암 환자의 우울증 (Depression in Cancer Patients)

  • Bidirectional relationships exist between cancer and depression; the prevalence of depression in cancer patients is higher than in the general population, and depression predicts cancer progression and mortality. The mechanisms through which depression contributes to the progression of cancer are related with dysregulation of the hypothalamic-pituitary-adrenal axis and impairment of immune function. However, depression in cancer patients tends to be underdiagnosed and not appropriately treated. The methods of diagnosis and assessment of depression in cancer patents have been debated because physical symptoms of depression mimic both cancer symptoms per se and the side effects of cancer treatment. Many studies have shown that various psychosocial and/or pharmacological interventions are effective at improving de-pressive symptoms and quality of life in cancer patients. Furthermore, antidepressant treatments are effective for various physical symptoms related to cancer, such as fatigue, anorexia, pain, hot flashes, and itching. This article reviews and discusses current knowledge about depression in cancer patients.

한열변증 점수를 이용한 한증과 열증의 건강 상태와 미병 특징 비교 (Comparison of Health Status and Mibyeong Characteristics between Cold Syndrome and Heat Syndrome by Cold Heat Syndrome Differentiation Score)

  • Objectives: The objective of this study is to develop the diagnostic tool to distinguish between cold syndrome (CS) and heat syndrome (HS). Methods: A total of 1,753 subjects were divided into three groups, those are CS group, intemediate group, and HS group, by the mean and standard deviation of the cold heat syndrome differentiation score using 7 point scale consisting of 9 items. Demographic characteristics, diseases history, health status, Mibyeong, syndrome differentiation were analyzed. Results: CS is characterized by women, elderly, and low body mass index. CS has a history of thyroid disease, cataract, depression, osteoporosis, and HS has a history of prostatomegaly. CS receives less social psychosocial stress than HS, and the quality of life associated with health status is lower than HS. CS group has the tendency to be tired, painful, sleepless, dyspeptic and anxious. Conclusions: CS is a set of symptoms associated with decreased energy metabolism and decreased metabolic function, and is more likely to be unhealthy than HS.

대학생 흡연 관련 행태 및 흡연에 의한 건강문제 (A Review of Cigarette Smoking-related Behaviors and Health Problems among University Students)

  • Objectives: This article reviewed research on smoking behaviors and smoking-related health problems among university students to suggest evidences for developing smoking control strategies for them. Methods: Domestic research papers were screened through Research Information Sharing Service (RISS), Koreanstudies Information Service System (KISS), Medical Library Information System (MEDLIS), Korean Medical Database (KMbase), and digital national assembly library. International papers were searched mainly via PubMed. Results: Smoking prevalence among male college students were found to be over 50% in majority of Korean studies. While studies on smoking-related health problems were scant in Korea, several recent studies overseas showed possible effects of smoking on health among young adults, including decreased lung function, respiratory symptoms, cardiovascular problems, glucose intolerance, buccal disease, depressive symptoms, and so forth. Besides, smoking behaviors were closely related with other risk behaviors including drinking, and several psychosocial factors. Conclusions: To explore the smoking problem among college students, the first step should be a national representative survey with scientific methods. More research should be focused on the smoking-related problems among college students. To prevent smoking among college students, smoking prevention education, and smoking cessation counseling, and the initiatives of smoke free campus are needed.

신장공여자와 신장수혜자를 가진 가족들의 사회심리적 적응 (Psychosocial Adjustment in Families with Kidney Donor or Recipient)

  • The purpose of this study was to generate a grounded theory of how families with kidney donor or recipient coped with kidney transplantations. Interview data from twelve families involved in kidney transplants was analyzed using the grounded theory method. Data analysis revealed that “protecting the family” was the main theme that represented family member experiences. In order to maintain family function and to protect the family from breaking up, family members had to adjust the family structure from the traditional style of a husband-centered family, to one that was patient health -centered. The process of this adjustment was a very long and difficult one, taking several years from the recognition of the kidney disease to the kidney transplants. Family members, especially spouses, employed nine different strategies to deal with various problems and conflicts which occurred during the process : 1) paying attention to the patient's illness and complications ; 2) accepting the patient's illness as the family's illness as well ; 3) managing the patient's illness and complications that occured ; 4) being thrifty ; 5)supporting the kidney donor ; 6) accepting and replacing the lost roles of the patient ; 7) keeping composure and encouraging the patient ; 8) sustaining the patient's independence ; 9) self-restraining sexual desires. These findings suggest that there is a developmental process where family members adjust to a kidney transplant over time. There is also a need for increased social and psychological health services for all family members over the course of kidney transplants.

