Completeness Estimation of the Korean Medical Insurance Data in Childhood Asthma : Using Capture-Recapture Method (소아 천식을 통해서 본 의료보험 상병 자료의 완전성 추정 : Capture-Recapture 분석방법의 적용)

  • Objectives : The purpose of this paper is to estimate the completeness of the Korean Medical Insurance Data in childhood asthma. Methods : Capture-recapture method was used to estimate the prevalence of childhood asthma and case ascertainment rate(completeness) of Korean Medical Insurance Data using two source model, 'Korean Medical Insurance Committee Data (KMICD)' and 'Nationwide Study of Asthma and Allergies in Korean Children'. The asthma cases were restricted to those who were born from 1981 to 1989 and were identified by their Resident Register Number. Asthma cases in Korean Medical Insurance Data were defined as cases coded by ICD-9 493 and ICD-10 J45. In 'Nationwide Study of Asthma and Allergies in Korean Children', asthma cases were defined as the children who had been diagnosed asthma and had experienced symptoms of asthma during the past 12 months. The defined cases in two data sources were matched by 13 digits Resident Register Number. The numbers of matched patients in two data sources were 245 of 32,825 eligible total subjects. Chapman and Wittes' nearly unbiased estimation was used for capture-recapture analysis of two data sources. Results : Observed prevalence rate of childhood asthma was 5.3% and estimated prevalence rate by capture-recapture analysis was 11.6%. The highest prevalence rate was observed in 6-7 age group and the older the rate decreased. The completeness (the proportion of cases ascertained by KMICD to the total observed cases by two data sources) was 20.6%, and ranged form 10.8% to 28.8% by area. Conclusions : Invalid diagnosis of cases might overestimate the prevalence of child-hood asthma and might underestimate the completeness of Korean Medical Insurance Committee Data in this study.

Survey of Corrosion Cost in China and Preventive Strategies

  • A national consultative project entitled "corrosion cost survey in China and preventive strategies" was funded by the Chinese Academy of Engineering in 1998. Soon afterwards, an expert group was organized jointly by the Institute of Metal Research, CAS and Chinese Society of Corrosion and Protection. The report on corrosion cost survey in China was published in 2003. According to this report the overall annual corrosion cost in China estimated by the Uhlig Method and Hoar Method at 1997-2001 was found to be 200.7 billion Yuan RMB and 228.8 billion Yuan RMB respectively, which is equivalent to 2% of the gross national product of China. However the total cost of corrosion including the direct and indirect cost was estimated to be more than 500 billion Yuan RMB per year in China. Among them, corrosion cost of infrastructure ranked in first comparing with other sectors. Although corrosion costs in some sectors, such as electric power, petrochemical, oil pipeline and railway in China has reduced in the past years, significant losses are still being encountered in most sectors of industries and cost-effective methods have not always been implemented. Both successful and unsuccessful cases in corrosion control and corrosion management were collected. As the investment in capital construction continues increasing rapidly in China, the maintenance and life extension of the infrastructures will become a big issue. INTERPRETING A SINGLE ANTISTREPTOLYSIN O TEST: A COMPARISON OF THE 'UPPER LIMIT OF NORMAL' AND LIKELIHOOD RATIO METHODS

  • Single serologic tests may occasionally influence clinicians in making diagnoses. The antistreptolysin O (ASO) test is a frequently used tool for detecting recent Streptococcus pyogenes infection and is helpful in the diagnosis of diseases like rheumatic fever. Using data from a 1989 prospective study of 600 healthy male military recruits, in which 43% experienced S. pyogenes upper respiratory tract infection (2-dilution rise in ASO), this report compared two methods of interpreting a single ASO titer. Using the 'upper limit of normal' (80 percentile) method, recruits with an ASO titer of greater than 400 showed evidence of recent S. pyogenes infection. This method had a sensitivity and specificity of only 65.9 and 81.9% respectively. In contrast to the 'yes-no'. dichotomy of the 'upper limit of normal' method. the likelihood ratio method statistics were ASO value specific, more consistent with clinical judgment, and better emphasized the caution clinicians must use in interpreting a single ASO test.

Review on Geumnaeng Method - Focus on Chinese Medical Articles - (금냉법(金冷法)에 대한 고찰 - 중국 논문을 중심으로 -)

  • Objectives : 'Geumnaeng method'' is a well-known folk remedy, but It has not been studied in academia. This study was conducted to review the chinese published articles on Geumnaeg method (Jinleng method) of Chinese traditional medicine. Methods : The author searched Chinese published papers from 2000 to 2018 via CNKI(China National Knowledge Infrastructure) database by using keyword 'Jinleng', 'Jinleng method', and analyzed the papers covered Jinleng method health preservation, and classified them including periods, type of study, target symptoms, and comparison between countries. Results : 17 studies were reviewed. The study of Jinleng method in China began in 2005. 8 articles (47.1%) were review articles, 4 articles (23.5%) of the case report and case series, and 5 articles (29.4%) were clinical studies. Clinical studies have increased since 2008. Most of the studies related to genital disorders and sexual function were mainly performed. There were differences between Japan, Korea and China in Jinleng method. Conclusions : Various disease and symptoms was researched with Jinleng method in China. Research in China is more active than other country. We need to increase the level of evidence of Jinleng method's effectiveness through additional studies in the future.

