치아 치수 세포는 치아 손상에 따르는 병리적인 상황에서 골과 상아질 기질을 형성하는 능력을 가진 것으로 생각된다. 본 연구에서는 MTA가 사람 치수세포의 성장에 미치는 영향과 상아질 형성에 관여하는 유전자의 발현을 유도하는지를 알아보고자 하였다. 또한 상아질 형성의 잠재적 지표인 alkaline phosphatase(ALP) activity에 미치는 영향을 평가하였다. 유전자 발현 검사를 위해 glyceraldehyde-3-phosphate dehydrogenase, type I collagen, alkaline phosphatase, osteonectin(SPARC), and dentin sialoprotein primer set을 이용하여 MTA 처리 2일과 4일 후 reverse transcriptase polymerase chain reaction(RT-PCR)을 시행하였다. cell viability assay(세포 생존력 측정) 에서 5일간 MTA에 노출된 치수 세포의 비율이 대조군보다 높았다. 대조군에 비해 MTA를 처리한 군에서 ALP와 SPARC가 증가되었다. 이상의 결과를 종합하여 보면, 이 연구에 사용한 dental pulp culture system은 MTA를 포함한 치과재료의 처리 후 치수세포의 성장과 분화 그리고 상아질 형성 유도 기전을 연구하는 데 유용한 모델로 사용할 수 있다. MTA 처리는 사람 치수세포에 세포독성을 유도하지 않으며, ALP 활성도와 유전자 발현 그리고 osteonectin (SPARC) 유전자 발현을 증가시켜 수복상아질을 형성할 것으로 사료된다.
치아우식은 주로 mutans streptococci에 의해 야기되는 감염성 질환으로서 주 원인균에는 streptococcus mutans가 있다. S. mutans가 치아우식을 유발하는 분자 생물학적 기전은 몇 가지 단계를 포함한다. 먼저 S. mutans는 AgI/II와 같은 세포 표면의 섬유성 단백질을 매개로 치면의 타액성 피막에 일차적으로 부착한다. 두번째 단계에서 자당의 존재하에 glucosyltransferase(GTF)는 glucan과 같은 다당체를 합성하게 된다. 마지막으로 이렇게 합성된 glucan은 glucan binding proteins와 상호작용하여 치면세균막을 형성해서 세균의 군집화를 가능하게 한다. 많은 실험과 임상연구에서 S. mutans의 주요 항원(Ag I/II, GTFs, GBPs)들이 치아우식 병리기전에 영향을 준다고 알려져 왔고, 따라서 이런 항원들이 면역계에 작용하여 치아우식을 막는 백신으로 이용 가능하다. 본 실험은 streptococcus mutans GS-5로부터, GTFb, GTFc, GTFd 유전자를 복제하고 염기서열분석을 하였으며, 이중 GTFd가 먼저 재조합 단백질 생산을 위해 발현 벡터에 클로닝 되었으며, 이로부터 단백질이 발현됨을 확인하였다. 이번 실험에서 얻은 순수 GTF 항원은 동물실험을 통해 특정 GTF 활성부위에 대한 항체 생산에 이용될 수 있을 것이다.
제1대구치의 이소맹출은 국소적 원인과 유전적 원인의 조합에 의한 다양한 요소에 의해서 유발된다. 주기적인 검진과 방사선학적 검사를 통해 비가역적 이소맹출로 판단 될 경우, 제2유구치 조기 상실에 의해 제1대구치의 근심경사나 회전 및 복잡한 교정적인 문제를 일으킬 수 있으므로 적절한 개입이 필요하다. 본 증례는 항암치료의 기왕력이 있는 만성 골수성 백혈병 환아에서 나타난 제1대구치의 이소맹출을 Halterman appliance를 이용하여 정상적인 맹출로 유도할 수 있었다.
