종격동 종양의 임상적 고찰 (Clinical Evaluation of the Mediastinal Tumors)

  • Generally early surgical extirpation is considered to be beneficial for the treatment of mediastinal tumors and prevention of its complication. The review of the clinical course, histopathologic characteristics and current diagnostic and treatment modalities will help us to decide when to treat and how to treat mediastinal tumors. For this purpose clinical analysis of the 42 patients who were surgically treated from July 1984 to August 1993 in the Chung Ang University Hospital is done in this report. There were 24 males and 18 females[1.3:1], and their age ranged from 1 year to 75 years. Nonspecific symptoms such as chest pain, cough and dyspnea were the most frequently encountered symptoms. Asymptomatic patients were relatively common[21.4%]. The tumors were found most frequently in the posterior mediastinum[45.2%]. Anterior mediastinum was the next and middle and upper mediastinum showed relatively rare occurrence of the tumor. Neurogenic tumor was the most common histopathologic type . Germ cell tumor, benign cyst and thymoma followed it. Curative resection was possible in 27 patients[64%], partial resection in 6 patients[14.3%]and just open biopsy in 7 patients[16.7%]. There was no early operative mortality. Postoperative complications were occurred in 5 patients[11.9%] and most of them were not serious.

Thoracic Interdural Aspergillus Abscess Causing Rapid Fatal Spondylitis in the Presence of Posterior Mediastinitis

  • Most primary spinal abscesses, irrespective of pathogens and anatomical locations, have better prognosis than that of secondary abscesses with spondylitis. We report a 68-year-old man, previously undertaken pulmonary resection due to tuberculosis, presented with paraparesis. Imaging studies showed primary intraspinal abscesses at T-1 and T-3 vertebral levels, semi-invasive pulmonary Aspergillosis and inflammation of the posterior mediastinum. Operative procedure and histopathological examination revealed interdural Aspergillus abscess. Despite chemotherapy, he deteriorated progressively, and spondylitis developed at corresponding vertebrae. He eventually died 6 weeks postoperatively due to pulmonary complication. The authors intended to inform that such an extradural inflammatory lesion of Aspergillus abscess should be treated carefully.

고령의 환자에서 발생한 다발성 후경부 신경절신경종 1예 (A Case of Multiple Posterior Cervical Ganglioneuromas in Elderly Patient)

  • Ganglioneuroma (GN) is benign neurogenic tumor arising from ganglia of the sympathetic nervous system. They are mostly found at posterior mediastinum, retroperitoneum, and adrenal gland, whereas only 1-5% occurred in the cervical region. GN usually present as a single, painless and slow-growing mass, but multiple cervical occurrences are extremely rare. An 80-year-old woman came to our clinic complained of posterior neck mass for three years. We performed surgical excision, and it was finally diagnosed as GN. We report the unique and rare disease entity with a brief literature review.

  • A 22 year old soldier was discovered on routine study for discharge to have a mass in the posterior mediastinum He was admitted due to high fever and improved by antibiotic treatment for 2 weeks. Chest PA, apicogram, myelogram and CT scan demonstrated enlargement of the neural foramen at the T1 level with erosion of the posterior aspect of the vertebral body and the pedicle contiguous with the intrathoracic mass, A myelogram showed a large extradural defect at the T1 level on the left. There were no clinical signs of cord compression. A standard posterolateral thoracotomy incision was made with extension to high thoracic vertebra. The 3rd rib was resected and the angles of the posterior portion of 1st and 2nd ribs were cut and rib heads were removed. Extrapleural neurilemmoma 6x6 cm was resected intrathoracically. And after removal of the pedicle and the lamina, intraspinal extradural mass 3 X 2 cm was resected carefully with trivial tearing of the dura which was sealed with gel-foam and pleura. There was ma postoperative neurological complication.

후종격동에 발생한 모세혈관상 혈관종 -1예 보고- (Capillary Hemangioma in the Posterior Mediastinum -A case report-)

  • 종격동에 발생한 혈관종은 전체 종격동 종양의 0.5% 미만을 차지할 정도로 드문 질환이다. 후종격동에 발생한 혈관종의 경우는 전종격동에 비해 더 드문 것으로 알려져 있다. 환자는 21세 여자로 단순흉부 방사선촬영상 후종격동에 비정상적인 음영을 보여 본원으로 전원되어 시행한 흉부 전산화 단층촬영과 흉추 자기공명 영상촬영에서 척수 경막까지 도달되어 있는 아령모양의 후종격동 종양 소견을 보였다. 이에 수술적 절제를 시행하였고 병리 조직학적 검사결과 3${\times}$4${\times}$2 cm의 모세혈관상 혈관종의 소견이 관찰되었다.

