  • IgG4-related disease is an immune-mediated fibro-inflammatory disease, characterized by lymphoplasmacytic infiltration composed of IgG4-positive plasma cells of various organs with elevated circulating levels of IgG4. This disease is now reported with increasing frequency and usually affects middle-aged men. Massive pleural effusion in children is an uncommon feature in IgG4-related disease. Here, we report a case of a 16-year-old male patient with extensive IgG4-related disease presenting with massive pleural effusion, mediastinal mass, and mesenteric lymphadenopathy.

Effects of Soyoligosaccharides and Varing Dietary Fats on Intestinal microflora. Lipid Profiles and Immune Responses in Rats (대두올리고당이 흰쥐의 장내 균총, 지질 대사 및 면역 능력에 미치는 영향)

  • The aim of this study was to evaluate the effects of soyoligosaccharides on intestinal microflora transit time lipid profiles and immune responses. Forty-eight male rats of Sprague-Dawley strain were divided into six groups(SYS: soybean oil-sucrose SYO: soybean oil-oligo SES; esame oil-sucrose SEO:sesame oil-oligo BFS : beef tallow-sucrose BFO : beef tallow-oligo) soyoligo-groups(SYO, SEO, BEO) were fed the diet containing 5% soyoligosaccharides but control groups not fed them and all groups were fed 20% fats for 4 weeks. The number of fecal bifidobacteria tended to increase after soyoligosaccharides feeding. The significant increase was found in SEO group compared to SES in that of fecal bifidobacteria,. Stool wet weight dry fecal weight and stool water content were increased significantly in soyoligo-group(SYO, SEO, BFO) compared to control groups(SYS, SES, BFS) and gastrointestinal transit time(TT) tended to increase. Soyoligosaccharides lowered plasma cholesterol in SEO group significantly more than did other groups. soyoligo-groups(SYO, SEO, BFO) tended to decrease liver triglycerides compared to control groups. Plasma IgG concentration increased in beef tallow groups(BFS, BFO) than that in other dietary fat groups. Soyoligosaccharides and kinds of fats had effects on plasma C3 concentration and mitogenesis of the spleen cells. Soyoligosaccharides had no effects on plasma leptin and insulin concentration but kinds of fats did so that sesame groups were decreased significantly in plasma leptin concentration but increased significantly in plasma insulin concentration compared to other dietary fat groups. In conclusion dietary soyoligosaccharides changed the composition of the intestinal microflora beneficially with sesame oil and partially had effect on plasma and liver lipid profiles .(Korean J Nutrition 33(6) : 597∼612, 2000)

Influence of Environmental Living Standards on Helicobacter pylori Infection in Korean Elementary School Children (서울 지역 초등학생의 생활환경과 Helicobacter pylori 양성률)

  • Purpose: We measured anti-H. pylori IgG in Korean elementary school children living in Shinchon area of Seoul, Korea to evaluate the influence of environmental living standards on H. pylori infection. Methods: IgG antibodies to H. pylori were measured in plasma using a commercial ELISA kit (GAP IgG Helicobacter pylori, Bio-Rad Laboratories Inc., Hercules, CA, USA). Information on environmental status such as place of birth, parental income, type of housing, number of persons in the household, parents' occupation, family history of peptic ulcer disease and gastric cancer was obtained. Statistical analysis was done by Chi-square and logistic regression test using SPSS $7.0^{TM}$ for Windows. Results: Study subjects consisted of 571 children, and the age distribution ranged from 6.0 to 13.6 years with a mean of $9.6{\pm}1.8$ years. Male-to-female ratio was 1.1:1. The seropositive rates of H. pylori infection ranged from 10.4% in children aged 6 years to 30.9% in 12 year-old group, overall 16.8%. The prevalence of H. pylori infection progressively increased with age, but there was no significant difference in seropositive rates among children in different age groups (p=0.06). Seropositive rates of anti-H. pylori IgG on the basis of gender, place of birth, parental income, type of housing, parents' occupation, family history of peptic ulcer disease and gastric cancer showed no statistically significant difference. Interestingly, however, seropositive rate of anti-H. pylori IgG showed statistical significance in relation to number of persons in the household (p=0.003; Odds ratio 1.50 by logistic regression test). Conclusion: These results suggest that number of persons in the household is the most important factor among environmental living standards, and that risk of H. pylori infection increases by increment of 1.5 times as the number of persons in the household increases by one.