하악 제2소구치의 선천결손은 비교적 흔히 나타나는 이상으로, 방사선사진에서 결손으로 확인되는 경우 여러가지 치료방법을 고려할 수 있다. 환자의 연령, 제2유구치 치근의 흡수상태, 총생의 정도, 악골의 성장, 측모의 형태, 절치의 돌출도, 하안 모고경 등을 고려하여 치료방법을 선택하게 된다. 그러나, 선천결손의 진단과 치료방법의 결정에 있어서 주의해야 할 점이 있다. 하악 제2소구치는 분화와 석회화과정에 있어서 가장 많은 변이를 보이는 치아이다. 경조직 형성은 $2\frac{1}{4}\sim2\frac{1}{2}$세에 시작되며, 대부분 적어도 $3\sim3\frac{1}{2}$세에는 시작된다. 그러나, 이 시기는 다른 영구치에 비해 그 범위가 매우 넓어서 $5\sim6$세, 드물게는 그 이후에 발육이 시작되는 경우도 있다. 그러므로 하악 제2소구치의 선천결손을 조기에 진단하기에는 어려움이 있으며, 선천결손으로 진단한 경우라도 오진할 가능성이 있음을 인식하는 것이 중요하다. 저자는 두 가지 증례를 통한 임상연구와 문헌고찰을 통해 다음과 같은 결론을 얻었다. 1. 두 증례 모두에서 하악 제2소구치의 두드러진 발육지연이 관찰되었다. 2. 치배형성 이후의 석회화 속도는 거의 정상적으로 나타난 것을 보아, 발육지연은 석회화 과정보다는 분화과정에 있었던 것으로 판단되었다. 3. 이와 유사한 증례를 접하였을 경우, 치아의 선천결손으로 단정하기 보다는 발육지연의 가능성을 함께 고려 할 필요가 있을 것으로 사료되었다.
교정치료 시 고정원은 항상 염두에 두면서 치료를 진행해야 하는 중요한 요소이다. 전통적인 고정원을 이용한 치료는 환자의 적극적인 협조를 필요로 하고 원하는 치아이동에 대한 반작용이 나타날 수 있는 등의 부작용을 가지고 있다. 그러므로 보다 견고한 구강 내 고정원이 요구되어져 왔으며 이를 위해 miniscrew가 임상에서 흔히 이용되고 있다. Miniscrew를 통한 고정원의 확보는 구외력 의존도 감소, 치료기간의 단축, 식립 후 즉시 교정력 적용, 환자의 협조도 불필요, 식립의 간편성, 저렴한 비용 등의 장점을 가지고 있다. 그러나 성장기 환자는 성인에 비해 골질이 좋지 않아 miniscrew의 성공률이 성인에 비해 낮다. 그러나 일단 고정에 성공하면 많은 교정치료 시 매우 유용하게 이용할 수 있다. 본 증례는 혼합치열기 환자를 miniscrew를 이용하여 정중이개 및 정중선 불일치, 매복치에 대한 양호한 교정치료를 하였기에 보고하는 바이다.
In 2015, the population of elderly people in Thailand was 16% of the total population and is predicted to be over 20% by 2021 and nearly 28% by 2031. The increase of the elderly population in Thailand has also increased the proportion of dependent elderly people, and caring for them poses many challenges for both families and the government. This descriptive method research aimed to survey the health status of dependent older people in the rural community of Lampang province in northern Thailand. The participants consisted of 62 older people and 62 primary family caregivers from Hong Ha Health Promoting Hospital, Lampang, Thailand (totaling 124 people). The researchers assessed the health status of older people and their activities of daily living (ADL). In addition, researchers assessed the health status and stress of caregivers. All the participants were interviewed about their experiences with caregiving. The results showed that most of the older respondents were female with an average age of 78.15 years. Based on the ADL assessment, 50 of the 62 older persons were homebound while the rest were bedridden. The majority of older people had chronic or long-term conditions that required hospitalization from time to time. Their frequent health problems included oral disorders such as tooth decay or caries/gum disease/no teeth, reduced sight, psychological disorders, knee pain and risk of falling, low BMI, risk of malnutrition, and urinary leakage and incontinence (58.06%, 66.13%, 62.90%, 70.97%, 38.71%, 66.13%, and 37.10%, respectively). Usual care provided by the family members included personal hygiene care, food preparation and feeding, medication management, housekeeping and organizing necessary equipment, supply of needed equipment, prevention of falls, helping with travel for medical checkups and treatment, and providing companionship. Families experienced shortages of medical supplies, daily use equipment, lack of employment, inadequate income, and difficulty accessing health care services due to lack of transportation. Some caregivers experienced caregiving stress related to a lack of social interactions as well as routine caregiving activities. Families need different types of support in order to promote the well-being of older people and caregivers. This highlights the need for a community participation model for the care of older people in order to reflect sustainable long-term outcomes.