소아암 아동과 가족에 관한 국내 연구 동향 (Trends in Research on Children with Cancer and Their Families in Korea)

  • Purpose: This study was designed to analyze recent trends in pediatric oncology research in Korea and suggest future research directions in this area. Methods: Studies (105) selected from http://www.riss4u.net for last 15 yr were used. They were analyzed by publication type, field and design of the study, study participants, main theme, and outcome variables used in intervention studies. Results: 1. Of the 105 studies, 67 were master and doctoral dissertations and 49.5% of studies were conducted in nursing. 2. There were 73 (69.5%) quantitative studies and 20 (19.1%) qualitative research studies and the most frequently used study design was that of a descriptive study. 3. Children with cancer undergoing treatment and their parents, mostly mothers, were the participants most frequently studied. 4. Most themes were psychological/spiritual problems, coping, and family function. 5. Most frequently measured outcome variables were nausea/vomiting, fear/anxiety and adjustment. Conclusion: The results indicate that future research should include more well-designed intervention studies to develop new intervention protocols and to confirm the effect of previous study findings. It is also necessary to use an interdisciplinary approach to deal with physical and psychosocial needs of these children and their families including siblings and fathers of children with cancer.

Psychiatric understanding and treatment of patients with amputations

  • Amputation changes the lives of patients and their families. Consequently, the patient must adapt to altered body function and image. During this adaptation process, psychological problems, such as depression, anxiety, and posttraumatic stress disorder, can occur. The psychological difficulties of patients with amputation are often accepted as normal responses that are often poorly recognized by patients, family members, and their primary physicians. Psychological problems can interfere with rehabilitation and cause additional psychosocial problems. Therefore, their early detection and treatment are important. A multidisciplinary team approach, including mental health professionals, is ideal for comprehensive and biopsychosocial management. Mental health professionals could help patients set realistic goals and use adaptive coping styles. Psychiatric approaches should consider the physical, cognitive, psychological, social, and spiritual functions and social support systems before and after amputation. The abilities and limitations of physical, cognitive, psychological, and social functions should also be considered. To improve the patient's adaptation, psychological interventions such as short-term psychotherapy, cognitive behavioral therapy, mindfulness meditation, biofeedback, and group psychotherapy can be helpful.

학령기 거부집단 아동의 친구관계 특성 (Friendship Characteristics of Rejected Children in Middle Childhood)

  • This study investigated children's friendship factors such as number of friends, friendship quality, and friends' characteristics as a function of children's sociometric status. The concordance between children's and their friends'perceptions of friendship quality was examined. Subjects were fifth grade students recruited from two public schools. The peer nomination index assessed peer rejection and acceptance. Positive and negative nominations as well as social preference identified children into rejected and popular groups. Children who nominated each other on their list of friends were considered mutual friends. Children reported the quality of their friendships using the friendship quality scale. The results showed that rejected children had fewer mutual friends compared to popular children. Rejected children as well as their friends perceived their friendships less positively. Moreover, rejected children's perception showed lower correlations with their friends'perceptions than popular children's perceptions. Furthermore, rejected children were similar with friends in respect to peer rejection level. It was concluded that rejected children's problems in their peer group and dyadic friendships may mutually influence each other. The findings from this study highlight the need for future research on friendship quality and psychosocial adjustment for rejected children.

