Relationship Between Perceived Risk and Physician Recommendation and Repeat Mammography in the Female Population in Tehran, Iran

  • Iranian women are at high risk of low compliance with repeat mammography due to a lack of awareness about breast cancer, negative previous experiences, cultural beliefs, and no regular visits to a physician. Thus research is needed to explore factors associated with repeated mammography participation. Applying the concept of perceived risk as the guiding model, this study aimed to test the fit and strength of the relationship between perceived risk and physician recommendation in explaining repeat mammography. A total of 601 women, aged 50 years and older referred to mammography centers in region 6, were recruited via a convenience sampling method. Using path analysis, family history of breast cancer and other types of cancer were modeled as antecedent perceived risk, and physician recommendation and knowledge were modeled as an antecedent of the number of mammography visits. The model explained 49% of the variance in repeat mammography. The two factors of physician recommendation and breast self-examination had significant direct effects (P < 0.05) on repeat mammography. Perceived risk, knowledge, and family history of breast cancer had significant indirect effects on repeat mammography through physician recommendation. The results of this study provide a background for further research and interventions not only on Iranian women but also on similar cultural groups and immigrants who have been neglected to date in the mammography literature.

Prediction of the Type of Delivery using Fuzzy Inference System

  • In this paper a new fuzzy prediction is designed and developed to predict the type of delivery based on 7 factors. The developed system is highly needed to give a recommendation to the family excepting baby and at the same time provide an advisory system to the physician. The system has been developed using MATLAB and has been tested and verified using real data. The system shows high accuracy 95%. The results has been also checked one by one by a physician. The system shows perfect matching with the decision of the physician.

미국 캘리포니아주에 거주하는 한인여성들의 자궁경부암 수검권고에 영향을 미치는 보건의료 접근성 및 보건의료인의 특성 분석 (Effect of healthcare access and healthcare provider status on recommendation for Pap test among Korean American women in Alameda and Santa Clam Counties, California)

  • 연구목적: 3년마다 정기적인 자궁경부암 조기검진을 받도록 권고하고 있는 수검권고안은 검진행동 유발요인으로써 자궁경부암 검진참여의 가능성을 증진시키는 주요 영향요인으로 작용하고 있다. 본 연구는 보건의료 접근성과 보건의료인의 특성이 자궁경부암 조기검진 수검권고에 미치는 영향을 살펴보고자 시도되었다. 연구방법: 본 연구에서 재분석된 한인여성 건강조사자료는 미국 캘리포니아주 알라메다와 산타클라라 카운티에 거주하는 한인여성 865명을 대상으로 2002년에 전화설문조사로 수행되었다. 수검권고안에 영향을 미치는 주요 예측인자를 살펴보기 위해 로지스틱 회귀분석을 수행하였다. 연구결과: 조사대상자 중 37.9%만이 지난 3년간 자궁경부암 조기검진 참여에 관한 수검권고를 받은 것으로 나타났다. 수검권고의 주요 예측인자는 건강보험 가입여부, 지난 1년간의 의사방문횟수, 정기적으로 방문하는 보건의료인의 특성인 것으로 나타났다. 또한 보건의료 접근성에서는 공공의료보험 또는 사보험 여부와 관계없이 건강보험에 가입한 여성들이 수검권고를 더 많이 받은 것으로 나타났다. 특히 보건의료인의 특성 중 인종과의 관련성을 살펴보면, 한인이 아닌 타 인종의 여성 의사로부터 가장 많이 검진참여에 관한 수검권고를 받았고(OR=6.21, 95% CI=2.63, 14.66), 다음으로 한인 남성 의사(OR=2.19, 95% CI=1.30, 3.68), 비한인 남성 의사로부터 수검권고를많이 받았다(OR=2.07, 95% CI=1.15, 3.71). 결론: 캘리포니아주의 2개 카운티에서 거주하는 한인여성을 대상으로 보건의료 접근성과 보건의료인의 특성이 자궁경부암 조기검진 수검권고에 미치는 영향을 분석한 결과, 한인여성들의 건강보험 가입여부 및 의사방문 수준, 보건의료인의 인종이 수검권고와 관련이 있는 것으로 나타났다. 이는 한인여성들의 자궁경부암 조기검진 수검률 향상 및 자궁경부암 이환률과 사망률을 감소시키기 위한 수행전략 개발에 중요한 기초자료로 활용될 수 있다.

농촌지역주민의 암 조기검진과 관련 요인에 관한 연구 (Cancer Screening Rate and Related Factors in Rural Area)

