미니어쳐 핀셔견에서 발생한 확장성 심근질환 (Primary Dilated Cardiomyopathy in a Miniature Pinscher Dog)

  • 5개월간의 일시적인 기절현상(syncope)을 주증상으로 내원한 9개월령의 1.97kg 미니어쳐 핀셔견에서 호흡곤란, 심초음파에서의 현저한 심실/심방의 확장, 전반적인 심비대, 약한 맥박, 폐수종 등 DCM의 진단 지표를 기준을 통해 확장성 심근병을 진단하였다. 본 증례에서 심초음파 검사는 심장의 확장과 심무력을 유발하는 다른 심장 질환을 배제하고 DCM을 확진 하는데 중요한 역할을 하였다. 치료를 위해 산소공급과, nitroglycerine patch, furosemide, pimobendan, benazepril, potassium gluconate gel을 19일간 처방한 결과 초기 내원시 보였던 대부분의 임상증상이 개선되었다.

요크셔테리어 개에서 심장초음파를 이용하여 진단된 심장내 종괴 2 증례 (Echocardiographic Diagnosis of Intracardiac Masses in Yorkshire Terrier Dogs : 2 Cases)

  • 심장에 발생하는 원발성 그리고 전이성 종양은 개에서는 흔치 않다. 우리는 본 연구에서 심장초음파적 진단을 이용하여 요크셔테리어 2마리에서 발생한 심장내 종괴를 확인하였다. 첫째 증례에서의 종괴는 상행 대동맥과 폐동맥 사이에 위치하고 있었으며, 중증도의 대동맥 역류와 좌심실의 확장을 유발하고 있었다. 이 증례의 환자등급은 ISACHC II 등급의 심부전으로 분류되었다. 환자에 대한 심장처방으로는 furosemide, enalapril, pimobendan 그리고 경구적인 화학요법으로는 lomustine가 처방되었다. 둘째 증례에서의 종괴는 좌심방의 2/3를 차지하고 있었으며, 좌심방의 확연한 확장과 심각한 이첨판 역류 (~5 m/s) 를 유발하고 있었으나, 환자의 심부전 등급은 ISACHC Ib 등급으로 심각한 울혈성 심부전은 나타나지 않았다. 종괴의 성상은 cardiac myxoma로 추정되었으나, 조직생검검사는 보호자의 거절로 인하여 수행되지 않았다. 환자에 대한 심장처방으로는 ramipril, clopidogrel 그리고 기관지 확장제는 aminophylline이 처방되었다. 본 증례의 두 환자들은 여전히 생존해 있으며, 정기적인 검진을 받고 있다.

Transient Myocardial Thickening in a 4-year-old Korean Domestic Shorthair Cat

  • A 4-year-old neutered female domestic shorthair cat weighing 5.1 kg was referred to Jeju National University Hospital with acute onset respiratory distress, weakness, and anorexia. The patient had a history of stressful antecedent events that involved bullying by a newly introduced cat. Thoracic radiography and echocardiography revealed a stage C hypertrophic cardiomyopathy phenotype based on the American College of Veterinary Internal Medicine classification system with pulmonary edema, pleural effusion, and pericardial effusion at the same time. The patient was treated with furosemide, pimobendan, and rivaroxaban. Pericardiocentesis was performed because pericardial effusion was identified. Reevaluation after 30 days revealed a normal respiratory rate on physical examination, normal cardiac shape on thoracic radiographs, and normal cardiac measurements on echocardiography. The patient was tentatively diagnosed with transient myocardial thickening (TMT) and all medications were discontinued. Six months after the initial hospitalization, the cat continued to do well without any clinical signs or left ventricular wall thickening. This case is the first report describing feline TMT in Korea. Moreover, it involves a rare case in which pulmonary edema, pleural effusion, and pericardial effusion, which induce cardiac tamponade, occurred simultaneously due to TMT-related congestive heart failure.

잡종 개에서 이첨판 폐쇄 부전증이 합병된 난원공 개존증 (Patent Foramen Ovale Complicated with Chronic Mitral Valvular Insufficiency in A Mixed Dog)

  • 12연령의 암컷 잡종개 (체중 6.2 kg)가 심한 복부 팽만, 지속적 기침 그리고 운동 불내성을 주증으로 진료 의뢰 되었다. 진단상에 복수, 경정맥 노장, 심한 좌우 심첨부 수축기 잡음 (4/6 단계), 후대정맥 확장을 동반한 전반적 심비대와 좌심방 확장이 관찰되었다. 심장 초음파에서는 퇴행성 이첨판막 그리고 삼첨판막 심내막증과 양심실성 울혈성 심부전이 관찰되었다. 좌우단락의 난원공 개존증 또한 있었다. 좌심실 수축능은 용적 과부하의 정도와 관계되어 떨어져 있었다. 진단적 결과에 기초하여, 본 증례는 ISACHC IIIa 심부전의 이첨판막과 삼첨판막 심내막증이 합병된 난원공 개존증으로 진단되었다. 이 개는 푸로세마이드 (2 mg/kg, q12hr, PO), 피모벤단 (0.3 mg/kg, q12hr, PO)와 스피로노락톤 (1 mg/kg, q12hr)으로 치료되었다. 임상 증상들은 약물 치료 후에 점차적으로 개선되었다.

