  • Chemical Process industry in Korea has over 30 year's of history and is likely to face potential incidents. The traditional risk analysis and control system in Chemical Process industry focuses on mechanical defects, overlooking the human performance control. Although development of automation technology and controlling technology was necessary, human decision factor is essential to preventing accidents in the Chemical Process. Almost all serious accidents take place when inappropriate humanperformance and mechanical defects of safety equipments simultaneously occurs. The AHRA(Advanced Human Reliability Analyzer) software has been developed to collect failure data and analyze human error probability (Reliability) in Chemical Process Industry in Korea. This paper describes the HRA analysis result of PIF(Performance Influencing Factor) evaluation, HEP(Human Error Probability) and root cause of accidents by applying a Chemical Process Industry related accident data. This analysis result should present a scheme that, by controlling human error factor other than putting safety management funds into the machinery in plants, can reduce cost and maximize the safety in Chemical Process Industry.

  • Increasing safety requirements of automobile are asking companies to find out solutions, based on the ISO 26262 which is a functional safety standard. ISO 26262 is an adaptation of the IEC 61508 for automotive electric/electronic systems. ISO 26262 provides a V model for ECU (Electronic Control Unit) development process to secure safety against vehicle. It well describes the requirements, necessary works and their resulting products for each development phase. However, it is difficult to apply to product development for achieving functional safety in the electric/electronic systems of an automobile because it lacks explanation on the working steps to follow and the methodologies and tools to be used in each step. In this paper, we introduce the outline of the ISO 26262 product development process and present a DFSS (Design For Six Sigma) road-map based on the ISO 26262 product development process as a way to operate efficiently the ISO 26262 product development process. The DFSS road-map consists of five phases: Define, Measure, Analyze, Design, and Verify. The detailed activities, tools, inputs, and work products are given for each phase.

A Study on Dental Technology Students' Attitudes toward Safety and Their Practice of Safety (치기공과 학생들의 안전에 대한 태도 및 실천도 조사)

  • Objectives: Practice of dental technology poses great potential risk since the manufacturing processes are complex and involve dangerous devices and the students are asked to handle various materials in the casting and processing process. Lacking an understanding of the overall practice process, the beginners of practice are exposed to greater danger due to their unsafe attitudes and poor practice attitudes. This study set out to investigate the students of dental technology for their attitudes toward safety and their practice of safe actions during practice and to provide basic data for safety education to prevent any possible safety incidents during practice. Methodology: The subjects include total 408 dental technology majors in their sophomore and junior year. Based on literature study and previous studies, a structured questionnaire was made, distributed to the subjects right after practice, and gathered on the spot. The questionnaire contained total 60 items, and data were collected from April 20 to May 25, 2006. Results: Collected data were analyzed in frequency and $X(C)^{\sim}$-test using the SPSS 12.0 version. The analysis results show that the students believed that they could prevent safety accidents by following safety rules and guidelines as far as safety attitudes were concerned. However, they did nothing more than paying attention to many practice processes even though they thought they were dangerous. Conclusion: Overall the subjects practiced proper safety actions during practice with significance differences in their behavioral characteristics according to grade, gender and age. The significant differences according to grade seem to derive from safety education before practice, and those according to gender and age have more to do with individual actions and common sense according to gender and age than education. Thus practice teachers need to develop safety education materials for each practice process and provide systematic safety education before practice so that the students can practice safety actions.

  • Heat transfer oils are used in applications such as chemical plant heating systems, refinery heat exchange systems, certain gas processes, injection molding systems, and pulp and paper processing. These oils are extremely stable and resistant to thermal and oxidative degradation. In the event of a spill or accidental release of heat transfer oils, it can be ignited easily when there is an ignition source. This study discusses the status of safety management through the actual status of the heat transfer oils to prevent fire and explosion accidents in industries for process safety management. The actual status of the heat transfer oils in process system of industries surveyed by a questionnaire developed. The results of this study can be used to help establishment of safety management to prevent fire and explosion accidents, such as the management of heat transfer oils, safe operation and maintenance in heat transfer oil processes.

  • Based on latest research, the parametric quality statistics cannot be used in non-normal process with demand pattern of many-variety and small-volume, since it involves extremely small sample size. The research proposes nonparametric quality statistics according to the number of lot or batch in the non-normal process. Additionally, the nonparametric Process Capability Index (PCI) is used with 14 identified non-normal distributions.

  • An accident from machine work is often fatal to the worker. This accident is mostly preventable through perfect process jig. In case of this machine work, however, the disaster frequently occurred because of the design which is not considered the beginning of product design, post-process and mass process of production. As for sand casting method, this has the merits of the production; the product is easily produced by manual labor. On the other hand, this method has the demerits of a bigger dimensional error contrary to other mass process of production. When the sand casting product is in machine work, there are various problems such as unsafe fix and excessive cutting, product desorption and rapid life depreciation of equipment and tools. Considering the characteristics of sand casting method, it is accepted as difficulty to improve the problems. In this study, it suggests shell mold method for mold instead of the machine work after the sand casting of the circle shape container product. And the surface accomplishes the following average of surface roughness Ra$9.94{\mu}m$ of machine work with the casting of flask mold by shell mold method. In accordance with the simplification of processing process and reducing the average thickness variation by mass production of product with detailed appearance, it has a good influence on safety accident prevention caused by production and damaged product. It is confirmed that making higher degree of size precision of all machine work product is possible to increase the safety and productivity, reduce the processing process and improve environment.

  • The government has announced its Hydrogen Economy Roadmap to strengthen global competitiveness on the hydrogen economy by focusing on hydrogen fuel cell electric vehicles and fuel cells. In this regard, the interest on the economics and safety of the infrastructure of hydrogen stations has also increased. In this study, a test bed similar to an actual hydrogen charging facility was built, and a prototype of a piston-type compressor was modeled. In this model, the piston was hydraulically compressed to progressively test leakage, leakage rate, and durability and to check for any malfunction. Moreover, the leakage rate, cylinder leak performance, and compressor operation durability were evaluated for safety; it was confirmed that there were no abnormalities. Nevertheless, an investigation of the long-term use and operating pressure of the compressor is necessary to verify the safety of developing a100-MPa domestic compressor in the future.
