Clinical Investigation of 11 Cases of Chronic Eosinophilic Pneumonia Reported in Korea (국내에서 보고된 만성 호산구성 폐렴 11예에 대한 임상적 고찰)

  • Background: Chronic eosinophilic pneumonia(CEP) presents with profound systemic symptoms, including fever, malaise, night sweats, weight loss, and anorexia together with localized pulmonary manifestations such as cough, wheeze, and sputum. It is an illness occurring predominantly in women. The chest radiogragh shows fluffy opacities that often have a characteristic peripheral configuration. The hallmark of CEP is the peripheral blood eosinophilia and a prompt response to oral corticosteroid therapy. We investigated characteristics of eleven patients of chronic eosinophilic pneumonia, reported in Korea. Method: There were eleven reports of CEP from 1980 to 1996, including three cases experienced in our hospital. The journals were analysed in respects of clinical history, laboratory, and radiographic findings. Results: 1) Male vs. female ratio is 3 : 8. The peak incidence occurred in forty and fifty decades. The atopic diseases were present in 6 cases. Asthma was the commonest manifestation 2) The presenting symptoms were as follows: cough, dyspnea, sputum, weight loss, fever, general weakness, night sweats, urticaria with the descending incidence. 3) Peripheral blood eosinophilia was present in all patients(mean ; 38.4%) and serum IgE level was elevated in nine patients(mean ; 880IU/ml). Conclusion: The diagnosis of chronic eosinophilic pneumonia is based on classic symptoms, including fever, night sweats, weight loss with a typical roentgenogram of peripheral pulmonary infiltrates and peripheral blood eosinophilia, and that is confimed by lung biopsy and/or bronchoalveolar lavage. Chronic eosinophilic pneumonia is responsive to corticosteroid promptly and recommended at least 6 months of therapy to prevent relapse.

An Empirical Study on the Effect of CRM System on the Performance of Pharmaceutical Companies (고객관계관리 시스템의 수준이 BSC 관점에서의 기업성과에 미치는 영향 : 제약회사를 중심으로)

  • Facing a complex environment driven by a decade, many companies are adopting new strategic frameworks such as Customer Relationship Management system to achieve sustainable profitability as well as overcome serious competition for survival. In many business areas, CRM system advanced a great deal in a matter of continuous compensating the defect and overall integration. However, pharmaceutical companies in Korea were slow to accept them for usesince they still have a tendency of holding fast to traditional way of sales and marketing based on individual networks of sales representatives. In the circumstance, this article tried to empirically address current status of CRM system as well as the effects of the system on the performance of pharmaceutical companies by applying BSC method's four perspectives, from financial, customer, learning and growth and internal process. Survey by e-mail and post to employers and employees who were working in pharma firms were undergone for the purpose. Total 113 cases among collected 140 ones were used for the statistical analysis by SPSS ver. 15 package. Reliability, Factor analysis, regression were done. This study revealed that CRM system had a significant effect on improving financial and non-financial performance of pharmaceutical companies as expected. Proposed regression model fits well and among them, CRM marketing information system shed the light on substantial impact on companies' outcome given profitability, growth and investment. Useful analytical information by CRM marketing information system appears to enable pharmaceutical firms to set up effective marketing and sales strategies, these result in favorable financial performance by enhancing values for stakeholderseventually, not to mention short-term profit and/or mid-term potential to growth. CRM system depicted its influence on not only financial performance, but also non-financial fruit of pharmaceutical companies. Further analysis for each component showed that CRM marketing information system were able to demonstrate statistically significant effect on the performance like the result of financial outcome. CRM system is believed to provide the companies with efficient way of customers managing by valuable standardized business process prompt coping with specific customers' needs. It consequently induces customer satisfaction and retentionto improve performance for long period. That is, there is a virtuous circle for creating value as the cornerstone for sustainable growth. However, the research failed to put forward to evidence to support hypothesis regarding favorable influence of CRM sales representative's records assessment system and CRM customer analysis system on the management performance. The analysis is regarded to reflect the lack of understanding of sales people and respondents between actual work duties and far-sighted goal in strategic analysis framework. Ordinary salesmen seem to dedicate short-term goal for the purpose of meeting sales target, receiving incentive bonus in a manner-of-fact style, as such, they tend to avail themselves of personal network and sales and promotional expense rather than CRM system. The study finding proposed a link between CRM information system and performance. It empirically indicated that pharmaceutical companies had been implementing CRM system as an effective strategic business framework in order for more balanced achievements based on the grounded understanding of both CRM system and integrated performance. It suggests a positive impact of supportive CRM system on firm performance, especially for pharmaceutical industry through the initial empirical evidence. Also, it brings out unmet needs for more practical system design, improvement of employees' awareness, increase of system utilization in the field. On the basis of the insight from this exploratory study, confirmatory research by more appropriate measurement tool and increased sample size should be further examined.

