좌심실우심방 단락치험 1 (Left ventricular-right atrial canal:report of one case)

  • Left ventricular-Right atrial canal is a rare congenital heart disease. The vast majority of the cases reported in the literature are clinically diagnosed as atrial septal defect or ventricular septal defect. The method of choice in establishing the diagnosis of left ventricular-right atrlal canal is selective left ventriculography. Recently we experienced one case of left ventricular-right atrial canal which was diagnosed as ventricular septal defect preoperatively. The type of defect was tricuspid perforation of infravalvular type, and repaired with direct suture. Postoperative course was uneventful and discharged without complication.

대동맥판막 및 상행대동맥 대치이식술 1례 - Bentall씨 수술 변형술 - (A Successful Replacement of Ascending Aorta and Aortic Valve With a composite Graft)

  • A forty-eight-year-old female patient with ascending aortic aneurysm with aortic insufficiency underwent a modified Bentall operation. The ascending aorta and the aortic valve were replaced with a composite graft containing a St. Jude valve. The coronary orifices were anastomosed to the tubular Dacron prosthesis by means of a second smaller Gore-Tex tube, and a fistula between the aneurysmal sac and the right atrial appendage was created to drain oozing from the prosthesis. The postoperative course was uneventful and the patient was discharged without complication. She is doing well on the 14 months follow-up.

식도 평활근종의 임상적 고찰 (Clinical Evaluation of Esophageal Leiomyoma)

  • The five patients with leiomyoma of esophagus were treated from 1976, to 1990. The patients were 4 men and 1 woman whose ages ranged from 28 to 53 years. One of them was asymptomatic, two had mainly dysphagia and the others complained indigestion and epigastric discomfort. There was no relationship between the severity of symptoms and the size of tumor. The preoperative diagnosis was made by esophagoscopy and esophagogram, and all of them were treated by thoracotomy and enucleation. There was no postoperative complication and the results were excellent.

외상성 기관지 파열의 수술 치험 -3례 보고- (Surgical treatment of bronchial rupture by chest trauma -3 cases report-)

  • Traumatic rupture of the main bronchus is comparatively very rare. With the advent of widespread mechanization and high-speed era, the incidence of traumatic rupture of the tracheobronchial tree has been increased considerably. Rupture of the bronchus is an unusual result of nonpenetrating trauma to the chest. Early recognition of bronchial rupture and emergency thoracotomy and management is essential for reducing of morbidity, mortality and late complications. We experienced 3 cases of bronchial rupture caused by nonpenetrating chest trauma with or without rib fracture. Patients were suffered from dyspnea and chest pain. After closed thoracostomy, corrective surgery was performed. Postoperative courses were uneventful and discharged without any complication.

기관지 암양종 (Carcinoid) 수술치험 1례 보고- (Bronchial Carcinoid - One case report -)

  • Bronchial carcinoid was classified as one of the bronchial adenoma group and it was regarded as benign tumor because of its uniform histologic feature, predictable clinical course and good survivability after surgical resection. But recently, many authors described that bronchial carcinoid has low grade malignancy for its lymphatic spread and distant metastasis, especially atypical carcinoid. We had a case of typical bronchial carcinoid in 20 year old female patient, and successful operation was performed by bilobectomy of middle and lower lob of right lung. Postoperative course was uneventful and discharged without any complication.

  • The usual treatment for aortic aneurysms is surgical replacement with a prosthetic graft; however the associated morbidity and mortality rates must be considered. Endovascular placement of self-expandable stent-graft is a safe noninvasive treatment that can be an alternative to the surgical repair, the postoperative course of the 2 cases of thoracoabdominal aortic aneurysms was uneventful and no complication has been associated with the stent-graft during the 17 months and 5 months follow-up studies,.

  • Forty patients with spontaneous pneumothorax underwent vertical axillarty thoracotomy for surgical bullectomy of spontaneous pneumothorax between June, 1991 and september, 1992. We evaluated the method in terms of postoperative pain, wound complication, days of hospital stay and cosmetic result. It`s concludid that verical axillary thoracotomy provides satisfactory exposure for limited procedures within the thoracic cavity and offers the specific advantages of minimal and cosmetically acceptable results when compared with the thoracotomy methods.

전폐절제후 늑막강으로부터 체액의 소실 -1례 보고- (Disappearance of Fluid From the Pneumonectomy Space (1 case report))

  • One case is presented in which there was radiographic evidence that pleural space fluid disappeared at the 15th day after pneumonectomy. Clinical course was uneventful and the space was refilled at the postoperative fifth month. This complication was probably due to the presence of small a bronchopleural fistula, in spite of the difficulty experienced in its demonstration. Conservative management is recommended with frequent clinical and radiographic observations, so that early surgical intervention may be undertaken if an overt bronchopleural fistula results.

  • Life-threatening cardiac arrhythmia is a frequent complication of open heart surgery. There are many causes of postoperative cardiac arrhythmias. Electrolyte imbalance such as hypokalemia and acidemia are major causes of ventricular arrhythmias. Infrequently, however, antiarrhythmic agents and /or hypomagnesemia induce[s] a ventricular arrhythmia such as "torsade de pointes" by increasing the repolarization time of myocardium, Recently, we have experienced two cases of "Torsade de pointes" associated with hyp-omagnesemia after replacement of mitral valve and one of whom after use of procainamide. Intravenous infusion of magnesium immediately and successfully abolished the torsade de pointes in both cases.intes in both cases.

좌심실 부전증에서의 IABP 치험 - 5예 보고- (Intra-Aortic Balloon Pump in the Left Heart Failure)

  • From July 1986 to June 1987, five patients were underwent IABP [intra aortic balloon pulsation] because of sever left heart failure in spite of maximum medication. These patients were reviewed as prophylactic IABP [1 patient], During operation [3 patients] and postoperative IABP [1 patients]. All patients were showed stable hemodynamic status with improved LV function during and after IABP. there was no IABP related complication or mortality. Advanced disease needs more effective methods of mechanical circulatory assistance and heart replacement.
