Purpose: This study investigated predictive factors for severe neonatal thrombocytopenia, which greatly increases the need for intensive care and is associated with a high mortality rate in premature infants. Factors adopted for prompt identification of at-risk newborns include blood test results and birth history. This study analyzed the relationship between the presence of severe neonatal thrombocytopenia and the mortality rate. The causes of thrombocytopenia in premature infants were also examined. Methods: This retrospective study evaluated 625 premature infants admitted to the neonatal intensive care unit (NICU) at Chung-Ang University Medical Center. The neonates were classified into 3 groups according to the severity of thrombocytopenia: mild ($100{\times}10^9/L{\leq}platelet<150{\times}10^9/L$), moderate ($50{\times}10^9/L{\leq}platelet<100{\times}10^9/L$), or severe (platelet<$50{\times}10^9/L$). Analysis of blood samples obtained at the onset of thrombocytopenia included platelet count, white blood cell (WBC) count, hemoglobin level, hematocrit level, absolute neutrophil count, and high-sensitivity C-reactive protein level. Results: Of the 625 premature infants admitted to our NICU, 214 were detected with thrombocytopenia. The mortality rate in thrombocytopenic neonates was 18.2% (39/214), whereas a mortality rate of only 1.0% was observed in non-thrombocytopenic neonates. The major causes of thrombocytopenia were perinatal insufficiency and sepsis in premature infants. Severe thrombocytopenia was noted more frequently in premature infants with higher WBC counts and in those with a younger gestational age. Conclusion: Platelet count, WBC count, and gestational age are reliable predictors for severe neonatal thrombocytopenia. The major causes of thrombocytopenia were perinatal insufficiency and sepsis in premature infants.
The purpose of this study is to provide basic data for the medical nutrition therapy of premature infants. The general characteristics, presence of metabolic disorders, hematological profile and feeding methods were compared between the premature infant group (<37 weeks, n=61) and the full-term infant group (37$\sim$42 weeks, n=165). Birth weight (p<0.0001), birth length (p<0.005), head circumference (p<0.0001), chest circumference (p<0.0001), and Apgar scores (p<0.0001) of the neonates were all statistically lower in the premature infant group. Jaundice cases (p<0.0001) were statistically higher in the premature infant group. White blood cell counts (WBC: p<0.005), mean corpuscular volume (MCV: p<0001), mean corpuscular hemoglobin (MCH: p<0.005), mean corpuscular hemoglobin concentration (MCHC: p<0.005), and mean platelet volume (MPV: p<0.05) were statistically lower in the premature infant group. The premature infant group were fed a higher rate of premature formula than breast milk and the full-term infant group were fed a high rate of human milk at a higher rate, showing differences in kinds of feeding methods (p<0.0001) between the two groups. An infant's birth weight showed a significantly positive correlation with the infant's birth length (p<0.0001), head circumference (p<0.0001), chest circumference (p<0.0001), and Apgar scores(p<0.0001). The birth length also showed a significantly positive correlation with both head circumference (p<0.05) and chest circumference (p<0.05). Head circumference showed a significantly positive correlation with chest circumference (p<0.0001) and Apgar scores (p<0.0001). Chest circumference showed a significantly positive correlation with Apgar scores (p<0.0001). In addition, the Apgar Score at of 1 minute after birth showed a significantly positive correlation with the Apgar score at of 5 minute after birth (p<0.0001).
Purpose: This study tries to explore mothers' stress patterns and the related factors influencing mothers' stress over time after giving birth to premature babies. Method: Eighty four mothers who had given birth to premature babies were selected from Hospitals in B city. Data was collected using a self-reporting questionnaire that the mothers' stress level. Result: The mothers' stress after giving birth to premature babies gradually diminished and the stress pattern of mothers changed over time. Mother's age, occupation, income level, gestational period of the measures baby, weight at birth, nutrition type, lactation mode, number of complications, and existence or non-existence of an operation were analyzed as the factors that affected the mother's stress. Conclusion: The stress pattern of mothers giving birth to premature babies changed overtime. Based on the study results, it is considered that the nursing intervention programs should be developed in order to reduce the stress of premature baby's mothers with time elapse.
