폐암 환자의 기능상태에 관한 연구 (Functional Status in Lung Cancer Patients)

  • Purpose: To exam how functional status varied according to age, gender, stage, treatment protocols, and pathologic types of cancer in lung cancer patients. Method: A Cross-sectional and descriptive study was used. Functional status was measured with the Medical Outcome Study Short Form-36. A total of 106 lung cancer patients participated. Their mean age was 61 years. Majorities were male, in advanced stages, and receiving chemotherapy. Result: Functional status of the subjects were relatively low compared to the results of previous studies. Women had more severe functional limitations in Role-Emotion(t=2.17, p <.05). Generally, older patients(> 60 yrs.) had relatively more severe limitations in all subcategories, but the difference was not statistically significant. Subjects in late lung cancer stage(stage III & IV) had more severe functional limitation in all subcategories. But the statistical difference was found only in General Health(t=2.10, p<.05). In terms of treatment protocol, no-current treatment group had lower General Health than those of the chemotherapy group(F=3.42, p<.01). There were no statistical differences in functional status among pathological cancer cell types. Conclusion: The results of this study suggest that effective management may be achieved when these factors are considered on individual basis in the clinical management of lung cancer patients.

Local radiotherapy for palliation in multiple myeloma patients with symptomatic bone lesions

  • Purpose: To evaluate the clinical outcomes of symptomatic bone lesions in patients with multiple myeloma (MM) who received local radiotherapy (LRT). Materials and Methods: Fifty-one patients with 87 symptomatic bone lesions treated via LRT were analyzed. LRT was delivered at a median total dose of 21 Gy (range, 12 to 40 Gy) in a median of 7 fractions (range, 4 to 20 fractions). The clinical outcomes of LRT and the factors affecting treatment response were assessed. Results: After a median follow-up time of 66.7 weeks, symptom relief was achieved for 85 of 87 lesions (97.7%). The median time to symptom relief was 7 days from the start of LRT (range, 1 to 67 days). The duration of in-field failure-free survival ranged from 1.1 to 450.9 weeks (median, 66.7 weeks). The radiation dose or use of previous and concurrent chemotherapy was not significantly associated with in-field failure for LRT (p = 0.354, 0.758, and 0.758, respectively). Conclusion: Symptomatic bone lesions in patients with MM can be successfully treated with LRT. A higher radiation dose or the use of concurrent chemotherapy may not influence the in-field disease control. A relatively low radiation dose could achieve remission of symptoms in patients with MM.

원발성 이하선 비호지킨 림프종: 증례보고 (Primary Parotid Non-Hodgkin's Lymphoma: A Case Report)

  • Purpose: Primary malignant lymphomas of the salivary glands are uncommon, representing only 1.7% to 3.1% of all salivary neoplasms and 0.6% to 5% of all tumors and tumor-like lesions of the parotid gland. Lymphomas of the parotid glands are usually manifestations of a systemic disease process but primary lymphomas of the parotid glands are rare. Most of these lesions are classified as extranodal non-Hodgkin's lymphoma. We report the clinicopathological features of primary malignant lymphoma of the parotid gland based on an analysis of our cases. Methods: The subject was a 48-year-old male patient with a malignant lymphoma originating in the parotid gland, which had been slowly increasing in size over previous 6 months. The diagnosis was established by MRI and a superficial lobectomy. After diagnosis, the patient was referred to an oncologist for staging and medical treatment. Results: The stage was IIIA. The patient was treated with chemotherapy following surgery with rituximab and CHOP (Cyclophosphamide, Adriamycin, Vincristine, Prednisolone). The tumor was controlled successfully by chemotherapy. The patient was followed up for 1 year with no relapse. Conclusion: A case of primary non-Hodgkin's lymphoma of the parotid gland was treated with a superficial parotidectomy and chemotherapy. The disease was well controlled after a 1 year follow-up.

