부자이중탕(附子理中湯)의 효능(效能)에 관(關)한 실험적(實驗的) 연구(硏究) (Studies on the effects of Bujaleejungtang)

  • In order to investigate the effect of Bujaleejungtang, by means of oral medication to rats and mice, to isolated intestine and stomach, and the effect to pyloric ulcer, indomethacin-induced ulcer, secretion of gastric juice, and to transport ability of intestine content were studied as the action to G-I tract. The effect to normal rats and resperpine-treated rats were studied as the action to thermo-regulation. The results were as follows: 1. Bujaleejungtang showed the inhibitory effect on the smooth muscle contraction induced by acethylcholine chloride and barium chloride in the isolated mice ileum. 2. Bujaleejungtang showed inhibitory effect on the contraction induced by acetylcholine chloride and barium chloride in the rat fundus-strip. 3. Inhibitory effect of Bujaleejungtang on pyloric ulcer, indomethacin-induced gastric ulcer in rats was statistical recognized(p<0.05). 4. No inhibitory effect of Bujaleejungtang on gastric juice secretion in Shay rats was recognized. 5. Inhibitory effect of Bujaleejungtang on gastric free acidity and total acidity in Shay rats was recognized only when Bujaleejungtang was medicated in high thickness(2000mg/kg) (p<0.001). 6. Inhibitory effect of Bujaleejungtang on pepsin output in Shay rats was recognized only when Bujaleejungtang was medicated in high thickness(2000mg/kg)(p<0.001). 7. Inhibitory effect of Bujaleejungtang on barium sulfate transport in the small intestine of mice was recognized only when Bujaleejungtang was medicated in high thickness(2000mg/kg)(p<0.05). 8. Inhibitory effect of Bujaleejungtang on barium sulfate transport in the large intestine of mice was recognized(p<0.05). 9. Inhibitory effect of Bujaleejungtang on rectal temperature in normal rats was recognized. 10. Inhibitory effect of Bujaleejungtang on rectal temperature in reserpine-treated rats was recognized only when Bujaleejungtang was medicated in high thickness(2000mg/kg)(p<0.05).

한국 재래산양의 위장관에 출현하는 내분비세포의 면역조직화학적 연구 (An immunohistochemical study of the endocrine cells in gastrointestinal tract of the Korean native goat)

  • 한국 재래산양의 위장관 내분비세포를 면역조직화학적으로 관찰하였던 바 5-HT, somatostatin, Gas/CCk, glucagon, chromogranin, PP 면역 반응세포들이 동정되었다. 한국 재래산양의 위장관에 있어서 이들 면역반응세포의 부위별 분포와 출현빈도의 특징은 다음과 같다. 5-HT 면역반응세포는 제 4 위 및 유문부에서 보다 대장과 소장에서 더 많이 출현하였다. Somatostatin면역반응세포는 대장과 소장에서 보다 제 4 위와 유문부에서 다수 출현하였다. Gas/CCk 면역반응세포는 유문부에서 가장 다수로 분포하였으며 기타 부위에서는 소수로 출현하였다. 대장과 소장에서는 중등도의 glucagon 면역반응세포가 관찰되었으나, 제 4위와 유문부에서는 소수로 관찰되었다. Chromogranin 면역반응세포는 전 위장관에서 골고루 그리고 아주 많이 출현하였다. PP면역반응세포는 대장에서 중등도로, 회장에서 소수로 분포하였다. Insulin 면역반응세포는 전 위장관에서 관찰할 수 없었다.

  • 한국산 고슴도치 유문부에 있어서 내분비세포를 전자현미경적으로 관찰하여 EC, ECL, D1 및 G형 등의 4종류의 세포를 분류하였다. EC세포는 고전자밀도의 다형태성 과립을 가지며, 과립내에는 더 높은 전자밀도의 dense body가 보였다. 과립의 크기는 $160{\sim}530nm$였다. ECL세포는 원형 또는 난원형의 과립을 가지며, 이들 과립내용물의 전자밀도는 높고, 편재하거나 공포상으로 출현하였다. 과립의 크기는 $200{\sim}560nm$였다. D1세포는 원형의 과립을 가지며, 과립의 전자밀도는 낮고, 때로 한계막과 과립내용물 사이에 좁은 halo를 나타내었다. 과립의 크기는 $130{\sim}400nm$였다. G세포는 원형 또는 난원형의 과립을 가지며, 비교적 낮은 전자밀도의 과립내용물과 한계막 사이에 좁은 halo를 보였다. 과립의 크기는 $140{\sim}370nm$였다.

Pylorus-Preserving Gastrectomy for Gastric Cancer

  • Pylorus-preserving gastrectomy (PPG) is a function-preserving surgery for the treatment of early gastric cancer (EGC), aiming to decrease the complication rate and improve postoperative quality of life. According to the Japanese gastric cancer treatment guidelines, PPG can be performed for cT1N0M0 gastric cancer located in the middle-third of the stomach, at least 4.0 cm away from the pylorus. Although the length of the antral cuff gradually increased, from 1.5 cm during the initial use of the procedure to 3.0 cm currently, its optimal length still remains unclear. Standard procedures for the preservation of pyloric function, infra-pyloric vessels, and hepatic branch of the vagus nerve, make PPG technically more difficult and raise concerns about incomplete lymph node dissection. The short- and long-term oncological and survival outcomes of PPG were comparable to those for distal gastrectomy, but with several advantages such as a lower incidence of dumping syndrome, bile reflux, and gallstone formation, and improved nutritional status. Gastric stasis, a typical complication of PPG, can be effectively treated by balloon dilatation and stent insertion. Robot-assisted pylorus-preserving gastrectomy is feasible for EGC in the middle-third of the stomach in terms of the short-term clinical outcome. However, any benefits over laparoscopy-assisted PPG (LAPPG) from the patient's perspective have not yet been proven. An ongoing Korean multicenter randomized controlled trial (KLASS-04), which compares LAPPG and laparoscopy-assisted distal gastrectomy for EGC in the middle-third of the stomach, may provide more clear evidence about the advantages and oncologic safety of PPG.