  • For the purpose of evaluation of clinical characteristics and histopathological properties in mediastinal tumor, and to provide the guidelines of surgical management of mediastinal tumor,238 patients with mediastinal tumors treated during the period from January 1983 to December 1991 were reviewed at Seoul National University Hospital. There were 106 males & 132 females, and their ages ranged from 3 months to 73 years, with average 33.6 years. The most frequently encountered tumor site was anterosuperior mediastinum followed by posterior, and middle mediastinum. In the pathological viewpoint, thymoma was the most frequent type followed by neurogenic tumor. 81% of the tumor were benign and 19% were malignant. Half of the malignant tumors were neurogenic tumors. Malignancy rate was high in pediatric patients compared to adults as 40% and 19% respectively. 65% of patients were asymptomatic at diagnosis.There was no operative mortality. Post operative complications were occurred in 35 cases. Most frequent complications were adjacent peripheral nerve injuries. But other usual operative complications, such as bleeding, chylothorax, infection, were relatively rare.

종격동 평활근종의 1례 (Mediastinal Leiomyoma -Report of a case-)

  • The benign tumor of smooth muscle, the leiomyoma, may arise anywhere in the body, in such tissues as the muscularis of the gut and the media of blood vessels, although by far its most common location is the uterus. Indeed, the mediastinal leiomyomas are extremely rare, especially when one excludes those arising in the esophagus. No dogmatic statements can be made with regard to age, sex distribution, symptoms, or clinical course because of the extreme paucity of cases available for evaluation. Although the majority of the reported cases are in the posterior mediastinum, thus suggesting esophageal origin. We have experienced a right inferior mediastinal leiomyoma in 36 years old housewife. She complained hemoptysis and right lower chest pain associated with intermittent low-graded fever and chillness. Routine X-ray film revealed a large irregular cystic mass density in the right lower thoracic cavity. On exploratory thoracotomy, a huge round yellow-gray colored solid mass, measured about 2.5kg in weight, was located in the right inferior mediastinum just above right hemidiaphragm, medially. The tumor mass was removed, being proved to be leiomyoma on histopathologic examination. Her postoperative course was uneventful and discharged without complication.

"DUMBBELL" 신경종양 -1례 보고- ("DUMBBELL" Neurilemmoma -A Case Report-)

  • Neurogenic tumors of the mediastinum may have an intraspinal component connected by a narrowed segment of tumor in the intervertebral foramen,hence the descriptive term dumbbell.Recently we had an occasion to remove a dumbbell neurilemmoma in a 62 years old woman using an approach designed to allow wide posterolateral thoracotomy and concomitant laminectomy for a single stage removal of the entire tumor. The mass in the posterior mediastinum was discovered on routine chest roentgenography. CT scan demonstrated a dumbbell shaped soft tissue mass density compressing spinal canal but preserving spinal cord. There were no neurologic signs. A standard posterolateral thoracotomy incision was made to remove tumor mass and then T5 unilateral laminectomy has done by Neurosurgeon. 7 x 7 cm sized extrapleural neurilemmoma was round, cystic, soft mass which covered parietal pleural with invaded regional vertebrae. There was no postoperative neurological complication.

Mediastinal Anaplastic Ependymoma

  • Ependymomas arise from ependymal cells and can grow at any site in the central nervous system (CNS), as well as in some locations outside of the CNS. The latter is rare, contributing to the frequent misdiagnoses of such cases. Herein, we present the case of a 54-year-old man with a history of lower limb weakness and numbness. Magnetic resonance imaging revealed an extradural, heterogeneously enhanced solid lesion with a regular and well-defined border in the posterior mediastinum. A post-resection histopathological examination revealed tumor-forming perivascular pseudo-rosettes that showed immunoreactivity against glial fibrillary acidic protein, epithelial membrane antigen, and vimentin, as well as a high Ki-67 labeling index. Based on pathological features, a diagnosis of anaplastic ependymoma was established.

종격동 종양의 전산화단층촬영 소견 (CT findings of the Mediastinal tumors)

  • 30례의 종격동 종양의 CT분석 결과, 저자들은 다음과 같이 요약 할 수 있었다. 1. 가장 흔한 종양은 흉선질환 이었으며, 그 다음으로는 기형종, 림프종, 기관지성 낭종, 신경종, 심막낭종의 순이었다. 2. 5례의 흉선종은 균일한 충실성 음영의 종괴로 보였으며, 석회침착, 소엽형성이 각각 1례에서 보였다. 악성흉선종 중 1례에서 피낭형성이 잘된 낭성 종괴로 보였으며, 흉선암종은 주위 경계의 소엽형성을 보인 균일한 음영의 종괴로 보였다. 3. 전 례의 가형종은 모두 낭성종괴로 보였으며, 지방과 석회음영은 각각 2례, 4례에서 보여졌다. 4. 신경종은 4례 모두에서 후종격동에 위치한 균일한 음영의 종괴로 보여졌다. 5. 기관지성 낭종은 기관분기부 하방, 부흉곽지역에 각각 1례, 후기관부에 2례 있었으며, 모두 균일한 음영의 낭성 종괴로 보여졌다. 6 심막낭종은 심장 주위 경계를 따라 난형모양의 낭성 종괴로 보여졌다. 결론적으로 종격동 종괴의 진단에 CT를 실시함으로써 종괴의 정확한 위치, 크기 및 특징적인 구성성분을 관찰할 수 있으며, 이러한 소견으로 종괴의 감별진단에 도움을 얻을 수 있다.
