  • Tetralogy of Fallot with absent pulmonic valve is a rare cardiac malformation. The most significant symptoms during early infancy are secondary to bronchial compression resulting from the dilated pulmonary arteries. The clinical diagnosis was confirmed by echocardiography, cardiac catheterization and cardioangiography. A case of TOF with absent pulmonic valve was successful operated upon without insertion of the pulmonic valve. The narrow pulmonic valve annulus was enlarged with a transannular pericardial patch graft. The postoperative course was uneventful.

  • A 17-year-old male patient was referred with symptoms of dyspnea. Multi-detector computerized tomography (MDCT) and echocardiography evaluation revealed quadricuspid aortic and pulmonary valves, an atrial septal defect (ASD), and pulmonary stenosis. We closed the ASD using a bovine patch and performed a commissurotomy of the pulmonary valve. Quadricuspid semilunar valves are very rare congenital abnormalities that are reported to occur nine times more frequently in the pulmonic valve than in the aortic valve. According to the Hurwitz Roberts classification, the aortic valve was type A, and the pulmonic valve was type B. The aortic valve normal function, but the pulmonic valve was stenotic and had abnormal function.

  • A 17-month-old intact male Cocker Spaniel was presented with primary complaints of severe ascites, exercise intolerance, and diarrhea. Diagnostic studies revealed tricuspid and pulmonic regurgitation on phonocardiogram, right ventricular enlargement on the electrocardiogram, typical right cardiac enlargement signs on the thoracic radiography and tricuspid valve malformation and marked enlargement of the right atrium and right ventricle on the echocardiography and tricuspid and pulmonary regurgitation on the color spectral echocardiography, suggesting tricuspid valve dysplasia and pulmonary hypertension. Using angiography and cardiac catheterization, pulmonary hypertension was ruled out. Further echocardiographic study revealed membranous valvular structures cranial to pulmonary annulus causing pulmonary regurgitation. Based on these findings on the diagnostic investigation, the case was diagnosed as tricuspid valve dysplasia complicated with pulmonic regurgitation. The dog was medically managed with furosemide, enalapril, nitroglycerine transdermal patch and pimobendan after the ascitic fluid removal.

  • A 3-year-old, intact female, Shih-tzu dog was presented with a 15-day history of vomiting, depression, and anorexia. On physical examination, systolic ejection murmurs with precordial thril atthe left heart base were detected. A diagnosis of congenital pulmonic stenosis (PS) was made mainlyfrom the thoracic radiography, electrocardiography, and echocardiography. On complete blood counts andconfirmed that main pulmonary artery was tremendously buldged and electrocardiography was suggestiveof severe right ventricular hypertrophy. Echocardiographic findings revealed the pulmonic valve stenosiscontaining valvular dysplasia and poststenotic dilation. On Doppler echocardiography, ejection velocityof the lesion accounted for 3.38 m/sec, meaning mild velocity through the stenotic area. The dog'sproblem and resulted in death. However, there has been no reliable relation between PS and CRF. Primarymalformation of pulmonic valve was confirmed at necropsy after death.

  • Bacterial endocarditis has been well recognized as an important complication of congenital heart disease, such as ventricular septal defect, patent ductus arteriosus or pulmonary stenosis. The incidence of right sided bacterial endocarditis is lesser than left sided bacterial endocarditis. Also, pulmonic valve vegetation has been thought to be relatively uncommon. And pulmonary embolism is common in the patients with right sided bacterial endocarditis. So in a patient with fever and evidence of recurrent pulmonary infarction, changing heart murmurs and scattered pneumonic infiltrates, one should direct attention to the heart as a possible source of the infection. Echocardiography with M-mode, 2-D and Doppler mode represents the only noninvasive technic available for detecting vegetations in bacterial endocarditis. In fact, the technic is more sensitive in identifying these lesions than angiography. We experienced a case of ventricular septal defect with bacterial endocarditis, pulmonic valve vegetation and multiple pulmonary embolism diagnosed with Echocardiogram and lung scan, and confirmed by operation. Patch repair of ventricular septal defect, resection of pulmonic valve and vegetation and artificial valve formation with pericardium were done.

  • A 6-month-old intact male Maltese dog (2.5 kg of body weight) was presented with the primary complaint of heart murmur and exercise intolerance. Diagnostic studies revealed severe pulmonic stenosis. Since the pressure gradient in stenotic pulmonic valve area was higher than 200 mmHg (maximum flow velocity = 7.1 m/s), the dog was underwent balloon valvuloplasty. With this procedure, the peak pulmonic velocity was reduced to 3.0 m/s (PG = 36 mmHg). Periodic re-evaluations performed in monthly interval revealed no further deterioration even after 4 month of procedure. This is the first case report of pulmonic stenosis fixed by non-surgical balloon valvuloplasty in Korea.

  • A 6-month-old mature intact female Siamese cat presented with exertional dyspnea. Diagnostic studies revealed pleural effusion, grade 4/6 left basal systolic murmur, deep S-wave in electrocardiograph leads I, II, and III, cardiomegaly with pleural effusion on radiography, pulmonic systolic (~5.8 m/sec) and tricuspid (3.6 m/sec) regurgitant jets, atrial septal defect, and a hypoplastic right outflow tract. Based on these results, the case was diagnosed as pulmonic stenosis with atrial septal defect. To the best of our knowledge, this is the first case report describing pulmonic stenosis with atrial septal defect in a cat in Korea.

  • A 1.3 year-old Castrated male Bedlington terrier (10.0 kg of body weight) was presented with heart murmur and occasional coughing. Diagnostic imaging studies revealed supravalvular pulmonic stenosis (maximal velocity 2.91 m/s) from abnormal membranous structure (aperture) distal to the pulmonary valve in the main pulmonary artery. Further study also revealed pulmonic regurgitant (1.82 m/s of peak velocity, 13.2 mmHg of pressure gradient) jets started from the abnormal membranous structure. Since the dog had no obvious clinical signs related to heart failure, no interventional therapy including balloon dilation was considered in this case. Instead, medical treatment for preventing further deterioration of clinical signs related to PS was done with enalapril. This case report described a rare case of type III supravalvular PS in a dog, which has never been reported. Further deterioration of clinical signs has yet been recognized after medical treatment.

  • Bacterial endocarditis has been well recognized as an important complication of congenital heart disease. The most common. form of congenital heart disease is the VSD, of which natural history is spontaneous closure, pulmonary vascular disease, symptoms, and endocarditis. The incidence of endocarditis is relatively low. But endocarditis is almost universally fatal if untreated. Two cases of VSD with endocarditis, 4 \ulcorneryear male and 17 \ulcorneryear female, were treated at Department of Thoracic and Cardiovascular Surgery, Chonbuk National University. In the First case, the VSD was perimembranous type and vegetation located on the septal leaflet of the tricuspid valve. After 7 week medical treatment, simple closure of the VSD, removal of vegetation, and tricuspid annuloplasty were performed. In the second case. the VSD was subpulmonic type and the pulmonic valve was destructed due to vegetation. So the VSD was closed with interrupted 4 \ulcorner0 Prolene sutures and the pulmonic valve was excised. Postoperative course of all cases was uneventful.