Plaster models were constructed and orthopantomographs were taken for 86 male and 70 female primary school pupils, whose upper and lower permanent 4 incisors and 1st molars were completely erupted without crowding; whose deciduous canines and molars were found almost uniformly even without any visible tooth fractures, dental caries or restorations on proximal surfaces of the teeth. Certain reference points on the orthophantomograph were set up and measured and the values were compared with actual or predicted values from the models. The following results were obtained: 1. In regards to available space, the values from the orthopantomograph were greater than the values from the models by a mean of 3.24% on the upper and 10.06% on the lower for males; 3.05% on the upper and 10.01% on the lower for females. 2. In regards to total mesiodistal widths of permanent canine, 1st and 2nd premolars, the values from the orthopantomograph were greater than the presumed values based on the size of lower permanent 4 incisors from the models by a mean of 18.50% on the upper and 24.09% on the lower for males; 14.54 on the upper and 20.51% on the lower for females. 3. Comparing the magnified values of total mesiodistal widths of permanent canine, 1st and 2nd premolars with those of available space, the regression constants of regression equation (Y = a + bX) between them were a=3.2336, b=0.6533 on the upper and a=5.0138, b=0.3290 on the lower for males; a=2.5994, b=0.6521 on the upper and a=3.0113, b=0.6586 on the lower for females. 4. The correlation coefficients between the magnified values of available space and permanent canine, 1st and 2nd premolars were moderately positive as 0.6474 in the upper and 0.505 on the lower for males; 0.6493 on the upper and 0.6183 on the lower for females. 5. In regards to magnified values of the available space from the orthopantomographs there were no significant difference between sexes, (P>0.05) but of the total mesiodistal widths of permanent canine, 1st and 2nd premolars, a significant difference between sexes was found.(p<0.01).
The primary cause of tooth loss after 30 years of age is periodontal disease. Destruction of alveolar bone by periodontal disease is done by bone resorbing activity of osteoclasts. Understanding differentiation and activation mechanism of osteoclasts is essential for controling periodontal disease. The purpose of this study is to identify the possible effects of Vitamin D and cytokines affecting osteoclasts and its precursor cells. Four to six week-old mice were killed and humerus, radius, tibia and femur were removed aseptically and washed two times with Hank's solution containing penicillin-streptomycin and then soft tissue were removed. Bone marrow cells were collected by 22 gauge needle. Cells were cultured in Hank's solution containing 1 mg/ml type II collagenase, 0.05% trypsin, 41mM EDTA. Supernatant solution was removed 5 times after 15 minutes of digestion with above mentioned enzyme solution, and remained bone particles were maintained in alpha-MEM for 15 minutes and $4^{\circ}C$ temperature. Bone particles were agitated for 1 minute and supernatant solution containing osteoclast precursor cells were filtrated with cell stainer. These separated osteoclast precursor cells were dispensed with 100-mm culture dish by $1{\times}10^7$ cells unit and cultured in ${\alpha}$- MEM containing 20 ng/ml recombinant human M-CSF, 30 ng/ml recombinant human soluble osteoclast differentiation factor and 10% fetal calf serum for 2 and 7 days. Total RNA of osteoclast precursor cells were extracted using RNeasy kit. One ${\mu}g$ of total RNA was reverse transcribed in $42^{\circ}C$ for 30 minutes using SuperScriptII reverse transcriptase. Expression of transcribed receptors of each hormone and cytokine were traced with 1 ${\mu}l$ of cDNA solution by PCR amplification. Vitamin D receptor WAS found in cells cultured for 7 days. TNF-${\alpha}$ receptor was found in cells cultured for 2 days and amount of receptors were increased by 7 days. IL-1 type I receptor was not found in cells cultured 2 and 7 days. But, IL-1 receptor type II was found in cells cultured for 2 days. TGF-${\alpha},{\beta}$type I receptor was found in cells cultured 2 and 7 days, and amount of receptors were increased by 7 days of culture. These results implies Vitamin D and cytokines can affect osteoclasts directly, and affecting period in differentiation cycle of osteoclasts is different by Vitamin D and cytokines.
