Relation between Blood Alcohol Concentration and Clinical Parameters in Trauma Patients (외상 환자에서의 혈중 알코올 농도와 임상 관련 인자간의 관계)

  • Purpose: The aim of this study was to evaluate the effects of blood alcohol concentration (BAC) on the clinical parameters in trauma patients. Methods: From January 2011 to March 2013, the records of a total of 102 trauma patients with BAC data were analyzed retrospectively. The revised trauma score (RTS), injury severity score (ISS), presence of shock, use of mechanical ventilation and blood transfusion, length of hospital stay, and mortality were collected. Patients were divided into four groups in accordance with the level of BAC: group A (<100 mg/dL), B (100~200 mg/dL), C (200~250 mg/dL), and D (>250 mg/dL). Patients were also divided into two groups depending on the presence of the shock, and gender, ISS, BAC, and presence of active bleeding were compared between these two groups. Results: No statistically significant differences in the ISS, RTS, presence of active bleeding, use of mechanical ventilation, and mortality were noted between groups A to D. However, the presence of shock was significantly higher in group D. After patients with severe chest injuries had been excluded, mechanical ventilation was found to have been applied more frequently in the higher BAC groups (C and D). A logistic regression analysis of these factors showed that extremely high BAC (>250 mg/dL) was an independent indicator of shock. Conclusion: High BAC is a predicator of shock and the need for mechanical ventilation in patients with trauma, regardless of injury severity. Alcohol intoxication leads to an overestimate of the clinical condition and aggressive management for trauma patients. Thus, a guideline for the diagnosis and treatment of patients intoxicated with alcohol is necessary.

  • Bioelectrical Impedance method for measuring human body composition is based upon the principle that the electrical conductivity of the fat-free mass (FFM) is far greater than that of the (at. Deter- mination o( impedance was nlade in 30 healthy and obese men aged $40.9{\pm}14.7$yr (mean${\pm}$SD) and 25 healthy and obese women aged $44.0{\pm}10.2$ yr. A commercial impedance body fat analyzer was used with a four electrode arrangement that introduced a painless constant current(ImA at 50 kHz) into the body. Linear relationships were found between impudence values and FFM(r=-0.786) and between weight and FFM(r=0.837). Signigicant increases in the correlation coefficients were ob served when the predicator Hta/z was regressed against FFM(r=0.912) where Ht Is height and Z Is impedance. A linear regression equation, FFM=0.586(Htf/z) +0.317(Weight) -1.674(r=0.977), was found. The correlation coefficient of % body fat between the impedance and infrared methods was 0.898. These data Indicate that the bioelectrical impedance technique is a reliable and valld approach for the estimation of human body composition. This method Is safe, noninvasivr, prouides rapld measure menu, requires little operator skill and subject cooperation, and Os portable. Finally measurement of percent body fat was made possible using both the regression equation and a developed impedance measuring device which measures impedance between wrist and ankle.

The Effects of the Individual and Family Relational Variables Perceived by Adolescents on Adolescents' Problem Behaviors (청소년의 개인변인과 청소년이 지각한 가족변인이 문제행동에 미치는 영향)

  • The purpose of this study was to show general trends in the psychological environment of family and problem behaviors perceived by adolescents and examine possible changes in such trends in accordance with the individual variables of adolescent, and then find out the effect of these variables on adolescents' problem behavior. The subject were 1374 adolescents of middle school in Busan(male 698, female 676). The main results were as follows : (1) General trends in the degree of parental monitoring showed that girls had more high than boys, and in the degree of family discord, boys showed more high than girls. In the degree of openness of parent-adolescent communication perceived by adolescents showed that girls had more open communication with mother than boys. Such trends in problem behaviors indicated that adolescents had the attention problem most. According to gender, girls had more problems in withdrawal, somatic complain, anxiety/depression, thought problems, attention problems, destructivity/identity, internalizing problems, total behavior problems in comparison to boys. Boys did delinquent behaviors more than girls. (2) For boys, the monthly income of their family, family discord, and the mother-adolescent communication have a significant direct effect on internalizing problems, externalizing problems and total behavior problems. Besides mother's employment and the type of family have a significant indirect effect on behavior problem. For girls, mother's employment, parental monitoring, family discord, and father-adolescent communication have a significant direct effect on internalizing problems. Mother's employment, family discord, mother-adolescent communication have a significant indirect effect on externalizing problems and total behavior problems. Bisides mother's employment, grade, the type of family, dating have a significant indirect effect on behavior problem. (3) The variables to have a significant influence on the parental monitoring showed as the monthly income of their family, dating, mother's employment, the type of family, the family discord showed as mother's employment on the parent-adolescent communication showed as the type of family. (4) Family discord was the most powerful predicator of problem behaviors of middle school students.
