임상실습을 경험한 간호대학생의 코로나바이러스감염증-19 (COVID-19) 관련 정보이해력, 위험인식 및 위기소통이 예방행위에 미치는 영향 (Effects of Information Literacy, Risk Perception and Crisis Communication Related to COVID-19 on Preventive Behaviors of Nursing Students in Clinical Practice)

  • 본 연구에서는 간호대학생의 코로나바이러스감염증-19 (Corona Virus Disease-19, COVID-19)에 대한 정보이해력, 위험인식 및 위기소통이 COVID-19 관련 예방행위에 미치는 영향을 확인하기 위해 시도되었다. 본 연구는 간호대학생 3,4학년 187명을 대상으로, 2020년 6월 25일부터 7월 3일까지 구조화된 설문지를 사용하여 자료를 수집하였고, 자료분석은 SPSS/WIN 26.0 프로그램을 이용하였다. 회귀분석 결과, 위기소통(β=0.30 p<.001), 정보이해력(β=0.29, p<.001), 위험인식(β=0.19 p=.004) 순으로 예방행위에 영향을 미치는 것으로 나타났으며, 총 모델 설명력은 27%였다. 본 연구는 간호대학생의 COVID-19 예방행위 향상을 위한 프로그램 개발시에 COVID-19와 관련하여 개인, 가족 및 사회 간의 위기사건에 대한 정보를 교환하는 소통과정을 활성화하고, 정보접근 및 이해력 수준을 향상시킴과 동시에 질병에 대한 위험인식을 높여야함을 시사하고 있다.

지역사회 기관 간 금연사업 네트워크 모델: 블록모델링을 중심으로 (Interorganizational Networks for Smoking Prevention and Cessation: A Blockmodeling Approach)

  • Purpose: This study examined characteristics and patterns of interorganizational networks for smoking prevention and cessation in Korea. Methods: We surveyed two community health centers, ninety-five hospitals or clinics, ninety- two pharmacies, and sixty-five health welfare organizations in two districts of Seoul in 2020. Data on the organizations' characteristics of smoking cessation and interorganizational activities for information sharing, client referral, and program collaboration were collected and analyzed using network statistics and blockmodeling. Results: Network size was in the order of information sharing, client referral, and program collaboration networks. Network patterns for interorganizational activities on information sharing, client referral, and program collaboration among four organizations were similar between the two districts. Community health centers provided information and received clients from a majority of the organizations. Their interactions were not unidirectional but mutual with other organizations. Pharmacies were involved in information sharing with health welfare organizations and client referrals to hospitals or clinics. Health welfare organizations were primarily connected with the community health centers for client referrals and program collaboration. Conclusion: A community health center is the lead agency in interorganizational activities for smoking prevention and cessation. However, hospitals or clinics, pharmacies, and health welfare organizations also participate in interorganizational networks for smoking prevention and cessation with diverse roles. This study would be evidence for developing future interorganizational networks for smoking prevention and cessation.

Optimization of Cost and Downtime for Periodic PM Model Following the Expiration of Warranty

  • This paper develops the optimal periodic preventive maintenance policies following the expiration of warranty: renewing warranty and non-renewing warranty. After the warranty period is expired, the system undergoes the PM periodically and is minimally repaired at each failure between two successive PMs. Firstly, we determine the expected cost rate per unit time and the expected downtime per unit time for the periodic PM model. Then the overall value function suggested by Jiang and Ji(2002) is applied to obtain the optimal PM period and the optimal PM number. Finally, the numerical examples are presented for illustrative purpose.

요통 처방을 통하여 본 한약제제 효능 개정의 필요성 (Necessity for Revising Efficacy of Herbal Medicines Based on Low Back Pain Prescription)

  • Objective : There are many documents that explain efficacy of herbal medicine products (HMP), however, explanations on efficacy are not consistent with the clinical use. The objective of this study is to identify the differences between herbal medicines used in clinics and Korean Medicine literatures. The disease was restricted in low back pain (LBP). Method : In order to investigate HMP for the treatment of LBP, we selected five Korean medical documents as reference and searched for key words related to LBP. Five databases were Information on Herbal prescription published by Ministry of Food and Drug Safety, 56 Herbal Medicines covered with Health Insurance, Guideline for prescribing herbal medicines announced by Ministry Health & Welfare, Explanation on Herbal Medicines published by Korean Pharmaceutical Association, website of Korea Pharmaceutical Information Center (http://www.health.kr/). The keywords were 'low back pain', 'lumbar', 'pain', 'myalgia', 'neuralgia', 'arthralgia', 'arthroneuralgia', and 'sciatica'. We also utilized the result of retrospective cross-sectional study in five university hospitals to investigate HMP used in practice for LBP. Results : From five databases, the number of searched HMP was 25, 12, 40, 12 and 38 respectively and 83 remained after removing duplications. There were 43 kinds of HMP used in clinical practice and only 20 (46.51 %) were included in one or more databases. Conclusion : This study suggests the necessity for reorganizing efficacy of herbal medicine. Standardizing explanation on herbal medicine should reflect the clinical conditions in further study.

