Purpose: The purpose of this study was to identify minimum datasets for ulcer assessment and to map the minimum datasets to paper-based nursing records for pressure ulcer care in homecare setting. Methods: To identify minimum datasets for pressure ulcer assessment, the authors reviewed four guidelines for pressure ulcer care. The content validity of the minimum datasets was assessed by three homecare nurse specialists. To map the minimum datasets to nursing records, the authors examined 107 pressure ulcer events derived from 45 pressure ulcer patients who received home nursing from two hospitals in Gyeonggi Province. Results: The minimum datasets for initial assessment were anatomical location, stage, size, tissue, exudate, condition of periwound skin, undermining, odor, and pain. 'Location' was recorded best, accounting for a complete recording rate of 98.1%. 'Exudate' and 'pain' showed the poorest record, accounting for 2.8% and 0%, respectively. The minimum datasets for progress assessment were wound size, tissue, and exudate, each accounted for 31.8%, 2.8%, and 4.7%, respectively. Conclusion: This study concluded that data on pressure ulcer assessment was not sufficient homecare and it can be improved by adopting minimum datasets as identified in this study.
본 연구는 기록연구사의 정보공개청구 업무과정을 밝히고 이와 관련된 문제점을 살펴보고자 하였다. 이를 위해 11명의 현직 기록연구사들과 직접 심층 면담을 시도하여 담당자의 의견과 고충을 들었다. 이들은 주로 정보공개 처리 현황 관리자로서 역할을 이행하고 있었는데 이로 인해 외부 정보공개청구인과 기관 내 구성인 양자로부터 압력을 받는 상황에 놓이게 되는 경우가 빈번하였다. 또한 행정 소송으로 인한 업무 스트레스도 상당한 것으로 밝혀졌는데, 이러한 정보공개 업무의 스트레스는 주업무인 기록관리 업무 수행에 심각한 영향을 미치는 것으로 드러났다.
Purpose: To demonstrate the importance of comprehensive skin observation as an effective intervention for pressure injury prevention in elderly long-term care hospital patients. Methods: The survey was conducted with 70 nursing staff members working at two long-term care hospitals with 200 beds or less in D city. Data were collected from October 16 to October 23, 2019 and analyzed using descriptive statistics, the chi-square test, and the independent t-test with the SPSS 25.0 program. Results: Nursing care knowledge for pressure injury was similar between the two groups. Conversely, nursing care performance for pressure injury degree of the nursing staff in the hospital using skin observation records performed better than those who did not (t=6.11, p<.001). Furthermore, comprehensive skin assessments in long-term care hospitals using skin observation records showed a lower incidence rate of pressure injury than that showed using general skin assessments (t=-5.28, p=.006). Conclusions: Comprehensive skin assessment is important for pressure injury prevention in elderly long-term care hospital patients. To implement this effectively, it is necessary to devise institutional regulations, guidelines, and systematic education programs.

Background: Although work absenteeism is in the focus of occupational health, longitudinal studies on organizational absenteeism records in hospital work are lacking. This longitudinal study tests time pressure and lack of time autonomy to be related to higher sickness absenteeism. Methods: Data was collected for 180 employees (45% nurses) of a Swiss hospital at baseline and at follow-up after 1 year. Absent times (hours per month) were received from the human resources department of the hospital. One-year follow-up of organizational absenteeism records were regressed on self-reported job satisfaction, time pressure, and time autonomy (i.e., control) at baseline. Results: A multivariate regression showed significant prediction of absenteeism by time pressure at baseline and time autonomy, indicating that a stress process is involved in some sickness absenteeism behavior. Job satisfaction and the interaction of time pressure and time autonomy did not predict sickness absenteeism. Conclusion: Results confirmed time pressure and time autonomy as limiting factors in healthcare and a key target in work redesign.
Subsurface pressure gauge has many advantages in measuring a wide range of wave spectra in coastal waters from wind waves to long waves. However, a shortcoming of the gauge is related to the difficulties in recovering surface wave spectra from subsurface pressure records. In this study, the effect of current on the pressure transfer function of the pressure gauge, and hence on the surface wave energy spectrum, was investigated by analyzing the subsurface pressure data based on the linear wave theory. For this purpose, laboratory experiments were carried out in a wave-current flume. Subsurface pressure records, as well as the surface elevation data, were obtained simultaneously under different wave and current conditions. Pressure transfer functions were obtained and compared with those estimated from the linear wave theory, both with and without inclusion of the current-effect. It was established that wave spectra obtained from subsurface pressure gauge were in closer agreement with those from surface wave gauge when current-effect on the pressure transfer function was taken into consideration for analysis.
