Purpose: The aim of this study was to evaluate the result of combined first metatarsal and calcaneal osteotomy for static cavovarus deformity of the foot. Materials and Methods: We performed a dorsal closing wedge $1^{st}$ metatarsal osteotomy and a lateral and upward displacement calcaneal osteotomy for 9 patients, 12 feet (6 male and 3 female). The mean age at the time of operation was 37 years and the mean followup period was 27 months. The causes of deformity were 2 poliomyelitis, 1 cerebral palsy, 1 Charcot-Marie-Tooth disease and 5 idiopathic type. Five lateral ligament reconstructions of the ankle and six percutaneous Achilles tendon lengthenings were added. The surgical results in terms of pain, function and alignment of the foot were evaluated by means of AOFAS ankle-hindfoot score and talo-$1^{st}$ metatarsal, calcaneus-$1^{st}$ metatarsal and calcaneal pitch angles were checked with weight bearing radiographs in lateral projection. Results: Talo-$1^{st}$ metatarsal and calcaneal pitch angles were reduced from the mean preoperative values of $21^{\circ}$ and $25^{\circ}$ to $12^{\circ}$ and $19^{\circ}$, respectively, at last followup. Also, calcaneus-$1^{st}$ metatarsal angle was increased from the mean $114^{\circ}$ to $114^{\circ}$. The mean AOFAS score was improved from 44.5 points preoperatively to 89.2 points at followup. There were 1 metatarso-cueiform joint nonunion, 1 sural nerve injury and 3 remaining symptomatic claw toes. Conclusion: Combined first metatarsal and calcaneal osteotomy appears to be an effective procedure for the treatment of adult static cavovarus foot.
Purpose: To determine the clinical and radiographic results of arthroereisis using the $Kalix^{(R)}$ implant (Newdeal, Lyon, France) for the treatment of flexible flatfoot deformity. Materials and Methods: From February 2005 to February 2007, we performed the subtalar arthroereisis on 8 patients (9 feet) of symptomatic flexible flat feet after more than 6 months of conservative treatment. Average age was 14.5 years ($11{\sim}29$ years) old. We checked the functional status with AOFAS functional score pre-operatively and at final follow-up. Radiologically, we took weight bearing anterior to posterior and lateral view of the feet, and measured the talo-first metatarsal angle, calcaneal pitch angle in pre-operatively and at final follow-up. Results: Mean follow up period was 34.4 months. Average AOFAS score improved from preoperatively 65.6 to postoperatively 94.8. Average lateral talo-first metatarsal angle reduced from $12.8^{\circ}$ preoperatively to $1.6^{\circ}$ at final follow-up. Average anterior to posterior talo-first metatarsal angle was reduced from $15.1^{\circ}$ preoperatively to $8.3^{\circ}$ at final follow-up. Average calcaneal pitch angle was increased from $9.5^{\circ}$ preoperatively to $12.0^{\circ}$ at final follow-up. Conclusions: Subtalar arthroereisis with Kalix$K^{(R)}$ implant can be considered to be one of treatment options symptomatic flatfoot deformity patients.
Purpose: We evaluate the results of subtalar arthroereisis with $Kalix^{(R)}$ implant (Newdeal, Lyon, France) that were performed in painful flatfoot deformity. Materials and Methods: We performed the subtalar arthroereisis on 16 feet of children symptomatic flexible flat feet after more than 6 months of conservative treatment. Average age was 11 years (8-14 years) old. We checked the functional status with AOFAS functional score in pre-operatively and at final follow-up. Radiologically, we took weight bearing anterior to posterior and lateral view of the feet, and measured the talo-$1^{st}$ metatarsal angle, calcaneal pitch angle, cuboid-surface height. Finally, we asked to patient's parents for satisfaction of the surgery. Results: Mean follow up period was 34.1 months. Average AOFAS score improved from preoperatively 71.9 to postoperatively 91.3. Only one patient has subtalar pain. Average lateral $1^{st}$ metatarsal angle reduced from $-18.2^{\circ}$ preoperatively to $-4.6^{\circ}$ at final follow-up. Average anterior to posterior $1^{st}$ metatarsal angle was reduced from $18.9^{\circ}$ preoperatively to $6.5^{\circ}$ at final follow-up. Average calcaneal pitch angle was increased from $8.6^{\circ}$ preoperatively to $12.6^{\circ}$ at final follow-up. Average cuoboid-surface height was improved from 12.1 mm preoperatively to 16.0 mm at final follow-up. All patients had excellent or good satisfaction. Conclusions: Subtalar arthroereisis with $Kalix^{(R)}$ implant is a viable surgical alternative for painful flatfoot deformity of children.
