An Analysis of the Disagreement in Disease Coding in South Korean Medical Institutions: Focusing on the Health Insurance Claim Data of Outpatients (우리나라 의료기관의 질병 코딩 불일치성 분석 : 외래환자 건강보험 청구 자료를 중심으로)

  • The purpose of this study was to use the data from the Health Insurance Review and Assessment Service to analyze the disagreement in disease coding given by different medical institutions on the same disease of the same patient and provide basic data that could help improve the quality of national public health statistics. 9,976,826 patients' data records from the Health Insurance Review and Assessment Service-National Patient Sample (HIRA-NPS) of 2014 were analyzed. The disagreement in disease coding differed by movement paths for medical institutions; the disagreement rate tended to increase when moving from a medical institution other than public health centers to a public health center and decrease remarkably when moving from a specialized general hospital to another. Therefore, this analysis of disagreement in disease coding among medical institutions suggests the need to supplement the system so that domestic medical institutions can realize consistent disease coding.

A Study on the Development of Educational Services to Collaborate between Cultural Heritage Institutions (문화유산기관의 협업을 통한 교육서비스 개발에 관한 연구 - 기록관·도서관·박물관을 중심으로 -)

  • Traditionally, archives, libraries, and museums have been known to have common responsibilities in providing public service, despite of their differences in the nature of holdings. Recently, these institutions, which are keeping archival materials, increasingly collaborate with each other to provide effective integrated user service in educational programs. The study highlights the importance of collaboration among cultural institutions and analyzes the exemplary cases inside and outside Korea. By analyzing the current educational services conducted by the institutions, focusing on weaknesses and strengths, the authors mention that the analysis results can be referred to them when developing a model of educational services. Based on the analysis, this study finally offers a plan to activate the collaboration of cultural heritage institutions in Korea and develop a model for educational services using the ADDIE model.

  • As research of oral history improved, not only public institutions but also private sectors actively collected and managed too. Despite these demands, however, there was not enough systematic analysis of systems targeted at oral archives. In this study, which started from this problem equation, the characteristics of oral archives and systems were analyzed and the institution that manages and serves current oral archives was selected to derive problems. It also analyzed the information required by the Presidential Archives, which collects oral archives but plans to build a system design. I will present suggestions for the establishment of a systematic oral archives management system focusing on problems and improvement measures that arise in the process.

The Present State and Solutions for Archival Arrangement and Description of National Archives & Records Service of Korea (국가기록원의 기록물 정리기술의 현황과 개선방안)

  • Archival description in archives has an important role in document control and reference service. Archives has made an effort to do archival description. But we have some differences and problems about a theory and practical processes comparing with advanced countries. The serious difference in a theory is that a function classification, maintenance of an original order, arrangement of multi-level description are not reflected in practical process. they are arranged in shelves after they are arranged by registration order in a unit of a volume like an arrangement of book. In addition, there are problems in history of agency change or control of index. So these can cause inconvenience for users. For improving, in this study we introduced the meaning and importance of arrangement of description, the situation and problem of arrangement of description in The National Archives, and a description guideline in other foreign countries. The next is an example for ISAD(G). This paper has chapter 8, the chapter 1 is introduction, the chapter 2 is the meaning and importance of arrangement of description, excluding the chapter 8 is conclusion we can say like this from the chapter 3 to the chapter 7. In the chapter 3, we explain GOVT we are using now and description element category in situation and problem of arrangement of description in Archives. In the chapter 4, this is about guideline from Archives in U.S.A, England and Australia. 1. Lifecycle Date Requirement Guide from NARA is introduced and of the description field, the way of the description about just one title element is introduced. 2. This is about the guideline of the description from Public Record Office. That name is National Archives Cataloguing Guidelines Introduction. We are saying "PROCAT" from this guideline and the seven procedure of description. 3. This is about Commomon Record Series from National Archives of Australia. we studied Registration & description procedures for CRS system. In the chapter 5, This is about the example which applied ISAD to. Archives introduce description of documents produced from Appeals Commission in the Ministry of Government Administration. In the chapter 6, 7. These are about the problems we pointed after using ISAD, naming for the document at procedure section in every institution, the lack of description fields category, the sort or classification of the kind or form, the reference or identified number, the absence description rule about the details, function classification, multi-level description, input format, arrangement of book shelf, authority control. The plan for improving are that problems. The best way for arrangement and description in Archives is to examine the standard, guideline, manual from archives in the advanced countries. So we suggested we need many research and study about this in the academic field.

