Validity of Self-reported Hypertension and Factors Related to Discordance Between Self-reported and Objectively Measured Hypertension: Evidence From a Cohort Study in Iran

  • Objectives: Self-reporting can be used to determine the incidence and prevalence of hypertension (HTN). The present study was conducted to determine the validity of self-reported HTN and to identify factors affecting discordance between self-reported and objectively measured HTN in participants in the Ravansar Non-Communicable Diseases (RaNCD) cohort. Methods: The RaNCD cohort included permanent residents of Ravansar, Iran aged 35-65 years. Self-reported data were collected before clinical examinations were conducted by well-trained staff members. The gold standard for HTN was anti-hypertensive medication use and blood pressure measurements. The sensitivity, specificity, positive and negative predictive values, and overall accuracy of self-reporting were calculated. Univariate and multivariate logistic regression were used to examine the discordance between self-reported HTN and the gold standard. Results: Of the 10 065 participants in the RaNCD, 4755 (47.4%) were male. The prevalence of HTN was 16.8% based on self-reporting and 15.7% based on medical history and HTN measurements. Of the participants with HTN, 297 (18.8%) had no knowledge of their disease, and 313 (19.9%) had not properly controlled their HTN despite receiving treatment. The sensitivity, specificity, and kappa for self-reported HTN were 75.5%, 96.4%, and 73.4%, respectively. False positives became more likely with age, body mass index (BMI), low socioeconomic status, and female sex, whereas false negatives became more likely with age, BMI, high socioeconomic status, smoking, and urban residency. Conclusions: The sensitivity and specificity of self-reported HTN were acceptable, suggesting that this method can be used for public health initiatives in the absence of countrywide HTN control and detection programs.

Clinical characteristics of heat stroke in bath facilities, and analysis of predictive factors of multiple major complications (목욕시설에서 발생한 열사병의 임상양상 및 열사병의 다발성 주요 합병증에 관한 예측인자 분석)

  • Objective: The aim of this study was to evaluate the clinical characteristics of heat stroke in a bath facility and investigate predictive factors of multiple major complications in heat stroke patients. Methods: This was a retrospective study on heat stroke patients who visited an urban emergency center from January 2010 to March 2018. We compared clinical characteristics, complication, and outcomes of heat stroke patients in bath and non-bath facilities. Multivariate logistic regression analysis were performed to identify independent predictors of multiple major complications in heat stroke patients. Results: A total of 67 heat stroke patients with heat stroke were enrolled, of which 42 (62.6%) were in a bath facility and 25 (37.3%) were in a non-bath facility. Patients with heat stroke in the bath facility were characterized by old age, past medical history of hypertension and diabetes mellitus, and high incidence of hypotension compared with those in the non-bath facility but also low incidence of acute renal failure, seizure, and multiple major complications. In the multivariate analysis, predictive factors of multiple major complications in heat stroke patients were non-bath facility (odds ratio [OR], 5.4; 95% confidence interval [CI], 1.2-29.9), Glasgow Coma Scale (GCS)${\leq}8$ (OR, 8.2; 95% CI, 1.3-49.4), and mean arterial pressure (MAP), body temperature above $40.5^{\circ}C$ (OR, 8.1; 95% CI, 1.1-58.8) <60 mmHg (OR, 14.8; 95% CI, 1.8-122.9). Conclusion: Heat stroke in the bath facility resulted in less major complications, and high body temperature, GCS ${\leq}8$, and MAP <60 mmHg were independent predictive factors of multiple major complications in heat stroke patients.

The Reliability and Validity of Smart Insole for Balance and Gait Analysis (균형과 보행분석을 위한 스마트 인솔의 신뢰도와 타당도 분석)

