Effect of the Exchange of Saline Used in Surgical Procedures on Surgical Site Infection (수술에 사용하는 생리식염수의 교환이 수술부위감염에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of the exchange of saline used in surgical procedures on surgical site infections. Method: Patients with stomach cancer were assigned to the experimental group or to the control group by random sampling, respectively. The experimental group received an exchange of saline during the operation right after the excision of the stomach in a gastrectomy but the control group did not. Data were collected from the medical charts of 34 patients from Dec. 1, 2002 through May 31, 2003. Result: The surgical site infection rate of the experimental group was 5.9% while surgical site infection rate of the control group was 17.6%. In total, the surgical site infection rate was 11.8%. The experimental group maintained a normal level of WBC on post operative day 3; however, the control group, showed an increase of WBC on post operative day 3. Conclusion: The exchange of saline used in an operation immediately after the excision of the stomach in a gastrectomy decreases the contamination level of saline used in the operation, and can prevent surgical patients from a surgical site infection.

Factors Related to Surgical Site Infections in Patients Undergoing General Surgery (일반외과 환자의 수술부위 감염 관련 요인 분석)

  • Purpose: To identify risk factors for surgical site infections in patients undergoing general surgery, to analyze the prolonged hospital stay and extra cost for antibiotics, and to provide basic data for control of surgical site infections. Method: Surgical site infection was defined using the definition of the CDC and the data were analyzed by $x^2$-test and unpaired t-test. Results: The prevalence of surgical site infections was 9.7%, and it was related to wound class, duration of operation, number of operations, whether the operation was an emergency, trauma, drains, preoperative stays, presence of remote infection during operative period, and previous history of recent surgery. The mean duration for post-operative stay when a surgical site infection occurred was 9.5 days and in 56.9 % of the patients the surgical site infection appeared 7 days after the operation. Post-operative stays for infected patients were 20.3 days longer than that of uninfected patients. The mean cost of antibiotics for infected patients was higher than that for uninfected patients by 561,067 won per person. Conclusion: Surgical site infection results in an increased length of stay and extra-cost, thus, hospitals need to create strategies to reduce nosocomial infections through effective infection surveillance and by considering factors related to surgical site infections.

  • The feature based modeling approach is useful for post-CAD related works such as process planning and NC part programming. This paper describes the development of 'FeaTURN' system which is feature based NC part programming system for turning operation. The programming task in 'FeaTURN' system becomes easy and effective with the assistance of feature icons. The manufacturing attributes can be handled toghther with the features during input procedure. The cutter location data (CLD) is determined by the processor module. The post process module converts the CL data to machine control data (MCD). Also, the system graphically displays the tool path.

  • Purpose: The purpose of this study was to evaluate the effects of mobilization application for the lower extremity after fibula Pilon fracture operation patient. Methods: The subjects was 62 years old male who was injury of Lt. fibula shaft Fx., 3 cuneiform & cuboid Fx., 2.3.4 metatarsal bone Fx., We were compared to result of physical therapy between pre and post exercise for 2weeks. Results: The results of this study were summarized below; The mobilization application of the Lt. lower extremity was significantly differences of the ROM at pre and post therapy after 2 weeks, especially in knee flexion ($40^{\circ}$). The increased of accessary movement was evaluated to increased of the physiologic movement about the joints of the lower extremity. Conclusion: We consider that factors of therapy result were not only fracture types, operation and reduction methods for the fibula Pilon fracture but also the ability of physical therapist's manual techniques.

  • The aim of this study is to develop an assessment program of speech rehabilitation for children having some language and speech disorders. Patients of dysarthria, tonsillectomy, tonsilhypertrophy, and nasal obstruction were selected for this experimental clinical phonetic study. Formant variations ($F_1\;&\;F_2$) show pre- and post-operation differences in tonsillectomy and cleft palate patients. Nasal formants ($NF_1\;&\;NF_2$) show pre- and post-operation differences in nasal obstruction. The articulation reaction time (ART) as a parameter was used to assess Voice Onset Time(VOT). It was shown longer duration for hypokinetic dysarthria and shorter for atoxic dysarthria.. The diadochokinetic rate was measured by Visi-pitch. Lower diadochokinetic rate appeared to spastic and dysarthria in comparison with the control group. It was shown that the nasalance of tonsilhypertrophy, nasal obstruction, and cleft palate patients was seen to increase after operation. In addition, the assessment of nasality can be measured only by simple vowels such as /a/ and /i/.

  • This study examines the effectiveness of a new community facility operation and management system that were implemented by one of the major construction companies. This system was an unprecedented approach in a sense that it provided not only necessary facilities but also a specialized management staff, an operational manual, and a subsidy for various programs. According to POE conducted among the residents, it can be concluded that the residents' satisfaction will be increased under the following conditions; 1) when a program manual associated with the community facility is provided, 2) when there is a professional staff who can coordinate various programs effectively, 3) when the facilities are well designed in accordance with the program requirements, 4) when the users adhere to regulations and management standards, 5) when all three components- i.e. facility, people and programs- are well coordinated, and 6) when affordable user fees are charged. This research assures that professional management staff and well-planned management system play a significant role in the enhancement of the residents' satisfaction as well as vitalizations of the community.

