RELATIONSHIP BETWEEN BISPECTRAL INDEX (BIS), SEDATION SCORE AND PLASMA CONCENTRATION FOR EVALUATION OF MONITORING EFFECT OF BISPECTRAL INDEX IN MIDAZOLAM SEDATION (Midazolam 진정요법 시 Bispectral index (BIS) Monitoring 효과의 평가를 위한 혈장농도, 진정지수(Sedation Score) 사이의 상관 관계 분석)

  • Purpose : We examined the relationship between BIS, sedation score and plasma midazolam concentration to verify the usefulness of BIS to assess the patient's consciousness during sedation. Patients and Methods : Twenty-five young, healthy adult volunteers participated in this clinical study. Midazolam was administered intravenously up to 0.08 mg/kg to induce unconsciousness and we monitored the patient's physiological and conscious status until complete recovery from sedation. BIS and sedation score were measured before sedation, 10, 20, 30 minutes after midazolam administration. Plasma midazolam concentration was measured 10 minutes after midazolam administration. BIS was measured using A-2000 BISTM monitor (Aspect Medical Systems, USA) and the degree of sedation was evaluated with the sedation score. Results : The BIS score correlated with the sedation score (r = 0.676; P < 0.05). With the decreased plasma midazolam concentration, the correlation was better with sedation score (r = -0.656). Although BIS values did not correlate with calculated plasma concentration of midazolam (r = 0.467) at 10 minutes after midazolam administration, values after sedation were well distinguished from those before sedation. Conclusions : BIS is known for an effective predictor of patient's hypnotic state, and it is correlated with the sedation score. But, it doesn't always coincide with the clinical parameters of depth of sedation. So more attention is needed using BIS only during sedation, and it is advisable that the patient's consciousness is monitored with variable sedation score systems every several minutes.

  • Current status on consumption and packaging of sweet persimmons in USA was surveyed by visiting the market, and three sea shipments were conducted as model export trial from Korean to USA with measurement in ship container temperature and fruit quality. Strategy to promote the export of Korean persimmons was derived. There have been gradual decrease in the price of persimmons due to their increases production, but there seems potential growth in consumption of the fruits from Asian, Hispanic and American people. Compared to the fruits from other countries, Korean persimmons are desired to have higher soluble solid content with stronger red color, but too large sizes are not favored in American market. There has often been temperature fluctuation in shipment container during the sea transport to USA, resulting in surface blackening, skin black spotting and flesh softening. Plastic bag packages with inappropriate unitizing crimping were found to sometimes build up unproper modified atmosphere (high $O_2$ and low $CO_2$) giving high rate of physiological injury.

Comparative Genome-Scale Expression Analysis of Growth Phase-dependent Genes in Wild Type and rpoS Mutant of Escherichia coli

  • Numerous genes of Escherichia coli have been shown to growth phase-dependent expression throughout growth. The global patterns of growth phase-dependent gene expression of E. coli throughout growth using oligonucleotide microarrays containing a nearly complete set of 4,289 annotated open reading frames. To determine the change of gene expression throughout growth, we compared RNAs taken from timecourses with common reference RNA, which is combined with equal amount of RNA pooled from each time point. The hierarchical clustering of the conditions in accordance with timecourse expression revealed that growth phases were clustered into four classes, consistent with known physiological growth status. We analyzed the differences of expression levels at genome level in both exponential and stationary growth phase cultures. Statistical analysis showed that 213 genes are shown to, growth phase-dependent expression. We also analyzed the expression of 256 known operons and 208 regulatory genes. To assess the global impact of RpoS, we identified 193 genes coregulated with rpoS and their expression levels were examined in the isogenic rpoS mutant. The results revealed that 99 of 193 were novel RpoS-dependent stationary phase-induced genes and the majority of those are functionally unknown. Our data provide that global changes and adjustments of gene expression are coordinately regulated by growth transition in E. coli.

