  • Purpose: This study aimed to compare the effects of ischemic compression (IC) therapy and extracorporeal shock wave therapy (ESWT) in patients with upper trapezius myofascial pain syndrome and to present an effective treatment method for these patients. Methods: A total of 42 patients with upper trapezius myofascial pain syndrome were randomly assigned to the IC group (n = 21) and ESWT group (n = 21). IC therapy and ESWT were performed in the IC and ESWT groups, respectively. Treatment was applied to the trigger point of the upper trapezius muscle. Visual analog scales (VAS), pain pressure threshold (PPT), range of motion, neck disability index (NDI), and fear-avoidance belief questionnaire (FABQ) were evaluated before and after the intervention to compare its effectiveness. Results: Both groups showed significant differences before and after the intervention in VAS, PPT, NDI, FABQ, flexion, extension, right side bending, and left side bending (p <.05). In addition, there were significant differences in the IC group compared to the ESWT group in VAS, PPT, and NDI (p <.05). Conclusion: IC therapy and ESWT applied to patients with upper trapezius myofascial pain syndrome are mediating methods of pain, function, and psychosocial effects. In addition, IC therapy may be a more effective mediating method for pain and dysfunction than ESWT.

  • This study was designed to evaluate the Personality characteristics of orthognathic surgery patients preoperatively and postoperatively through the MMPI. Comparison was made between the personality characteritics of preoperative and postoperative groups. Motivation for undergoing orthognathic surgery and postoperative satisfaction were examined through the questionnaire. Subjects used in this study consisted of 50 patients under presurgical orthodontic treatment and 22 patients who had received orthognathic surgery. From this study, the results were followings. 1. In preoperative patients group, all T-scores were within the normal range except for K-scale. The T-score of K-scale is 57.7, this showed that these patients underwent some or less stress. 2. In postoperative patients group, all T-scores were within the normal range. 3. Only in K-scale, there was statistically significant increase after surgery (p<0.05). 4. In male patients group, all T-scores were within the normal range except for that of preoperative K-scale. Postoperatively, there were statistically significant increase in K and Hs scales(p<0.05) 5. In female patients group, all T-scores were within the normal range. 6. Motivation for undergoing orthognathic surgery could be divided into 4 categories. esthetic goal($28.2\%$), functional goal($32.1\%$), TMJ problem($9.0\%$), and psychosocial reason($30.7\%$). 7. The degree of postoperative satisfaction was quite high($85.7\%$). $14.3\%$ of all patients were very satisfied, $71.4\%$ were satisfied, $9.5\%$ were neither satisfied or dissatisfied, and $4.8\%$ were very dissatisfied.

  • The relationship of stress and psychopathology with subjective or objective tinnitus intensity was investigated in 43 patients with tinnitus. Global assessment of recent stress scale, Beck depression inventory and symptom checklist-90-revision were used to measure stress perception and psychopathology. Subjective tinnitus intensity was assessed by subjective tinnitus severity inventory, whereas objective tinnitus intensity was assessed by tinnitogram. Scores of perceived stress related to interpersonal relationship and sickness or min had significantly positive correlation with subjective tinnitus intensity. In psychopathology, scores of somatization, interpersonal sensitivity, depression, anxiety, phobia, paranoid ideation, psychoticism had significantly positive correlation with subjective tinnitus intensity. However, stress perception and psychopathology did not significantly correlate with objective tinnitus intensity. These findings suggest that tinnitus may be associated with stress perception and multiple psychopathology including anxiety and depression. Thus, it is emphasized that Psychosocial intervention as well as effective consultation-liasion activity is needed for evaluation and treatment of patients with tinnitus.

