근골격계질환에 대한 물리적/심리적요인에 대한 연구 (Physiological and Psychological analysis of musculoskeletal symptoms)

  • 본 논문에서는 근골격계질환, 특히 직업성요통에 관련되는 위험요인의 영향을 종합적으로 살펴보고자 하였다. 첫째로 물리적인 요인의 영향을 알아보기 위하여 해석적인 인체역학 모델과 NIOSH(National Institute for Occupational Safety & Health) Guidelines를 이용하여 자동차 조립작업의 직업성 요통관련 위험도를 평가하였다. 총 246 작업자에 대한 분석을 수행하였는데, 분석결과 총 20개의 작업에서 L5/S1에서의 압축력이 300kg이 넘었으며 44개의 작업은 NIOSH Guidelines에 의한 분석결과, 최저허용기준치(ACTION LIMIT)을 넘는 것으로 나타났다. 둘째로 사회심리적 요인과 직업성요통의 관계에 대해 분석하였다. 사회심리적 요인의 영향을 알아보기 위하여 증상과 PWI(Psychosocial Well-being Index)를 토대로 한 설문을 이용하여 246명 작업자 모두에게 설문조사를 실시하였다. 그 결과 207명의 작업자가 직업성 요통증상을 경험한 적이 있으며 특히 27명의 작업자는 유병자로 분류된 바 있는 사람들이었다. PWI 점수를 근거로 하여 나눈 두 집단(저스트레스군, 고 스트레스군)은 증상이나 진단결과에 있어서 통계적으로 유의한 차이를 보이지 않았다. 또한 사회심리적 스트레스와 작업강도와의 관계를 살펴보면 주로 작업자세요인과 사회심리적 스트레스가 유의한 관계를 가지는 것으로 나타났다. 마지막으로 직업성요통의 발생에 대하여 경로분석을 수행하였다. 분석결과 인구학적 요인, 사회심리적 요인, 작업특성 요인이 직업성 요통에 직접적으로 영향을 미쳤으며, 개인특성요인 즉, 작업경험, 흡연, 수면시간 등은 직업성요통에 직접적인 영향을 미치는 않은 것으로 나타났다. 그러나 개인특성요인은 직업성 요통에 유의한 영향을 미치는 사회심리적 요인과는 밀접한 관계를 가졌다. 향후 다양한 직종에서의 이와같은 경험적 연구결과가 축적되어 우리나라의 조직문화적 특성을 고려한 직업성 요통에 대한 위험모델에 구축되어야 한다고 사료된다.

Factors associated with the decision to undergo risk-reducing salpingo-oophorectomy among women at high risk for hereditary breast and ovarian cancer: a systematic review

  • Purpose: This systematic review aims to identify factors associated with risk-reducing salpingo-oophorectomy (RRSO), including the uptake rate and decision timing, among women at high risk for hereditary breast and ovarian cancer (HBOC). Methods: We found 4,935 relevant studies using MEDLINE, Embase, CINAHL, and PsycINFO on July 6, 2020. Two authors screened the articles and extracted data. Twenty-four studies met the inclusion criteria. Quality assessment of articles was conducted using the Risk of Bias for Nonrandomized Studies tool. Results: Five types of factors were identified (demographic factors, clinical factors, family history of cancer, psychological factors, and objective cancer risk). The specific significant factors were older age, having child(ren), being a BRCA1/2 carrier, mastectomy history, perceived risk for ovarian cancer, and perceived advantages of RRSO, whereas objective cancer risk was not significant. The uptake rate of RRSO was 23.4% to 87.2% (mean, 45.2%) among high-risk women for HBOC. The mean time to decide whether to undergo RRSO after BRCA testing was 4 to 34 months. Conclusion: RRSO decisions are affected by demographic, clinical, and psychological factors, rather than objective cancer risk. Nonetheless, women seeking RRSO should be offered information about objective cancer risk. Even though decision-making for RRSO is a complex and multifaceted process, the psychosocial factors that may influence decisions have not been comprehensively examined, including family attitudes toward RRSO, cultural norms, social values, and health care providers' attitudes.

일부 남자 고등학생들의 사회·심리적 스트레스와 우울과의 관련성 (Relationship between Self-Perceived Psychosocial Stress and Depressive Symptoms among Male High School Students)

  • 본 연구는 남자 고등학생들의 사회 심리적 스트레스와 우울과의 관련성을 파악하며, 우울에 영향을 미치는 요인을 규명할 목적으로 실시하였다. 조사대상은 D광역시의 남자 고등학생 573명으로 하였으며, 2015년 10월에 자기기입식 설문조사를 통해 자료를 수집하였다. 연구결과, 조사대상 학생들의 사회 심리적 스트레스의 분포는 건강군 14.8%, 잠재적 스트레스군 71.9%, 고위험스트레스군 13.3%이었으며, 우울수준의 분포는 정상군 71.4%, 경도 우울군 25.3%, 중등도 우울군 3.0%, 중증 우울군 0.3%이었다. 조사대상 학생들의 우울수준에 영향을 미치는 요인으로는 주관적인 건강상태, 아침식사 여부, 수면시간, 학교성적, 고민거리, 학교생활에 대한 만족도, 가정의 경제 상태 및 사회 심리적 스트레스가 선정되었으며, 특히 사회 심리적 스트레스는 우울수준과 높은 관련성이 있음을 시사하고 있어 학생들의 스트레스 관리에 대한 필요성이 강조된다.

