Purpose : Ovarian dysgerminoma is a highly radiosensitive malignant tumor occurring in young age group. The conventional treatment was total abdominal hysterectomy and bilateral salpingo-oophorectomy followed by radiotherapy. We retrospectively analyzed the treatment results of Patients who had received radiotherapy in the era before chemotherapy was widely used. Material and Method : Twenty two patients with ovarian dysgerminoma were treated at the Department of Therapeutic Radiology, Seoul National University Hospital between August, 1980 and May, 1991. Four patients were excluded from this study, because three patients received incomplete treatment and one received combined chemotherapy. Sixteen patients received postoperative radiotherapy and two patients had radical radiotherapy as tumor was unresectable. Median follow-up period was 99 months (range, 51-178) Median age was 22 years (range, 11-42). Among the postoperatively treated patients, three Patients were in stage IA, eight in stage IC, two in stage II, and three in stage III. One patient had Turner's syndrome. Radiotherapy was performed with high energy photon (telecobalt unit or linear accelerator, either 6MV or 10MV), The radiation dose to the whole abdomen was 1950-2100cGy (median, 2000) and 1050-2520cGy was added to the whole pelvis, the total dose to the whole pelvis was 3000-4500cGy (median, 3500). Prophylactic Paraaortic area irradiation was done in six Patients (dose range, 900-1500cGy). One patient who had positive Paraaortic node, received radiation dose of 1620cGy, followed by additional 900cGy to the gross mass with shrinking field. Total dose to the paraaortic node was 4470cGy. Six patients, including one who had paraaortic node metastasis, received Prophylactic irradiation to mediastinum and supraclavicular area (2520cGy). Of the two patients with unresectable tumors who received radical radiotherapy, one was in stage III and the other was in stage IV with left supraclavicular lymph node metastasis. The stage III patient received radiation to the whole abdomen (2000cGy), followed by boost to whole pelvis (2070cGy) and paraaortic area (2450cGy). Stage IV patient received radiation to the whole abdomen (2000cGy), followed by radiation to the whole pelvis and paraaortic area (2400cGy), mediastinum (2520cGy) , and left supraclavicular area (3550cGy) .Results : The 5 year local control rate was $100\%$ in patients who received postoperative adiuvant radiotherapy after total abdominal hysterectomy and bilateral salpingo-oophorectomy. Only one patient in stage III who did not receive prophylactic irradiation to mediastinum developed mediastinal metastasis. but was salvaged by chemotherapy. So. the 5 year overall survival rate uras also $100\%$. Two patients who received radiation only, are alive without disease at 112 and 155 months. Conclusion : Postoperntive adjuvant radiotherapy as well as radical radiotherapy in unresectable ovarian dysgerminoma was very effective. aut chemotherapy is also an effective treatment modality We now recomrneifd chemotherapy for Patients who need to save their ovarian functien and reserve radiotherapv fov chemo-resistant tumor or recurrence alter che motherapy.
The adjuvant effect of a muramyl dipeptide (MDP) derivative, MDP-Lys(L18), on enhancing of antitumor immunity induced by X-irradiated tumor cells against highly metastatic B16-BL6 melanoma cells was examined in mice. Mice immunized intradermally (i.d.) with a mixture of X-irradiated B16-BL6 cells and MDP-Lys (L18) [Vac+MDP-Lys (L18)] followed by an intravenous (i.v.)inoculation of $10^4$ viable tumor cells 7 days after immunization, showed a significant inhibition of experimental lung metastasis of B16-BL6 melanoma cells. The most effective immunization for the prophylactic inhibition of tumor metastasis was obtained from the mixture of $100{\;}\mu\textrm{g}$ of MDP-Lys (L18) and $10^4$ X-irradiatied tumor vaccine. Furthermore, immunization of mice with Vac+MDP-Lys(L18), 3 days after tumor challenge, resulted in a significant inhibition of lung metastasis of B16-BL6 melanoma cells in an experimental lung metastasis model. Similarly, the administration of Vac+MDP-Lys(L18), 1 or 7 days after tumor removal, markedly inhibited tumor metastasis of B16-BL6 in a spontaneous lung metastasis model. When Vac+MDP-Lys (L18) was i.d. administered 3 days after subcutaneous (s.c.) inoculation of tumor cells ($5{\times}10^5/site$) on the back, mice treated with Vac+MDP-Lys(L18) showed inhibition of significantly tumor growth on day 20. These results suggest that MDP-Lys (L18) is able to enhance antitumor activity induced by X-irradiated tumor vaccine to reduce lung metastasis of tumor cells, and is a potent immunomodulating agent which may be applied prophylactically as well as therapeutically to treatment of cancer metastasis.
