Development of job satisfaction scale for dental hygienist (치과위생사의 직무만족에 관한 측정도구 개발)

  • Objectives: This study was conducted to develop a dental hygienist's job satisfaction scale and to evaluate its validity and reliability. Methods: A total of 34 preliminary questions were composed based on the components commonly defined in previous studies and in-depth interviews. Results: The results of the study were practice activity, professional satisfaction, income, working conditions, patient relations, and co-worker relations using the exploratory factor analysis. Goodness of fit of the model assessed using confirmatory factor analysis generally met the criteria. On verification of the internal consistency, the Cronbach's alpha for the factors were as follows: 0.821 for practice activity, 0.822 for professional satisfaction, 0.847 for income, 0.765 for working condition, 0.743 for patient relations, and 0.747 for co-worker relations, which implied that the measurement tool was reliable. Through exploratory and confirmatory factor analyses, a total of 24 items for six factors: six items for practice activity factors, four items for professional satisfaction, three items for income, four items for working conditions, three items for patient relationships, and three items for coworker relations. Conclusions: Based on the aforementioned results, the validity and reliability of the dental hygienist's job satisfaction measurement tool were satisfactory. Thus, it may be used for assessing the job satisfaction of the dental hygienists in future.

Level of perception of professional ethics education and the knowledge and attitudes related to medical ethics of students and professors in dental hygiene (치위생학 전공 학생과 교수자의 직업윤리교육 의식과 의료윤리 관련 지식과 태도)

  • Objectives: This study aimed to analyze the dental hygiene students and professors' knowledge and attitudes related to medical ethics and their level of perception of professional ethics education. Methods: This was a cross-sectional survey study of 330 students and professors in the field of dental hygiene using a structured questionnaire. The sub-factors of knowledge and attitudes related to medical ethics were classified into patient relations, medical and social relations, and individual specialized fields, and analyzed for comparison. Results: In the university curriculum, the professional ethics completion rate was 34.8% for students and 22.1% for professors, but 98.5% of respondents said that professional ethics is important. Their knowledge and attitudes related to medical ethics were average, ranging from 5 points to 3.37 points, followed by "relationships with patients (3.75 points)", "medical and social relations (3.19 points)", and "individual specialties (3.16 points)." Conclusions: Students and professors who major in dental hygiene recognize the importance of professional ethics, but the level of knowledge and attitude related to medical ethics has been found to be moderate. Therefore, it is suggested that medical ethics, including professional ethics, should be treated as an essential subject in the university curriculum.

Conversation Analysis for Improving Nursing Communication (간호 커뮤니케이션 향상을 위한 대화 분석적 접근)

  • Purpose: Nursing communication has become more important than ever before because quality of nursing services largely depends on the quality of communication in a very competitive health care environment. This article was to introduce ways to improve nursing communication using conversation analysis. Methods: This was a review study on conversation analysis, critically examining previous studies in nursing communication and interpersonal relationships. Results: This study provided theoretical backgrounds and basic assumptions of conversation analysis which was influenced by ethnomethodology, phenomenology, and sociolinguistic. In addition, the characteristics and analysis methods of conversation analysis were illustrated in detail. Lastly, how conversation analysis could help improve communication was shown, by examining researches using conversation analysis not only for ordinary conversations but also for extraordinary or difficult conversations such as conversations between patients with dementia and their professional nurses. Conclusion: Conversation analysis can help in improving nursing communication by providing various structures and patterns as well as prototypes of conversation, and by suggesting specific problems and problem-solving strategies in communication.

Interprofessional Education Programs for Nursing Students: A Systematic Review (간호대학생을 위한 전문직 간 교육 프로그램에 대한 체계적 문헌고찰)

  • Purpose: The purpose of this study was to investigate how interprofessional education has been designed, implemented, and evaluated in undergraduate programs in nursing through a systematic review. Methods: The literature was searched using the PubMed, CINAHL, EMBASE, and Cochrane central databases to identify interventional studies including teaching-learning activities among nursing students and other disciplines in English between January 2000 and May 2017. Thirty studies were selected for the analysis. Results: Twenty-four studies out of 30 were designed as a pre-post, no control group, quasi-experimental study design. Interprofessional education learners were primarily engaged in medicine, physical therapy, dentistry, occupational therapy, pharmacy, and respiratory therapy. Patient care related activity was the most frequently selected topic and simulation was the most common teaching-learning method. Evaluation of learning outcomes was mainly based on the aspects of teams and collaboration, professional identity, roles and responsibilities, patient care, and communication skills. Nursing students in 26 out of the 30 reviewed studies were found to benefit from interprofessional education, with outcome effects primarily related to changes in learning outcomes. Conclusion: The development and integration of interprofessional education with collaborative practices may offer opportunities in nursing education for training professional nurses of the future.

Primary Care Physicians' Action Plans for Responding to Results of Screening Tests Based on the Concept of Quaternary Prevention

  • Since noncommunicable diseases (NCDs) are generally controllable rather than curable, more emphasis is placed on prevention than on treatment. For the early detection of diseases, primary care physicians (PCPs), as well as general practitioners and family physicians, should interpret screening results accurately and provide screenees with appropriate information about prevention and treatment, including potential harms. The concept of quaternary prevention (QP), which was introduced by Jamoulle and Roland in 1995, has been applied to screening results. This article summarizes situations that PCPs encounter during screening tests according to the concept of QP, and suggests measures to face such situations. It is suggested that screening tests be customized to fit individual characteristics instead of being performed based on general guidelines. Since screening tests should not be carried out in some circumstances, further studies based on the concept of prevention levels proposed by Jamoulle and Roland are required for the development of strategies to prevent NCDs, including cancers. Thus, applying the concept of QP helps PCPs gain better insights into screening tests aimed at preventing NCDs and also helps improve the doctor-patient relationship by helping screenees understand medical uncertainties.