한국산 두더지의 위장췌내분비세포에 관한 면역조직학적 연구 (An Immunohistochemical study of the gastro-entero-pancreatic endocrine cells of the insectivorous Korean mole, Talpa micrura coreana)

  • 한국산 두더지의 위장관 점막과 췌장에 분포하는 내분비세포들의 분포와 출현빈도 및 종류를 알아보기 위하여 면역조직화학적으로 관찰하였던 바, 5-HT 면역반응세포는 극소수내지 중등도의 빈도로 전 장관에 고루 분포하였다. Glucagon 면역반응세포는 대장에 국한하여 출현하였다. 또 bovine CG, BPP 및 somatostatin 면역반응세포들은 각각 십이지장, 공장 그리고 위저부를 제외한 전 장관에 다양한 빈도로 고루 분포하였다. Gas/CCK 면역반응세포는 유문부와 소장에서만 다수 그리고 극소수로 관찰되었으나, insulin 면역반응세포는 전 장관에서 관찰할 수 없었다. 한편 췌도의 주변부에서는 glucagon, somatostatin과 BPP 면역반응세포들이, 중심부에서는 insulin 면역반응세포가 높은 빈도로 동정되었으며 또한 외분비에서는 이들 면역반응세포들이 단독 또는 소수의 집단으로 출현하였다. 이상의 결과로써 두더지의 소화관내분비세포의 분포패턴는 고슴도치의 그것과 유사하였으나 십이지장에서 세포의 종류와 출현빈도가 낮은 것을 알 수 있었다.

  • Morphology, histology, and histochemical characteristics of the digestive tract of the guirine goby (Rhinogobius giurinus: Gobiidae), collected from the coast of Jeju Island, Korea, were investigated. The digestive tract of R. giurinus, which is a brackish water species, has a short (relative length of gut=0.42), simple, and narrow gut. The gastric glands are well developed in the stomach, but pyloric caeca are absent. The mucosal folds are regularly branched, and the muscularis extern a is thickest in the esophagus, which also contained the most mucus-secreting goblet cells (P<0.05). In R. giurinus, digestive function occurs in the anterior portion of the digestive tract, where mucus protects the tract from the actions of digestive enzymes and functions to activate digestion.

소아의 구토에 대한 방사선학적 진단 (Radiological Diagnosis of vomitting in infant and children)

  • Vomiting in pediatric patient is frequently encountered problem in emergency room or outpatient clinic. In differential diagnosis, age of the patient or accompanying symptoms should be considered in the differential diagnosis. Accurate diagnosis is very important because surgical treatment is necessary in some of the conditions. Imaging diagnosis of conservative modality such as upper gastrointestinal series or colon study is still important radiological examination in the initial differential diagnosis, but recently ultrasonography offers accurate diagnosis in many situations. The cause of vomiting in pediatric are diverse according to the age group :neonatal sepsis, necrotizing enterocolitis, or hypertrophic pyloric stenosis in neonates : gastroesophageal reflux, viral enteritis, or intussusception in infant: midgut volvulus, appendicitis, metabolic disorders, or increased intracranial pressure also an be the cause. knowledge of radiological findings of normal gastrointestinal tract is important to recognize abnormalities. A discussion of radiological findings in variable surgical conditions to present as vomiting in pediatric patients is offered.

  • Collagenases are generally defined as enzymes capable of degrading the polypeptide backbone of native collagen under conditions which do not denature the protein. Two types of proteases with collagenolytic activity have been reported and thought to play different physiological functions. Metallo-collagenases, firstly discovered in tadpole tissue explants are zinc-containing enzymes requiring calcium for optimum activity and stability, and These enzymes have been widely studied from various mammalian tissues as well as from bacteria, such as Bacillus cereus, Clostridium histolyticum, Achromobacter, Vibrio alginolyticus and Clostridium perfringens and snake venoms. 위점막하 혈관종증에 의한 위-십이지장형 장중첩증 (Gastroduodenal Intussusception due to Gastric Submucosal Hemangiomatosis)

  • Gastroduodenal intussusception, an invagination of a part of the gastric wall through the pyloric canal into the duodenum is a rare condition. Gastroduodenal intussusception is caused by a mobile usually benign gastric tumor. However, gastroduodenal intussusception by gastric submucosal hemangiomatosis is not documented. We have managed a case of gastric submucosal tumor leading to gastroduodenal intussusception in 2 years and 10 months old boy. The tumor was $10{\times}5{\times}3cm$ in size in posterior wall of gastric antrum. Laparotomy, manual reduction of the intussusception, and wedge resection of posterior gastric wall including the tumor were performed. Pathologic diagnosis was a submucosal hemangiomatosis.