Clinical characteristics and nursing diagnoses of pediatric patients hospitalized with inflammatory bowel disease: a single-center retrospective study in South Korea

  • Purpose: This study aimed to identify clinical characteristics of South Korean pediatric inflammatory bowel disease (IBD) in a children's hospital over the past 5 years, with a specific focus on comparing the features observed between Crohn's disease (CD) and ulcerative colitis (UC). Additionally, it aimed to examine the nursing diagnoses given to patients. Methods: This retrospective study analyzed the medical records of Korean pediatric patients under 18 years of age who were diagnosed with IBD and hospitalized at a children's hospital in Seoul, South Korea, from January 2017 to December 2021. Results: The number of pediatric patients diagnosed with IBD steadily increased. This finding was particularly prominent for CD patients, the majority of whom were male. Pediatric patients with CD had significantly higher rates of abdominal pain and perianal lesions, while pediatric patients with UC had a higher rate of bloody stool. Laboratory findings indicated that CD patients had higher levels of inflammatory markers and lower albumin levels than UC patients. The nursing diagnoses given during hospitalization mostly related to safety and protection, physical comfort, and gastrointestinal function. Conclusion: This study provides insights into Korean pediatric IBD patients, enabling early detection and the development of nursing intervention strategies. From a comprehensive perspective, nursing care should not only address patients' physical needs but also their psychosocial needs.

척수 손상 환자에 관한 실태 조사 (A Study of the Evaluation of the Spinal Cord Injuries)

  • The incidence of spinal cord injury increase due to traffic accident, industrial accident and leisure sports. Spinal cord injury damages motor and sensory function below the injury level, also affects autonomic functions associated with voiding and defecation. Sexual dysfunction and psychosocial, vocational maladaptations are also some of the unwanted consequences of injury. The purpose of this study is look for means to prevent and to manage complications in spinal cord injury through investigation and analysis. The subjects of this study in spinal cord injured patients were admitted to the department of physical therapy, Kwangju christian Hospital, Nam Kwang Hospital, Chun Nam university Hospital and Cho Sun university Hospital, from April, 1, 1995 to March, 31, 1996. The results are as follows: 1. The subjects comprised 96 cases of spinal cord injury, ranging from 17 to 85(mean-40.8 yrs) and included 72 males and 24 females. Among these patients, 58 were cervical injury, 20 were thoracic injury and 18 were lumbar injury. 2. As for a major causative of spinal cord injury were traffic accident(59.4%), fall down (27.1 %), and motocycle(4.2%).. 3. The bladder control were taken by indwelling cathetar(41.7%), Crede maneuver(37.5%) and self voiding(16.7%). The bowel control were taken by all aid(61.5%), assitance(32.3%) and self defecation(6.2%). 4. Possible of sexual function were 35 cases (47.9%). 5. The device of transfer used wheel chair(69.8%) and bed(16.7%). 6. The patients with higher cord lesion got more serious pain than lower cord lesion. Also the patients with higher cord lesion got a serious spasticity. 7. The incidence of decubitus ulcer among 96 patients were in case 46(47.9%). The largest group of the pressure sore sites were sacral portion(82.0%), less than 1 month of onset occured a large numbers(50%). Incidence of pressure sore by spasticity occured many patients in case of mild or moderate. Incidence of pressure sore by pain occured many patients in case of severe pain.

비만이 소아의 삶의 질에 미치는 영향 (Impact of Obesity on Health-Related Quality of Life among Children)

  • Objectives To examine the relationship between health-related quality of life (HRQoL) and body mass index (BMI) of obese children. Methods This cross-sectional study included 387 children. HRQoL was measured with the PedsQLTM 4.0 Generic Core Scale. BMI was classified according to the World Health Organization Asia-Pacific obesity guidelines. Psychosocial factors (body image, self-esteem, and depression/anxiety) were also measured. Data were analyzed using one-way analysis of variance, Pearson's correlation and Path analysis. Results Obese participants reported lower score for physical evaluation, self-esteem, appearance evaluation component of body image, total HRQoL score, and physical/emotional/social function components of HRQoL ; they reported higher score for depression/anxiety. In addition, results indicated that as BMI increased, appearance evaluation, total HRQoL score, and social function component of HRQoL dropped. Path analysis revealed that BMI did not directly affect HRQoL ; however, BMI directly affects body image and self-esteem, indirectly mediates depression/anxiety, and thereby impacts on an individual's HRQoL. Conclusions Body image and self-esteem, rather than BMI itself, have more influence on HRQoL. This reinforces the importance of therapeutic intervention to enhance body image and self-esteem among obese children.