Evaluation of the Immune-Stimulating Activity of Samul-tang, a Traditional Korean Herbal Medicine, Standardized by HPLC-PDA

  • Objectives: We performed simultaneous determination of five constituents by HPLC in Samul-tang (SMT). Additionally, we investigated the immune-stimulatory potential of SMT on specific cellular and humoral immune responses in ovalbumin (OVA)-immunized mice. Methods: Reverse-phase chromatography using a Gemini C18 column operating at $40^{\circ}C$, and photodiode array (PDA) detection at 190-400 nm, were used for quantification of the five components of SMT. Mobile phase using a gradient flow consisted of two solvent systems. Solvent A was 1.0% (v/v) aqueous acetic acid and solvent B was acetonitrile with 1.0% (v/v) acetic acid. C57BL/6 mice were immunized intraperitoneally with OVA/alum ($100{\mu}g/200{\mu}g$) on days 1, 8, and 15. The extract of SMT (1000 mg/kg) was given to mice orally for 21 days (from day 1 to day 21). At day 22, OVA-, lipopolysaccharide (LPS)- and concanavalin A (Con A)-stimulated splenocyte proliferation and OVA-specific and total antibodies were measured in plasma. Results: Calibration curves were acquired with $r^2$>0.9999, and the relative standard deviation (RSD, %) for intra- and inter-day precision were both less than 3.5%. The recovery was in the range of 95.69-115.12%, with an RSD less than 6.0%. The contents of five components in SMT were 1.08-15.30 mg/g. SMT significantly enhanced Con A-induced splenocyte proliferation in OVA-immunized mice (p<0.01). Also, SMT significantly enhanced OVAspecific IgG, IgG1 and total IgM levels in plasma compared with the OVA-immunized group. Conclusions: The established method will be applied for the quantification of major components and immunestimulating activity in OVA-immunized mouse model of SMT.

Effect of Threonine Addition to a Low Protein Diet on IgG Levels in Body Fluid of First-Litter Sows and Their Piglets

  • First-litter gilts were used to determine how different dietary treatments during gestation affect the reproductive performance of gilts and immunity development of their piglets. Twenty-two crossbred $Landrace{\times}Yorkshire$ gilts were randomly assigned to three dietary treatments, Following conception, the gilts were fed experimental diets until farrowing occurred. The diet for treatment I was low protein diet (8% CP), treatment 2 had an additional supplement of 0.14% threonine that was added to the low protein diet, and treatment 3 was a control diet containing 12% CP. During gestation, net body weight gain of sows in treatment group 2 was higher than in treatment group 1 (p=0,075), However, during lactation there was no difference between all treatments groups on body weight loss and their live piglets at birth. Although milk IgG between treatments did not differ, treatment groups 2 and 3 were slightly higher than treatment group I was, Plasma IgG concentrations in piglets were however equal within all treatment groups at birth and at 7 days of age, at 21 days of age, it was higher in treatment group I than it was in the other two groups (p<0.01), Threonine supplementation to a low protein diet during gestation slightly increases milk IgG of sows, It is beneficial for piglets to acquire more passive immunity, but a suppressive effect was also noted on the endogenous IgG synthesis in piglets. A gestation diet of 8% CP for gilts can stimulate immuno-system of her piglets.

  • This study was performed to investigate the effect of oral administration of GSRE against the asthma. Asthma was induced to Balb/c mouse by i.p. injection and aerosol immunization with ovalbumin. It was observed the change of the eosinophil number in the BALF. Concentrations of IL-4, IL-5 in BALF and splenocyte were assessed by ELISA, IgG and IgE from serum were calculated by same method. Concentration of IL-4 in splenocyte was significantly decreased in GSRE group compared with control group. Concentrations of IL-5 from BALF and splenocyte were significantly decreased in GSRE group compared with control group, respectively. Level of IgE in serum was significantly decreased in GSRE group compared with control group, but not IgG. We found that the effect of GSRE extract in asthma was implicated in reductions of IL-4, IL-5 released from Th2 cell, and decreses of IgE, from plasma cell. These findings suggest that GSRE extract can produce anti-asthmatic effect, which may play a role in allergen-induced asthma therapy.