본 연구의 목적은 자연어처리에 의해 생성된 욕창간호진술문의 특성을 파악하고, 욕창 단계판별 예측정확도를 평가하기 위함이다. 욕창관련 간호기록은 서술통계를 이용하여 분석하였고, 워드클라우드 생성기를 활용하여 욕창예방 간호기록에서 단어의 특성을 파악하였다. 딥러닝을 이용하여 욕창단계판별 정확도(accuracy ratio) 를 구하였다. 연구결과, 욕창의 단계에 대한 기록 중 2단계와 심부조직손상의심단계가 각각 23.1% 와 23.0 % 로 가장 많았고, 빈도수가 높은 핵심단어는 홍반, 수포, 가피, 부위, 크기 등으로 나타났다. 예측의 정확도가 높은 단계는 0단계, 심부조직손상의심단계, 2단계 순으로 나타났다. 따라서, 이를 활용하여 임상적 의사결정지지 시스템으로 개발된다면, 임상간호사의 욕창관리역량 향상 전략 개발에 기초가 될 수 있을 것이다.
Purpose: The purpose of this study was to examine the accuracy of general nurses' assessments of pressure ulcers in hospitalized patients. Methods: A total of 129 nursing records of assessments on pressure ulcers were analyzed. Assessment records of pressure ulcers by general nurses were compared to those by Wound, Ostomy, Continence Nurses (WOCN) on the same pressure ulcers. A WOCN of a nursing unit was a nurse certified by the hospital after completion of a formal WOCN course and passing a cyber education course, both offered by the hospital. The formal WOCN course was taught by an internationally certified WOCN. The inter-rater reliability among WOCNs was 98.2%. General nurses in this study did not receive a structured pressure ulcer education. Results: The accuracy for nursing assessment of pressure ulcers by general nurses compared to WOCNs' were evaluated in various ways and resulted in as follows; the existence of pressure ulcer 91.4%, site of pressure ulcer 85.3%, stage of pressure ulcer 85.3%, Braden scale 36.3%, size of pressure ulcer 51.9~64.3%, details of pressure ulcer 0~100%. Conclusion: The structured education about pressure ulcer assessment is important to enhance the accuracy of pressure ulcer assessment of hospitalized patients by general nurses.
Evacuation performance, starting transient, and plume blowback at diffuser breakdown of a straight cylindrical supersonic exhaust diffuser with no externally supplied secondary flow are investigated. Pressure records in the transitional periods are measured by a small-scale cold-gas simulator. Flow-fields evolving in the diffuser-type ejector are solved by preconditioned Favre-averaged Navier-Stokes equations with a low-Reynolds number $k-{\varepsilon}$ turbulence model edited for turbulence compressibility effects. The present RANS method is properly validated with measured static wall pressure distributions and evacuation level at steady operation as well as the pressure records during the transition regime.
해수면과 해저파압을 연계하는 모수 모형을 개발하기 위한 시스템 식별법을 제시하였다. 비선형 고정변수를 포함한 선형 시불변 모형 구조를 가정하고 추가적인 입력자료를 갖는 자기회귀모형 (ARX)을 이용하여 해저파압 시계열자료로부터 해수면 시계열자료를 또는 해수면 시계열자료로부터 해저파압 시계열자료를 추출하는 방법을 제시하였다. 임의로 선정된 해수면과 해저 파압 자료를 이용하여 모형을 검증하였으며, 유사한 해저수심의 파압자료와 다른 해상 기상조건으로 생성된 해수면 스펙트럼 자료를 통해 재검증하였다. 시스템 식별법을 이용한 방법이 전통적인 선형파 이론을 이용한 선형전송함수(LTF) 방법보다 전반적으로 더 정확하게 수행됨을 확인하였다. 또한 본 논문에서 제시된 방법으로 추정된 해저 파압 시계열모의는 수정 선형전송함수(corrected LTF)에 의한 결과와 유사함을 확인하였다.
다양한 지층에 설치된 대형 뉴메틱케이슨의 강성기초에 작용하는 접지압분포를 파악하기 위하여 영종대교하부기초로 채택된 뉴메틱케이슨 시공시 실시한 현장계측자료를 분석하였다. 뉴메틱케이슨의 침설시 케이슨의 하부 접지면에 케이슨의 침설에 저항하여 저항력이 작용할 것이다. 이 저항력을 뉴메틱케이슨 기초저면 모서리에 설치한 반압계로 측정할 수 있었다. 그리고 이 저항력측정기록을 이용하여 접지압을 산정하였다. 측정기록의 분석결과 기반암구간에서 뉴메틱케이슨기초에 작용하는 접지압분포는 아래로 볼록한 형상의 접지압분포를 보이는 반면에 해성퇴적층과 풍화암층에서의 접지압분포는 위로 볼록한 형상을 보인다. 그리고 이들 접지압분포는 대체적으로 모든 지층에서 대칭분포를 나타내고 있다. 대형 뉴메틱케이슨기초에 작용한 최대접지압에 의하여 제시된 접지압분포는 Kgler(1936)와 Fang(1991)이 강성기초를 대상으로 제시한 접지압분포특성과 잘 일치하는 것으로 나타났다.
