Percutaneous ultrasound-guided thrombin injection is effective even in infants with external iliac artery pseudoaneurysms

  • Iatrogenic pseudoaneurysms are extremely rare in children. Conventional management of pseudoaneurysms in adults has included surgical repair, ultrasound-guided compression, and more recently, endovascular embolization. However, in infants and children, there is little information regarding the applicability of such treatment modalities, which have been effective in adults, because of its rarity. Here, we present the case of a 6-month-old infant who developed a postprocedural pseudoaneurysm of the external iliac artery, which was successfully treated with ultrasound-guided percutaneous thrombin injection.

  • A 49-year-old female presented with severe dyspnea. She was diagnosed with cardiac tamponade combined with ascending aortic pseudoaneurysm and rupture, which was caused by Klebsiella pneumoniae infection. This extremely rare condition was managed by an emergency pericardiostomy and two separate aortic operations. Antibiotics active for the K. pneumoniae isolate were used throughout. The patient was well for nine months after discharge and continues to be followed up for signs of possible reinfection.

Traumatic Aneurysm of the Callosomarginal Artery-Cortical Artery Junction from Penetrating Injury by Scissors

  • Traumatic intracranial aneurysms (TICAs) are rare and are associated with high rates of morbidity and mortality. TICAs usually result from head injuries caused by traffic accidents, industrial accidents, or gunshots. We report a traumatic aneurysm of the callosomarginal artery-cortical artery junction arising from a penetrating injury by scissors. A 51-year-old woman was admitted to our hospital after suffering a penetrating injury caused by scissors. Computed tomography (CT) and CT-angiography demonstrated a right orbital roof fracture, subarachnoid hemorrhage, frontal lobe hemorrhage, intraventricular hemorrhage, and a traumatic aneurysm of the right callosomarginal artery-cortical artery junction. We trapped the traumatic aneurysm and repositioned a galeal flap. Postoperative CT showed a small infarction in the left frontal lobe. Follow-up angiography two months later showed no residual aneurysm. We suggest that an aggressive surgical intervention be performed whenever TICA is diagnosed.

  • Aortobronchial fistula (ABF) involves the formation of an abnormal connection between the thoracic aorta and the central airways or the pulmonary parenchyma and is associated with an increased risk of mortality. An ABF typically manifests clinically with symptoms of hemoptysis, and currently, there is a lack of defined guidelines for its treatment. Here, we report the cases of two patients who suffered from recurrent hemoptysis due to ABF with pseudoaneurysm. We propose that removal of the aorta with concomitant lung resection and coverage of the aorta using the pericardial membrane is a definite treatment to lower recurrence of ABF and persistent infection.

  • Undifferentiated pleomorphic sarcoma (UPS) arising from the descending thoracic aorta is a rare type of tumor. To our knowledge, only a few cases have been reported in the literature. We present computed tomography (CT) and magnetic resonance imaging findings of a 43-year-old male patient with undifferentiated pleomorphic sarcoma of the descending thoracic aorta, which showed enhancement on only magnetic resonance imaging (MRI). MRI with contrast enhancement may be useful in differentiating an aortic tumor from atherosclerotic disease.

  • The spleen is the most commonly injured organ after blunt abdominal trauma. Nonoperative management (NOM) is the standard treatment for blunt splenic injuries in haemodynamically stable patients without peritonitis. Complications of NOM include rebleeding, new pseudoaneurysm formation, splenic abscess, and symptomatic splenic infarction. These complications hinder the NOM of patients with blunt splenic injuries. We report a case in which a large haemorrhagic fluid collection that occurred after angio-embolisation was resolved by percutaneous drainage in a patient with liver cirrhosis who experienced a blunt spleen injury.

Concurrent Intracranial and Extracranial Arterial Aneurysms: Report of Three Cases

  • Systemic multiple aneurysms are rare, and an association between intracranial and visceral arterial or abdominal aortic aneurysm in the same patient is a very rare occurrence. We report herein three such cases. In one case, aneurysms of the right internal carotid artery(ICA) and the right middle cerebral arterial bifurcation(MCAB) coexisted with the inferior pancreaticoduodenal arterial pseudoaneurysm and two ileal arterial aneurysms. In another case, the patient had the A-com arterial aneurysm and the right renal arterial aneurysm. And in the other patient, he had the right vertebral artery dissecting aneurysm with the abdominal aortic aneurym. Initially, all patients were referred to our hospital with subarachnoid hemorrhage(SAH), and thereafter first two patients developed visceral arterial aneurysm rupture in the course of hospital stay and in the last patient, the abdominal aortic aneurysm was detected incidentally during carotid angiogram for Guglielmi detachable coil(GDC) embolization of vertebral dissecting aneurym. After thorough review of our cases together with pertinent literatures, we emphasize the possibility of underlying extracranial aneurysms in ruptured intracranial arterial aneurysm patient and it's uncommon but fatal complication.

대동맥장루 -1예 보고- (Aortoenteric Fistula -one case report-)

  • 이차성 대동맥장루는 아주 드물며 대동맥-장골동맥의 재건수술의 중대한 합병증으로 발생할 수 있다. 이 질환은 진성 대동맥장루와 paraprosthetic enteric fistula 로 분류할 수 있다. 이러한 합병증의 예방, 진단 및 치료는 추구해야할 문제로 되어 있다. 염증성 가성동맥류로 복부대동맥 및 장골동맥을 인조혈관으로 치환한 후 41개월에 내시경으로 Paraprosthetic enteric Fistula로 진단되어 수술을 시행한 39세 남자환자를 치험하였기에 보고한다.

흉부둔상에 의한 무명동맥 파열 -치험 1례- (Innominate Artery Ruplure Caused by Blunt Chest Trauma -A Case Report)

  • 무명동맥은 길이가 짧고 흉곽에 의해 잘 보호되기 때문에 흉부둔상에 의한 무명동맥의 손상은 매우 드물다. 본 증례는 흉부둔상을 수상한 37세의 남자에서 발생한 무명동맥 파열로 흉부W와 대동맥 조영술로 진단되었다. 정중흉골절개를 통한 응급 수술을 시행한 바 무명동맥의 근위부에 길이와 폭이 각각 3 cm의 가성동맥류를 형성하고 있었고 혈관 내피의 완전파열을 관찰할 수 있었다 무명동맥의 근위부를 대동맥궁 쪽에서 폐쇄시키고 손상된 부위를 절제한 후 10 m Gore-tex를 이용하여 대동맥-무명동맥 우회술을 실시하였다. 수술후 환자는 20일째에 신경학적 합병증없이 퇴원하였고 양측상지의 혈압도 동일하게 측정되었다.

  • 흉부 둔상으로 인한 무명동맥 손상은 드문 것으로 보고되고 있다. 본원에서는 교통사고로 인한 흉부 둔상으로 발생한 40세 남자 환자의 무명동맥 박리 및 가성 동맥류를 흉부 전산화 단층촬영으로 진단하여 상행대동맥과 무명동맥 간에 인조혈관 우회로 조성술로 성공적으로 치료하였다. 환자는 술 후 별다른 합병증 없이 1주일만에 퇴원할 수 있었으며 현재 외래 추적 중이다.