Trends Analysis of Suicidal Poisoning in Korea- Comparison before and after the IMF Crisis and the Economic Recession (IMF와 경기침체 전후 음독자살 환자에 대한 분석 -1997년부터 2004년까지 일개 대학병원 응급센터 연구조사를 중심으로-)

  • Purpose: Nowadays the Korean society is in the center of time of the social change. Because of dissociation of traditional family, IMF (International Monetary Fund) crisis and economic recession, the mortality rate of suicidal attempts are increasing annually. The majority of suicidal attempts were by poisoning considering the characteristics of korean society. Therefore we studied to find out the characteristics of suicidal poisoning before and after social economic stress, and to suggest the direction for the proper management. Methods: We reviewed medical records of 547 patients of suicide by poisoning who visited the emergency medical centers of St. Mary's hospitals from 1997 to 2004. For the annual trends analysis, we analyzed the demographic and toxicologic data of these patients compared with before and after IMF crisis (1998) and the economic recession ($2003\~2004$). Results: At the time of IMF and the economic recession, the number of suicidal poisoning increased, especially in fifth decade. The refusal rate of toxicological emergency treatment increased. Also the mortality rate and the admission rate to ICU (Intensive Care Unit) increased during the same period. In the result of the comparison analysis, clinical severity and mortality of social problem group were higher. However they couldn't be treated appropriately because of financial problem and the family indifference. Conclusion: At the economic recession period, the severity of suicidal poisoned patients was high. But the refusal rate of toxicologic treatment also increased, so the patients had lost the chance for proper toxicologic treatment. Therefore they would be supported by medical institution and public health.

A Study on the Exhibition through the Web with Open Source Software OMEKA (공개 소프트웨어 OMEKA를 이용한 기록 웹 전시 방안 연구)

  • Korea has a high standard of IT environment to serve exhibit programs through the web with internet propagation and IT technology. However, the web exhibition of public institutions not only seem to introduce off-line exhibitions but also not to invigorate. It is caused by the lack of awareness, the cost of system installation and the lack of professional manpower. In this situation, OMEKA could suggest practical solutions to archives where need their own exhibition through the web. Especially, it would helpful for small record management organizations which are not enough budget and personal. OMEKA is an open source software program for digital collection and contents management. It has an affinity with users unlike traditional archives service programs. It also has been variously used by libraries, museums and schools because of exceptional exhibit functions. In this article, we introduce to the installation of a practical use about OMEKA. Regarding to OMEKA features, we consider it to raise exhibit effects. OMEKA would reduce the cost related to plans of exhibitions because it could display various contents and programs which reflecting characteristics of institutions. In addition, the availability of installation and widespread technological environment would lessen burden of public institutions. Using OMEKA, they would improve service level of public institutions and, make users satisfy. Therefore, they can change the social recognition of public institutions. OMEKA can contribute to various exercises of public records. It is not just the stereotypical system but, serves exhibition and collections with the strategy which each public institution would like to display. After all, it not only to connect to users with producers but also to improve the public image of institutions positively. Then, OMEKA would bring the great result through this interaction between public institutions and users.

A Necessary Conditions of Building University Archives: For the Tentative Application of an Immature Archival Method and Program prior to Building Archives (대학기록관 설립의 필요조건: '미성숙한' 기록관리 방법의 시험 운용 방안)