Purpose: This study, using a pre-posttest design of non-equivalence comparative groups, was done to determine effects of KC (kangaroo care) on weight and SH (stress hormone) of premature infants. Method: Participants were 31 premature infants assigned to KC(14) or CG(control group)(17). Data were collected from June 2006 to February 2007. KC was given for 60 minutes at a time, Mon, Wed, and Fri for a 10 times. Body weight was measured at a fixed time daily and serum cortisol was examined as part of routine blood testing, frequent for premature infants. Results: The weight for infants in the KC group was higher than the control. The difference was significant between both the two groups, and the 2 measurement times(F=12.7, p=.001). SH(cortisol) in infants of KC group decreased than CG, but there was no valid difference from the statistical viewpoint. Conclusion: The result of the study suggest that KC contributes to weight increase in premature infants and also that KC is effective for SH reduction in spite of the lack of statistical validity. Therefore, our conclusion is that KC can be used as an independent nursing intervention for development and advance of premature infants.
Purpose: The aim of this study was to determine the predictive factors for maternal role development for mothers of premature infants. Methods: A descriptive correlational study was conducted. A total sample of 121 mothers of premature infants following discharge from the neonatal intensive care unit were recruited using two strategies; an internet-based survey and an in-person data collection in a tertiary university hospital in Korea. A self-report questionnaire was used to collect data regarding personal, birth variables, marital intimacy, maternal attachment, maternal identity and maternal role development. Results: A hierarchical multiple regression analysis indicated that parity, maternal attachment, marital intimacy and maternal identity were predictors for maternal role development for mothers of premature infants, accounting for 70% of the variance. Among these variables, maternal attachment is the most powerful predictor for maternal role development. Conclusion: Nursing interventions during hospitalization to post-discharge education that includes parents of premature babies with positive interaction between couples strengthening marital intimacy and promotes maternal attachment that leads to integrate maternal identity should be considered by priority. Community-based family services such as home visits should be focused on maximizing the predictive factors for maternal role development in transition to motherhood that can contribute to maternal health as well as optimal growth and development of premature infants.
Purpose: Knowing the accurate gestational age(GA) is critical in nursing care of premature infants. A descriptive study was performed to examine the reliability and clinical applicability of the New Ballard examination(NBE) in premature infants. Method: A NBE was performed to measure GA by assessing the neuromuscular and physical maturity in the course of physical examination of a convenient sample of 74 premature infants. Result: 1. The highest item of NBE score was posture (mean=2.82) and the lowest item was breast (mean=1.84). 2. There was a highly correlation between both the GA by LMP(GA-LMP) and GA by NBE (GA-NBE)(r=.844, p=.000). 3. There was a greater positive relationship in neuromuscular maturity than physical maturity in the GA-NBE of the premature infants(r=.786 vs r=.933). 4. There was a positive correlation between neuromuscular, physical, total maturity, GA-LMP and GA-NBE in the birth weight, length, 1, 5 minute apgar score. 5. There was no significant difference neuromuscular, physical, total maturity in NBE by delivery history. Conclusion: The study supports the reliability a clinical relevance of NBE in assessment of the accurate GA in premature infants.
Purpose: This study examined the effects of a positioning education program through the brochure or oral explanations for the parents of premature infants with a brain lesion and investigated the satisfaction level of physical therapy of parents of premature infants with a brain lesion. Methods: Forty parents of premature infants with a brain lesion participated in this study. The recruited premature infants were randomized into groups A (n=20) and B (n=20) for the purpose of the positioning education method through brochure or oral explanations. The level of satisfaction and recognition for the positioning education program was investigated by the parents of 12 month old infants. The gross motor function, measure-88, was examined at three months, six months, nine months, and twelve months of the corrected age. Results: No significant differences in the recognition of the position education program, level of satisfaction of the environment, and the attitude of the therapist were noted (p>0.05). Significantly high levels of satisfaction with the program of group B given the brochure were noted. The gross motor function measure-88 was also similar in both groups (p<0.05). Conclusion: This study suggests that the parents were satisfied with the positioning education program of the brochure. The differences in educational methods did not affect the development of premature infants.