대부분의 기계는 여러 종류의 금속으로 구성된다. 특히 선박의 축계는 프로펠러 날개의 황동과 스테인리스로 된 축으로 이루어져 있다. 이 이종금속이 바닷물의 전해액에 들어가면 볼타 전지를 이루고, 기전력이 발생된다. 이 기전력은 축계를 받치고 있는 베어링과 축을 전기부식 시키는 원인이 된다. 선박에서는 이 부식을 막기 위해 선박에서는 축 접지 시스템을 설치하여 운용하고 있다. 본 연구는 가변피치 프로펠러의 축기전력을 측정하기 위하여 추진축의 전압과 주기관의 회전수를 동시에 측정하였다. 측정장치는 내셔널인스트루먼트사의 24bit A/D컨버터를 사용하여 측정하였고, 프로그램은 LabVIEW를 사용하였다. 주기관의 회전수와 축기전력의 발생, 블레이드 각도에 따른 기전력과, 배의 항해 방향에 따른 축기전력을 측정하고 분석하였다.
Purpose: The aim of this study was to investigate the radiological characteristics of the osteoarthritis of the second metatarsophalangeal (MTP) and tarsometatarsal (TMT) joint. Materials and Methods: Between January 2002 and August 2010, 27 patients (33 feet) who had second metatarsal osteoarthritis (OA) were reviewed retrospectively. Group 1 was 14 patients (17 feet) with second MTP joint OA. Group 2 was 13 patients (16 feet) with second TMT joint OA. Group 3 was 24 patients (25 feet) had hallux valgus without second metatarsal (MT) OA as control. Weight bearing foot anteroposterior (AP) and lateral view were checked, and measured hallux valgus angle, metatarsus adductus angle (MAA), second MT functional length, first and second MT length by Hardy & Clapham method on AP view, angle of second MT with horizontal plane, calcaneal pitch, talo-first MT on lateral view. Results: On weight bearing foot AP view, second MT functional length of group 1, 2, 3 was 2.4 mm, -0.1 mm, 0.7 mm and MAA of group 1, 2, 3 was $17.7^{\circ}$, $17.7^{\circ}$, $14.5^{\circ}$. Second MT functional length of group 1 was longer than control group and it was statistically significant. MAA was significant different between group 1-3 and group 2-3. Angle of second MT with horizontal plane of group 2 was smaller than control group and it was statistically significant. Other radiographic parameters have no statistical significance. Conclusion: Group 1 has long functional length of second MT and group 2 has small angle of second MT with horizontal plane.
실제 시스템 적용에 있어서, 수중비행체(Underwater Flight Vehicle : UFV)의 자율제어(autonomous control)를 위한 3-D 장애물회피(obstacle avoidance) 시스템은 다음과 같은 문제점들을 가지고 있다. 즉, 소나(sonar)는 지역적 탐색영역 내에서 장애물의 거리(range)/방위(bearing) 정보를 제공하며, 자율수중운동체(Autonomous Underwater Vehicle : AUV) 관점에서 에너지 소비 및 음향학적 소음이 적은 시스템을 필요로 하며, 최대 피치 및 심도와 같은 UFV 운용 제약조건을 가진다. 나아가, 구조와 파라메터의 관점에 있어서 용이한 설계 절차를 요구한다. 이 문제를 해결하기 위해서 진화 전략(Evolution Strategy : ES) 및 퍼지논리 제어기(Fuzzy Logic Controller : FLC)를 이용하는 지능형 3-D 장애물회피 알고리즘이 제안되었다. 제안된 알고리즘의 성능을 검증하기 위해 UFV의 3-D 장애물회피가 수행되었다. 시뮬레이션 결과는 제안된 알고리즘이 실제 시스템에 존재하는 문제점들을 효과적으로 해결하고 있음을 보여준다.
This research develops an experimental method to measure the motion of a FDB spindle system with a 3.5' disk by using three capacitance probes fixed on the xyz-micrometers, and it shows that a FDB spindle system has the whirling, flying and tilting motion. It also shows that the whirling, flying and tilting motion converge very quickly to the steady state at the same time when the rotor reaches the steady-state speed. However, they are quite large even at the steady state when they are compared with the 10nm flying height of a magnetic head. For the FDB spindle system used in this experiment, the whirl radius and the peak-to-peak variation of flying height and tilting angle at the steady-state speed of 7,200rpm are 0.675m, 30nm and $5.758\times10^{-3^{\circ}}$, respectively, so that the radial motion of the FDB spindle system exceeds a track pitch of a 3.5' HDD with 90,000 TPI.