Stimulation of the Immune Response by Herbal Formulas for Wind-Cold and Heat Pain Symptom (대표적인 풍한열(風寒熱)에 의한 통증 치료 처방의 면역 활성화 비교 연구)

  • Three herbal formulas (Bangpungtongsung-san, Ohyaksungi-san, and Ojeok-san) for wind-cold and heat pain symptom were applied to investigate the immunological activities on antigen (Ag)-specific or Ag-non-specific immune responses in murine macrophage cell line (RAW 264.7) and ovalbumin (OVA)-immunized mice. This study was carried out in nitric oxide (NO) synthesis in RAW 264.7 cells and cellular proliferation in mouse splenocytes according to three herbal formulas. C57BL/6 mice were immunized intraperitonially with OVA/aluminium ($100\;{\mu}g/200\;{\mu}g$/mouse) on day 1, 8, and 15. Three herbal formulas were administrated to mice orally for 3 weeks from day 1. On day 22, OVA-, lipopolysaccharide (LPS)-, and concanavalin A (Con A)-stimulated splenocyte proliferation and antibodies (OVA-specific antibodies of the IgG, lgG1, and total IgM classes) in plasma were measured. Ohyaksungi-san increased NO synthesis in RAW 264.7 cells. Ojeok-san and Ohyaksungi-san significantly enhanced cellular proliferation by LPS and Con A in splenocytes from OVA-immunized mice (p<0.001). Three herbal formulas for wind-cold and heat pain symptom also significantly enhanced plasma OVA-specific IgG, IgG1, and total IgM levels compared with the OVA/Alum group. These results suggested that three herbal formulas for wind-cold and heat pain symptom could be used as stimulator of immune response.

Hens immunized with live attenuated Salmonella strains expressing virulence-associated genes in avian pathogenic Escherichia coli passively transfer maternal antibodies to chicks

  • We investigated whether maternal antibodies (mAbs) elicited by dams immunized with recombinant vaccine candidates against avian pathogenic Escherichia coli (APEC) can passively confer protective immunity to chicks. In the present study, pBP244 plasmids carrying selected antigens of APEC were transformed into Salmonella Typhimurium JOL912, which was used as a vaccine candidate against APEC. The hens were immunized with the vaccine candidates using prime or booster doses. The levels of IgG and sIgA specific to the selected antigens increased significantly following prime immunization. To evaluate the persistence of passively transferred mAbs, the levels of IgY and IgA were determined in egg yolks and whites, respectively. The eggs from the immunized group showed consistently increased levels of IgY and IgA until week 16 post-laying (PL) and week 8 PL, respectively, relative to the control group. The presence of mAbs was observed in chicks that hatched from the hens, and titers of plasma IgY were consistently raised in those from the immunized hens by day 14 post-hatching. Further, chicks from the immunized hens were protected from challenge with a virulent APEC strain, whereas those from non-immunized hens showed acute mortality.

  • Immunoglobulin G4 (IgG4)-related disease is a newly recognized condition characterized by fibroinflammatory lesions with dense lymphoplasmacytic infiltration, storiform-type fibrosis and obliterative phlebitis. The pathogenesis is not fully understood but multiple immune-mediated mechanisms are believed to contribute. This rare disease can involve various organs and pleural involvement is even rarer. We report a case of IgG4-related disease involving pleura. A 66-year-old man presented with cough and sputum production for a week. Chest radiography revealed consolidation and a pleural mass at right hemithorax. Treatment with antibiotics resolved the consolidation and respiratory symptoms disappeared, but the pleural mass was unchanged. Video-assisted thoracoscopic surgery was performed. Histopathology revealed dense lymphoplasmacytic infiltration and storiform fibrosis with numerous IgG4-bearing plasma cells. The serum IgG4 level was also elevated. Further examination ruled out the involvement of any other organ. The patient was discharged without further treatment and there is no evidence of recurrence to date.

The Effects of Juglandis Semen Extract on the Experimental Asthma induced by Ovalbumin (호도(胡桃) 물추출물이 Ovalbumin으로 유도된 천식동물모델에 미치는 영향)

  • Objective : Juglandis Semen has a function that to invigorate the lung and kidney. And It is commonly used as a supporting agent in the treatment of coughing and bronchitis. This study was performed to investigate the effect of oral administration of Juglandis Semen Extract (JSE) against the experimental asthma induced by ovalbumin. Methods : Asthma was induced to Balb/c mouse by i.p. injection and aerosol immunization with ovalbumin. It was observed the change of the cell number in the BAL fluid. Concentrations of IL-4, IL-5 in splenoc yte were assessed by ELISA, IgG and IgE from serum were calculated by same method. Results : 1. Number of macrophage in BAL fluid was significantly decreased in JSE group compared with control group, but not eosinophil and lymphocyte. 2. Levels of IgG and IgE in serum were significantly decreased in JSE group compared with control group, respectively. 3. Concentration of IL-4 in culture supernatant of splenocyte was significantly decreased in JSE group compared with control group, but there was no significant in IL-5. Conclusion : We found that the effect of JSE extract in asthma was implicated in reduction of IL4released from Th2 cell, and decreases of IgG and IgE from plasma cell. These findings suggest that JSE can produce anti-asthmatic effect, which may play a role in allergen-induced asthma therapy.
