순환운동이 뇌졸중 후 우울증 환자의 뇌 세로토닌 합성에 미치는 영향 (The Effect of Circuit Class Training on the Synthesis of Central Serotonin in People with Post-stroke Depression)

  • PURPOSE: The purpose of present study was to determine effects of circuit class training (CCT) on the synthesis of central serotonin in people with post-stroke depression. METHODS: Forty patients with mild post-stroke depression were participated in current study. All subjects were assigned to two groups of CCT (circuit class training) group and SW (stretching and weight shifting) group. Both groups were performed for 80 minutes. The change of blood f-Trp, BCAAs, f-Trp/BCAAs and serotonin according to period training time were examined and the following results were obtained. RESULTS: In the CCT group, f-Trp, BCAAs, and f-Trp/BCAAs increased according to the time point. However, after 24 hour of circuit class training, all of these were significantly decreased by those before training. The change in blood levels of serotonin was the highest in immediately after the training in the CCT group while it was not changed significantly in the SW group. CONCLUSION: It can be seen that CCT can change the serotonin level and have an effect on the synthesis of central serotonin in people with post-stroke depression.

  • Objectives : This study aimed to investigate the prevalence and risk factors of depression within two weeks after stroke. Methods : A total of 362 stroke patients were recruited. Depression (major and minor depressive disorders) was diagnosed by applying DSM-IV criteria. Data on socio-demographic characteristics (age, gender, education, marital state, living state, religion, occupation, income, life event, and social support), stroke severity (NIHSS), disability (BI), stroke hemisphere and location, vascular risk and disease, and previous history of stroke and depression were obtained. Results : Depression was present in 90 (24.9%) patients: major depression 29 (8%) and minor depression 61 (16.9%) patients. In the univariate analyses, depression was associated with older age, higher number of stressful life event, poorer social support, severe disability, anterior stroke location, previous history of stroke and depression. In the multivariate analyses, depression was independently associated with higher number of stressful life events and poorer social support. Conclusion : Depression was common and was determined by premorbid levels of stress and social support in stroke patients at acute stage. More intensive psychiatric care and intervention is needed for the high risk group.

  • 이 연구의 목적은 뇌졸중 후 발생가능한 정서적, 사회인지 문제의 중재를 위해 작업 치료에 기초한 인지자극순환프로그램(CSCP ; Cognitive Stimulation Circulative Program)의 효과를 확인하는 것이다. 이 연구에는 12명의 뇌졸중 환자가 참여하였고 CSCP는 인지과정을 촉진시키는 방법으로 조직화되었으며 총 16회기의 순환 프로그램으로 실시되었다. 프로그램의 효과를 알아보기 위하여 우울증 척도와 관계 변화 척도, 삶의 질 평가 도구를 사용하였다. 연구결과 피험자의 우울증 척도와 역할의 물리적 및 사회적 인지 기능 능력을 평가하기 위해 사용된 관계 변화 척도는 통계적으로 유의미한 변화가 있었다(p<.05). 또한, 삶의 질 영역 중에서 신체적 역할, 사회적 기능의 항목에서 의미있는 향상이 있었다. CSCP가 뇌졸중 환자의 우울을 감소시키고 사회인지 기능을 향상시켜 삶의 질을 향상시키는 효율적인 중재 프로그램으로 사용 가능함을 입증하였다.

  • Objective : The purpose of this study was to examine the mood disorder, especially depression and anxiety which have an effect on rehabilitation in stroke patients. Methods : Subjects were 32 stroke patients who were inpatients and outpatients at Woosuk University oriental hospital. The degree of depression and anxiety was measured by the Beck Depression Inventory(BDI) and State-Trait Anxiety Inventory(STAI, State Anxiety Inventory : SAI, Trait Anxiety Inventory: TAI). Other questionnaire included sex, age, onset of stroke, degree of education, econommic class, religion, relationship with family, expectancy on treatment, side of hemiplegia, weakness grade of hemiplegia. Results : Results were as follows. 1. The average scores of SAI, TAI and BDI were 45.7, 46.7 and 19.9. The percentages of patients who had relatively high SAI, TAI and BDI scores were 75%(>36 point), 87.5%(>36 point) and 56.2%(>16 point). 2. Review of Pragmatic Clinical Trials on Acupuncture

  • Background : Over the last thirty years, majority of researches on clinical effectiveness of acupuncture have been explanatory (or experimental) randomized controlled trial. The benefits of acupuncture in clinical trials are still controversial and most studies concluded that further control studies were required. Standardized combinations of acupuncture points for all the experimental subjects in various past studies have been criticized because such treatments do not reflect current routine clinical treatment. Objective : This paper aims to review pragmatic clinical trials on the effect of acupuncture treatment and to develop the ideal clinical research methodology of acupuncture study. Method : Clinical studies of acupuncture relevant with pragmatic or individualized trials were searched mainly in Pubmed and Science direct databases. All articles were fully reviewed by researchers, and data were evaluated by usage of a standardized form. Results & Suggestion : Pragmatic acupuncture researches were tried for various symptoms (eg. low back pain, hypertension, depression during pregnancy, sleep quality in HIV disease, chronic poststroke leg spasticity, headache, etc). Individualized acupuncture treatments based on oriental disease pattern diagnosis reflexes practical treatments which is more effective than unified and fixed acupuncture treatments without any theoretical basis of oriental medical philosophy. Conclusion : To overcome the controversies and limitations of past explanatory acupuncture trials, more individualized and tailored acupuncture trials with the theoretical basis of oriental medical diagnosis is highly recommended. Also clear definition and categorization of pattern identification should be established for further active clinical researches and applications of acupuncture.