The molecular mechanisms control the function of PDL(periodonta1 ligament) cells and/or fibroblasts remain unclear. PDLsl7, PDL-specific gene, had previousely identified the cDNA for a novel protein from cultured PDL fibroblasts using subtraction hybridization between gingival fibroblasts and PDL fibroblasts. The purpose of this study was to determine the regulation by growth factors and cytokines on PDLsl7 gene expression in cultured human periodontal ligament cells and observe the immunohistochemical localization of PDLsl7 protein in various tissues of mouse. Primary PDL fibroblasts isolated by scraping the root of the extracted human mandibular third molars. The cells were incubated with various concentration of human recombinant $IL-1{\beta}$, PDGF-BB and TGF\;${\beta}$ for 48h nd 2 weeks. At each time point total RNA was extracted and the levels of transcription ere assessed by reverse transcription-polymerase chain reaction (RT-PCR assay). polyclonal antiserum raised against PDLsl7 peptides, CLSVSYNRSYQINE and SEAVHETDLHDGC, were made, and stained the tooth, periodontium, developing bone, bone marrow and mid-palatal suture of the mouse. The results were as follows. 1. PDLsl7 mRNA levels were increased in response to PDGF (10ng/ml) and $TGF\;{\beta}$(20ng/ml) after treatment of the $IL-1{\beta}$, PDGF-BB and $TGF{\beta}$for 48 h. 2. PDLsl7 was up-regulated only by $TGF{\beta}$(20 ng/ml) after treatment of the $IL-1{\beta}$, PDGF-BB and $TGF\;{\beta}$ for 2 weeks and unchanged by the other stimulants. 3. PDLsl7 was a novel protein coding the 142 amino acid peptides in the ORF and the nucleotide sequences of the obtained cDNA from RT-PCR was exactly same as the nucleotides of the database. 4. Immunohistochemical analysis showed that PDLsl7 is preferentially expressed in the PDL, differentiating osteoblast-like cells and stromal cells of the bone marrow in the adult mouse. 5. The expression of PDLsl7 protein was barely detectable in gingival fibroblasts, hematopoetic cells of the bone marrow and mature osteocytes of the alveolar bone. These results suggest that PDLsl7 might upregulated by PDGF-BB or $TGF{\beta}$ and acts at the initial stage of differentiation when the undifferentiated mesenchymal cells in the bone marrow and PDL differentiate into multiple cell types. However, more research needs to be performed to gain a better understanding of the exact function of PDLsl7 during the differentiation of bone marrow mesenchymal and PDL cells.
본 조사는 한국의 소아치과학 연구 분야와 방향이 어떠한 변화를 거쳐왔는지를 연대별로 파악하여 우리의 연구관심이 시대별로 어디에 집중되어 왔는가를 알아보고, 향후 우리가 어느 분야에 관심을 더 쏟을 필요가 있는가를 파악하려는 의도로 시도되었다. 이에 저자는 1974년부터 2001년에 걸쳐 한국소아치과학회지에 게재된 논문들을 주제별로 분류하고 연구주제의 변화양상을 파악하여 이를 보고하는 바이다. 1. 치과용 재료와 장비에 관한 연구는 지속적인 증가추세를 보였다. 특히, 치과용 재료에 관한 연구는 논문 수가 많을 뿐 아니라, 지속적인 증가추세를 보였다. 2. 치아우식증, 우식의 예방, 장애아동 및 전신질환에 관련된 연구는 전 기간에 걸쳐 균일하게 높은 분포를 보였다. 특히, 우식예방에 관한 연구는 제 1기에 비해 제 2, 3기에 들어서면서 상대적으로 늘어나는 경향을 보였다. 3. 부정교합의 원인과 치료. 구강병리와 소수술은 제 2기부터 증가하다가 근래에 들어 감소추세를 보였다. 4. 부정교합의 골격연구, 치아의 발육과 맹출, 치아의 발육장애, 치수치료, 치열공간의 관리에 대한 연구는 과거부터 지속적인 감소추세를 보였다. 5. 신체의 발육, 행동조절, 구강악습관, 유치열과 혼합치열기의 교합에 관한 연구는 전 기간에 걸쳐 1%에 못 미치는 낮은 분포를 보였다.

제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
