As the convergence technology is developing rapidly and the portable mobile devices are spreading in ubiquitous smart healthcare, users were able to get medical information easily through the smart health platform. According to such rapid industrialization, wellness care, aging society, information society, changes in disease aspects and life style, user-centered healthcare, and health promotion contents are being offered. In this study, we proposed the mining based mental health and blood pressure management service for the smart health. The proposed method provides the mental health management service and the blood pressure management service for chronic disease patients within the mining based smart health platform. Users receive optimized healthcare services regardless of time and place in the PHR based smart health platform. For the performance evaluation of the proposed mining based mental health and blood pressure management service, F-measure verification are conducted.

Purpose: This study was conducted to construct evidence based clinical guidelines and to develop nursing process based performance measures for prevention and management of pressure ulcers, falls and pain. Method: Clinical guidelines were drafted through a comprehensive review of relevant literature, national guidelines and hospital protocols. The proposed guidelines were reviewed by a panel of experts and 90 hospital nurses, and refined on the basis of their suggestions. Nursing process based performance measures were developed based on the clinical guidelines and content validity was examined by surveys from 90 hospital nurses. Results: All items, except timetable for position change and pressure ulcer nursing record, in the guidelines for prevention and management of pressure ulcer were appropriate. Most items, except fall risk assessment tools, were appropriate for the guidelines of fall prevention. All other items, except the purpose of pain management, were appropriate for the guidelines of pain management. Performance measures developed in this study were acceptable as a tool to evaluate quality of nursing care. Conclusion: Nursing process based performance measures provide important indicators to monitor whether necessary nursing care is implemented and can be used as the primary resources to improve quality of nursing services.
Technology has had a tremendous impact on our daily lives. Recently, technology and its impact on aging has become an expanding field of inquiry. A major reason for this interest is that the use of technology can help older people who experience deteriorating health to live independently. In this paper we give a brief review of the in-home monitoring technologies for the elderly. In the pilot study, we analyze the possibility of employing the data generated by a continuous, unobtrusive nursing home monitoring system for predicting elevated(abnormal)pulse pressure(PP) in elderly(PP=systolic blood pressure-diastolic blood pressure). Our sensor data capture external information(behavioral) about the resident that is subsequently reflected in the predicted PP. By continuously predicting the possibility of elevated pulse pressure we may alert the nursing staff when some predefined threshold is exceeded. This approach may provide additional blood pressure monitoring for the elderly persons susceptible to blood pressure variations during the time between two nursing visits. We conducted a retrospective pilot study on two residents of the TigerPlace aging in place facility with age over 70, that had blood pressure measured between 100 and 300 times during a period of two years. The pilot study suggested that abnormal pulse pressure can be reasonably well estimated (an area under ROC curve of about 0.75) using apartment bed and motion sensors.
Purpose: The purpose of this study was to establish novice user guidelines for efficient external laryngeal manipulation for intubation in difficult airway management. Methods: This study included 59 pre-qualified junior and senior emergency medical service students. The participants were instructed at random to intubate a manikin equipped with a cervical collar, thus simulating a difficult airway, using three types of external laryngeal manipulation: bimanual laryngoscopy, backward-rightward pressure, and cricoid pressure. The resultant intubation time and glottic view grade scores were estimated. Results: Intubation time was longest using the bimanual manipulation method, followed by cricoid pressure and backward-rightward pressure. A low Cormack-Lehane glottic view score was obtained regardless of the assisted compression method used. Conclusion: Backward-rightward pressure may be the most efficient method of external laryngeal manipulation on the basis of the intubation time and improvement in glottic view.
Objective: The current study was to test if the developed sphygmomanometer was working well and blood pressure information could be collected and monitored systematically through the internet. We tested if the sphygmomanometer and services for blood pressure controlled high blood pressure significantly and the ubiquitous monitoring could be used further. Methods: Kyungwon University, KT Co., Gil Medical Center, LIG Nex1 Co., and Sujeong Health Center conducted an ubiquitous high blood control project in Sujeong-gu, Sungnam, Korea from Mar. 5 to May 16. We developed and applied sphygmomanometer. We distributed the devices to 27 high blood pressure patients. The blood pressures of the residents were monitored through the internet when they measured blood pressures in their homes. A nurse monitored and consulted their blood pressures in the monitoring center in Kyungwon University during the demonstration period. The consultant called them and consulted on their blood pressures in few seconds they used the sphygmomanometers. For the significance of change in blood pressure, we tested statistically with Generalized Additive Model(GAM) and Multi-level Analysis. Results: Both GAM and Multi-level Analysis showed that the blood pressures of persons with ubiquitous blood pressure management decreased significantly as time passed. Conclusions: The internet monitoring and services are considered to be promising because most of the participants were satisfied especially because somebody was caring their health. The decrease of blood pressures was significant by GAM and Multi-level Analysis. Thus, we can apply ubiquitous blood pressure management to health promotion projects.
This study empirically investigated inter-relationships among the influence factors on the adoption of e-commerce, which are classified as cause factor (i.e., perceived environmental uncertainty and competition), facilitators (i.e., perceived economic benefits, inter-organizational trust, suppliers' pressure and capability and asset specificity) and support factor (i.e., top management's support and organizational resource capability). The results of regression analyses showed that competition, inter-organizational trust, suppliers' pressure and capability, top management's support, and organizational resource capability have a positive impact on the adoption of e-commerce. In the analyses of inter-relationships among the influence factors, it was found that perceived environmental uncertainty positively influences suppliers' pressure and capability and perceived economic benefits, and competition has a positive impact on asset specificity. It was also observed that perceived economic benefits, inter-organizational trust and suppliers' pressure and capability positively affect top management's support, and inter-organizational trust and suppliers' pressure and capability have a positive impact on organizational resource capability. With mediating regression analyses, it was found that competition has an indirect impact on the adoption of e-commerce through the effect on perceived economic benefits, suppliers' pressure and capability, organizational resource capability and top management's support. The results of mediating regression analyses also showed that suppliers' pressure and capability and perceived economic benefits have indirect effects on the adoption of e-commerce through the effects on top management's support and organizational resource capability. From these results, it is concluded that intense competition causes the activation of facilitators, and the facilitators contribute to both the enhancement of top management support and the creation of organizational resource capability, which are directly linked to the adoption of e-commerce.