Serum 2,3,7,8-Tetrachlorodibenzo-p-dioxin Levels and Their Association With Age, Body Mass Index, Smoking, Military Record-based Variables, and Estimated Exposure to Agent Orange in Korean Vietnam Veterans

  • Objectives: The aim of this study was to examine the levels of serum 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) and evaluate their association with age, body mass index, smoking, military record-based variables, and estimated exposure to Agent Orange in Korean Vietnam veterans. Methods: Serum levels of TCDD were analyzed in 102 Vietnam veterans. Information on age, body mass index, and smoking status were obtained from a self-reported questionnaire. The perceived exposure was assessed by a 6-item questionnaire. Two proximitybased exposures were constructed by division/brigade level and battalion/company level unit information using the Stellman exposure opportunity index model. Results: The mean and median of serum TCDD levels was 1.2 parts per trillion (ppt) and 0.9 ppt, respectively. Only 2 Vietnam veterans had elevated levels of TCDD (>10 ppt). The levels of TCDD did not tend to increase with the likelihood of exposure to Agent Orange, as estimated from either proximity-based exposure or perceived self-reported exposure. The serum TCDD levels were not significantly different according to military unit, year of first deployment, duration of deployment, military rank, age, body mass index, and smoking status. Conclusions: The average serum TCDD levels in the Korean Vietnam veterans were lower than those reported for other occupationally or environmentally exposed groups and US Vietnam veterans, and their use as an objective marker of Agent Orange exposure may have some limitations. The unit of deployment, duration of deployment, year of first deployment, military rank, perceived self-reported exposure, and proximity-based exposure to Agent Orange were not associated with TCDD levels in Korean Vietnam veterans. Age, body mass index and smoking also were not associated with TCDD levels.

우리나라 사망등록자료에서 의사에 의한 사망진단 분율의 양상과 관련 요인 (Proportion of Death Certificates Issued by Physicians and Associated Factors in Korea, 1990-2002)

  • Objectives : Previous studies showed that death certification by physicians was an important predictor to improve the quality of death certificate data in South Korea. This study was conducted to examine the proportion of death certificates issued by physicians and associated factors in South Korea from 1990 to 2002. Methods : Data from 3,110,883 death certificates issued between 1990 and 2002, available to the public from the National Statistical Office of Korea, were used to calculate the proportion of death certificates issued by physicians and to examine associated factors with logistic regression analysis. Results : The overall proportion of death certificates issued by physicians increased from 44.6% in 1990 to 77.6% in 2002 (mean: 63.5%). However, the proportion was greatly influenced by the deceased's age. In 2002, more than 90% of the deceased aged 51 or less were certified by physicians. A higher proportion was found among deceased who had tertiary education (college or higher) living in more developed urban areas. Conclusion : The information regarding the cause of death for younger, well-educated deceased in urban areas of South Korea may show a higher level of accuracy. Epidemiologic research using information on causes of death may well benefit from the continually increasing proportion of death certificates issued by physicians in the future in South Korea.

노인 인구에서 육체적 활동과 고관절 골절 발생간의 관련성 : 코호트 연구 (Physical Activity and Hip Fracture in Elderly People : A Cohort Study in Korea)

  • Objective : To evaluate the relationship between physical activity and the risk of hip fracture in the elderly Korean people. Methods : The study population was a Physical Activity Subcohort (n=8,908) extracted from the Korean Elderly Pharmacoepidemiological Cohort (KEPEC). Physical activity information was obtained from a mailed questionnaire surveys. The outcome data was collected from claims data gathered between Jan. 1993 and Dec. 1998. A hospital survey relating to potential cases was conducted to confirm the final diagnoses. The abstracted data was reviewed by a medical doctor before the final diagnoses were confirmed. A mailing questionnaire survey was performed to obtain information on potentially confounding variables, including alcohol intake, smoking habits, weight, height and postmenopausal duration. There were 79 confirmed cases hospitalized due to hip fractures between Jan. 1993 and Dec. 1998. Relative risk of physical activity scores on the hip fracture, and their 95% confidence intervals, were estimated by a Cox's proportional hazard model using SAS for Windows ver.6.12. Results : Compared to the reference group, the adjusted relative risk of hip fracture associated with the most physical active category: after controlling for age, weight and alcohol intake in the males, and for weight, alcohol intake and postmenopausal duration in the females, were 1.04 (95% CI=0.35-3.06) and 0.f4 (95% CI=0.26-0.77), respectively. Conclusions : Physical activity may protect elderly women from hip fracture.

Cohort Study Protocol: A Cohort of Korean Atomic Bomb Survivors and Their Offspring

  • In 1945, atomic bombs were dropped on Hiroshima and Nagasaki. Approximately 70 000 Koreans are estimated to have been exposed to radiation from atomic bombs at that time. After Korea's Liberation Day, approximately 23 000 of these people returned to Korea. To investigate the long-term health and hereditary effects of atomic bomb exposure on the offspring, cohort studies have been conducted on atomic bomb survivors in Japan. This study is an ongoing cohort study to determine the health status of Korean atomic bomb survivors and investigate whether any health effects were inherited by their offspring. Atomic bomb survivors are defined by the Special Act On the Support for Korean Atomic Bomb Victims, and their offspring are identified by participating atomic bomb survivors. As of 2024, we plan to recruit 1500 atomic bomb survivors and their offspring, including 200 trios with more than 300 people. Questionnaires regarding socio-demographic factors, health behaviors, past medical history, laboratory tests, and pedigree information comprise the data collected to minimize survival bias. For the 200 trios, whole-genome analysis is planned to identify de novo mutations in atomic bomb survivors and to compare the prevalence of de novo mutations with trios in the general population. Active follow-up based on telephone surveys and passive follow-up with linkage to the Korean Red Cross, National Health Insurance Service, death registry, and Korea Central Cancer Registry data are ongoing. By combining pedigree information with the findings of trio-based whole-genome analysis, the results will elucidate the hereditary health effects of atomic bomb exposure.