The HDD (hard disk drive) using Load/Unload (L/UL) technology includes the benefits which are increased areal density, reduced power consumption and improved shock resistance than those of contact-start-stop (CSS). Dynamic L/UL has been widely used in portable hard disk drive and will become the key technology for developing the small form factor hard disk drive. The main design objectives of the L/UL mechanisms are no slider-disk contact or no media damage even with contact during L/UL, and a smooth and short unloading process. In this paper, we focus on lift-off force, pitch static attitude (PSA), roll static attitude (RSA) and dimple point. The "lift-off" force, defined as the minimum air bearing force, is another very important indicator of unloading performance. A large amplitude of lift-off force increases the ramp force, the unloading time, the slider oscillation and contact-possibility. PSA and RSA are also very important parameters in L/UL system and stability of slider is mainly determined by PSA and RSA. Dimple point by PSA and RSA is also important indicator. Therefore we find the optimal dimple point of SFF HDD suspension for improving the unloading performance.
Purpose: The author evaluated the clinical and radiological results after wearing the medial arch supports in children. Materials and Methods: 103 patients who had symptomatic flat feet were evaluated from march, 2002 to may 2009. All patients wore the medial arch supports according to the symptoms. We measured parameters at weight-bearing radiographs before and after medial arch support were worn. We also evaluated the clinical scores using the AOFAS score. Results: Mean age of patients was 97 months (11-204 months), all foot of patients involved bilaterally. Mean talo-first metatarsal angle of right foot was $17.7{\pm}9.4$ and left foot was $19.96{\pm}9.5$ degrees at AP radiograph in pre-wearing state. Mean calcaneal pitch angle of right foot was $12.0{\pm}5.3$ and left foot was $11.9{\pm}5.8$ degrees at lateral radiograph in pre-wearing state. Mean talo-first metatarsal angle of right foot was $14.4{\pm}8.05$ and left foot was $13.1{\pm}8.77$ degrees at AP radiograph in post-wearing state. Mean calcaneal pitch angle of right foot was $16.4{\pm}5.75$ left foot was $16.5{\pm}5.6$ degrees at lateral radiograph in post-wearing state. The radiographic angles between pre-wearing and post-wearing state were statistically significant (p<0.05). Mean pre-wearing AOFAS hindfoot score was $66.7{\pm}9.25$, midfoot score was $60.0{\pm}9.34$ forefoot score was $57.1{\pm}11.8$. Mean post-wearing AOFAS hindfoot score was $73.2{\pm}9.73$, midfoot score was $68.1{\pm}10.1$, forefoot score was $67.2{\pm}11.4$. The forefoot score was highest improving scores among the AOFAS scores. Conclusion: From our study, we concluded that medial arch support was effective for symptomatic flat feet of children in radiological and clinical results from our study.
열방성 액정상을 형성하는 새로운 하이드록시프로필 키토산과 이의 2가지의 유도체, 즉 6-콜레스테릴옥시카보닐펜톡시프로필 키토산들과 이들의 아크릴산 에스터들을 합성하였다. 또한 액정상태에 있는 6-콜레스테릴옥시카보닐펜톡시프로필 키토산의 아크릴산 에스터들을 광가교시켜 액정질서를 지닌 가교필름들을 제조하였다. 모든 시료들의 액정특성과 아세톤 중에서의 가교필름의 팽윤거동을 검토하였다. 하이드록시프로필 키토산과 판이하게 콜레스테릴을 지닌 모든 미가교시료들은 좌측방향의 나선구조를 지닌 단방성 콜레스테릭 상들을 형성하며 콜레스테릭 상의 전 범위에서 반사색깔들을 나타내었다. 이것이 키토산유도체가 가시광 영역의 반사대를 지닌 열방성 콜레스테릭 상을 형성한다고 하는 최초의 보고이다. 6,콜레스테릴옥시카보닐펜톡시프로필 키토산과 이의 아크릴산 에스터의 광학 피치들 (λ$_{m}$ 'S)은 온도상승 혹은 주어진 온도에서는 콜레스테릴 함량의 증가에 의해 감소한다. 그러나 콜레스테릴 함량이 동일한 경우, 동일한 온도에서 나타내는 λ$_{m}$ 은 6-콜레스테릴옥시카보닐펜톡시프로필 키토산이 이의 아크릴산 에스터에 비해 크다. 모든 가교시료들은 반사색깔을 나타내지 않았으며, 이러한 사실은 6-콜레스테릴옥시카보닐펜톡시프로필 키토산의 아크릴산 에스터의 콜레스테릭 구조가 가교에 의해 현저하게 변화됨을 시사한다. 모든 가교시료들에 있어서 액정가교겔의 특징적인 2차원적 이방성 팽윤이 관찰되었다.