Efficacy of High Dose Radiotherapy in Post-operative Treatment of Glioblastoma Multiform - A Single Institution Report

  • Background: Glioblastoma multiform (GBM) is a highly aggressive tumor with median survival of approximately 14 months. Management consists of maximal surgical resection followed by post-operative chemoradiation with concurrent then adjuvant temozolamide. The standard radiotherapy dose is 60Gy in 2-Gy fractions recommended by the radiation therapy oncology group (RTOG). With the vast majority of tumor recurrences occurring within the previous irradiation field and the poor outcome associated with standard therapy, regimens designed to deliver higher radiation doses to improve local control and enhance survival are needed. In this study, we report a single institutional experience in treatment of 68 consecutive patients with GBM, treated with resection, and given post-operative radiotherapy followed by concurrent and/or adjuvant chemotherapy. Results: Of the 80 patients who entered this study, 68 completed the treatment course; 45 (66.2%) males and 23 (33.8%) females with a mean age at diagnosis of $49.0{\pm}12.9$ (21-75) years. At a median follow up of 19 months, 39 (57.3%) patients had evidence of tumor progression and 36 (52.9%) had died. The median over all survival for all patients was 16 months and progression free survival for all patients was 6.02 months. All potential prognostic factors were analyzed to evaluate their effects on overall survival. Age ${\leq}50$ year, concurrent and adjuvant chemotherapy and extent of surgery had significant p values. We found lower progression rate among patients who received higher doses of radiotherapy (>60Gy). Higher radiation doses improved progression free survival (p=0.03). Despite increasing overall survival, this elevation was not significant. Conclusions: This study emphasize that higher radiation doses of (>60Gy) can improve local control and potentially survival, so we strongly advise prospective multi centric studies to evaluate the role of higher doses of radiotherapy on GBM patient outcome.

Outcomes of Preoperative Chemoradiotherapy and Combined Chemotherapy with Radiotherapy Without Surgery for Locally Advanced Rectal Cancer

  • Purpose: To evaluate the treatment outcomes of patients with locally advanced rectal cancer treated with preoperative concurrent chemoradiotherapy (CCRT) or combined chemotherapy together with radiotherapy (CMT-RT) without surgery. Materials and Methods: A total of 84 patients with locally advanced rectal adenocarcinoma (stage II or III) between January $1^{st}$, 2003 and December $31^{st}$, 2013 were enrolled, 48 treated with preoperative CCRT (Gr.I) and 36 with combined chemotherapy and radiotherapy (CMT-RT) without surgery (Gr.II). The chemotherapeutic agents used concurrent with radiotherapy were either 5-fluorouracil short infusion plus leucovorin and/or capecitabine or 5-fluorouracil infusion alone. All patients received pelvic irradiation. Results: There were 5 patients (10.4%) with a complete pathological response. The 3 year-overall survival rates were 83.2% in Gr.I and 24.8 % in Gr.II (p<0.01). The respective 5 year-overall survival rates were 70.3% and 0% (p<0.01). The 5 year-overall survival rates in Gr.I for patients who received surgery within 56 days after complete CCRT as compared to more than 56 days were 69.5% and 65.1% (p=0.91). Preoperative CCRT used for 12 of 30 patients in Gr.I (40%) with lower rectal cancer demonstrated that in preoperative CCRT a sphincter sparing procedure can be performed. Conclusions: The results of treatment with preoperative CCRT for locally advanced rectal cancer showed comparable rates of overall survival and sphincter sparing procedures as compared to previous studies.

