The Surgical management of Ebstein's anomaly: A Report of 4 cases of tricuspid valve replacement and plication of the atrialized right ventricle (Ebstein 심기형의 개심술 4례 (삼첨판막 대치이식술 및 우심실 Plication))

  • Ebstein`s anomaly had been amenable to reparative surgery since 1962. However, neither the role of surgery in Ebstein`s anomaly nor the surgical procedure of choice for its correction are clearly defined. Whether or not the atrialized right ventricle, which plays a major role in the functional abnormalities, should be obliterated in all cases remains unsolved. Four cases of Ebstein`s anomaly treated surgically at Seoul National University Hospital were reported. All had closure of the atrial septal defect, obliteration of the atrialized right ventricle by plication, and insertion of a tricuspid bioprosthesis and an epicardial ventricular pacemaker. One patient had a pulmonic valvotomy due to stenotic bicuspid pulmonic valve also. All but one had discharged with a good result.

Peritoneopericardial diaphragmatic hernia in a immature dog (미성숙견에서 선천성 Peritoneopericardial diaphragmatic hernia 발생례)

  • A 5-month-old, 5kg, intact male mixed dog was admitted to the Veterinary Medical Teaching Hospital of the College of Veterinary Medicine Cornell University because of severe systolic murmur. On physical examination, the dog appeared to be thin and slightly small for his age. Radiographic studies with barium sulfate clearly showed the presence of loops of intestines in the pericardium. Based on these findings peritoneopericardial diaphragmatic hernia was diagnosed. Many abnormalities were found in dog's heart : ventricular septal defect, pulmonic stenosis with moderate pulmonic valve insufficiency, and mild mitral and tricuspid valve insufficiency. Puppy's longterm prognosis was fair. His intestines were removed from his pericardium and the defect in his diaphragm was closed. The surgery went smoothly and puppy recovered very nicely from anesthesia.

Mid term experience with CarboMedics Medical Valve (CarboMedics 기계판막의 임상경험)

  • The CarboMedics valve is a bileaflet prosthesis with excellent hemodynamic characteristics, but the long term surgical experience with this valve, its durability and its biocompatibility are unknown. During a 5 year period from october 1988 to July 1993, 748 prostheses [402 mitral, 261 aortic, 58 tricuspid, 27 pulmonic] were inserted in 552 patients [mean age 40.2 years]. The operative mortality was 6.6% [37/560, 13.2% in age group below 15 years and 5.7% above 15 years]. and the main causes of death were complex congenital malformation and left ventricular failure. Follow up was totaled 1182 patient- years and mean follow up was 28.3 months/patient. No structural failure has been observed. Hemorrhage was the most frequent valve related complication[1.78% / Patient-year]. Embolism occurred at a rate of 0.93% / Patient-year. There were 5 cases of valve thrombosis [0.42% / Patient-year, two fatal]. There occurred 11 late deaths[6 valve related] and 42 valve related complications. Actuarial survival at 5 years is 97.18 0.94% and actuarial complication free survival at 5 years is 89.07 1.54%. In summary, the CarboMedics valve stands for a durable valve substitute, with low valve related complications.

  • Despite low mortality and excellent long-term results after repair of Tetralogy of Fallot, certain associated anomaly such as single pulmonary artery continues to be told to have a high operative mortality and morbidity, and there is still some debate on appropriate surgical intervention. During the 4 year period from 1981 to 1984, surgical repair was performed on 5 patients with tetralogy of Fallot and congenital [4 cases] or acquired [1 case] absence of left pulmonary artery. Previous left pneumonectomy had been performed in the patient with acquired absence of a pulmonary artery. Transannular patch or RVOT patch alone with or without pulmonic valvotomy was used with some modifications to reduce pulmonic insufficiency in individual patient. In contrast with previous reports, all patients survived operation and have exhibited marked symptomatic improvement without pulmonic valve insertion nor valved conduit.

Growth of Pulmonary Autograft in Swine, I.Feasibility of the Operation (신생돈을 이용한 이식 자가 폐동맥의 성장에 관한 연구, 제1보, 수술의 적합성)

  • In order to test the hypothesis that the pulmonic valve, when used to replace the aortic root as a pulmonary autograft, will remain a viable anatomical structure and will grow and develop normally along with the host, we performed aortic valve replacement with the pulmonary autograft in 15 neonatal piglets. The weight of the donor was 9.3 $\pm$ 0.2 kg, the recipient 9.6 $\pm$ 0.3 kg. Measured diameters of pulmonic annulus were 14 $\pm$ 0.2 mm for autograft and 14.2 $\pm$ 0.2 mm for pulmonary artery homograft. Operation was performed under cardiopulmonary bypass with deep hypothermia [20oC at low flow perfusion [70 ml/kg/min . The mean operation time was 227 $\pm$ 10 min., bypass time 152$\pm$ 7.6 min. and aortic cross clamp time 73$\pm$ 4.6 min.. 9 piglets survived more than 12 hours. One survived 12 days and died of pneumonia and the latest one survived in good condition and sacrificed at postoperative 6th week for cardiac catheterization and pathologic examination that revealed the viability and growing of the pulmonary autograft. Currently we are able to complete the operation with good preservation of cardiac function, and our postoperative care has evolved to the extent that we are now confident enough of having an acceptable percentage of long term survivors to undertake a definite study in this regard.