A study was conducted to monitor the changes in serum protein profile, cholesterol and blood glucose during endotoxic shock in buffalo calves and also to assess the role of prophylactic supplementation of vitamin E and selenium in alleviating the endotoxic effects. Fifteen male buffalo calves (6-8 months of age) were divided into three groups: Group I (control)-infused with 0.9% saline solution; Group II-infused with E. coli endotoxin at 5${\mu}g/kg$ body weight in normal saline solution; Group III- supplemented prophylactically with 250 mg vitamin E and 7.5 mg selenium by i/m injections at weekly intervals for one month prior to the induction of endotoxic shock. The blood samples were collected at 0, 1, 3, 6, 9, 12, 24, 48 and 72 h after the induction of shock. Endotoxin caused a significant (p<0.05) hypoproteinemia from 3-12 h post infusion in group II but this hypoproteinemia was less pronounced and only from 3-9 h post infusion in vitamin E and selenium supplemented calves. Hypoglycemia was observed in group II from 3-24 h and blood glucose level returned to normal at 72 h. However hypoglycemia was mild in group III and blood glucose returned to normal at 48h. Hypocholesterolaemia and hypoalbuminemia were found in both groups II and III but these changes were less pronounced in group III i.e. vitamin E and Se supplemented calves. Serum electrophoretic protein patterns of group III were quite similar to those of control group but animals of group II had different electrophoretic pattern. It was concluded that the antioxidant effects of vitamin E and Se prevent the liver against oxidative stress during endotoxic shock.

Distal caries of the second molar is common indication for the mandibular third molar surgery and there are no universally acceptable predictive criteria for distal caries of the second molar. To analyze the correlation of the distal caries of the second molar and the eruption state of the mandibular third molar using panoramic radiographs statistically and propose the acceptable guideline for preventive extraction of the mandibular third molar. 786 patients who were extracted the mandibular third molar from 2002 to 2006 at Samsung medical center were examined. The presence and absence of distal caries of mandibular second molar, age, gender, angulation, impaction degree, distance between distal cementoenamel junction of the second molar and mesial cementoenamel junction of the mandibular third molar were assessed. 79.6% of third molars had a mesial angulation of between $40^{\circ}$ and $80^{\circ}$. The mean age of third molar removal for distal caries of second molar was $33.86{\pm}9.81$. The prophylactic removal of a mesio-angular third molar about $40^{\circ}$ and $80^{\circ}$ could prevent distal cervical caries forming in the mandibular second molar.

Although schistosomicidal drugs and other control measures exist, the advent of an efficacious vaccine remains the most potentially powerful means for controlling this disease. In this study, native fatty acid binding protein (FABP) from $Fasciola$$gigantica$ was purified from the adult worm's crude extract by saturation with ammonium sulphate followed by separation on DEAE-Sephadex A-50 anion exchange chromatography and gel filtration using Sephacryl HR-100, respectively. CD1 mice were immunized with the purified, native $F.$$gigantica$ FABP in Freund's adjuvant and challenged subcutaneously with 120 $Schistosoma$$mansoni$ cercariae. Immunization of CD1 mice with $F.$$gigantica$ FABP has induced heterologous protection against $S.$$mansoni$, evidenced by the significant reduction in mean worm burden (72.3%), liver and intestinal egg counts (81.3% and 80.8%, respectively), and hepatic granuloma counts (42%). Also, it elicited mixed $IgG_1/IgG_{2b}$ immune responses with predominant $IgG_1$ isotype, suggesting that native $F.$$gigantica$ FABP is mediated by a mixed Th1/Th2 response. However, it failed to induce any significant differences in the oogram pattern or in the mean granuloma diameter. This indicated that native $F.$$gigantica$ FABP could be a promising vaccine candidate against $S.$$mansoni$ infection.

Organophosphorus compounds are irreversible inhibitors of cholinesterase enzyme. Exposure causes a progression of toxic signs, including hypersecretion, tremor, convulsion, respiratory distress, epileptiform seizure, brain injuries and death. To protect brain injuries, administration of diazepam as a neuroprotectant is now considered essential for severely exposed nerve agent casualties. However, studies have shown diazepam to provide less than total protection against the neuropathological consequences of nerve agent exposure. In this context, extensive studies have been carried out to find out effective alternative drugs to protect brain from epileptiform seizures induced by organophosphate compounds intoxication. It has been reported that a combination of carbamate and anticholinergic or antiglutamatergic can be a very effective medical countermeasure in dealing with the threat of organophosphorous poisoning. In this study, experimental animals including rats and guinea pigs were implanted with microelectrodes on their brain sculls, and treated with various centrally acting drugs such as physostigmine and procyclidine prior to soman challenge, and then its electroencephalography(ECoG) was monitored to see anticonvulsant effects of the drugs. It was found that seizure activities in ECoG were not always in proportion to clinical signs induced by soman intoxication, and that combinative pretreatment with physostigmine plus procyclidine effectively stopped the seizures induced by organophosphorous poisoning.