Analysis of Researches on Nursing Intervention for Elderly Health Promotion from 1994 to 2004 (국내 노인 건강증진 간호중재연구 분석 - 1994년에서 2004년까지 -)

  • Purpose: The purpose of this study was to analyse researches on nursing intervention for elderly health promotion. Method: A literature review of 38 researches was carried out using guidelines developed by the present researchers. Result: The research papers studied in this research were theses written for doctoral or master's degree and published between 1994 to 2004. These researches applied nursing intervention for health promotion of elders and used experimental study design. The average period of nursing intervention was about 9 weeks and the average frequency was three times per week. As for major characteristics of subject groups, most of them were over 65 years old (76.3%) and resided in community (65.8%), and each group consisted of 10 to 29 elders. The dependent variables used in these studies included physiological variables (blood pressure, blood cholesterol level, pulmonary function, fasting blood sugar, blood cortisol level, body mass etc.), psychologic variables (depression, quality of life, life satisfaction, loneliness, anxiety etc.), cognitive variables (perceived benefits, perceived barriers, self-efficacy, self-concept, subjective health status etc.), activities of daily livings, health promotion behavior, pain, risks of fall, and variables related to Trans Theoretical Model. The majority of these studies (78.9%) applied exercise programs for health promotion including exercise motivating programs (15.8%). Conclusion: We suggest that more various nursing interventions must be applied to promote elders' health and to take care of their chronic diseases.

Factors Affecting Subjective Health Status in Middle-aged Adults with Hypertension (고혈압이 있는 중년의 주관적 건강상태에 대한 영향요인)

  • This study was conducted to identify factors that affect subjective health in middle-aged with hypertension. Hypertension, a representative chronic disease, is a disease of increasing prevalence from middle age. To effectively manage hypertension from middle age, it is necessary to revise lifestyle based on subjective health awareness. The subjects of this study were 2,254 middle-age with hypertension who were 30-64 years of age among the 2014-17 national health and nutrition survey. Study variables consisted of general, physical, and psychological factors. For data analysis, compound statistics were made using SPSS 25.0 program and then complex statistics. The results of this study showed that the factors that significantly influenced the subjective health of the subjects were sex, age, economic level, number of households, home ownership, stress, quality of life, and their explanatory power was 20.4% (F = 12.58, p<.001). The results of this study can be used as a basis for the intervention to improve the subjective health of middle-aged with hypertension. Appropriate lifestyle modifications are needed to effectively manage hypertension based on good subjective health awareness.

Smart Elderly-care System using Smart-phone (스마트폰을 이용한 고령자용 스마트 간병 시스템)

  • In this paper, we propose a system to monitor the condition of elderly people who are uncomfortable by using smart-phone and biometric sensor at any time and for care-givers to provide with the best medical service anytime and anywhere. The proposed system monitors the status of the elderly through various bionic sensors installed in the hospital ward based on the Arduino system and not only provides the physiological and medical services required by the elderly, but also informs the guardian so that he can cope with the emergency. In conclusion, this paper suggests that a reading light used by elderly people can operate as a home server with a biosensor using Arduino and Android applications (Apps.), and the smart-phone of care-givers is configured as a remote management and a emergency call system. Therefore, this study suggests important ways to improve the satisfaction of medical service for the both elderly people and care-givers of chronic diseases in the future.

'Healthy Japan 21' : A New Perspective on Health Promotion Policy for Japan in the 21st Century

  • 'Healthy Japan 21' is a new health policy that has been proposed for the 21st century: it embodies a totally new concept for its viewpoints and methods. To start with, for its goal, the focus is placed on the 'quality of life' or a life that is free of diseases, rather than mere prolongation of life. For its doctrine, the emphasis has shifted dramatically from improving the health of the entire population(the traditional approach for health improvement) to 'achieving an ideal health status for each individual. The ultimate aged society that arrives first in Japan is a society in its ultimate form for human being. Why did Japan become westernized, giving up her traditional culture? Why did she go through industrialization, sacrificing her nature? And why does she try so hard to industrialize the developing countries? These efforts are all preparation for the arrival of a ultimate aged society. During the 20th century, we believed in unlimited possibilities and expanded our social frontier. In the 21st century, on the other hand, a super-aged society(the ultimate society), a glimpse of which we have witnessed from time to time, will descend on us sooner or later. It is expected to arrive first in Japan. 'Healthy Japan 21' is intended to prepare for the arrival of the hitherto unheard of super-aged society by building the physiological basis of people. This policy is social experimentation on an immense social scale, in which questions are posed on the understanding of health, the relationship between individuals and society, the relationship between administration and citizens, the manner by which central and local governments operate, and the new relationship between prevention and therapy, 'Healthy Japan 21' may be summarized as an experiment on a huge scale directed to the ultimate form of human society, in which Japan and each of her citizens play a role and set an example for the rest of the world. Even just by considering various approaches newly suggested for this venture, one may be convinced that it is a policy with features suitable for a country that has already achieved the world's highest longevity.