  • By using Siregnost FD 91 body plethysmograph, we measured thoracic gas volume (TGVe) at end of expiration in 19 healthy subjects aged 20-43 years in order to compare with functional residual capacity (FRC) measured by closed circuit method. The results obtained were as follows 1. Mean values of TGVe and FRC were $3.395{\pm}0.5851{\ell},\;and\;3.393{\pm}0.618{\ell}$, respectively. 2. A advantage of the body phethysmographic method for measuring thoracic gas volume was that it were rapid, safe, and easy to perform, requires no gas sample for chemical analysis, and measured TGVe several times.

Demand Forecasting for Developing Drug Inventory Control Model in a University Hospital (한 종합병원 약품 재고관리를 위한 수요예측(需要豫測))

  • The main objective of this case study is to develop demand forecasting model for durg inventory control in a university hospital. This study is based on the pertinent records during the period of January 1975 to August 1981 in the pharmacy and stock departments of the hospital. Through the analysis of the above records the author made some major findings as follows: 1. In A.B.C. classification, the biggest demand (A class) consists of 9 items which include 6 items of antibiotics. 2. Demand forecasting level of an index or discrepancy in A class drug compared with real demand for 6 months is average 30.4% by X-11 Arima method and 84.6% by Winter's method respectively. 3. After the correcting ty the number of bed, demand forecasting of drug compared with real demand for 6 months is average 23.1% by X-11 Arima method and 46.6% by Winter's method respectively.

  • Since guided missiles with the characteristics of the one-shot system remain stored throughout their entire life cycle, it is important to maintain their storage reliability until the launch. As part of maintaining storage reliability, period of preventive test is set up to perform preventive periodic test, in this case failure detection rate has a great effect on setting up period of preventive test to maintain storage reliability. The proposed method utilizes failure rate predicted by the software on the basis of MIL-HDBK-217F and failure mode analyzed through FMEA (Failure Mode and Effect Analysis) using data generated from the actual field. The failure detection rate of using the proposed method is applied to set periodic test of the actual guided missile. The proposed method in this paper has advantages in accuracy and objectivity because it utilizes a large amount of data generated in the actual field.

A Study on a Preventive Replacement Model by the Dynamic Programming Method (동적 계획법에 의한 예방교체모형에 관한 연구)

  • This paper is deals with the preventive replacement for the equipment which fails only when the total amount of damage reaches a prespecified failure level. Most of replacement model use time as their decision variable, but it is not appropriate for the cases in which failures dependent on their cumulative damage levels. In this paper, a new type preventive replacement model is introduced in which an equipment is replaced before failure when the cumulative damage reaches a certain level or replaced on failure, whichever occures first. The optimal replacement damage levels which minimize total expected cost are obtained by the Dynamic programming Method when the number of use of the equipment is finite. A numerical example is also presented. The optimal preventive replacement policy when the equipment will be used for a finite time span is also discussed.

  • Purpose: The purpose of this study is to propose a method to calculate the optimal preventive maintenance cycle of radar used in the aviation weapon system of ROKAF. Methods: A hybrid model is used to estimate the optimal preventive maintenance cycle in a system that can perform condition based predictive maintenance (CBPM) through continuous diagnosis. The failure data of the radars operating in the military were used to calculate the reliability. Results: According to the research results, the reliability threshold of the radar began to decrease after 5 flights, and decreased rapidly after 12 flights. Since the second check, costs have continued to decline. Conclusion: A method is proposed to determine the cycle of optimal preventive maintenance of radar within operational budget through modeling results between reliability limit and cost for radar. The results can be used to determine the optimal preventive maintenance cycle and frequency of various avionics equipment.

A Comparative Study on Evaluation Methods of Permanent Impairment in Korea (우리나라의 신체장애평가법에 관한 비교연구 - 신체장해등급법, McBride법, 미국의학협회 (AMA)기준법을 중심으로 -)

  • In this study, literature review was done to examine and compare the current status and problems of different evaluation approaches toward permanent impairment in Korea. Alternatives and improvements in the current approaches in Korea were suggested. Series of cases were also examined to compare different approaches applied to the real cases, using 105 cases from a hospital data and another 207 cases from a insurance company data. The main findings of the literature review are as follows; 1. The current evaluation methods of permanent impairment in Korea are grouped into two categories, grading and rating. Gradings of impairments are expressly specified in 17 various statutes. 2. In Grading methods, the rigid system of 74 different grades has been adopted uniformally for the convenience of administration, which may not be, appropriate or valid from medical and scientifical aspect. 3. The adventage of McBride method is assessment of occupational disability rate. However the classified compensable occupations are only 280 and limited to manufacturing industries in 1960s'of U.S.A., which is not appropriate to current Korean circumstances. Especially, the job list does not include managerial officers or mental workers. 4. AMA Guides is the scientific and reasonable method for the assessment of physical impairment rate. However compensation and reparation of impairment case is difficult because this method cannot assess the disability rate according to occupation, age, etc. The results of cases comparative study are as follows: 5. The physical Impairment could be compared in 167 out of total 312 cases, and for the cases of complex impairment, McBride method underestimate physical impairment rate compared with AM A method. 6. When disability late was assessed, occupation was considered the compensation of only 85 cases, and age was used in only 21 cases. This was because occupation and age compensation in McBride method are unreasonable. 7. The most Ideal alternative is to assess physical impairment according to AMA method and then to develop a compensation method appropriate for the circumstances of Korea society.