ER71/ETV2 Promotes Hair Regeneration from Chemotherapeutic Drug-Induced Hair Loss by Enhancing Angiogenesis

  • Chemotherapy-induced alopecia and hair loss can be stressful in patients with cancer. The hair grows back, but sometimes the hair tends to stay thin. Therefore, understanding mechanisms regulating hair regeneration may improve the management of chemotherapy-induced alopecia. Previous studies have revealed that chemotherapeutic agents induce a hair follicle vascular injury. As hair growth is associated with micro-vessel regeneration, we postulated that the stimulation of angiogenesis might enhance hair regeneration. In particular, mice treated with 5-fluorouracil (5-FU) showed delayed anagen initiation and reduced capillary density when compared with untreated controls, suggesting that the retardation of anagen initiation by 5-FU treatment may be attributed to the loss of perifollicular micro-vessels. We investigated whether the ETS transcription factor ETV2 (aka ER71), critical for vascular development and regeneration, can promote angiogenesis and hair regrowth in a 5-FU-induced alopecia mouse model. Tie2-Cre; Etv2 conditional knockout (CKO) mice, which lack Etv2 in endothelial cells, presented similar hair regrowth rates as the control mice after depilation. Following 5-FU treatment, Tie2-Cre; Etv2 CKO mice revealed a significant reduction in capillary density, anagen induction, and hair restoration when compared with controls. Mice receiving lentiviral Etv2 injection after 5-FU treatment showed significantly improved anagen induction and hair regrowth. Two-photon laser scanning microscopy revealed that enforced Etv2 expression restored normal vessel morphology after 5-FU mediated vessel injury. Our data suggest that vessel regeneration strategies may improve hair regrowth after chemotherapeutic treatment.

골육종에서 술전 항암화학요법의 효과 판정 (Evaluation of Neoadjuvant Chemotherapy Effect in Osteosarcoma)

  • 목적: 골육종에 대한 술전 항암화학요법의 효과를 조기에 비침습적으로 평가하기 위해 다양한 영상 진단 기법들을 사용해 왔다. 저자들은 골육종에 대한 술전 항암화학요법의 효과를 예측하는 데 있어 단순 촬영 및 양전자 방출 단층 촬영과 같은 영상 검사들의 효용성을 평가하고 양성자 방출 단층 촬영에서 얻어진 값들을 해석하는 데 있어서의 통칙을 정해 보고자 하였다. 대상 및 방법: 2주기 술전 항암화학요법 후 외과적 절제를 시행한 18명의 골육종 환자들을 대상으로 하였다. 남자 13명, 여자 5명이었으며, 연령의 중위수는 19세였다. 술전 항암화학요법의 전후로 단순 촬영 및 양전자 방출 단층 촬영을 시행하였다. 절제된 종양에 대해 병리검사를 시행하여 조직학적 반응 등급을 확인하였다. 조직병리학적 괴사 정도와 방사선학적 소견, 술전 항암화학요법 후 최대 표준섭취계수(maximum standardized uptake value), 평균 표준섭취계수(average standardized uptake value), 대사종양용적(metabolic tumor volume) 및 해당 값들의 감소율에 대해 통계학적 분석을 시행하였다. 결과: 조직학적 평가 상, 8명의 환자에서 술전 항암화학요법에 대해 좋은 반응을 확인하였다. 최대 표준섭취계수 감소율의 중위수는 좋은 반응을 보인 군에서 74 (23-77) %였고, 나머지에서 42 (-32-76) %였다. 대사종양용적 감소율의 중위수는 좋은 반응을 보인 군에서 93.5 (62-99) %였고 나머지에서 46 (-81-100) %였다. 방사선학적 소견의 범주는 조직학적 반응에 따른 차이가 없었지만 (p=1.0), 최대 표준섭취계수의 감소율은 유의한 차이를 보였다(p=0.041). 대사종양용적의 감소율 차이는 통계학적 유의성에 근접하였다(p=0.071). 결론: 본 연구에서 방사선학적 소견의 범주는 골육종에 대한 술전 항암화학요법의 효과를 평가하는 데 있어 신뢰할 수 없었지만, 최대 표준섭취계수는 유용한 척도였다. 양전자 방출 단층 촬영에서 얻어지는 값들은 여러 요인에 의해 영향을 받으므로, 각 기관은 기왕의 연구들을 참고로 각자의 판단 기준을 마련하기 위해 노력해야만 한다.

