대학생들의 수면의 질과 우울증상과의 관계 : 긍정심리의 조절효과 (The Relationship between Sleep Quality and Depressive Symptoms among University Students : Moderating Effect of the Positive Psychology)

  • Objectives : The aim of study was to estimate the prevalence of poor sleep quality and depressive symptoms as well as to examine the moderating effect of positive psychology on the relationship between poor sleep quality and depressive symptoms among Korean university students. Methods : A total of 2,338(1,071 male and 1,267 female) university students at seven universities of four cities in Korea were randomly selected to participate in a survey. Data were collected by self-report questionnaires, including Pittsburgh Sleep Quality Index(PSQI), Beck Depression Inventory(BDI), Satisfaction with Life Scale(SWLS), Positive Psychotherapy Inventory(PPTI), and sociodemographic characteristics. Results : The prevalence was 44.8% for poor sleep quality and 14.4% for depressive symptoms among Korean university students. By moderated regression analysis, the results showed that poor sleep quality(${\beta}=0.453$, p<0.001) aggravated depressive symptoms of university students, and life satisfaction(${\beta}=-0.087$, p<0.001) and happiness(${\beta}=-0.145$, p<0.001) respectively buffered the negative effect of the poor sleep quality on depressive symptoms. Conclusion : This study showed very high prevalence of poor sleep quality and depressive symptoms among Korean university students. Overall, these findings demonstrates that positive psychology can be effective in the enhancement of subjective and psychological well-being and may help to reduce depressive symptom levels. In the context of public health, positive psychology can be used as preventive, easily accessible and non-stigmatizing tools.

Subcortical Ischemic Change as a Predictor of Driving Cessation in the Elderly

  • Objective Motor, perceptual, and cognitive functions are known to affect driving competence. Subcortical ischemic changes on brain magnetic resonance imaging (MRI) can reflect reduction in cognitive and motor performance. However, few studies have reported the relationship between subcortical ischemic changes and driving competence of the elderly. Thus, the objective of this study was to investigate the association between subcortical ischemic changes on MRI and driving abilities of the elderly. Methods Participants (n=540) were drawn from a nationwide, multicenter, hospital-based, longitudinal cohort. Each participant underwent MRI scan and interview for driving capacity categorized into 'now driving' and 'driving cessation (driven before, not driving now)'. Participants were divided into three groups (mild, n=389; moderate, n=116; and severe, n=35) depending on the degree of white matter hyperintensity (WMH) on MRI at baseline. Driving status was evaluated at follow-up. Statistical analyses were conducted using ${\chi}^2$ test, analysis of variance (ANOVA), structured equation model (SEM), and generalized estimating equation (GEE). Results In SEM, greater baseline degree of WMH was directly associated with driving cessation regardless of cognitive or motor dysfunction (${\beta}=-0.110$, p<0.001). In GEE models after controlling for age, sex, education, cognitive, and motor dysfunction, more severe change in the degree of WMH was associated with faster change from 'now driving' state to 'driving cessation' state over time in the elderly (${\beta}=-0.508$, p<0.001). Conclusion In both cross-sectional and longitudinal results, the degree of subcortical ischemic change on MRI might predict driving cessation in the elderly.

Effects of an Online Imagery-Based Treatment Program in Patients with Workplace-Related Posttraumatic Stress Disorder: A Pilot Study

  • Objective We developed easily accessible imagery-based treatment program for patients with post-traumatic stress disorder (PTSD) related to workplace accidents and investigated the effects of the program on various PTSD related symptoms. Methods The program was based on an online platform and consisted of eight 15-min sessions that included script-guided imagery and supportive music. Thirty-five patients with workplace-related PTSD participated in this program 4 days per week for 4 weeks. Its effects were examined using self-report questionnaires before and after the take-home online treatment sessions. Results After completing the 4-week treatment program, patients showed significant improvements in depressed mood (t=3.642, p=0.001) based on the Patient Health Questionnaire-9 (PHQ-9), anxiety (t=3.198, p=0.003) based on the Generalized Anxiety Disorder seven-item (GAD-7) scale, and PTSD symptoms (t=5.363, p<0.001) based on the Posttraumatic Stress Disorder Check List (PCL). In particular, patients with adverse childhood experiences exhibited a greater degree of relief related to anxiety and PTSD symptoms than those without adverse childhood experiences. Conclusion The present results demonstrated that the relatively short online imagery-based treatment program developed for this study had beneficial effects for patients with workplace-related PTSD.