Immunogenicity of Escherichia coli pilus and LT were evaluated in 20-week-old hens. The antigens were consisted of K88, K99, 987p pilus and heat labile toxin purified from enterotoxigenic Escherichia coli. The durations of antibody titers in sera and egg yolk were investigated by an enzyme-linked immunosorbent assay(ELISA). After first inoculation, antibody titers in sera reached at peak 2 weeks postinoculation. However, peak antibody titers in egg yolk were detected 4 weeks postinoculation, indicating that transfer of immunoglobulin from serum to egg yolk took about two weeks period. Although there were slight reduction in titers, the specific antibodies in egg yolk lasted up to 3 months. Immune responses against monovalent and combined antigens were showed as almost same patterns. The transfer rate of antibodies from serum to egg yolk didn't show any significant differences among three pilus antigens in this study. Considering the concentrations of antigens in each inoculated group, multivalent antigens containing heat labile toxin of E coli were found to be more immunogenic than monovalent antigen in producing specific antibodies. From this experiment, it was demonstrated that multivalent antigens containing three pilus and heat labile toxin could be a promising candidate for the production of egg yolk antibodies for prophylactic use in preventing swine colibacillosis in future.
In volleyball, the most common injuries are anterior cruciate ligament (ACL) tears. For this reason, volleyball players frequently use knee brace as prophylactic and rehabilitation measures. The purpose of the study was to investigate the effects of knee brace on anterior cruciate ligament injuries risk factors during spike take off in female volleyball players. Fifteen female volleyball players were recruited and performed randomly spike take off with and without knee brace. Kinematics and ground reaction data were collected to estimate the anterior cruciate ligament injuries risk factors. The ACL risk factors are knee maximum flexion angle, thigh maximum adduction angle, thigh maximum internal rotation angle, shank maximum abduction angle, shank maximum external rotation angle, knee maximum extension moment and knee maximum abduction moment. Data were analyzed with paired samples t-test with Bonfferoni collection. Female volleyball players with knee brace had no significant results in knee maximum flexion angle, thigh maximum adduction angle, thigh maximum internal rotation angle, shank maximum abduction angle and shank maximum external rotation angle compare to without knee brace. Female volleyball players, however, with knee brace showed more reduced knee maximum extension moment and knee maximal abduction moment than without knee brace. In conclusion, Female volleyball players with knee brace reduced anterior cruciate ligament stress.

Background In recent years, breast implants have been frequently placed in the subcutaneous pocket, in the so-called prepectoral approach. We report our technique of prepectoral implant-based breast reconstruction (IBR), as well as its surgical and aesthetic outcomes, in comparison with subpectoral IBR. We also discuss relevant considerations and pitfalls in prepectoral IBR and suggest an algorithm for the selection of patients for IBR based on our experiences. Methods We performed 79 immediate breast reconstructions with a breast implant and an acellular dermal matrix (ADM) sling, of which 47 were subpectoral IBRs and 32 were prepectoral IBRs. Two-stage IBR was performed in 36 cases (20 subpectoral, 16 prepectoral), and direct-to-implant IBR in 43 cases (27 prepectoral, 16 subpectoral). The ADM sling supplemented the inferolateral side of the breast prosthesis in the subpectoral group and covered the entire anterior surface of the breast prosthesis in the prepectoral group. Results The postoperative pain score was much lower in the prepectoral group than in the subpectoral group (1.78 vs. 7.17). The incidence of seroma was higher in the prepectoral group (31.3% vs. 6.4%). Other postoperative complications, such as surgical site infection, flap necrosis, implant failure, and wound dehiscence, occurred at similar rates in both groups. Animation deformities developed in 8.5% of patients in the subpectoral group and rippling deformities were more common in the prepectoral group (21.9% vs. 12.8%). Conclusions The indications for prepectoral IBR include moderately-sized breasts with a thick well-vascularized mastectomy flap and concomitant bilateral breast reconstruction with prophylactic mastectomy.
Peritoneal metastasis (PM) frequently occurs in patients with gastric cancer (GC) and confers a dismal prognosis despite advances in systemic chemotherapy. While systemic chemotherapy has poor peritoneal penetration, intraperitoneal (IP) chemotherapy remains sequestered, resulting in high peritoneal drug concentrations with less systemic side-effects. The first application of IP treatment was hyperthermic intraperitoneal chemotherapy (HIPEC) with cytoreductive surgery (CRS) for gastric cancer peritoneal metastasis (GCPM); but was associated with an increased morbidity and mortality rate without significantly improving overall survival (OS). While CRS confers limited benefit, the potential role of prophylactic HIPEC and laparoscopic neoadjuvant HIPEC are currently being evaluated. Combination systemic and IP chemotherapy (SIPC) gained popularity in the 1990s, since it provided the benefits of IP treatment while reducing surgical morbidity, demonstrating promising early results in multiple Phase II trials. Unfortunately, these findings were not confirmed in the recent PHOENIX-GC randomized controlled trial; therefore, the appropriate treatment for GCPM remains controversial. Small observational studies from Japan and Singapore have reported successful downstaging of PM in GC patients receiving SIPC who subsequently underwent conversion gastrectomy with a median OS of 21.6-34.6 months. Recently, the most significant development in IP-directed therapy is pressurized IP aerosol chemotherapy (PIPAC). Given that aerosol chemotherapy achieves a wider distribution and deeper penetration, the outcomes of multiple ongoing trials assessing its efficacy are eagerly awaited. Indeed, IP-directed therapy has evolved rapidly in the last 3 decades, with an encouraging trend toward improved outcomes in GCPM, and may offer some hope for an otherwise fatal disease.
