health control and following-up control of dentures.

Affection Factors of Health-Related Quality of Life in Married Women by Job (직업에 따른 기혼여성의 건강관련 삶의 질 영향요인)

  • This study was conducted to identify the factors affecting the health-related quality of life(HRQOL) in married women according to their occupations. The study subjects were 2,555 married women in the 7th National Health and Nutrition Examination Survey, and the SPSS 25.0 program was used for data analysis. The results are as follows. In the case of office workers, the age, household income, obesity etc., were significant factors, and the explanatory power was 8.9% (p<.001). In the case of service workers, the age, education level, weight control, subjective health, and stress were significant factors, and the explanatory power was 16.1% (p<.001). In the case of simple labor, the age, education level, number of household members, hypertension, obesity, weight change, weight control, sitting time, subjective health, subjective body image, and stress were significant factors, and the explanatory power was 24.7(p<.001). In the case of unemployed, the age, household income, number of household members, number of days taking a walk per week, sleeping time, sitting time, subjective health, subjective body image, and stress were significant factors, and the explanatory power was 29.9% (p<.001). In the future, when developing a program to improve the HRQOL, the individual management programs considering the factors affecting the HRQOL according to occupation should be applied.

Effects of the Brain waves according to participation in Therapeutic recreation programs on the Depression, Sleep Disturbance and Quality of Life in the Elderly with Dementia (치료레크리에이션 프로그램에 따른 치매노인의 뇌파 변화가 우울감 및 수면장애와 삶의 질에 미치는 영향)

  • The purpose of this study was to identify the effects of brain wave change through therapeutic recreation programs on depression, sleep disturbance and quality of life among elderly with dementia. The subjects of this quasi-experimental study consisted of two groups, one experimental group (N=14) and one control group (N=18), after excluding 8 participants from a total of 40 participants. The subjects of experimental group were randomly selected from the elderly (order than 65 years old) of senior care center in Daejoen and participated in 3-month therapeutic recreation program. On the other hand, the subjects of control group did not participated in any therapeutic recreation program. Each group's pre-post brain wave change, depression, sleep disturbance and quality of life were estimated. Through ANCOVA and Analysis of Structural Equation Modeling with SPSS window 17.0 and AMOS 7.0, this study found followings. Frist, the therapeutic recreation program group indicated significant improvement of brain waves, sleep disturbance and quality of Life. In addition, depression was significantly reduced in the therapeutic recreation program group. Second, significant causal relationships was found among brain waves, depression, sleep disturbance, and Quality of Life.

A Study on The Performance Verification and Economic Evaluation of ESS for Frequency Regulation Application (주파수조정용 ESS의 성능검증 및 경제성평가 알고리즘에 관한 연구)

  • Recently, the installation of energy storage systems (ESSs) has increased in parallel with the extension of renewable energy resources. However, there has been no concrete analysis ofthe performance verification and economic evaluation of ESSs,which makes it difficult to perform aneffective installation and operation of an ESS. In particular, there are no international technical standards and guidelines on electric ESS for frequency regulation applications. Therefore, acomprehensive study on the power quality, impact on grid, extent of contribution, and cost benefit study of ESS are strongly being required. Under these backgrounds, this paper proposes a performance verification algorithm on ESS for frequency regulation application based on ananalysis of the AGC(Automatic Generation Control) performance verification method of PJM in USA. In addition,this paper proposes an economic evaluation algorithm on a 500 MW installation of ESS for frequency regulation applications using the account settlement of an expensive gas-fired generation plant and coal-fired power generation plant. From the simulation with real ESS operation data and 500 MW installation case, it wasconfirmed that the ESS showssuperior performance toany other conventional generators and provides anannual benefit of 500 MW ESS are between 345~429 billon won.

