Studies on the Treatment and Prevention of Dementia by Green-Tea extracts (녹차(綠茶)추출물에 의한 치매 치료 및 예방에 관한 연구)

  • Alzheimer's disease (AD) is characterized by amyloid deposition and associated loss of neunons in brain regions involved in learning and memory processes. Several causes of evidence support that the congnitive disturbance is closed associated with the deficit of cerebral acetylcholine neurotransmission, and the effect of carboxyl terminal 105 amino acid fragment (CT105) of the amyloid precursor protein (APP) on the gene expression of proinflammatory cytokines. We tested it on the scopolamine-induced amnesia model of the ICR mouse using the Morris water maze with repeated orally administration of 1st Green-Tea extract (200 mg/kg) and 2nd Green-Tea extract (200 mg/kg). The Green-Tea prevents impairment of learning and memory and neuronal loss in mouse models of cognitive disturbance and it demonstrated selectivity for inhibition of acetylcholinesterase (AChE). Furthermore, the repeated administration of Green-Tea, CT105-induced alzheimer's mouse model showed central cholinergic activity by ameliorates learning and memory impairment, and isolation of CD14 microglia showed significantly decreases intracellular release of the proinflammatory cytokines tumor necrosis factor-${\alpha}$, interleukin-$1{\beta}$ and reactive oxygen species (ROS). Because of its composite profile, oral therapeutic index and a prophylactic, Green-Tea is considered the better therapeutic candidate for the treatment of Alzheimer's disease.

  • The aim of the double blind test was to assess the prophylactic and therapeutic efficacy of the preparation GINSANA, containing the standardized ginseng extract PHARMATON G115,. The determination was carried out with special emphasis on the following features : general physical condition, physical performance, mental performance, enjoyment of life/mood, concentration and memory, as well as sleeping habits. Sixty test persons took part in the study, men and women between the ages of 22 and 80. The 90-day test was carried out in the form of a double blind experiment. Experimental measurements were made and the persons were also questioned. The reaction time, the optical merging threshold, the coordination of both hands and the recovery quotients, as well as the recovery rates were analyzed. The results for the serum group were clearly better than those for the placebo group, particularly for the characteristics: general physical condition, physical performance and sleeping habits. The results of the test methods used, especially with regard to the reaction time, the coordination of both hands, the recovery quotient and the recovery period, permit the following conclusion to be drawn: when administered for several weeks, GINSANA has a positive action, in the sense of an activation of the entire personality by the ginseng glycosides contained in the standardized Extracts G115.

Emphysema following air-powder abrasive treatment for peri-implantitis

  • Background: Subcutaneous emphysema refers to swelling caused by the presence of air or gas in the interstices of loose connective tissue. In the head and neck area, it may follow the fascial planes and is characterized by sudden swelling, crepitus on palpation, infrequent pain, and air emboli on radiography. It usually occurs as a complication in dental treatment. Some reports have described subcutaneous emphysema caused by dental procedures; however, severe emphysema related to peri-implantitis after treatment has not been documented. Accordingly, the current report describes a rare case of subcutaneous cervical emphysema resulting from the use of an air-powder abrasive device to treat peri-implantitis. Case presentation: Based on a review of the existing literature and the present case, nine cases of subcutaneous emphysema due to air-powder abrasive device have been reported. In most cases, the emphysema resolved over time after treatment with prophylactic antibiotics; among these, two were related to peri-implantitis management. Conclusion: Considering the frequent use of air-powder abrasive devices to treat peri-implantitis, the potential risk of iatrogenic emphysema related to this procedure needs to be addressed more extensively.

Fluoride penetration from three orthodontic adhesives: an experimental study

  • Objective: To examine the prophylactic potential of 3 orthodontic bonding adhesives: Fuji Ortho SC, Illuminate, and Resilience. Methods: Thirty-six Wistar Wag rats were randomly divided into 4 groups consisting of 9 rats each. One of the groups received no treatment and was used as a control. In the other groups, individual bands coated with one of the 3 adhesives were cemented to the lower incisors. Enamel samples were obtained after 6 and 12 weeks and analyzed using scanning electron microscopy in combination with energy dispersive spectrometry. Results: Six weeks after band cementation, no fluoride was found in the enamel of the lower incisors. After 12 weeks, there was no fluoride in the enamel of teeth coated with the Resilience composite. However, in the case of the Illuminate composite and the resin-modified glass ionomer Fuji Ortho SC cement, the depth of fluoride penetration reached $2{\mu}m$ and $4.8-5.7{\mu}m$, respectively. Conclusions: Fluoride ions from orthodontic adhesives can be incorporated into the surface layer of the enamel. Orthodontists may apply orthodontic adhesives, such as the Fuji Ortho SC, to reduce the occurrence of caries during orthodontic treatment with fixed appliances.

Ginseng alleviates microbial infections of the respiratory tract: a review

  • The detrimental impact of air pollution as a result of frequent exposure to fine particles posed a global public health risk mainly to the pulmonary disorders in pediatric and geriatric population. Here, we reviewed the current literature regarding the role of ginseng and/or its components as antimicrobials, especially against pathogens that cause respiratory infections in animal and in vitro models. Some of the possible mechanisms for ginseng-mediated viral inhibition suggested are improvements in systemic and mucosa-specific antibody responses, serum hemagglutinin inhibition, lymphocyte proliferation, cell survival rate, and viral clearance in the lungs. In addition, ginseng reduces the expression levels of proinflammatory cytokines (IFN-γ, TNF-α, IL-2, IL-4, IL-5, IL-6, IL-8) and chemokines produced by airway epithelial cells and macrophages, thus preventing weight loss. In case of bacterial infections, ginseng acts by alleviating inflammatory cytokine production, increasing survival rates, and activating phagocytes and natural killer cells. In addition, ginseng inhibits biofilm formation and induces the dispersion and dissolution of mature biofilms. Most clinical trials revealed that ginseng, at various dosages, is a safe and effective method of seasonal prophylaxis, relieving the symptoms and reducing the risk and duration of colds and flu. Taken together, these findings support the efficacy of ginseng as a therapeutic and prophylactic agent for respiratory infections.