  • Purpose: The Pedisole is a newly developed shoe-mounted wearable assessment system for analyzing balance and gait. This study aimed to determine the reliability and validity of the parameters provided by the system for static balance and gait analysis of healthy adults. Methods: This study included 38 healthy adults (22.4±1.9 years) with no history of injury in the lower limbs. All participants were asked to perform balance and gait tasks for undertaking measurements. For analysis of balance, both the smart Pedisole and Pedoscan systems were concurrently used to analyze the path length of the center of pressure (COP) and the weight ratio of the left and right for 10 s. Gait was measured using the smart Pedisole and GaitRite walkway systems simultaneously. The participants walked at a self-selected preferred gait speed. The cadence, stance time, swing time, and step time were used to analyze gait characteristics. Using the paired t-test, the intra-class coefficient correlation (ICC) was calculated for reliability. The Spearman correlation was used to assess the validity of the measurements. In total, data for balance from 36 participants and the gait profiles of 37 participants were evaluated. Results: There were significant differences between the COP path lengths (p<.050) derived from the two systems, and a significant correlation was found for COP path length (r=.382~.523) for static balance. The ICC for COP path length and weight ratio was found to be greater than .687, indicating moderate agreement in balance parameters. The ICC of gait parameters was found to be greater than .697 except for stance time, and there was significant correlation (r=.678~.922) with the GaitRite system. Conclusion: The newly developed smart insole-type Pedisole system and the related application are useful, reliable, and valid tools for balance and gait analysis compared to the gold standard Pedoscan and the GaitRite systems in healthy individuals.

The Prognosis of Glyphosate herbicide intoxicated patients according to their salt types (글라이포세이트 중독 환자에서 포함된 염의 종류에 따른 예후의 차이)

  • Purpose: Glyphosate herbicide (GH) is a widely used herbicide and has been associated with significant mortality as poisoned cases increases. One of the reasons for high toxicity is thought to be toxic effect of its ingredient with glyphosate. This study was designed to determine differences in the clinical course with the salt-type contained in GH. Methods: This was a retrospective study conducted at a single hospital between January 2013 and December 2017. We enrolled GH-poisoned patients visited the emergency department. According to salt-type, patients were divided into 4 groups: isopropylamine (IPA), ammonium (Am), potassium (Po), and mixed salts (Mi) groups. The demographics, laboratory variables, complications, and their mortality were analyzed to determine clinical differences associated with each salt-type. Addtionally, we subdivided patients into survivor and non-survivor groups for investigating predictive factors for the mortality. Results: Total of 348 GH-poisoned patients were divided as follows: IPA 248, Am 41, Po 10, and Mi 49 patients. There was no difference in demographic or underlying disease history, but systolic blood pressure (SBP) was low in Po group. The ratio of intentional ingestion was higher in Po and Mi groups. Metabolic acidosis and relatively high lactate level were presented in Po group. As the primary outcome, the mortality rates were as follows: IPA, 26 (10.5%); Am, 2 (4.9%); Po, 1 (10%); and Mi, 1 (2%). There was no statistically significant difference in the mortality and the incidence of complications. Additionally, age, low SBP, low pH, corrected QT (QTc) prolongation, and respiratory failure requiring mechanical ventilation were analyzed as independent predictors for mortality in a regression analysis. Conclusion: There was no statistical difference in their complications and the mortality across the GH-salt groups in this study.

Applicability Analysis of the FE Analysis Method Based on the Empirical Equation for Near-field Explosions (근거리 폭발에 대한 경험식 기반 유한요소해석 방법의 적용성 분석)

  • The blast analysis method entails the use of an empirical equation and application of the pressure-time history curve as an explosive load. Although this method is efficient owing to its simple model and short run time, previous studies indicate that it may not be appropriate for near-field explosions. In this study, we investigated why different results were observed for the analysis method by considering an RC beam under near-field explosion conditions with the scaled distance of 0.4-1.0 as an example. On this basis, we examined the application range of the empirical analysis method by using the finite element analysis program LS-DYNA. The results indicate that the empirical analysis method based on data from far-field explosion tests underestimates the impulse. Thus, the calculated deflection of the RC beam would be smaller than the measured deflection and arbitrary Lagrangian-Eulerian (ALE) analysis result. The ALE analysis method is more suitable for near-field explosion conditions wherein the structural responses are large.