정신과의 분자생물학 적용 (Molecular Application in Psychiatry)

  • The development of molecular biology has brought many changes in psychiatry. Molecular biology makes us possible to know the cause of mental disorders that provide the way to prevent the disorders, and to develop various accurate diagnostic and treatment methods for mental disorders. The author discusses the concept, cause, and treatment of mental disorders in the aspect of molecular biology. Importing the methods of molecular biology into psychiatry, we can anticipate to get a number of the goals of psychiatric genetics, including identification of specific susceptibility genes, clarification of the pathophysiological processes whereby these genes lead to symptoms, establishment of epigenetic factors that interact with these genes to produce disease, validation of nosological boundaries that more closely reflect the actions of these genes, and development of effective preventive and therapeutic interventions based on genetic counseling, gene therapy, and modification of permissive or protective environmental influences. In addition to their capacity to accelerate the discovery of new molecules participating in the nervous system's response to disease or to self-administered drugs, molecular biological strategies can also be used to determine how critical a particular gene product may be in mediating a cellular event with behavioral importance. Molecular biology probably enables us discover the environmental factors of mental disorders and allow rational drug design and gene therapies for mental disorders, by isolation of gene products that facilitate a basic understanding of the pathogenesis of these disorders. A specific genetic linkage may suggest a novel class of drugs that has not yet been tried. With respect to gene therapy, the hypothetical method would use a gene delivery system, most likely a modified virus, to insert a functional copy of a mutant gene into those brain cells that require the gene for normal function.

우울증 자살시도자들에서 표준우울평가척도들의 요인분석 : CRESCEND 연구 (Factor Analysis of Standardized Depression Scales in Suicidal Attempters among Patients with Depressive Disorders : CRESCEND Study)

  • Objective : The aim of this study was to find associations between specific symptoms and suicidal behavior by using global severity on depression scales. Methods : Data were obtained from 1,183 patients of CRESCEND study. Factor analysis of the 17-item Hamilton Rating Scale for Depression (HAM-D) and the Beck Depression Inventory-II (BDI-II) was used to compare symptom clusters between the past suicide attempters and the non-suicide attempters, among the patients with depressive disorders. Results : Factor analyses of HAM-D and BDI-II extracted three factors. Suicide attempters had significantly higher scores on core factor of HAM-D, including depressed mood, feeling of guilt, suicide, work, and activities. The higher scores on all factors of BDI-II or scores correlated with a total of 7 suicide attempts and with known risk factors for suicidal behavior. Conclusion : These findings suggest that the suicide attempters complained of more cognitive, affective, somatic symptoms on BDI-II, but the core factor on HAM-D was the only differentiated factor between the two groups. Clinician-rated scales as well as self-reported questionnaires were valid in measuring suicidal attempts, and the clinical profile may help in guiding the studies of biological correlates and the treatments to reduce suicide risk.

일 도시 감정노동자의 직무스트레스와 우울증의 연관성 (The Relationship Between Job Stress and Depressive Disorder Among Emotional Laborers in a City)

  • Objective : The purpose of our study was to investigate the relationship between job stress and depressive disorder among emotional laborers in a city. Methods : A total of 677 emotional laborers living in Jeonju-city participated in this study. The participants completed survey questions regarding demographic characteristics, job stress (using the Korean Occupational Stress Scale-24, KOSS-24), and depression (the Patient Health Questionnaire-9, PHQ-9). Logistic regression was performed to estimate the association between job stress and depression. Results : Our results indicated that 34.9% of the participants had a high risk of total job stress and 21.7% of participants had depression. In the multivariate logistic regression analysis, female sex (OR=1.66, 95% CI=1.01-2.72) and total job stress (OR=3.84, 95% CI=2.57-5.75) were significantly associated with depression after adjusting for confounding factors. Among the subscales of job stress, job demand (OR=3.06, 95% CI=1.99-4.72) and occupational climate (OR=1.88, 95% CI=1.19-2.97) were significantly associated with depression. But insufficient job control, interpersonal conflict, job insecurity, the organizational system and lack of rewards were not. Conclusion : Among the emotional laborers, job stress, particularly job demand and occupational climate, were major contributing factors associated with depression. Early screening and preventive strategies focusing on job stress could reduce the impact of depression in emotional laborers.

한국 성인의 자살태도측정도구의 요인구조 : 2013년 자살실태조사 자료를 바탕으로 (Factor Structure of Attitudes Toward Suicide (ATTS) of Adults : Based on a Nationwide Survey in South Korea)