표준 항암화학요법에 실패한 진행성 비소세포폐암에서 ZD1839 ($Iressa^{TM}$)의 효과 (The Efficacy of ZD1839 ($Iressa^{TM}$) in Patients with Advanced Non-small Cell Lung Cancer which has Progressed After Previous Chemotherapy)

  • 배 경 : 비소세포폐암에서 2차 항암화학요법에 대한 치료반응과 역할은 아직 정립되어 있지 못한 실정이다. 한편, 최근 개발된 ZD1839는 상피세포성장인자 수용체 억제제로서 악성 고형 종양, 특히 비소세포폐암에서 항종양효과를 보이는 것으로 알려져 있다. 저자들은 기존의 표준 항암화학요법에 실패한 진행성 비소세포폐암 환자들을 대상으로 ZD1839를 투여하여 그 반응 및 안전성을 평가하고자 하였다. 대상 및 방법 : 원자력병원에서 비소세포폐암으로 조직학적 진단을 받고 임상 병기 IIIB 이상인 환자들 중에서 1차 이상의 항암화학요법을 시행 받은 후 치료실패로 판정된 환자들을 대상으로 2002년 1월부터 2003년 9월까지 ZD1839를 투여 하였다. 투여 용량은 하루 250 mg이었고, 반응 및 부작용을 평가하기 위해 1개월 간격으로 흉부 방사선 검사 및 외래 추적 검사를 실시하였다. 이 환자들 중 반응 및 독성의 평가가 가능하였던, 1개월 이상 투여 받은 83명의 환자들을 대상으로 하였다. 환자들의 중앙 연령은 59(33-76)세였고, 임상 병기는 IIIB가 12명, IV가 71명이었으며, ECOG 전신수행상태는 0-1이 10명, 2는 42명, 3은 31명이었다. ZD1839 투여일수는 중앙값이 90일이었다. 결 과 : ZD1839 투여 후 부분 반응은 12명(14.5%), 불변은 31명(37.3%), 진행은 40명(48.2%)이었고, 전체 생존기간과 병의 진행까지 기간의 중앙값은 각각 9.2 개월과 3.1 개월이었다. ZD1839의 부작용으로는 피부 발진을 보인 환자가 25명(25.8%)으로 가장 많았으며, 구토 및 설사를 보인 환자가 15(15.5%), 비정상적 간효소수치 상승 및 혈소판 감소를 보인 환자가 각각 1명(1.03%)이었다. Performance status 0, 1인 전신 상태와 선암종을 가진 환자군에서 반응률 및 생존률이 통계적으로 유의한 증가를 보였다. 결 론 : 기존의 항암화학요법에 실패한 일부 진행성 비소세포폐암 환자들에서 ZD1839 투여는 비교적 높은 반응률을 보였으며 부작용은 비교적 경미하였다. 향후 보다 대규모의 연구 결과에 대한 추시가 필요할 것으로 생각된다.

대장암 항암 화학요법의 처방 오류 평가 및 개선안 제시 (Guideline of Improvement and Evaluation of Prescribing Errors in Colorectal Chemotherapy)