List-event Data Resampling for Quantitative Improvement of PET Image (PET 영상의 정량적 개선을 위한 리스트-이벤트 데이터 재추출)

  • Multimodal-imaging technique has been rapidly developed for improvement of diagnosis and evaluation of therapeutic effects. In despite of integrated hardware, registration accuracy was decreased due to a discrepancy between multimodal image and insufficiency of count in accordance with different acquisition method of each modality. The purpose of this study was to improve the PET image by event data resampling through analysis of data format, noise and statistical properties of small animal PET list data. Inveon PET listmode data was acquired as static data for 10 min after 60 min of 37 MBq/0.1 ml $^{18}F$-FDG injection via tail vein. Listmode data format was consist of packet containing 48 bit in which divided 8 bit header and 40 bit payload space. Realigned sinogram was generated from resampled event data of original listmode by using adjustment of LOR location, simple event magnification and nonparametric bootstrap. Sinogram was reconstructed for imaging using OSEM 2D algorithm with 16 subset and 4 iterations. Prompt coincidence was 13,940,707 count measured from PET data header and 13,936,687 count measured from analysis of list-event data. In simple event magnification of PET data, maximum was improved from 1.336 to 1.743, but noise was also increased. Resampling efficiency of PET data was assessed from de-noised and improved image by shift operation of payload value of sequential packet. Bootstrap resampling technique provides the PET image which noise and statistical properties was improved. List-event data resampling method would be aid to improve registration accuracy and early diagnosis efficiency.

The Efficiency and Improvement of the Highway Wild-Life Fences for Decrease of Mammals Road-kill - In Case of Manjong~Hongchun Section on Jungang Highway - (포유류 로드킬 저감을 위한 고속도로 유도울타리 효율성 및 개선방안 연구 - 중앙고속도로 만종~홍천 구간을 사례로 -)

  • This study had targeted the Manjong Junction~Hongchun interchange section of Jungang highway in order to analyze the efficiency and improvement of the highway wild-life fences. Being analyzed wild-life fence as an effective facility for the prevention of road-kill, it had founded that the wild animals did not jump over even the two layer guardrail, concrete barrier and noise barrier, fence for rock fall. For the section with high road-kill decrease effect after installing the wild-life fence, the effect was higher at wild-life fence with the height of 1.0m where the door had not been installed and the well connected points of road facility than the area with fence height of 1.5m. The road-kill decrease effect was also high at the well managed areas even if the door has not been installed. Also, road-kill had occurred by concentrating around the end of wild-life fence after installing the fence, Moreover road-kill had also occurred around the cutting section. The door of wild-life fence had higher amount of road-kill occurrence as the installation interval was closer. it was analyzed that the door management has a lot of effect on road-kill decrease. The fence for rock fall, two layer guardrail and concrete barrier having the effect of wild-life fence installed on the road would have to be installed by connecting with wild-life fences through proper facility improvement. Although the door should not be installed if possible, it should be installed as automatic door or gravity door to prevent the door from leaving the door open. An escape route has to be formed for the prompt escape away from the road for the animals entered through the ending section of the fence. The eco-corridor has to be made by restoring the soil layer of dual purpose eco-corridor forming a planting area. Also, the dead body after the accident has to be disposed to the outer section of the road immediately in order to prevent the secondary road-kill by the predator from the road-kill. The fence has to be installed as 500m or longer in both ways, in other words 1,000m or longer, from the targeted spot of minimum accident prevention while connecting up to the bridge or box culvert, etc that are next road facilities if possible to guide wild animals safely to the eco-corridor.