남자 고등학생 흡연자의 영양상태 판정 및 흡연관련 요인분석 -II. 흡연과 관련된 사회심리적 요인에 대한 연구- (Assessment of Nutritional Status and Factors Related to Smoking in Adolescent Males -II. Psychosocial Factors Influencing Smoking among Male High School Students-)

  • The study purpose was to investigate psychosocial factors related to smoking among adolescent boys. The Theory of Planned Behavior provide the basis for the study. Twenty-five attitudinal beliefs, 9 normative beliefs and 20control beliefs were identified through questionnaire development. The data were analyzed using t-test and χ2-test. Thirty-three percent of 300 students were smokers. Most of the beliefs examined were significantly different between smokers(n=92) and nonsmokers(n=92). With respect to attitudinal beliefs, smokers responded less negatively on the items of bad health effects of smoking such as sore throat, headache, chest pain, risk of cancer and bad blood circulation(p<0.001), and decreased physical strength(p<0.05). Smokers believed less negatively on the items that smoking leads to bothering others, bad breath, yellow teeth and making them spend money(p<0.001). In contrast, smokers felt more positively on smoking as a means of stress management, relaxing, helping digestion(P<0.001). Smokers felt less pressure for not smoking from significant others. Especially mother, siblings, friends, girl friends, seniors of school were important sources of influence regarding subject's smoking. Smokers felt less confident in controlling the urge to smoke in several situations including; when they were with friends or asked to smoke by friends; after the meal, or drinking; when they were bored or stressed, upset, and when they felt unstable(p<0.001). Smokers also scored lower on specific skills to quit or control the urge to smoke as well as overall perceived control, compared to nonsmokers(p<0.001). These results suggest that interventions for adolescents incorporate diverse strategies to increase the perceived control over smoking in specific situations as well as overall perceived control, to help them realize and modify attitudinal beliefs, and to elicit support from significant others for not smoking. Occupational Health Problems and Lifestyle Changes Among Novice Working-From-Home Workers Amid the COVID-19 Pandemic

  • Background: Social distancing by working-from-home is an effective measure to decrease the spread of COVID-19. However, this new work pattern could also affect the well-being of workers. Therefore, the aim of the study was to study the magnitude of occupational health problems and lifestyle changes among workers who have only recently started working from home. Methods: A cross-sectional study was conducted using online self-administered questionnaires during the coronavirus disease 2019 pandemic in the Bangkok metropolitan area, Thailand. The participants were from any organization that allowed working from home. The demographic data including the analysis of the characteristics of working from home, the occurrence of occupational health problems, and the lifestyle changes caused by working from home were analyzed. Results: A total of 869 workers were included as study participants. The highest prevalence of physical health problems among all workers was identified to be weight gain at a rate of 40.97% (95% confidence interval = 37.69-44.24), and the highest prevalence of psychosocial problems was identified to be cabin fever at a rate of 31.28% (95% confidence interval = 26.66-35.90%) among full-time working-from-home workers. The health effects that were significantly related to the intensity of working from home (p for trends <0.05), either positively or negatively, included body weight changes, ergonomic problems, indoor environmental problems, and psychosocial problems. Meanwhile, the lifestyle changes related to work intensity included eating pattern, sleep habits, and exercise. Conclusions: Working from home can affect workers' well-being in various aspects. Hence, occupational health providers must prepare for risk prevention and health promotion in this "new normal" working life pattern and for future pandemics.

Working Conditions, Job Strain, and Traffic Safety among Three Groups of Public Transport Drivers

  • Background: Working conditions and psychosocial work factors have acquired an important role explaining the well-being and performance of professional drivers, including those working in the field of public transport. This study aimed to examine the association between job strain and the operational performance of public transport drivers and to compare the expositions with psychosocial risk at work of three different types of transport workers: taxi drivers, city bus drivers, and interurban bus drivers. Method: A sample of 780 professional drivers was drawn from three transport companies in Bogota (Colombia). The participants answered the Job Content Questionnaire and a set of sociodemographic and driving performance questions, including age, professional driving experience, work schedules, and accidents and penalties suffered in the last 2 years. Results: Analyses showed significant associations between measures of socio-labor variables and key performance indicators such road traffic accidents and penalties. Furthermore, multiple linear regression analysis contributed to explain significantly suffered accidents from key variables of the Job Demand-Control model, essentially from job strain. In addition, throughout post-hoc analyses, significant differences were found in terms of perceived social support, job strain, and job insecurity. Conclusion: Work stress is an issue that compromises the safety of professional drivers. This research provides evidence supporting a significant effect of job strain on the professional driver's performance. Moreover, the statistically significant differences between taxi drivers, city bus drivers, and interurban bus drivers in their expositions to work-related stress suggest the need for tailored occupational safety interventions on each occupational group.