  • Background: Colorectal cancer shows a significant increase in South Korea due to westernization of diet, lack of dietary fiber, drinking and smoking, irregular defecation. There are surgery, chemotherapy, radiation therapy in treatment of colorectal cancer. There may be a medication errors in the process of chemotherapy because of its high toxicity, narrow therapeutic index and the health status of cancer patients. Consequently medication errors can cause increasing the risk of death, prolonging hospital stay and increasing the cost. Among medication errors on medication use process, prescribing errors are of particular concern due to higher risk of serious consequences. It is important for pharmacist to prevent the prescribing errors before reaching patient. Therefore we analyzed the prescriptions of colorectal cancer, classified prescribing errors, suggested guideline to reduce prescribing errors and verified the importance of pharmacist's role in prevention of medication errors activity. Methods: We collected the numbers of prescriptions of colorectal cancer(n=2,373) through anti cancer management program and EMR and analyzed the errors of prescriptions by categories from Oct 1st 2011 to Sep 30th 2012 at Chungbuk National University Hospital. We reviewed the prescriptions as follows - patients' characteristics, the result of test, previous prescriptions, characteristics of antineoplastic agents and patients' allergy, drug sensitivity, adverse events. Prescriptions are classified into inpatient and outpatient and analyzed the errors of prescriptions by categories (dosage form, dose, input, diluents, regimen, product). Results: Total prescription number of inpatient and outpatient of colorectal cancer was 1,193 and 1,180 and that of errors was 107(9%) and 22(1.9%), respectively. In case of errors of categories, the number of errors of dosage form is 69 and 8, errors of dose is 15 and 5, errors of input is 9 and 9 in inpatient and outpatient prescriptions, respectively. Errors of diluents is 8, errors of regimen is 3, errors of product is 3 in only inpatient prescriptions. In case of errors of categories by inpatient department, the number of errors of dosage form is 34 and 35, errors of dose is 7 and 8, errors of input is 6 and 3, errors of diluents is 4 and 4, errors of regimen is 2 and 1, errors of product is 2 and 1 in SG and HO, respectively. In case of outpatient department, the number of errors of dosage form is 8 in HO, errors of dose is 5 in HO, errors of input is 5 and 4 in SG and HO, respectively. Conclusions: The rate of errors of inpatient is higher than that of outpatient. Junior doctors are engaged in prescriptions of inpatient and pharmacist need to pay attention to review all prescriptions. If prescribing errors are discovered, pharmacist should contact the prescriber and correct the errors without delay. The guideline to reduce prescribing errors might be upgrading software of anti cancer management program, education for physicians as well as pharmacists and calling prescriber's attention to preventing recurrence of errors.

Efficacy and Safety of Neurokinin-1 Receptor Antagonists for Prevention of Chemotherapy-Induced Nausea and Vomiting: Systematic Review and Meta-analysis of Randomized Controlled Trials

  • Objectives: Can addition of neurokinin-1 receptor antagonists (NK1-RAs) be considered as an ideal strategy for the prevention of chemotherapy-induced nausea and vomiting (CINV)? Researchers differ on this question. Materials and Methods: Electronic databases were searched for randomized control trials (RCTs) that evaluated the effectiveness and safety of NK1-RAs in preventing CINV. The primary end point was complete response (CR) in the acute, delayed, and overall phases after chemotherapy. Subgroup analyses evaluated the types of NK1-RAs, routines of administration, types of malignancies, regimens used in combination with NK1-RAs, and age of patients included in the studies. The incidences of different types of adverse events were also extracted to estimate the safety of NK1-RAs. Results: A total of 38 RCTs involving 13,923 patients were identified. The CR rate of patients receiving NK-RAs was significantly higher than patients in the control groups during overall phase (70.8% vs 56.0%, P<0.001), acute phase (85.1% vs 79.6%, P<0.001), and delayed phase (71.4% vs 58.2%, P<0.001). There were three studies including patients of children or adolescents, the CR rate was also significantly higher in the treatment group (overall phase: OR=2.807, P<0.001; acute phase: OR=2.863, P =0.012; delayed phase: OR=2.417, P<0.001). For all the other outcomes, patients in the NK1-RAs groups showed improvements compared to the control groups (incidence of nausea: 45.2% vs 45.9%, P<0.001; occurrence of vomiting: 22.6% vs 38.9%, P<0.001; usage of rescue drugs: 23.5% vs 34.1%, P<0.001). The pooled side effects from NK1-RAs did not significantly differ from previous reports and the toxicity rates in patients less than eighteen years old also did not diff between the two groups (P=0.497). However, we found that constipation and insomnia were more common in the patients of control groups, whereas diarrhea and hiccups were more frequently detected in patients receiving NK1-RAs. Conclusions: NK1-RAs improved the CR rate of CINV. They are effective for both adults and children. The use of NK1-RAs might be associated with the appearance of diarrhea and hiccups, while decreasing the possibility of constipation and insomnia.