The Relationship between of Calcium Intake and BMD in Middle-aged Women (중년여성의 칼슘섭취 수준과 골밀도와의 관계)

  • Purpose: This study was to determine the effects of calcium intake on bone mineral density (BMD) in pre and post menopause women and to provide basic data for enhancing bone health of middle aged women. Methods: A total of 700 middle-aged women living in Seoul and Geonggi Province were interviewed during the period from June 2003 through January 2004 to investigate their social. demographic and physiological characteristics, health and daily activity performance, and their dietary patterns, and bone mineral density was measured. The survey of dietary intake was 24 hour recalls, and the individual calcium intake was calculated using food frequency. Data of 618 subjects was used for the analysis. Of the calcium intake levels, BMD values of the subjects of 20% of high level. 60% of middle level and 20% of low level were analyzed and compared. Results: The level of calcium intake according to general characteristics of the subjects was significantly related to age (p=0.001), education levels (p=.003) and marital status (p=.001). The BMD of the lumbar vertebrae and femur of the subjects taking a high level of calcium showed significantly higher than that of the subjects taking a middle level and low level of calcium. Femoral T-score was also significantly higher in subjects taking a high level of calcium than that of those taking a middle level and lower levels of calcium. Lumber spine T-score was higher in the high level group than that of the middle level group. Conclusion: The study revealed that women taking a high level of calcium had better bone health. Therefore. calcium intake is extremely important in daily dietary intake so that the intake of calcium-rich foods is highly recommended.

Study on the Classificaition of Shoulder-Arm Pain in the Pre-Studies on Clinical Treatment of Shoulder-Arm Pain (견비통 치료 관련 선행연구에서 견비통의 유형 분류에 관한 연구)

  • To have effective treatment for shoulder arm pain, we searched the cause, symptom, etiology, classification of the pain areas, acupuncture points, and muscles along the meridians, and acquired the following results. Shoulder-pain is mainly divided into the malfunction of viscera and entrails, damage due to the weakness of essence and qi, abnormal status of muscle function, change of joints, disease in the nerve and vessel, and the internal injury due to seven modes of emotions. Pain of shoulder joints are pain in the local area of shoulder joints, referred pain of shoulder, neck, and shoulder-arm, numbnes and swelling of muscle, and muslce weakness. Shoulder-arm pain is classified as four types of pain: shoulder-joint pain, shoulder-back pain, shoulder-chest pain, and shoulder-arm-elbow pain. And shoulder-arm-elbow pain is again divided into the shoulder-blade pain, shoulder-arm pain, shoulder-elbow pain. The related meridians on shoulder pain are the three yin meridians of hand, Kidney Meridian, Conception Meridian, three yang meridians of hand, Bladder Meridian, Governor Meridian Acupuncture points for shoulder pain are in the acupuncture points of the 10 meridians and a-shi points. Thre related meridian muscles on shoulder-pain are the three yin and yang meridians of hand, and their related muscles are the ones that are connected with the front, back, and chest side muscles of shoulder joints, and the ones that are connected with the front and back side muscles of arm.

A Research for Removing ECG Noise and Transmitting 1-channel of 3-axis Accelerometer Signal in Wearable Sensor Node Based on WSN (무선센서네트워크 기반의 웨어러블 센서노드에서 3축 가속도 신호의 단채널 전송과 심전도 노이즈 제거에 대한 연구)

  • Wireless sensor network(WSN) has the potential to greatly effect many aspects of u-healthcare. By outfitting the potential with WSN, wearable sensor node can collects real-time data on physiological status and transmits through base station to server PC. However, there is a significant gap between WSN and healthcare. WSN has the limited resource about computing capability and data transmission according to bio-sensor sampling rates and channels to apply healthcare system. If a wearable node transmits ECG and accelerometer data of 4 channel sampled at 100 Hz, these data may occur high loss packets for transmitting human activity and ECG to server PC. Therefore current wearable sensor nodes have to solve above mentioned problems to be suited for u-healthcare system. Most WSN based activity and ECG monitoring system have been implemented some algorithms which are applied for signal vector magnitude(SVM) algorithm and ECG noise algorithm in server PC. In this paper, A wearable sensor node using integrated ECG and 3-axial accelerometer based on wireless sensor network is designed and developed. It can form multi-hop network with relay nodes to extend network range in WSN. Our wearable nodes can transmit 1-channel activity data processed activity classification data vector using SVM algorithm to 3-channel accelerometer data. ECG signals are contaminated with high frequency noise such as power line interference and muscle artifact. Our wearable sensor nodes can remove high frequency noise to clear original ECG signal for healthcare monitoring.