  • Objectives Suicide is a complex phenomenon not only caused by the individual's mental illness and economic situation, but also occurred in sociocultural contexts and relationships around the individual. Therefore, the attitudes, which include collective norms, thoughts and feelings of a population, toward suicide play a fundamental role in the prevention of suicide. Factor structure of Attitudes Toward Suicide (ATTS), developed by Renberg, is also various in different sociocultural contexts. This is the first study to investigate factor structure of ATTS along with nationwide sample of Korean adults. Methods This study analyzed the factor structure and reliability of ATTS in 1500 regionally stratified Korean adults. Exploratory factor analysis was performed by using principal axis factoring and varimax rotation. Results The selection of 33 ATTS items based on internal consistency and communality was conducted. Five factors were analyzed, which were named 'Good reason,' 'Negative gaze,' 'Predictability,' 'Means of escape,' and 'Common secret,' respectively. Explained total variance was 41.94%, and the internal consistency ranged from 0.516 to 0.740. Conclusions Each of the five factors contains different semantic dimensions. Consistent with previous studies, factors containing permissive attitude toward suicide was high in those with history of suicidal thought and of suicide attempt. It is notable that the 'Good reason' showed higher score in elderly. This may be related to socioeconomic or physical adversities which have been considered the major cause of suicide in elderly.

  • 연구목적:대한소아청소년정신의학회 창립 20주년을 맞아 한국 소아청소년정신의학의 발전 과정(과거)을 돌아보고, 현재의 상황과 문제점을 짚어보고, 앞으로 나아가야할 방향과 주요과제를 살펴 보고자함. 연구방법:학회기록, 관련논문, 원로들과의 개인통신, 저자개인기록과 참여경험을 토대로 함. 결 과:한국소아정신과의 공식출범은 1979년 서울대학교병원외래에 소아정신과클리닉의 개설과, 다음해 동대학병원에 소아정신분과 설립으로 볼 수 있음. 대한소아, 청소년정신의학회는 1983년 한국소아정신의학연구회로 시작하여 1986년 동학회가 공식 창립되었음. 창립후 지난 20년간 학회는 괄목할만한 성장과 발전을 하였는데 그 원동력은 소아청소년정신과 전임의 수련프로그람이 주요대학병원(현재 총 7개)에 생겼고, 1994년부터 한국소아청소년정신의학회지 발간, 1996년 대한소아청소년정신과 수련심사위원회의 발족과 역할수행에서 찾을 수 있음. 현재 한국에 77명의 소아청소년정신의학 분과전문의와 48명의 부가수련중인 전문의, 18명의 전임의가 수련 중에 있음. 현재 한국소아청소년정신과는 국내적으로는 중흥기를 맞이하고 있고 인접전문분야의 발전에도 크게 기여하고 있음, 국제적으로도 아시아소아청소년학회의 창립과 학술활동에 중추적 역할을 하고 있을 뿐 아니라 미국 및 국제 소아청소년정신의학회등 국제무대로 역할을 확대 중임. 결 론:한국소아청소년정신의학은 지난 20년간 성공적인 발전을 해왔고, 의학계와 일반 사회에 크게 기여하고 있음. 그러나 청소년문제에 대한 적극적 개입, 병원 및 정신질환 관련기관에서의 자문, 연결, 학교자문서비스에 좀더 적극적인 참여가 필요함. 우리나라는 지난 30여년간 급격한 정치적, 경제적, 사회문화적 변화를 겪었고, 특히 가족체계의 변화와 붕괴현상은 날로 심각해져, 소아청소년의 정신건강문제는 증가일로에 있는 것이 분명함으로 앞으로 소아청소년정신과 전문의의 역할은 중차대함. 뿐만 아니라, 우리는 아시아지역 국가들의 낙후된 소아청소년정신과서비스 개발과 발전에도 기여하여야할 사명이 있는 것으로 제안함.

Month and Season of Birth as a Risk Factor for Alzheimer's Disease: A Nationwide Nested Case-control Study

  • Objectives: Season of birth, an exogenous indicator of early life environment, has been related to higher risk of adverse psychiatric outcomes but the findings for Alzheimer's disease (AD) have been inconsistent. We investigated whether the month or season of birth are associated with AD. Methods: A nationwide nested case-control study including all community-dwellers with clinically verified AD diagnosed in 2005 to 2012 (n=70 719) and up to four age- sex- and region of residence-matched controls (n=282 862) residing in Finland. Associations between month and season of birth and AD were studied with conditional logistic regression. Results: Month of birth was not associated with AD (p=0.09). No strong associations were observed with season (p=0.13), although in comparison to winter births (December-February) summer births (June-August) were associated with higher odds of AD (odds ratio, 1.03; 95% confidence interval, 1.00 to 1.05). However, the absolute difference in prevalence in winter births was only 0.5% (prevalence of those born in winter were 31.7% and 32.2% for cases and controls, respectively). Conclusions: Although our findings do not support the hypothesis that season of birth is related to AD/dementia risk, they do not invalidate the developmental origins of health and disease hypothesis in late-life cognition. It is possible that season does not adequately capture the early life circumstances, or that other (postnatal) risk factors such as lifestyle or socioeconomic factors overrule the impact of prenatal and perinatal factors.