  • This essay introduces the basic method and program required to meet some necessary conditions of building university archives. By the phrase 'method and program', I intend the effective means of regularly but circumstantially controling the ways to the archival purposes proper which can be defined as the keeping of evidences and the broadening of information pool in terms of the evidential and informational natures or values of records. My starting point is about the matters of overcoming a standard method of induction which has long prescribed much passive procedures in the archival work. Considering the differences in the records and archives management-practices between West and Korea, I tentatively try to add some active elements to the archival work among which the collection for the expanding evidences and informations may firstly emphasized. While this collecting activity normally depends on the existing 'collections' and 'manuscripts', I cannot exclude the possibility of collections the materials, being likely to be registered in any poor or insufficient record groups. In the similar context, this kind of activity may and must be expanded beyond the university boundaries so at to arrange the cornerstones of archive-based local studies in the various disciplines. Here I premise another role of university archives, the role as 'science archives'. These archives within university archives seem likely to function in likewise the special collections within Western university libraries. What I mean here, however, is the archival groups purposedly gathered or acquisited according to more detail and narrower plan in order to meet the various demands from the different disciplines for the primary sources. The archival procedures from this revised method and program may, I hope, satisfy some of the preconditions of building university archives before the archives will actually function as a sub-institution of an university preserving legal, administrative and financial evidences, thus keeping identity and continuity of the university on the one hand, and as a local information center of supplying the archival contents on effectual demand from the field of local studies on the other. Finally, I conclude with a suggestion concerning the cooperation of all the parties of archival works. Proposing the 'Regional Research Center Program' in the field of technology and engineering as a model for the cooperation, I suggest that universities, private/public organizations, and central and local governments may work together for surveying the scattered ancient and modern documents as well as for building archives under the matching fund.

The Application of Customer Relationship Management for the Effective Prenatal Care (효과적인 산전관리를 위한 고객관계관리(CRM)의 도입)

  • The prenatal care is the preventive medical service to help the pregnant mother deliver the healthy baby. It's regular examines give some chances to check-up the healthy conditions. This thesis concentrates on the CRM system to support an effective prenatal care system and prove the effectiveness of it. As CRM is the adapted management related to the customer's own information, it is important to develop the CRM model classified by the patients characteristics. A general hospital in Busan operated the CRM system to carry out the effective prenatal care and there is an analysis to ensure the effectiveness of CRM system for the pregnant women in our maternity ward. The results can be summarized as follows: 1) According to the comparisons with the CRM system, we can conclude the system is desirable. (1) Maternal Age : In the age distribution, the prenatal visit frequency, triple marker freqency, oral GTT and targeted ultrasonography in the experimental group in 30 to 34 years old is higher on the whole. For over 35 years old group, the higher frequency comes out in the oral GTT and targeted ultrasonography and for 25 to 29 years old group the different figure shows just in the targeted ultrasonography. (2) Area of residence: There is a clear difference in all the items in Busan and near area but no sign of difference in prenatal visits and oral GTT in other residencial area. Especially in the targeted ultrasonography the higher figure shows in the experimental group located in the both areas. The targeted ultrasonography is known as the specific examination which should be examined by the specialists, on the contrary the other examinations can be operated in the small clinic. So the public information and seminars related with ultrasonography increases the check-up frequency. The clinic requests some ultrasonographical examinations to the specialists in general hospital. (3) Parity: The clear difference shows that the CRM system causes the prenatal visit frequency to become higher in experimental group. The figure is 9.7 times and 8.6 times each. This is opposite that the past study said multiparity reduced the average prenatal visits. But the result of CRM is considered as the method to help the multiparity understand the importance of the prenatal care. (4) Obstetrical history: In the experimental group of the spontaneous delivery group, the figure is higher in the prenatal visit frequency, triple marker, oral GTT and targeted ultrasonography but the Caesarean section delivery group has higher figure in targeted ultrasonography. (5) In the first check-up, the rate of targeted ultrasonography in under 16 week pregnancy, in the 16 week pregnancy to 32 week pregnancy and the over 32 week pregnancy in the experimental group is upper than the compared one. For the oral GTT, there is a difference in under 16 week pregnancy but no difference in prenatal visits and triple marker. 2) The analysis of characteristics of prenatal care through the decision tree resulted in the fact that the most important variable is the residential area. After the delivery frequency is following, the obstetrical history and maternal age are in order. It is the same result in the triple marker and oral GTT. Consequently it is the same order of important variables in CRM system. The effectiveness of CRM system is proved in this study. The CRM system is a marketing method to control and lead the customers through the segmentation of customer data. It increases the new customer aquisition, maintenance of loyal customers, augmentation of customers value, activation of potential customers and creation of life time customers. So eventually it can enlarge the customers value. The medical institution should make efforts to establish the data base enforced by the customer's information on the underlying ordinary data system to carry out the CRM system effectively. In addition, it should develop the a variety of marketing strategy in order to set up one to one marketing satisfying the needs of individual patients.