Clinical Study of Pulmonary Thromboembolism (폐혈전색전증의 임상적 연구)

  • Background : Pulmonary thromboembolism is relatively frequent and potentially fatal. However, it is commonly misdiagnosed. The incidence of pulmonary thromboembolism is not decreasing despite advances in diagnosis and effective prophylatic measures. Its potential for significant sequela necessitates a prompt diagnosis and treatment. Unfortunately, there are many difficulties and problems regarding accurate diagnosis. There is a low prevalence of deep vein thrombosis and pulmonary thromboembolism in Korea and only few reports on this subject are available. Method : The clinical features of 36 patients, who were diagnosed with pulmonary thromboembolism at the Korea University medical center, were reviewed. Results : 1) There was no significant difference in prevalence between men an women, and the mean age was 50.9 years in men 59.2 years in women. 2) The frequent causes of pulmonary thromboembolism were malignancies (22.2%), surgery (22.2%), and heart disease(8.2%). Specific causes were not identified in 33.3%. 3) The most common symptom was dyspnea(72.2%), and the most common sign was tachypnea(61.1%). 4) The EKG findings were normal in 28.6%, an S1Q3T3 pulmonale pattern in 25.7%, ST or QRS changes in others. 5) The chest X-ray findings indicated pulmonary infiltration in 37.5%, cardiomegaly in 15.6%, pleural effusion in 12.5%, and normal in 27.8%. The perfusion lung scan showed a high probability in 66.7%, and intermediate or low probability in 33.3%. 6) The pulmonary arterial pressure(PAP) in the high probability groups was 57.9mmHg with a higher mortality rate(35%). Conclusion : Pulmonary thromboembolism is not uncommon in Korea and its clinical features do not differ greatly from thase reported in the literature. When pulmonary thromboemblism of unknown causes are diagnosed, a search for an occult malignancy is recommended. Rapid diagnosis and treatment are achieved when thromboemblism is suspected.

COMPARISON OF MICROLEAKAGE WITH THREE DIFFERENT ADHESIVE SYSTEMS (수 종의 복합레진 접착 시스템에서의 미세 누출의 비교)

  • Recently, self-etching adhesive system have been developed and bonding procedures simplified into one or two steps, which are simultaneously applied to both enamel and dentin. These systems are easy to use and have the potential for good clinical success. The purpose of this study is to evaluate in vitro the microleakage on the cementum/dentin and enamel walls in composite resin restoration of Class V cavities, regarding the use of different adhesive systems. 30 human premolars were divided into 3 groups. A standardized Class V preparation was prepared on the buccal and lingual surface of each premolar. The preparation were made parallel to the cementoenamel junctions, with the gingival half of the preparation extending 1mm apical to the cementoenamel junction. After adhesive system was applied to teeth as manufacture's recommendation, hybrid resin composite was filled in bulk into the preparation and light polymerized according to manufacturer's recommendations. Specimen were stored in distilled water at $37^{\circ}C$ for 5 days and thermocycled 1000 times ($5^{\circ}C{\pm}2^{\circ}C\;and\;55^{\circ}C{\pm}2^{\circ}C)$, then immersed in a 2% methylene blue solution for 12 hours. After sectioning mesio distally through the restorations, the degree of dye penetration was scored under a stereomicroscope at ${\times}\;25$ magnification. The data were analyzed statistically using t-test and one-way ANOVA. The results were as follows: ${\cdot}$ There is no adhesive system which can prevent microleakage perfectly. ${\cdot}$ There is significant difference in microleakage between enamel margin and dentin margin (p<0.0001). ${\cdot}$ In enamel margin, self-etching primer systems did not show any significant difference comparing total-etching system. In denin margin, self-etching primer systems did not show any significant difference comparing one-bottle adhesive system used in combination with total-etching.