Post-processing of vector quantized images using the projection onto quantization constraint set (양자화 제약 집합에 투영을 이용한 벡터 양자화된 영상의 후처리)

  • In order to post process the vector-quantized images employing the theory of projections onto convex sets or the constrained minimization technique, the the projector onto QCS(quantization constraint set) as well as the filter that smoothes the lock boundaries should be investigated theoretically. The basic idea behind the projection onto QCS is to prevent the processed data from diverging from the original quantization region in order to reduce the blurring artifacts caused by a filtering operation. However, since the Voronoi regions in order to reduce the blurring artifacts caused by a filtering operation. However, since the Voronoi regions in the vector quantization are arbitrarilly shaped unless the vector quantization has a structural code book, the implementation of the projection onto QCS is very complicate. This paper mathematically analyzes the projection onto QCS from the viewpoit of minimizing the mean square error. Through the analysis, it has been revealed that the projection onto a subset of the QCS yields lower distortion than the projection onto QCS does. Searching for an optimal constraint set is not easy and the operation of the projector is complicate, since the shape of optimal constraint set is dependent on the statistical characteristics between the filtered and original images. Therefore, we proposed a hyper-cube as a constraint set that enables a simple projection. It sill be also shown that a proper filtering technique followed by the projection onto the hyper-cube can reduce the quantization distortion by theory and experiment.

  • Until recently post-processing of finite element model has been heavily relied on expensive graphic peripheral devices. With the aid of inexpensive microcomputers, very economical post-processor graphics program called MICRO-POST has been developed. Model geometry or results of analysis for the unlimited meshes can be easily presented in a number of low-cost graphic devices. The paper presents the procedure obtaining the device-independent graphics, and the structure and functions of the program. It also describes efficient I/O scheme to overcome the memory limitation, and dialogue-type input technique to control the plot operation in an interactive manner. Through the post processing examples for the general purpose finite element programs, it demonstrates the usefulness of the program.

  • Background : This study was designed to assess the effects of information on the satisfaction of parents and families whose children underwent elective surgery in the Pediatric Operating Room. Methods : We measured parent and family satisfaction with information given before and after offering informational interventions. The data were collected from 510 patients parents and families whose children had undergone elective surgery in the Pediatric Operating Room of Seoul National University Hospital in Seoul. South Korea. The data collection ran from May 3. 1999 to May 30. 1999 and from September 6, 1999 to September 30, 1999. The research instrument consisted of four categories (pre-operative period, intra-operative period, post-operative period, others) and 24 questionnaires. SPSS was used to analyze the data. Results : The major findings of this study are summarized as follows: 1. Before surgery, the satisfaction with the following was increased: the explanations of the necessary preparation for the surgery (6.0%), the time the patient would enter the operating room (20.6%), the operative procedure (2.0%), and the use of the waiting room (10.0%). 2. During the operation, the satisfaction with the situation board (15.1%), public announcements in the waiting room (8.4%), and the answering of the families questions (12.2%) was increased. however, the satisfaction with the surgeons explanations of the surgical outcomes decreased by 8.3%, even though the frequency of these explanations increased by 5%. 3. After surgery, the satisfaction with the following was increased: the explanations of patient's status of recovery(10.3%) the time the patient would be moved from the recovery room (17.6%), how the patient would be transferred to the ward (19.2%), and post-operative care (6.3%). Conclusion : Based upon the above findings, we concluded that pre-, intra-, and post-operative informational interventions were effective in increasing the satisfaction of the parents and families of children undergoing elective surgery.

Result of Staged Operation in Ruedi-Allgower Type II and III Open Tibia Pilon Fractures with Severe Comminution (분쇄가 심한 Ruedi-Allgower II, III형의 개방성 경골 천정 골절에서 단계적 수술의 결과)

  • Purpose: This study analyzed the clinical and radiological results of Reudi-Allgower type II and III open tibia pilon fracture patients who underwent plate fixation after the recovery of a soft tissue injury after external fixation. Materials and Methods: From 2010 to 2015, this study analyzed 14 patients who were treated for open tibial pilon fractures and could be followed up at least one year. The mean age was 49 years and the average follow-up period was 19 months. An emergency operation was performed for external fixation and open wounds, and secondary surgery was performed for definitive fixation using a plate. The radiological and clinical evaluations were analyzed retrospectively. Complications, such as post-traumatic osteoarthritis and wound infections were also analyzed. Results: The mean duration between two-staged surgery was 21 days and the mean bone union time was 9.2 months. Three cases of delayed union and one case of nonunion were reported. The malunion did not occur in all cases. The average American Orthopaedic Foot and Ankle Society (AOFAS) score was 68 points. A limitation of the ankle motion occurred in all cases. In four cases, wound infections due to initial open wounds occurred; one patient underwent a below the knee amputation due to chronic osteomyelitis. Post-traumatic arthritis occurred in 10 cases. Conclusion: Severe comminuted tibial plateau open fractures of Reudi-Allgower type II and III, which are high-energy injuries that result in extensive soft tissue damage, have a higher incidence of complications, such as ulcer problems and osteomyelitis, than closed tibia plateau fractures. Post-traumatic arthritis is the most common complication of tibia plateau open fractures, and staged surgery is recommended because of the relatively satisfactory clinical results.