Impact of Alcohol Consumption on Quality of Life, Depressive Mood and Metabolic Syndrome in Obstructive Lung Disease Patients: Analysis of Data from Korean National Health and Nutrition Examination Survey from 2014 and 2016

  • Background: The objective of this study was to investigate whether alcohol consumption might affect the quality of life (QOL), depressive mood, and metabolic syndrome in patients with obstructive lung disease (OLD). Methods: Data were obtained from the Korean National Health and Nutrition Examination Survey from 2014 and 2016. OLD was defined as spirometry of forced expiratory volume in 1 second/forced vital capacity <0.7 in those aged more than 40 years. QOL was evaluated using the European Quality of Life Questionnaire-5D (EQ-5D) index. Patient Health Questionnaire-9 (PHQ-9) was used to assess the severity of depressive mood. Alcohol consumption was based on a history of alcohol ingestion during the previous month. Results: A total of 984 participants with OLD (695 males, 289 females, age 65.8±9.7 years) were enrolled. The EQ-5D index was significantly higher in alcohol drinkers (n=525) than in non-alcohol drinkers (n=459) (0.94±0.11 vs. 0.91±0.13, p=0.002). PHQ9 scores were considerably lower in alcohol drinkers than in non-alcohol drinkers (2.15±3.57 vs. 2.78±4.13, p=0.013). However, multiple logistic regression analysis showed that alcohol consumption was not associated with EQ-5D index or PHQ-9 score. Body mass index ≥25 kg/m2, triglyceride ≥150 mg/dL, high-density lipoprotein <40 mg/dL in men and <50 mg/dL in women, and blood pressure ≥130/85 mm Hg were significantly more common in alcohol drinkers than in non-alcohol drinkers (all p<0.05). Conclusion: Alcohol consumption did not change the QOL or depressive mood of OLD patients. However, metabolic syndrome-related factors were more common in alcohol drinkers than in non-alcohol drinkers.

Seismic performance assessment of single pipe piles using three-dimensional finite element modeling considering different parameters

  • The present study investigates the non-linear soil-pile interaction using three-dimensional (3D) non-linear finite element models. The numerical models were validated by using the results of extensive pile load and shaking table tests. The pile performance in liquefiable and non-liquefiable soil has been studied by analyzing the liquefaction ratio, pile lateral displacement (LD), pile bending moment (BM), and frictional resistance (FR) results. The pile models have been developed for the different ground conditions. The study reveals that the results obtained during the pile load test and shaking cycles have good agreement with the predicted pile and soil response. The soil density, peak ground acceleration (PGA), slenderness ratio (L/D), and soil condition (i.e., dry and saturated) are considered during modeling. Four ground motions are used for the non-linear time history analyses. Consequently, design charts are proposed depended on the analysis results to be used for design practice. Eleven models have been used to validate the capability of these charts to capture the soil-pile response under different seismic intensities. The results of the present study demonstrate that L/D ratio slightly affects the lateral displacement when compared with other parameters. Also, it has been observed that the increasing in PGA and decreasing L/D decreases the excess pore water pressure ratio; i.e., increasing PGA from 0.1 g to 0.82 g of loose sand model, decrease the liquefaction ratio by about 50%, and increasing L/D from 15 to 75 of the similar models (under Kobe earthquake), increase this ratio by about 30%. This study reveals that the lateral displacement increases nonlinearly under both dry and saturated conditions as the PGA increases. Similarly, it is observed that the BM increases under both dry and saturated states as the L/D ratio increases. Regarding the acceleration histories, the pile BM was reduced by reducing the acceleration intensity. Hence, the pile BM decreased to about 31% when the applied ground motion switched from Kobe (PGA=0.82 g) to Ali Algharbi (PGA=0.10 g). This study reveals that the soil conditions affect the relationship pattern between the FR and the PGA. Also, this research could be helpful in understanding the threat of earthquakes in different ground characteristics.

Gold-Silver Mineralization of the Geojae Area (거제(巨濟)지역 금(金)-은(銀)광상의 광화작용(鑛化作用) 연구)

