Bilateral Pulmonary Sequestrations - A Case Report - (양측성 폐 격리증;1례보고)

  • Bilateral intralobar and extralobar pulmonary sequestration is an extremely rare anomaly. At present, four pathologically proven cases and two possible cases have been reported in the literature insofar as we know. We have found no previous reports in the literature in which simultaneous bilateral intralobar and extralobar pulmonary sequestration were present. We report a first case presenting as bilateral intralobar and extralobar pulmonary sequestration, recently performed staged bilateral resection, and emphasize the importance of computed tomography and angiography in diagnosis and operation.

  • This report describes a month-old female infant with a rare supracardiac type of total anomalous pulmonary venous connection which have intrapulmonary drainage and small left atrium. The left pulmonary vein drained into right hilum via transverse common pulmonary vein, and then both pulmonary veins drained into superior vena cava via ascending connecting vein. This anomaly was sucessfully repaired by double patch technique.

  • Pulmonary atresia with intact ventricular septum is a extremely rare congenital cardiac anomaly. With the history of cyanosis and failure to thrive, this anomaly should not be excluded and emergency management is necessary. Our patient was 69 day-old male with pulmonary atresia which was confirmed by cardiac angiography. Prostaglandin E1 was used for maintenance of pulmonary blood flow preoperatively. Right ventricular outflow reconstruction with pericardial patch and concomitant pulmonary valvotomy were done on beating heart for palliation. With this method, growing of right ventricle and tricuspid annulus are highly expected.

Surgical treatment of a pulmonary sequestration combined with gastric duplication (위 중복을 동반한 폐격리증 수술치험 1예)

  • Pulmonary sequestration is a rare congenital malformation of the lung and occasionally accompanied with upper gastrointestinal anomalies. Based on the embryologic development, they are grouped as broncho-pulmonary foregut malformation. We present one the case of the intralobar pulmonary sequestration with gastric duplication. The sequestrated pulmonary tissue was 9x7x8cm in dimension, multiseptated and multiloculated, and supplied by a systemic artery of 7mm diameter from the abdominal aorta. The gastric duplication was 8cm in diameter located at the posterior wall of the stomach without communication with the gastric lumen.

  • We managed a 30 years old female pulmonary aspergillosis patient concomitant with staged pneumothorax bilaterally whose lung function decreased severely. Operative indication was fetal hemoptysis. She discharged at four weeks after left upper lobectomy in good general condition. And we .concluded that early resection should be considered in patients with pulmonary aspergilloma and coexistent pulmonary tuberculosis because those are at greater risk of fetal hemorrhage, particularly once severe hemoptysis has occurred although poor lung function in our country.

  • A unilateral pulmonary artery agenesis, which develops occasionally as one of the associated anomalies in congenital cardiovascular defect like as tetralogy of Fallot, is very rare anomaly as an isolated congenital defect. The diagnostic approach for the pulmonary artery agenesis is first suggested by the unique appearance of the involved lung on a routinely checked chest roentgenogram, because most patients are asymptomatic unless pulmonary infection had been superimposed. We have recently experienced a case of left pulmonary artery agenesis, which was diagnosed by perfusion scan, digital subtraction angiogram and then treated by left pneumonectomy in a 9 year-old boy, and presented hereby with the review of relevant literature.

  • This is a case report of extralobar pulmonary sequestration with bronchogenic cyst located in aorto-pulmonary window area of left thorax in a 17 months old male patient. In this case, a bronchogenic cyst was presented immediately adjacent to the extralobar pulmonary sequestration. Communication between the extralobar pulmonary sequestration and bronchogenic cyst was not seen grossly or microscopically. The lesion was simply excised and the postoperatively course was uneventful.

A case of rescuing a patient with acute cardiovascular instability from sudden and massive intraoperative pulmonary thromboembolism by extracorporeal membrane oxygenation

  • Intraoperative pulmonary thromboembolism is a high mortality situation. Early mortality in patients with pulmonary thromboembolism varies from 2% in normotensive patients to 30% in patients with cardiogenic shock. The use of extracorporeal cardiopulmonary resuscitation can improve survival and neurologic outcomes of cardiac arrest. We report a case of intraoperative massive pulmonary thromboembolism with circulatory collapse and cardiac arrest during anesthesia for pelvic bone fracture surgery, which were rescued by extracorporeal membrane oxygenation.