Erysipelas of the Upper Extremity Following Surgical Therapy for Breast Cancer (유방암 치료 후 발생한 상지의 단독)

  • Purpose: Erysipelas is a bacterial infection of the dermis and hypodermis, mostly of streptococcal origin, and erysipelas of upper extremity following breast cancer treatment has never been reported in the Korean literature. Methods: 39-year-old female presented to our hospital complaining of fever and painful swelling of her left upper extremity. She had a history of breast cancer and was treated with breast conserving surgery with axillary lymph node dissection, chemotherapy, and radiation. On physical examination, her left upper extremity showed vesicle, bullae, local heatness and erythema with well-defined margin. With these distinctive features of a skin lesion, we gave a diagnosis of erysipelas and started treatment with intravenous antibiotics. Results: Resolution of the signs and symptoms of erysipelas occurred after 7 days of treatment. Conclusion: The diagnosis of erysipelas with distinctive feature of skin lesion is essential and we emphasize that the prevention of any trauma are very important in these patients for prophylactic measures.

A Clinical Review of the Cause of the Primary Spontaneous Pneumothorax (원발성 자연기흉의 원인에 대한 고찰)

  • By the early half of the 20th century, the most common cause of spontaneous pneumothorax was considered to be tuberculosis. But recently ruptures of the subpleural emphysema and/or blebs are considered as the major causes of spontaneous pneumothorax. To evaluate the causes of spontaneous pneumothorax, the authors reviewed the 96 patients who were consecutively diagnosed and treated by thoracotomy for spontaneous pneumothorax at the Department of Thoracic and Cardiovascular Surgery, Seoul Advenist Hospital, from May, 1988 to April, 1993. The patients who had clinical and radiological evidence of tuberculosis or other specific parenchymal lung diseases were excluded from this review. All patients were negative for tuberculous bacilli on sputum studies. The pathologic results were as follows : subpleural emphysema[25], blebs[27], subpleural emphysema and blebs[25], pleural fibrosis[10], tuberculosis[8], and parasitic granuloma[1]. Three of the patients who were diagnosed to be tuberculosis by pathologic findings were progressed to active pulmonary tuberculosis on the follow-up chest PA films. The authors conclude that all patients with spontaneous pneumothorax must be evaluated periodically for tuberculosis and that patients who were diagnosed to be tuberculosis by postoprative pathologic report need the administration of the prophylactic antituberculous drug, because the prevalence of tuberculosis remains relatively high rate in our country.

Effects of Gonadotropin-Releasing Hormone on Reproductive Performance of Early Postpartum Dairy Cows and Cystic Cows (Gonadotropin-Releasing Hormone의 투여(投與)가 산욕기(産褥期)의 유우(乳牛)와 난소낭종유우(卵巢囊腫乳牛)의 번식효율(繁殖效率)에 미치는 영향(影響))

  • Holstein-Friesian cows(n=284) were given $100{\mu}g$ of gonadotropin-releasing hormone(GnRH) or saline solution by intramuscular injection at 10 to 22 days after parturition, and were investigated their reproductive performance and frequency of ovarian cysts. Among them 28 cystic cows were injected with $150{\mu}g$ of GnRH intramuscularly and examined the recovery rate. The results obtained in this study were summarized as follows: 1. The interval from calving to 1st ovulation was reduced from 28.2 days in controls to 16.5 days for cows given GnRH (p<0.01). 2. The intervals from calving to 1st estrus and from calving to conception were extended significantly in control group (p<0.05). 3. Inseminations per conception and conception rate at 1st insemination did not reveal difference between two groups. 4. Frequency of ovarian cysts was reduced from 14.0% in control to 4.20% for cows given GnRH (p<0.05). 5. Of the 28 cystic cows receiving $150{\mu}g$ of GnRH, 23(82.1%) responded to 1st treatment and returned to estrus $24.2{\pm}4.3$ days after treatment. 6. These data provide evidence for reduction in infertility and reproductive disorders in early postpartum dairy cows given GnRH as a prophylactic.

Food Allergy, a Newly Emerging Food Epidemic: Is the Current Regulation Adequate?

  • Food allergy refers to an immunologically mediated adverse reaction to food, mainly to proteinaceous constituents. Health implications vary between those individuals who experience mild physical discomforts to those with fast-acting, life-threatening anaphylactic reactions. The prevalence of food allergy is higher in children than in adults, estimated around 4-8% and 1-2% respectively in developed countries. Food allergy has no effective cure at the present time and total avoidance of causative foods is the most reliable prophylactic method currently recommended by the medical community. To help food allergic patients to make informed choices of their foods, mandatory labeling of selected food allergens has been introduced in several countries. All food allergen labelling provisions specify a set of allergens common to the regulated countries. Policy divergence, however, exists between countries by inclusion of additional allergens unique to specific countries and enforcement of specific labelling requirements. Such variations in food allergen labelling regulations make it difficult to manage allergen labeling in imported pre-packaged food products. This paper addresses two current issues in food allergen regulation: 1) an urgent need to determine true prevalence of food allergy in the Asia-Pacific region. This will enable refinement to the food allergen regulation to be more country-specific rather than simply adopting CODEX recommendations. 2) There is an urgent need for harmonization of food allergen regulation in order to prevent food allergen regulation becoming a trade barrier.