Long Term Management of Congenital Ventricular Tachyarrhythmia in a Dog

  • A four month old, intact female Jindo dog presented to the Veterinary Teaching Hospital of Kangwon National University with collapse. Physical examination revealed a heart rate of more than 200 beats per minute. Blood tests showed mild anemia and mild neutrophilia, while thoracic radiography and ultrasonography revealed no remarkable findings. Electrocardiography showed ventricular premature complexes (VPCs). The dog was diagnosed with congenital ventricular tachyarrhythmia. The condition was improved by lidocaine infusion. After 10 days, the dog was discharged from the hospital with a prescription of atenolol, pimobendan, diltiazem, furosemide, spironolactone, and L-carnitine. This dog is still alive after 31 months. However, progressive cardiac remodeling was confirmed on radiography and ultrasonography. Congenital ventricular tachyarrhythmia is rare in dogs, and the prognosis of reported cases is poor. This report describes the long-term successful management of a dog with congenital ventricular tachyarrhythmia.

Management of Pulmonary Hypertension Due to Brachycephalic Obstructive Airway Syndrome in a Dog

  • A 15-year-old, neutered male, Shih-Tzu, was presented at the Chonnam National Veterinary Medical Teaching Hospital for evaluation of acute onset of persistent coughing, exercise intolerance, and abnormal heart sound. On thoracic auscultation, a split-second heart sound and a wheezing sound were detected on both sides of the chest walls. On physical examination, the dog's body condition score (BCS) was 7/9, and had stenotic nares. Thoracic radiographs revealed right-sided enlargement of the cardiac silhouette (vertebral heart score (VHS) 11.2; reference interval = 8.9-10.1), mild main pulmonary artery (MPA) bulging, mild interstitial infiltration, and hepatomegaly. The electrocardiogram showed right axis deviation, suggesting right ventricular hypertrophy. The echocardiographic study showed moderate pulmonary hypertension and moderate tricuspid regurgitation. There were no findings of a tracheobronchial disease, pulmonary thromboembolism, congenital shunt, left heart disease, or parasitic disease. Based on clinical signs and diagnostic findings, the dog was diagnosed with pulmonary hypertension secondary to brachycephalic syndrome. To rectify respiratory exacerbating factors, the dog was recommended weight control by restricting dietary intake and managing concurrent Cushing's syndrome. Treatments included sildenafil, pimobendan, furosemide, and ramipril. After five months of taking medications and weight control, the severity of pulmonary hypertension improved from moderate to mild. The clinical signs of the patient, including coughing and exercise intolerance, improved a lot. For 5 months of follow-up, the patient has not reported further recurrence of respiratory distress.

Double-Chambered Right Ventricle in an Old Standard Poodle Dog

  • A 12-year-old Standard Poodle presented with intermittent weakness and occasional dyspnea at the Veterinary Medicine Teaching Hospital of Kangwon National University. A grade of 4 out of 6 systolic murmur with an irregular tachycardic rhythm was auscultated on both sides of the chest. Systolic blood pressure was 140 mmHg. Panting was noticed in the hospital, but there was no crackle sound. Blood analysis revealed mild increases in liver panel levels (alanine aminotransferase 149 [reference interval, 19-70] U/L; and alkaline phosphatase, 185 [reference interval, 15-127] U/L) and severe increases in cardiac biomarker levels (n-terminal pro-brain natriuretic peptide, 4169 [reference interval, 50-900] pmol/L; and cardiac troponin I, 0.22 [reference interval, 0.03-0.12] ng/mL). On electrocardiography, irregularly irregular supraventricular tachycardic rhythm with an f-wave and no distinct p-wave was observed. Generalized cardiomegaly with an enlarged right atrium and left ventricle was confirmed on thoracic radiography. Moreover, hepatomegaly and an enlarged caudal vena cava were observed. Echocardiographic evaluation revealed a fibromuscular diaphragm in the right ventricle. Because of the obstructive lesion in the right ventricle, the right atrium and ventricle were enlarged (right atrial area index, 38.82 cm2/m2 [reference interval, 4.2-10.2 cm2/m2]; right ventricle end-diastolic area index, 14.152 cm2/m2 [reference interval, 4.9-10.92 cm2/m2]). Accordingly, the patient was diagnosed with double-chambered right ventricle (DCRV). Pimobendan, furosemide, enalapril, diltiazem, and S-adenosylmethionine (SAMe) were prescribed, and all symptoms were relieved. DCRV is a right-sided congenital heart defect resembling pulmonic valve stenosis. If symptoms are not severe, medical therapy can be facilitated without surgery or the balloon dilation.