Effect of job satisfaction on turnover intention and intention to stay in dental hygienists (치과위생사의 직무만족이 이직의도와 근속의사에 미치는 영향)

  • Objectives: This study was conducted to investigate the effects of dental hygienists' job satisfaction on turnover intention and intention to stay. Methods: A survey was conducted targeting 273 dental hygienists in dental clinics. The study data were u analyzed using PASW Statistics 20.0 Results: The level of job satisfaction of dental hygienists was 3.30±0.50, and the highest ranked item in the survey was 'I am satisfied with my relationships with patients' 3.72±0.73. Job satisfaction according to general characteristics showed significant differences in age, marriage, education, career, current work experience, position, and schedule for future turnover. The turnover intention according to general characteristics showed significant difference in age and schedule for future turnover. The intention to stay according to general characteristics showed significant differences in age, marriage, education, career, current work experience, number of dentists, number of dental hygienists, and schedule for future turnover. The factors affecting turnover intention were schedule for future turnover, income, co-worker relation and professional time, while those affecting intention to stay were overall professional satisfaction, co-worker relation, schedule for future turnover, professional time, income, number of dental hygienists, patient relations and current work experience. Conclusions: Job satisfaction factors that had a common effect on turnover intention and intention to stay were schedule for future turnover, income, co-worker relation and professional time. To reduce turnover and encourage longevity, adequate pay and positive interpersonal relationships are necessary.

Satisfaction in the National Cancer Screening Program for Breast Cancer with and without Clinical Breast Examination

  • The purpose of this study was to evaluate whether screening by clinical breast examination (CBE) in addition to mammography affected participant satisfaction in the National Cancer Screening Program (NCSP). Data were derived from the Quality Evaluation of National Cancer Screening satisfaction survey. This population-based nationwide telephone survey included participants who had been screened by the NCSP for breast cancer between June and August 2010 (n=2,370), and collected information on satisfaction with screening and screening service use. Five multiple regression models were used to determine satisfaction according to screening method, and according to each of five satisfaction measures (pre-screening information transfer, staff interpersonal skills, physical surroundings, reporting of results and general satisfaction). A total of 1,858 (78.4%) participants were screened by mammography alone and 512 (21.6%) by both mammography and CBE. Satisfaction was significantly higher in subjects screened by both mammography and CBE compared with those screened by mammography alone.

The Risk Factors Influencing Turnover Intention of Nurses (간호사의 이직의도에 영향을 미치는 예측 요인)

  • Purpose: This study was done to identify the risk factors influencing turnover intention of nurses. Method: The participants in this descriptive survey on causal relations were 756 nurses who were working at a tertiary university hospital in Seoul. The data were put in to multiple regression analysis to build a prediction model. Results: Turnover intention according to general features were shown as following.: Age, Clinical careers, Educational level, Marital status, Economic status(Yearly income). The relationship between turnover intention and job stress had positive correlation. But the relationship between turnover and other factors that job satisfaction, internal marketing, and organizational commitment had negative correlation. The causal factors of turnover intention were organizational commitment, the factors of organizational support and patient/caregiver relationships among subcategories of job stress and the factor of professional position among subcategories of job satisfaction. Conclusions: The findings of study suggest that board intervention program should be provided to prevent problems of turnover. It is also recommended that a program be developed that can help control the variables identified in this study along with follow up study to verify the model.

Mediating Effects of Hope and Therapeutic Relationship in the Relation between General Social Functions and Mental Health Recovery of Community People with Mental Illness (지역사회 정신장애인의 전반적 사회기능과 정신건강 회복 간의 관계에서 희망과 치료적 관계의 매개효과)

  • Purpose: This study tries to test mediating effects of hope and therapeutic relationship in the relation between general social functions and mental health recovery of community people with mental illness. Methods: This study was carried out in a cross-sectional research design. The participants included 217 people with mental illnesses who were enrolled at eight Mental Health Welfare Centers in the Gyeonggi Province. Data were collected from February to May, 2018. The collected data were analyzed using a regression analysis, and SPSS PROCESS macro was used to test the mediating effects. Results: This study analyzed the direct effects of general social functions on the hope, therapeutic relationship and mental health recovery. And general social functions had indirect effects on their mental health recovery via hope and therapeutic relationship. Both hope and therapeutic relationship had dual mediating effects in the influence of general social functions on mental health recovery. Conclusion: The role of hope and therapeutic relationship in the mental health recovery of community people with mental illness is important, and it is confirmed that hope is a powerful factor influencing mental health recovery.

Validity and Reliability of the Korean Version of Dental Hygienist Satisfaction Survey (한국어판 치과위생사의 직무만족 측정도구의 타당도와 신뢰도)

  • The purpose of this study was to develop a tool to assess the job satisfaction of dental hygienists and to evaluate the validity and reliability of the dental hygienist. A total of 54 items were translated into 24 items through content validity and content analysis. The results of this study are as follows: overall satisfaction, income, patient relations, professional time, personal time, Practice Management, Delivery of Care and co-worker relations were classified by exploratory factor analysis. The fit of the model through confirmatory factor analysis generally met the criteria. As a result of the internal consistency test, Cronbach's ${\alpha}$ was found to be a reliable measure of measurement, with an overall satisfaction of 0.918, an income of 0.833, a patient relationship of 0.804, an occupational time of 0.675, a personal time of 0.939, a work practice of 0.739, The validity and reliability of the dental hygienist 's job satisfaction measurement tool were verified to be appropriate. It can be used to study the job satisfaction of dental hygienist in the future.
