Utilization of Preventive Therapy in Korean Migraine Patients

  • Background: Migraine is a common neurological disorder that affects the quality of life and causes several health problems. Preventive migraine treatment can reduce migraine frequency, headache severity, and health care costs. This study aimed to estimate the utilization of migraine preventive therapy and associated factors in eligible patients. Methods: We studied 534 patients with migraine who were eligible for migraine preventive therapy using 2017 National Patient Sample (NPS) data from the Health Insurance Review and Assessment Service (HIRA). We estimated the migraine days by calculating the monthly average number of defined daily dose (DDD) of migraine-specific acute drug. Patients with a monthly average number of DDD of 4 or more were considered as subjects for preventive treatment. Chi-square test and multiple logistic regression analysis were used to determine the association between the preventive therapy and the influencing variables. Results: Less than half of the eligible patients for prophylaxis (n=234, 43.8%) were prescribed preventive therapy. Multiple logistic regression results show that migraine preventive therapy was influenced by age, the type of migraine, and some comorbidities. Patients over the age of 50 tend to receive less prophylactic treatment than under the age of 40. On the other hand, migraine patients with epilepsy or depression were more likely to receive preventive therapy. Sumatriptan was the most preferred medication for acute treatment, and propranolol was the most commonly prescribed drug for prevention. Conclusions: More than half of the patients who were candidates for migraine prophylaxis were not receiving suitable preventive treatment. Positive factors affecting the use of migraine prevention were the presence of comorbidities such as epilepsy and depression.

Management of anxiety using eye movement desensitization and reprocessing therapy in children undergoing extraction: a randomized controlled pilot study

  • Background: Eye movement desensitization and reprocessing (EMDR) therapy has been reported to be very efficacious for treating post-traumatic stress disorder (PTSD) and other anxiety-related conditions. However, a review of the literature reveals the sparse use of this therapy in the field of pediatric dentistry. This study aimed to evaluate anxiety trends in pediatric dental patients during local anesthesia and extraction with and without EMDR therapy. Methods: Children in the age range of 8-12 years who required dental extractions were assigned randomly into two groups: an EMDR group (group 1) and a routine behavior management therapy group (group 2; receiving more traditional interventions such as tender love and care behavioral modeling, and distraction). Anxiety scores were recorded at four levels using the visual facial anxiety scale (VFAS) preoperatively, after therapy, after the administration of local anesthesia (LA), and after extraction. Results: Reduced anxiety was observed after the delivery of EMDR therapy, after LA administration, and post-extraction in the EMDR group compared to pre-operative anxiety scores of anxiety (P < 0.001; unpaired Student's t and Mann-Whitney U tests). In the control group, mild reductions in anxiety after routine behavior management therapy were observed, accompanied by spikes in anxiety levels after LA and extractions. Conclusion: EMDR therapy was found to be valuable for reducing anxiety among pediatric dental patients during tooth extraction procedures.

Factors Associated with Hypertension Control and Antihypertensive Medication among Hypertensive Patients in a Community (지역사회 고혈압 환자에서 고혈압 조절 및 항고혈압제 복용에 관련된 요인)

  • Objectives : The main objectives of this study are to identify the factors associated with hypertension control and to determine the influencing factors associated with antihypertensive therapy. Methods : The study was conducted on 107 subjects who participated in the 1999 and 2002 Gwacheon Study and who had had uncontrolled hypertension (systolic$\geq$140 or diastolic$\geq$ 90mmHg) in 1999, We compared the characteristics of the controlled and uncontrolled hypertensive group and investigated the characteristics of those taking antihypertensive medication. Univariate associations between hypertension control and its characteristics and the association between antihypertension treatment and its characteristics were tested with $x^2$-test. We also peformed logistic regression analysis. Results : The participants who had their blood pressure checked within 6 months before their first measurement and those who had taken the antihypertensive medication showed significantly better hypertension control during the follow-up. The multivariate analysis showed that baseline antihypertensive therapy was the most important determinant factor of hypertension control during the follow-up. Socioeconomic level and life style did not affect hypertension control when controlled by the treatment variable in this study. The factors associated with antihypertensive therapy at follow-up were previous antihypertensive therapy, old age, and high educational level. Conclusions : Those who received antihypertensive therapy and those who had their blood pressure re-checked within 6 months both showed well controlled hypertension. The subjects with high educational level complied well with the antihypertensive regimen, but those in their forties did not.

  • In spite of different traditions, there are similar things between oriental psychotherapy and cognitive therapy. 1. Both of them stress the need of relations between patient and therapist.2. Exposure treatment and relaxation in cognitive behavioral therapy are similar with Qigong therapy and Kyungja-pyungji treatment in oriental psychotherapy.3. Both of them demand to avoid the avarice and stiff thoughts in life.4. Both of them are related to preventive psychiatry.