  • This qualitative study aims to understand family members' caregiving experience for middle-aged and elderly breast cancer survivors at early stages. A total of four families of breast cancer survivors were given focus group interviews(FGI), and an additional family was interviewed individually. The thematic analyses of the FGI and individual in-depth interview discussions revealed the following four themes: first, day-to-day feelings like walking on thin ice (entangled emotions with each treatment stage, regret of watching a survivor's pain, fear of recurrence and death, concerns of changes in family relationships, and various feelings experienced as a daughter), second, caregiving has become a mainstay in my life (unfamiliar and unskilled care, losing my life), third, beginning to recover and different experiences between a survivor and a family caregiver (not understanding a survivor's pain from the abyss, forgetting that the survivor is still a cancer patient and that treating him or her casually), and fourth, moving forward (hope that things will be better, taking care of myself to prevent exhaustion). Based on the results of this study, we proposed social welfare services and policy measures to reflect the psychosocial needs of family caregivers of breast cancer survivors and support family caregivers during the care process.

  • The purpose of this study was to develop and examine the effectiveness of 'Coping Resources Improvement Program' designed for emerging adults experiencing psychosocial difficulties during college to work transition. Based on transition model of Scholossberg(1995), the program was developed to intervene in 4S (Situation, Self, Support, Strategy) required in the transition. Participants included 31 job applicants who are senior or above and were assigned to the experimental group (N=10), comparison group (N=10), and control group (N=11). Data to verify effectiveness were collected pre-, mid-, and post treatment. Results indicated that the level of coping resources and psychosocial difficulties in the transition changed significantly after the program and those changes were still maintained after a month in the experimental group. However, there were no statistically significant changes on job-search burnout and career-adaptability in all groups. The implication and limitation of the study and the suggestions for the future studies were discussed.

  • Background: In principle, cervical tuberculous lymphadenitis (CTBL) is a medical disease that may require surgical treatment, particularly in young women who complain of psychosocial and cosmetic problems. We encountered 13 cases of aggravated CTBL treated surgically despite the appropriate course of antituberculous chemotherapy. We report the clinical characteristis of these cases. Methods: The clinical data of 13 patients with aggravated CTBL requiring surgical treatment from January 2000 to December 2006 at the Department of Chest Medicine, Internal Medicine and Plastic Surgery, National Medical Center was reviewed retrospectively. Results: Twelve of the 13 cases (92%) were female. The most common age was 21~30 years (69%). Multiple nodes were palpated in 11 cases (85%). The supraclavicular lymph nodes were sites the most commonly involved (54%). The other involved sites in the order of decreasing frequency were the jugular chain, posterior cervical, submandibular and infraauricular lymph nodes. A palpable mass was the most commonsymptom. Neck pain was reported in 3 cases (23%). General symptoms such as weight loss, fatigue, anorexia and night sweats were noted in 5 cases (38%). Respiratory symptoms such as cough, sputum, hemoptysis, dyspnea and chest pain were observed in 4 cases (31%). Pulmonary tuberculosis was noted in 11 cases (85%). Other extrapulmonary tuberculosis coexisted in 4 cases (31%). This suggests that surgical CTBLs may be manifestations of a systemic disease and might be difficult to treat. Most cases (92%) were stages 2 and 3 at the initial diagnostic period but all cases fell into stage 4 and 5 when reassesed before surgery. The average duration of anti-TB chemotherapy before and after surgery was 10.2 and 15.2 months, respectively. The 13 patients were followed up until June. 2008. Among them, 2 cases had newly developed CTBL and the other 11cases showed no recurrence. Conclusion: In principle, CTBL is the medical disease. However, despite the appropriate course of anti-TB chemotherapy, CTBL can progress to a more advanced stages and grow rapidly to a large-sized or fistulous mass with a persistent abscess. Surgical treatment may be inevitable for patients with psychosocial and cosmetic problems caused by these masses, particularly in young women.