Objectives : Little is known about the physician-related factors that are associated with the management of Hypertension. The purpose of this study was to determine the physician-related factors associated with blood pressure control in hypertensive patients. Methods : We surveyed 154 physicians at 117 public health (subhealth) centers in Gyeonggi-do, Forty-one physicians completed the survey (response rates : 26.6%) and 31 physicians were finally included as the study subjects. Using the information obtained from the self-reported survey, we measured the physician-related factors associated with hypertension control, including their perception of hypertension, prescription patterns (combination prescription rates, specific antihypertensives prescription rates among patients with diabetes mellitus), and sociodemographic factors. We then collected data on blood pressure and medication use in patients seen by these physicians from the health center#s information system. We compared the physicians# perceived hypertension control rates with the actual rates, and then evaluated the rate of high overestimation (overestimation by more than 25% of the median degree of hypertension control rate overestimation) among the physicians. The physicians# antihypertensive prescription patterns were also evaluated. Multiple logistic regression analysis was used to evaluate the independent association between hypertension control and physician-related factors. Results : The physicians tended to overestimate the proportion of their patients with controlled blood pressure (79.5% perceived vs. 57.8% actual). The percentage of physicians with high overestimation was 35.5% (11 physicians). The physicians with lower control rates were more likely to highly overestimate their patients# control rates. Physicians with below-median actual control rates tended to prescribe fewer combination treatments for patients with uncontrolled blood pressure and angiotensin-converting enzyme inhibitors or fewer angiotensin receptor blockers for patients with diabetes mellitus. The rate of high overestimation by physicians was 1.31 times higher in patients with uncontrolled blood pressure than in patients with other conditions (OR=1.31, 95% CI : 1.17-1.48). Conclusions : Physicians have a tendency to overestimate the rates of hypertension control in their patients. Because physicians have a direct role in treatment outcomes, physicians# overestimation about hypertension management contributes to inadequate blood pressure control. Thus, interventions for improving physician# awareness regarding the management of patients with hypertension are needed.
Purpose: The purpose of this study was to develop an algorithm for preventing and managing of pressure ulcer and to verify the its appropriateness. Methods: The first step was development of a pre-algorithm through a literature review and expert opinion. The second step was to establish content validity by submitting the algorithm questionnaires about the content to 12 experts. The third step was the revision of the algorithm. The fourth and last step was to establish the clinical validity of the algorithm with 25 experienced nurses. Results: For the ease of the practitioner the algorithm for prevention and the management of pressure ulcers was confined to one page depicting the main algorithm pathway and seven stepwise guidelines. The guidelines included skin care of pressure ulcer prevention, mechanical loading care, support surface care, reposition care of pressure ulcer, and Stages II, III and IV explanations along with debridement/wound irrigation and infection control. Most of all algorithm courses chosen more than 80% of agreement by expert index of content validity. The usefulness, appropriateness, and convenience of the algorithm were demonstrated through clinical validity with intensive care unit and ward nurses. Conclusion: The algorithm will improve the quality of pressure ulcer nursing care as it provides a model for decision making for clinical nurses as well as providing consistent and integrated nursing care for patients with pressure ulcer throughout an institution.

Pressure monitoring is expected to be expanded in a water distribution system according to accelerated development of smart water network management technologies caused by appearances of affordable digital infrastructures like computing, storage and bandwidth. However, the placement of pressure sensors has been determined by engineer's technical decisions since there is no well-defined criteria for deciding a suitable location of pressure sensor. This study presents a placement method of pressure sensors based on the consideration of allowable error in calibrating water network analysis modeling. The proposed method is to find a minimum set of pressure sensors for achieving a reliable management of water transmissions main and increasing the efficiency of their real-time operation. In the case study in Y area's transmission main, the proposed method shows equally distributed pressure sensors in terms of hydraulics. It is expected that the proposed method can be used to manage transmission mains stably and construct a robust real-time network analysis system as a minimal criteria.

Purpose: The purpose of this paper is to present a probability distribution of the burst pressure of API 5L X65 pipes for the reliability assessment of corroded gas pipelines. Methods: Corrosion is a major cause of weakening the residual strength of the pipe. The mean residual strength on the corrosion defect can be obtained using the burst pressure code. However, in order to obtain the pipe reliability, a probability distribution of the burst pressure should be provided. This study is concerned with estimating the burst pressure distribution using Monte Carlo simulation. A response surface method is employed to represent the distribution parameter as a model of the corrosion defect size. Results: The experimental results suggest that the normal or Weibull distribution should be suitable as the probability distribution of the burst pressure. In particular, it was shown that the probability distribution parameters can be well predicted by using the depth and length of the corrosion defect. Conclusion: Given a corrosion defect on the pipe, its corresponding burst pressure distribution can be provided at instant. Subsequently, a reliability assessment of the pipe is conducted as well.