The Effect of Franchisor's On-going Support Services on Franchisee's Relationship Quality and Business Performance in the Foodservice Industry (외식 프랜차이즈 가맹본부의 사후 지원서비스가 가맹점의 관계품질과 경영성과에 미치는 영향)

  • Introduction The purpose of this research is to develop overall model which involves the effect of ongoing support services by franchisor on franchisee's relationship quality(trust, satisfaction, and commitment) and business performance(financial and non-financial performance), and to investigate the relationships among trust, satisfaction, commitment, financial and non-financial performance. This study also suggests franchise business or franchise system should be based on long-term orientation between franchisor and franchisee rather than short-term orientation, or transactional relationship, and proposes the most effective way of providing on-going support services by franchisor with franchisee thru symbiotic relationship among franchisor and franchisee Research Model and Hypothesis The research model as Figure 1 shows the variables on-going support services which affect the relationship quality between franchisor and franchisee such as trust, satisfaction, and commitment, and also analyze the effects of relationship quality on business performance including financial and non-financial performance We established 12 hypotheses to test as follows; Relationship between on-going support services and trust H1: On-going support services factors (product category & price, logistics service, promotion, information providing & problem solving capability, supervisor's support, and education & training support) have positive effect on franchisee's trust. Relationship between on-going support services and satisfaction H2: On-going support services factors (product category & price, logistics service, promotion, information providing & problem solving capability, supervisor's support, and education & training support) have positive effect on franchisee's satisfaction. Relationship between on-going support services and commitment H3: On-going support services factors (product category & price, logistics service, promotion, information providing & problem solving capability, supervisor's support, and education & training support) have positive effect on franchisee's commitment. Relationship among relationship quality: trust, satisfaction, and commitment H4: Franchisee's trust has positive effect on franchisee's satisfaction. H5: Franchisee's trust has positive effect on franchisee's commitment. H6: Franchisee's satisfaction has positive effect on franchisee's commitment. Relationship between relationship quality and business performance H7: Franchisee's trust has positive effect on franchisee's financial performance. H8: Franchisee's trust has positive effect on franchisee's non-financial performance. H9: Franchisee's satisfaction has positive effect on franchisee's financial performance. Method The on-going support services were defined as an organized system of continuous supporting services by franchisor for the purpose of satisfying the expectation of franchisee based on long-term orientation and classified into six constructs such as product category & price, logistics service, promotion, providing information & problem solving capability, supervisor's support, and education & training support. The six constructs were measured agreement using a 7-point Likert-type scale (1 = strongly disagree to 7 = strongly agree)as follows. The product category & price was measured by four items: menu variety, price of food material provided by franchisor, and support for developing new menu. The logistics service was measured by six items: distribution system of franchisor, return policy for provided food materials, timeliness, inventory control level of franchisor, accuracy of order, and flexibility of emergency order. The promotion was measured by five items: differentiated promotion activities, brand image of franchisor, promotion effect such as customer increase, long-term plan of promotion, and micro-marketing concept in promotion. The providing information & problem solving capability was measured by information providing of new products, information of competitors, information of cost reduction, and efforts for solving problems in franchisee's operations. The supervisor's support was measured by supervisor operations, frequency of visiting franchisee, support by data analysis, processing the suggestions by franchisee, diagnosis and solutions for the franchisee's operations, and support for increasing sales in franchisee. Finally, the of education & training support was measured by recipe training by specialist, service training for store people, systemized training program, and tax & human resources support services. Analysis and results The data were analyzed using Amos. Figure 2 and Table 1 present the result of the structural equation model. Implications The results of this research are as follows: Firstly, the factors of product category, information providing and problem solving capacity influence only franchisee's satisfaction and commitment. Secondly, logistic services and supervising factors influence only trust and satisfaction. Thirdly, continuing education and training factors influence only franchisee's trust and commitment. Fourthly, sales promotion factor influences all the relationship quality representing trust, satisfaction, and commitment. Fifthly, regarding relationship among relationship quality, trust positively influences satisfaction, however, does not directly influence commitment, but satisfaction positively affects commitment. Therefore, satisfaction plays a mediating role between trust and commitment. Sixthly, trust positively influence only financial performance, and satisfaction and commitment influence positively both financial and non-financial performance.