Effect of Exercise on Blood Concentrations of Ethanol, Lactate and Glucose in Men Showing Facial Flush after Ethanol Ingestion (음주후(飮酒後) 얼굴 붉어지는 사람에 있어서 운동(運動)이 혈중(血中) 에타놀, 유산(乳酸) 및 포도당(葡萄糖) 농도(濃度)에 미치는 효과(效果))

  • To elucidate the effect of exercise on blood concentrations of ethanol, lactate and glucose in men who show facial flush after ethanol ingestion, 59 healthy male college students were studied. After 6 or more hours of fasting, the subjects were administered 3 ml of 25% ethanol solution(Soju) per liter of total body water. For control experiment Soju was replaced with the same dose of water. Exercise performed was vertical jumping on a rebounder for 3 min immediately after drinking. The subjects were classified into 6 groups: water ingestion(W), flushed (F) and non-flushed (N) groups after ethanol ingestion, water ingestion and exercise(WE), flushed(FE) and non-flushed (NE) groups after ethanol ingestion and exercise. Blood ethanol concentration in the exercise groups(NE, FE) was lower until 60 min after drinking than that in the non-exercise groups(N,F). Factor k representing the rate of ethanol absorption was markedly lower in the exercise groups than in the non-exercise groups. The flushed groups(F,FE) showed higher blood ethanol level than the non-flushed groups (N,NE) from 30 to 120 min after drinking. Blood lactate concentration in WE group was elevated immediately after exercise and returned to the resting level at 60 min after exercise. Ethanol increased blood lactate level from 30 to 120 min after ethanol drinking, Exercise after ethanol ingestion produced a sharp increase and then drop in blood lactate level which was stilled significantly higher than the resting level all the way through 120 min. Blood glucose concentration was decreased at 15 min after exercise. Ethanol-administered groups except F group showed a steady decrease in blood glucose level from 30 through 120 min. Heart rate was elevated by ethanol only in the flushed groups. Heart rate in F group was significantly increased at 4 min after ethanol and was maintained at high level until 120 min. In WE and NE groups, heart rate was significantly increased immediately after exercise and returned to the resting level at 60 min. The FE group, however, showed a consistently elevated heart rate throughout the 120-min experimental period. Taken together, the exercise alone produced a delayed ethanol absorption, a prompt increase in heart rate and blood lactate level and a decrease in blood glucose level early in the recovery period from exercise. After ethanol administration, blood lactate was elevated and blood glucose was lowered from 30 to 120 min. Flushed subjects showed rapid increase in heart rate after ethanol drinking and higher blood ethanol level than non-flushed ones from 30 to 120 min after drinking.

The Judgment of Criminal Liability and Psychiatric Evaluation for Mentally Defective Person (정신장애자의 형사책임능력 판단과 정신감정)

  • The Korean Criminal Code ${\S}10$ (1) provides that "The act of a person who, because of mental disorder, is unable to make discriminations or to control one's will, shall not be punished". Therefore, it'll not be able to be given criminal punishment if a mentally defective person is determined to lack the criminal liability. The problem about judging the criminal liability for the mentally defective person exists in areas where the criminal law intersects with psychiatry. Although the supreme court ultimately judges whether the criminal liability by mental defectiveness exists or not, the judgment of mental defectiveness, which is biological element, needs psychiatric knowledge and it is no wonder to rely on this. In particular, a change is required in the procedure and contents of mental examination for a mentally defective person as implementation of the Civil Participation in Criminal Jury Trial. It is needed the improvement of procedure to submit more accurate mental examination and the result of it in order that jurors are able to understand the result of mental examination and make an decision. This is because jurors consisting of ordinary citizens take part in trial. For guaranteeing the precise result of mental examination in the criminal justice procedure, it is necessary to establish the pool of manpower consisting of psychiatrists or psychologists who have completed the specific educational programs about the criminal justice and legal psychiatry, and it is desired to carry out the psychiatric test with selecting appraisers who belong to a pool of manpower. Furthermore, it is required to draw up and submit the written appraisal of mental examination which is easy to be known because of considering the nonprofessional of jurors consisting of ordinary citizens in the Civil Participation in Criminal Jury Trial. In order to gain a fair verdict of the jury about whether mental defectiveness exists or not, it is recommended the prompt submission of the written appraisal of mental examination, the presentation of the written appraisal of mental examination summarizing the important contents, and making out the written appraisal of mental examination for jurors to understand it easily.