  • The use of aortic valve homograft has been developed since 1962 when Ross and Barratt - Boyes independently replaced a diseased aortic valve with an orthotopically inserted homograft valve. And also surgical treatment of complex congenital cardiac malformations utilizing homograft extracardiac conduit has been tried with better result than any other prosthetic material. The present study was undertaken to clarify the safety tissue viability, sterility, after following our protocol of procurement of heart, dissection of aortic and pulmonic homograft, sterilization, cryopreservation, thawing and dilution, and transplantation on experimental animal, sheep. Tissue viability of valve and great artery was assessed by tissue culture. Sterility was evaluated by bacterial and fungal culture. The method used was proven no deleterious effect on the integrity of the valve. Tissue culture of valve tissue before, and after cryopreservation process resulted that active fibroblast growth was observed from homograft sterilized with antibiotics. And culture of the transplanted homograft from sacrificed animal showed active fibroblast growth. Pathologic examination of implanted valve tissue from sacrificed sheep showed mild calcification and minor change, but there were moderate and severe calcification of wall of great arteries.

  • Tricuspid regurgitation due to rupture of a chorda is a rare disease in newborns. Recently, we experienced one day old male with tricuspid regurgitation due to rupture of a chorda of anterior papillary muscle, and who had suffered from severe hypoxemia, acidosis, cyanosis, and bradycardia. Preoperative diagnosis was pulmonary atresia with intact ventricular s ptum, massive tricuspid regurgitation, and patent ductus arteriosus by echocardiogram, which demonstrated no flow through the pulmonic valve. At operation, the pulmonic valve was intact and a chorda of anterior papillary muscle was ruptured. Tricuspid regurgitation was corrected successfully with reconstruction of the chords. Postoperative course was complicated by pneumonia and sepsis, but the infant recovered and discharged at postoperative 20 days.

Ross Operation with Aortic Ring Annuloplasty (대동맥판륜 성형술을 동반한 Ross 술식)

  • Pulmonic autograft replacement of the aortic valve(Ross procedure) has been to have potential for growth in children, no complication of antiocagulation, and enhanced durability. Therefore, Ross operation is indicated in the young, in patients with an active life style, and when anticoagulation is contraindicated. However, late autograft valve has occurrd more frequently in patients with significant size discrepancy between the pulmonay valve annulus and the aortic valve annulus. In order to resolve this problem, we performed aortic annuloplasty with Duran ring for more accurate and strong reinforcement. We report a case using the Duran ring as a method of aortic annuloplasty during Ross procedure.

Successful Surgical Correction of Tricuspid Atresia with Complete Transposition of Great Arteries [S.D.D.]. (Modified Fontan 씨 수술법을 이용한 삼첨판막 폐쇄증과 완전대혈관전위증)

  • A 18-year-old female underwent surgical correction of tricuspid atresia and complete transposition of great arteries combined with atrial 8 ventricular septal defect and pulmonary stenosis. After the transection of main pulmonary artery just above the pulmonic valve, proximal portion of main pulmonary artery was closed with running suture and distal portion of main pulmonary artery anastomosed with right atrial appendage without valve insertion. Atrial septal defect was closed with running suture. Postoperative course was uneventful and she discharged on 18th postoperative day. Her condition is in very good until present. Modified Fontan`s operation without valve placement [in the condition of low pulmonary vascular resistance and good left ventricular function] may has a good result.

Viability Assay after $4^{\circ}C$ Cold Preservation & Cryopreservation of Aortic & Pulmonic Allograft Valves in Rabbits (토끼의 대동맥 및 폐동맥 판막 동종이식편의 냉장 및 냉동 보존후 생육성 평가(I))

  • Cardiac valve allografts have been used as replacements for diseased valves and right ventricular outflow tract reconstruction, the long term follow-up of which has been reported satisfactory. For a good long-term result, it is essential that the allograft be viable at implantation. In this study, we aimed at preparing the cardiac valve allografts aseptically, preserving them at cold- and cryo-conditions, and testing the viability of the allografts after preservation by four methods. We tested the viability of the cardiac valve allografts preserved in cold refrigerated state[4$^{\circ}$C in nutrient media & in liquid nitrogen tank[cryopreservation under -149$^{\circ}$C for pre-planned time periods. The testing methods were 1 glucose utility test 2 tissue culture 3 thymidine uptake test and 4 histologic evidence by light microscopy. We observed no differences in the viability between cold- & cryo-groups and similar results among the methods for testing the viability. In conclusion, there was no difference in the viability between cold- and cryopreserved-allografts at least for 14 days of preservation. And glucose utility test and thymidine uptake test were satisfactory in the evaluation of the allograft viability, since they were easy and rapid with relatively quantitative results.