심혈관질환의 심리사회적 위험요인으로써의 D유형 성격에 관한 논문 분석 (The Analysis of Type D Personality Research as a Psychosocial Risk Factor in Cardiovascular Disease for Elders with a Chronic Disease)

  • Purpose: The purpose of this paper was to examine the relationship between type D personality and cardiovascular disease, and to suggest future research directions. Method: A literature search was conducted from the following nine databases: 1) MEDLINE, 2) CINAHL, 3) Pubmed Unrestricted, 4) PsycINFO, 5) KISS, 6) RICHIS, 7) RISS4U, and 8) Nanet. The combinations of the words, "type D personality", "personality", "heart", "cardiovascular", and "coronary" were used for keyword searches to find relevant articles. Twenty eight studies were identified. Result: Type D personality has been associated with increased morbidity and mortality in patients with established cardiovascular disease. Type D patients are also at increased risk for impaired quality of life, and seem to benefit less from medical and invasive treatment. Conclusion: There is substantial evidence for a relationship between type D personality and clinical outcomes related to cardiovascular disease. Randomized clinical trials are needed to further evaluate the value of controlling type D personality to improve survival and reduce morbidity in patients with cardiovascular disease. Accumulating evidence from this analysis indicates the urgent need to adopt a personality approach in order to optimize the identification of patients at risk for stress related cardiac events.

  • 많은 연구들이 심혈관 질환의 병인과 병리적 기전에 있어 사회심리학적, 행동학적 위험요인의 역할을 조사해 왔다. A형 행동 유형과 심장 질환의 이환 및 사망과의 관련성을 확인한 여러 전향적 연구들을 바탕으로, A형 행동 유형은 관상동맥질환의 독립적인 위험요인으로 밝혀졌다. 또한 최근 제안된 새로운 성격유형인 D형 인격유형 역시 심혈관 질환의 이환율과사망률의 증가에 관여함이 보고되고 있다. 그러나 국내에서는 아직 이들과 관련된 연구가 부족한 실정이다. 따라서 저자는 심혈관 질환과의 관련성에 대해 널리 연구, 보고되고 있는 A형 행동유형과 D형 인격유형에 초점을 맞추면서 지금까지의 연구 문헌을 정리 검토하여 향후 연구에 도움을 주고자 하였다.

조기진통 임부의 조산 발생 영향요인: 전향적 코호트 연구 (Risk Factors for Premature Birth among Premature Obstetric Labor Women: A Prospective Cohort Study)

  • Purpose: To identify risk factors for premature birth among premature obstetric labor women. Methods: Participants were 129 hospitalized women who were diagnosed with potential premature obstetric labor with 20 weeks to 37 weeks of gestation. Data were analyzed using descriptive statistics, $x^2$ test, t-test, and binary logistic regression. Results: Of 129 women, 78(60.5%) gave premature birth and 51 (39.5%) gave full-term birth. Risk factors for premature birth were education level (${\leq}$bachelor's degree), abnormal bowel condition (constipation or diarrhea), time firstly diagnosed with a premature obstetric labor (below 28 weeks of pregnancy), and multiple pregnancy. There were also increased risks of premature birth for participants with high level of anxiety and high level of prenatal stress. In social support, there was an increased risk of premature birth for participants with low level of social support. Conclusion: Prenatal nursing programs should consider not only psychosocial factors such as anxiety, prenatal stress, and social support, but also some general and obstetric factors such as education level, abnormal bowel condition, time firstly diagnosed with a premature obstetric labor, and multiple pregnancy to increase maternal and child health.

Family Context Factors and the Risk of Smoking among Male Adolescents in Saudi Arabia

  • Background: Smoking behavior is related to numerous factors, including psychosocial parameters. This study investigated the association between family context factors and smoking among male adolescents. Materials and Methods: A cross-sectional, school-based study was conducted during 2014. The study sampled 900 students from intermediate and secondary schools in Madinah City, Saudi Arabia. Data concerning smoking status, sociodemographic, parental and friends' smoking behavior, and family factors were collected using a self-administered questionnaire. These data were employed to estimate the prevalence of smoking using appropriate statistical analyses including multivariate logistic regression. Results: Of 900 students, 870 completed the study questionnaire (96.7%). Of the respondents, 181 students (20.8%, 95% CI=18.1%-23.5%) were current smokers, and a much higher prevalence was observed among adolescents with most or all of their friends smoking (48.1%) and those living with neither parent (47.4%). The adjusted risk of smoking increased significantly among adolescents who lived with neither parent (OR=3.3; 95% CI=1.1-9.2) and among those who reported little or no parental supervision (OR=1.5; 95% CI=1.0-2.1). Conclusions: Family context factors are associated with an increased risk of smoking behavior among male adolescents in Saudi Arabia.