  • The electrum-silver-sulfide mineralization of the Geojae island area was deposited in three stages (I, II, and carbonate) of quartz and calcite veins that crosscut Late Cretaceous volcanic rocks and granodiorite(83 m.y.). Stages I and II were terminated by the onset of fractunng and breCCIation events. Fluid inclusion data suggest that the gold-sulfide-bearing stages I and II each evolved from an initial high temperature( near $370^{\circ}C$) to a later low temperature(near $200^{\circ}C$). Each of those stages represented a separate mineralizing system which cooled prior to the onset of the next stage. The relationship between homogenization temperature and salinity in stages I and II suggests a complex history of boiling, cooling and dilution. Evidence of boiling indicates a pressure of < 100 bars, corresponding to a depth of 500 to 1,250m assummg hthostatlc and hydrostatic pressure regimes, respectively. Fluid inclusion and mineralogical evidence suggest that the electrum-silver mineralization was deposited at a temperature of $220-260^{\circ}C$ from ore fluids with salinities between 1.9 and 8.1 equivalent wt.% NaCl. Total sulfur concentration is estimated to be $10^{-3}$ to $10^{-4}$ molal. The estimated $fs_2$ and $fo_2$ range from $10^{-11.8}$ to $10^{-14}$ atm and $10^{-35}$ to $10^{-36}$ atm, respectively. The chemical conditions indicate that the dominant sulfur species in the ore forming fluids was a reduced form($H_2S$). Rapid cooling and dilution of ore-forming fluids by mixing with less-evolved meteoric waters led to gold-silver deposition through the breakdown of the bisulfide complex($Au(HS)_2$) as the activity of $H_2S$ decreased.

Anger-coping types and hypertension in some employed men (일부 남자 고용집단에서 분노 대응형태와 고혈압)

  • This study examined the relation between anger-coping types and hypertension in employed men aged $40\sim60$ who consented to participate during the biannual physical checkup in the department of health management in 1988. The subjects analyzed were five hundred thirteen excluding those having hypertension history and/or current antihypertensive medications. Anger-coping types were constructed from the Harburg's model with two hypothetical anger-provoking situations involving wife and boss. Hypertensives were defined more than 140mmHg systolic blood pressure and/or 90mmHg diastolic blood pressure. Hypertensives were one hundred fifty two(29.6%) and those who suppressed their anger were 61.6% and 62.8% in wife and boss situations respectively. Items of anger, guilt, protest, and suppressed anger in wife situation showed odds ratios of 0.78-0.94 without statistical significance. But four items in boss situation showed odds ratios more than 1, especially anger-in types of anger item had 1.58 times the prevalence of hypertension of anger-out types(95% confidence intervals(CI) $1.06\sim2.35$) and subjects who indicated that suppressed their anger had 1.55 times the prevalence of hypertension of those who expressed their anger(95% CI $1.03\sim2.32$). For anger suppressed vs. expressed types of total suppressed anger index, prevalence of hypertension was 1.31 (95% CI $0.83\sim2.08$). After adjusting for age, body mass index, smoking and drinking, the odds ratios were slightly increased in both situations except guilt items compared with univariate analysis. These results suggest that the relation between Harburg's anger-coping model and hypertension is replicated partially in this subjects.

Association between Cancer and Selenium Concentration in Blood and Toenails (혈액과 발톱의 셀레니움 농도와 암과의 관련성)

  • A case-control study was conducted to investigate the association between the risk of cancer and selenium concentration in blood and toenails. Seventy three patients and two hundreds eighty three controls were, selected at the Yeungnam University Hospital between May and September in 1991. The selected cases were patients who had been hospitalized for stomach or colon cancer at the Depertment of General Surgery. The controls were people who visited to check physical examination at the Automated Mediscreening Center. The selenium concentration in whole blood and toenails were measured by atomic absorption spectrophotometer equipped with graphite furnace atomizer. The following information was ascertained for all cancer patients and controls : sex, age, body mass index, blood pressure, total serum cholesterol, and history of smoking and drinking. The mean selenium concentration in blood and toenail for all cancer patients were $143.6{\pm}10.8{\mu}g/l$ and $1.04{\pm}0.62{\mu}g/g$ and for the controls, $167.0{\pm}14.5{\mu}g/l$ and $1.15{\pm}0.55{\mu}g/g$, respectively. The difference in blood and toenail selenium concentrations of the two cancer sites was not statistically significant. Metastasis did not influence the concentration of selenium in blood and toenails. In the multiple logistic regression analysis, the blood selenium concentration($_aOR$: 0.888, 95% CI : 0.860-0.918), age, BMI and total serum cholesterol were significant variables for risk of cancer, but the selelenium concentration in toenail was not shown to be a significant variable in this regression analysis. The coefficient for blood selenium concentration adjusted for age, sex, diastolic blood pressure, total serum cholesterol, body mass index and smoking was -0.1184(p<0.01). These findings suggest that low selenium concentration is associated with gastrointestinal cancers. Further epidemiologic studies including important variables such as other antioxidant micronutrients will be necessary.