  • Objectives: The purpose of this study was to examine the characteristics and the psychosocial factors associated to the referral to psychiatric care in the suicide attempters visiting emergency center. Methods: We conducted a systematic chart review of 377 suicidal attempters visiting emergency center of the Korea University Ansan Hospital between January 2008 and December 2011. We gathered a data contain 20 items including psychosocial characteristics and factors related to suicide and factors related to psychiatric treatment. Multivariate logistic regression models were fitted to data to estimate the unique effects of sex, drunken status, companion, suicidal methods, place of suicide and current use of psychiatric medication on the referral to psychiatric care. Results: The female gender(OR=1.63, 95% CI=0.99-2.69), suicidal attempts at home(OR=3.40, 95% I= 1.21-9.56) and drunken state at visit(OR=2.34, 95% CI=1.10-5.01) are the factors that predict the risk of the non-referral of the patients to psychiatric intervention. Place of suicidal attempt was the most important factor do play a role in determining whether referral to psychiatric care will take place or not. Current use of psychiatric medication showed a trend toward significance(p=0.08, OR=1.67, 95% CI=0.95-2.95). Conclusions: These results suggest that when deciding whether to adapt or to refuse the referral to psychiatric care, the factors such as suicidal intent, lethality of suicide methods, familiar factors and alcohol may contribute onto the referral to psychiatric care. Additional research is required to investigate an association of these factors with referral to psychiatric care.

  • The purposes of this research are to examine the effects of muscle relaxation to the rheumatoid arthritis patients' physiological, psychological, and social adaptation, and to verify that the technique Is an effective tool for improving the patients' adaptation. This research utilized a qusi-experimental design that compares the pre-experiment measures and the post-experiment measures. The subjects of this study were 37 out-patients who were diagnosed for rheumatoid arthritis in P university hospital and K clinic between September 12, 1993 and November 30, 1993. The experimental group consisted of patients treated on Monday, Wednesday and Friday. The control group consisted of patients treated on Tuesday, Thursday and Saturday The pre-experimental survey utilized the following tools. The physiological adaptation was measured by a graphic rating scale for pain and activity of daily living. The psychological adaptation was measured by Zung's depression scale and Rosenberg's self-esteem scale : and the social adaptation was measured by Derogatis' psychosocial adjustment scale. The experimental group received muscle relaxation treatment for 15 minutes per day for the period of 2 weeks, the control group received no treatment and had quiet time. The post-experiment measurement was carried out similar to the pre-experiment survey, SPSS $PC^+$ is used to analyze the collected data. The reliabilities of the measurement tools were examined by Cronbach's ${\alpha}$ coefficients. The homogeneities between the experimental and control groups were tested by t-test and chi-square test, hypotheses were tested by t-test. This research found that the general characteristics between the groups were statistically homogenious. The physiological, psychological, and social adaptation between the groups in the pre-experimental survey were also statistically homogenious. The results of this research can be summarized as follows : 1. Concerning the Physiological adaptation, the experimental group showed a statistically lower pain score than the control group. Thus, muscle relaxation was effective to relieve the pain of rheumatoid arthritis patients (t=-2.95, p=.006). 2. Concerning the psychological adaptation, the experimental group showed a statistically lower depression score than the control group. Thus, muscle relaxation was effective to reduce the patient's depression(t=-4.00, p=.001). 3. Concerning social adaptation, the experimental group showed a statistically higher score for the health maintenance and disease control than the control groups. Thus, muscle relaxation was effective for the health maintenance and disease control (t=2.09, p=.004). This research showed that the muscle relaxation is a nursing intervention that can promote the physiological, psychological and social adaptation of the rheumatoid arthritis patients in terms of short-term and cognitive changes. However, more fundomental changes in behavior and long-term physiology could not be found through such a short-term relaxation treatment.

  • This study reviewed the important preceding studies of prevention and treatment of the crimes and therapies on the characteristics of mental illness, trying to investigate and identify more effective ways to prevent, decrease the crimes of the mentally ill and treat them. The main results were as follows. First, relationships between mental illness and crimes have been inconsistently reported, to investigate environmental stresses and triggers of the mentally ill additionally. Second, the term 'mentally illness' was to be defined more specifically, such as severe psychiatric disabled, antisocial personality disorder, intellectual disability, anger·impulse control disorder, and drug abuse. If medication and psychosocial treatments were properly treated to the severe psychiatric disabled, their crime incidence and recidivism will be decrease enough to live with their neighbors in community. But the ways and processes of treating antisocial personality disorder, intellectual disability, anger·impulse control disorder, and drug abuse were very different from it, requiring more intense psychological, medical and biological interventions and social seclusion. The national campaigns and projects for their human rights and community care will be needed to promote the therapeutic effects for them. The limitation and future tasks were discussed.