Aortopulmonary Window (대동맥폐동맥창)

  • Background: Aortopulmonary window (APW) is a very rare congenital heart anomaly, often associated with other cardiac anomalies. It causes a significant systemic to pulmonary artery shunt, which requires early surgical correction. Accurate diagnosis and surgical correction will bring good outcomes. The purpose of this study was to describe our 20-year experience of aortopulmonary window. Material and Method: Between March 1985 and January 2005, 16 patients with APW underwent surgical repair. Mean age at operation was $157.8{\pm}245.3$ ($15.0{\sim}994.0$) days and mean weight was $4.8{\pm}2.5$ ($1.7{\sim}10.7$) kg. Patent ductus arteriosus (8), atrial septal defect (7), interruptedaortic arch (5), ventricular septal defect (4), patent foramen ovate (3), tricuspid valve regurgitation (3), mitral valve regurgitation (2), aortic valve regurgitation (1), coarctation of aorta (1), left superior vena cavae (1), and dextrocardia (1) were associated. Repair methods included 1) division of the APW with primary closure or patch closure of aorta and pulmonary artery primary closure or patch closure (11) and 2) intra-arterial patch closure (3). 3) Division of the window and descending aorta to APW anastomosis (2) in the patients with interrupted aortic arch or coarctation. Result: There was one death. The patient had 2.5 cm long severe tracheal stenosis from carina with tracheal bronchus supplying right upper lobe. The patient died at 5th post operative day due to massive tracheal bleeding. Patients with complex aortopulmonary window had longer intensive care unit and hospital stay and showed more morbidities and higher reoperation rates. 5 patients had reoperations due to left pulmonary artery stenosis (4), right pulmonary artery stenosis (2), and main pulmonary artery stenosis (1). The mean follow-up period was $6.8{\pm}5.6$ (57.0 days$\sim$16.7 years)years and all patients belonged to NYHA class 1. Conclusion: With early and prompt correction of APW, excellent surgical outcome can be expected. However, optimal surgical method needs to be established to decrease the rate of stenosis of pulmonary arteries.

Clinical Usefulness of Rapid Antigen Test on the Diagnosis of Influenza (인플루엔자 진단을 위한 신속 항원검사의 유용성)

  • Purpose : Influenza is a respiratory disease which prevails widely every year and shows high morbidity and mortality among not only chronic invalids and the old, but also among infants and young children. To prevent community-acquired influenza infection, to facilitate prompt antiviral therapy and to avoid unnecessary use of antibiotics, an easy, rapid diagnostic method for the influenza virus is needed. We evaluated a lateral-flow immunoassay(QuickVue Influenza Test), compared to viral culture. Methods : During two consecutive years from Jan. 2004 to June 2004 and from Feb. 2005 to Jan. 2005, 408 patients who were suffering from fever, cough and/or sore throat and myalgia were enrolled in our study. A total of 408 patients were tested with $QuickVue^{(R)}$(Quidel Co., San Diego, USA) influenza rapid antigen test and virus cultures at the same time. Results : Of the 408 patients tested, children who showed positive results at the virus culture numbered 77; among them, 55(71.4 percent) were type A/H3N2 and 22(28.5 percent) were type B. QuickVue influenza test had a sensitivity of 71.4 percent and a specificity of 95.8 percent. The positive and negative predictive values were 79.7 percent and 93.5 percent, respectively. Conclusion : In our study, this test had comparable high sensitivity and high specificity and many advantages, such as being easy to perform and simple to interpret, and showing rapid results. If rapid influenza antigen tests are widely applied in the clinic, we can begin treatment more rapidly and reduce influenza complications and the abuse of antibiotics.