The Effects of Knee Muscle Taping Therapy on Parameter of Muscle Activities (슬관절 근육 테이핑요법이 근활성도에 미치는 영향)

  • The purpose of this study is to find out the effects of taping therapy on muscle activities between the group of taping therapy treatment and control group before or after exercises. The subjects are 18 S college's healthy male students who do not have any problems in the legs and could bend the knee joint normally. Considering physique, condition of health and capacity for locomotion, 3 groups of each 6 persons for the experimentation were classified. Three groups composed of control group without taping therapy treatment for control group, group I with taping therapy treatment only before the experimentation and group II with taping therapy treatment during 24 hour. before and after the experimentation. EMG MP150 was used to evaluate the parameter of the lower leg's muscle activities the exercise before 5 min. after 10 min. and after 24 hour. for the experimentation. Compared with control group, group I and group II had more increased muscle activities in vastus lateralis and rectus femoris and was statistically significant. And group I and group II also had more increase in muscle activities than control group in vastus medialis, although there was no statistically significant. We found that taping therapy increased muscle activity positively. The application of taping therapy could be used not only for preventive effects for sports man but also therapeutic effects for general people.

Application and Prospective of Aroma Therapy for Preservation of Health(養生) (양생(養生)을 위한 향기치료(香氣治療)의 한의학적 적용(適用)과 전망(展望))

  • Aroma therpay in the western classical medicine is compared with the mechanistic paradigm of Newton physics which is the foundation of western modern medicine. It is the therapy whose basic principle is very similar to the basic concepts of oriental medicine lasting thousands of years. We know that parts of their therapies are explained by Yin-Yan and five elements in Oriental Medicine. In the modern western medicine, as preventive medicine is becoming more and more important and moreover, health promotion and self healing care is being emphasized, Maintenanayce which is the oriental preventive medicine, acts in concert with the concept of New Age of Science and provides a way for the Oriental Medicine of the future. The health promotion and self healing care is similar to the thought of Maintenanayce in Oriental Medicine. Oriental Medicine has provided the concept of preventive ehaling care for a long time and built the foundation of the department of Maintenanayce. Among these method of Preservation of Health, We could find that of using fragrance throuth the several references. By comparing the department of Maintenanayce in oriental preventive medicine with the books and papers about aroma therapy, used in the fields of naturalpathy medicine, alternative medicine, and complementary medicine in the west, and by analyzing them, centering around references, to see whether they are reasonable and corresponding, the following conclusions have been reached. 1. We could see that aroma therapy of oriental preventive medicine could be applied to the fields of health promotion and self healing cure, recently appearing in the world of health medicine, as a modern way of Preservation of Health(養生). 2. We could see that western traditional aroma therapy take important position in alternative medicine and complementary medicine, and it could be developed as the transitional field of medical cure for the interchange between western and eastern medicine. 3. We could see that aroma therapy is not totally unknown therapy and there is some points of similarity in the traditional references of oriental medicine. Also, it is considered that the aroma therapy has a possibility of coming the front as an general therapy among various therapies. 4. aroma therapy will be actively applied to many fields such as oriental psychiatry, internal medicine of the respiratory system, pediatrics, oriental nursing, oriental dermatologic beauty, and preventive medicine. Therefore, we have a view that the combination between aroma therapy and oriental medicine will be fairly valuable to study as the general and transtional middle step on which it will prepare the situation of oriental medicine's reimportation from the west and we establish a bridgehead to export the oriental medicine.

The Relationship between ACE I/D Polymorphism and HDL Cholesterol (ACE 삽입 및 결손 유전자 다형성과 HDL 콜레스테롤과의 관련성)

  • Objectives : The purpose of this study is to evaluate the association of the angiotensin converting enzyme (ACE) insertion/deletion (I/D) polymorphism with cardiovascular disease risk factors. Methods : Out of a total of 608 middle-aged adults who visited local health centers, 424 subjects (104 male, 320 female) who had not been diagnosed with hypertension, diabetes mellitus, or hyperlipidemia were included in this study. ACE genotypes were determined in all subjects by polymerase chain reaction methods. Results : Statistical differences in high-density lipoprotein (HDL) cholesterol levels according to ACE genotype were observed using ANOVA (p<0.05), but no differences were found in other cardiovascular risk factors. Specifically, men with the DD and DI genotypes had significantly lower HDL cholesterol levels than those with the 11 genotype based on the LSD multi-comparison test (p<0.05). Conclusions : In men, the D-allele of the ACE I/D polymorphism was significantly associated with reduced HDL cholesterol levels. In the future, larger studies are needed to confirm this relationship between ACE I/D polymorphism and HDL cholesterol.