  • Objectives : Cancer is the second most frequent cause of death in Korea. Cancer screening tests can save lives through early detection. Enhancing the cancer screening rate is an important strategy for reducing cancer mortality. The purpose of our study was to evaluate the screening rate and related factors in a rural area. The study investigated relationships between sociodemographic characteristics, several preventive behaviors, and the experience of several cancer screening behaviors. Materials and Methods : The study population was recruited voluntarily from the three rural areas(Myen) in Chungju city. The participants completed structured questionnaire from July 21, 1990 to July 26, 1998. Results : The proportions of the study population who had previously received stomach, liver, breast, or cervix cancer screening tests were 24.5%, 18.5%, 27.0%, 59.2% respectively. The 1-year screening rates of stomach, liver, breast, and cervix cancer were 7.4%, 6.8%, 8.6%, 15.6% respectively. In multivariate logistic analysis, some sociodemographic variables, preventive behaviors, or psychological variables were significantly associated with several cancer screening tests. Those who had previously received a stomach cancer screening test were significantly associated with the presence of chronic disease, physician's recommendation, use of alcohol family history of cancer, or previous liver cancer screening test. Those who had previously received a liver cancer screening test were associated with education level, physician's recommendation and previous stomach cancer screening test. Those who had received a cervix cancer screening test were significantly associated with education level, presence of a transportation vehicle, physician's recommendation use of alcohol and previous breast cancer screening test. And those who had received a previous breast cancer screening test were significantly associated with age, marital status, and earlier cervix cancer screening test. Conclusion : Based on the results of this study a strategy to promote cancer screening and health objectives at the district level can be made.

Vietnamese Health Care Providers' Preferences Regarding Recommendation of HPV Vaccines

  • Physician recommendation is an important predictor of HPV vaccine acceptance; however, physician willingness and preferences regarding HPV vaccination may be influenced by factors including patient age, vaccine type, and cost. A cross-sectional survey was administered to a convenience sample of health care providers in Da Nang, Vietnam, to evaluate awareness, perceptions about HPV and HPV vaccines, and willingness to vaccinate a female patient. Willingness to vaccinate was evaluated using a full-factorial presentation of scenarios featuring the following factors: vaccine cost (free vs 1,000,000 VND), patient age (12, 16, or 22 years), and HPV vaccine type (bivalent vs quadrivalent). Responses from 244 providers were analyzed; providers had a mean age of $34{\pm}11.9$ years; a majority were female, married, and had children of their own. Thirty-six percent specialized in obstetrics/gynecology and 24% were providers in family medicine. Of the three factors considered in conjoint analysis, vaccine cost was the most important factor in willingness to vaccinate, followed by patient age, and vaccine type. The most favorable scenario for vaccinating a female patient was when the vaccine was free, the patient was 22 years of age, and the HPV4 vaccine was described. In multivariable analysis, older age, being a physician, being married, and having children were all associated with increased willingness to recommend HPV vaccination (p<0.05). Provider willingness is an important aspect of successful HPV vaccination programs; identifying preferences and biases in recommendation patterns will highlight potential areas for education and intervention.

  • Purpose: The purpose of this study was to investigate the effect of Situation-Background-Assessment-Recommendation (SBAR) reports on communication clarity and nurse-physician collaborative relationships. Methods: SBAR forms in one group were measured at baseline, at 2 weeks, and at 4 weeks postintervention. The subjects were 30 nurses who worked in a general ward of a general hospital in B city, Korea. A 45-minute SBAR education was provided to all subjects. Data was collected between September to October 2015. The collected data was analyzed using a repeated measure ANOVA. Results: After SBAR reports, levels of nurse's communication clarity and nurse-physician collaborative relationships were significantly higher than levels before SBAR reports. Conclusion: SBAR reports improved the communication clarity and nurse-physician collaborative relationships. It is recommended to use a lot of SBAR in clinical practice.

데이터 마이닝을 활용한 병원 재방문도 영향요인 분석 : 외래환자의 만족도를 중심으로 (On the Determination of Outpatient's Revisit using Data Mining)

  • Patient revisit to used hospital is a key factor in determining a health care organization's competitive advantage and survival. This article examines the relationship between customer's satisfaction and his/her revisit associated with three different methods which are the Chi Square Automatic Interaction Detection(CHAID) for segmenting the outpatient group, logistic regression and neural networks for addressing the outpatient's revisit. The main findings indicate that the important factors on outpatient's revisit are physician's kindness, nurse's skill, overall level of satisfaction, hospital reputation, recommendation, level of diagnoses and outpatient's age. Among these ones, physician's kindness is the most important factor as guidelines for decision of their revisit. The decision maker of hospital should select the strategy containing the variable amount of the level of revisit and size of outpatient's group under the constraint on the hospital's time, budget and manpower given. Finally, this study shows that neural networks, as non-parametric technique, appear to more correctly predict revisit than does logistic regression as a parametric estimation technique.

Increase in Potential Low-value Magnetic Resonance Imaging Utilization Due to Out-of-pocket Payment Reduction Across Income Groups in Korea: An Experimental Vignette Study

  • Objectives: This study examined the effect of out-of-pocket (OOP) payment reduction on the potential utilization of low-value magnetic resonance imaging (MRI) across income groups. Methods: We conducted an experimental vignette survey using a proportional quota-based sample of individuals in Korea (n=1229). In two hypothetical vignettes, participants were asked whether they would be willing to use MRI if they had uncomplicated headache and non-specific low back pain, each before and after OOP payment reduction. To account for the possible role of physician inducement, half of the participants were initially presented with vignettes that included a physician recommendation for low-value care. The predicted probability, slope index of inequality (SII), and relative index of inequality (RII) were calculated using logistic regression. Results: Before OOP payment reduction, the lowest income quintile was least likely to use low-value MRI regardless of physician inducement (36.7-49.6% for low back pain; 30.5-39.3% for headache). After OOP payment reduction, almost all individuals in each income quintile were willing to use low-value MRI (89.8-98.0% for low back pain; 78.1-90.3% for headache). Absolute and relative inequalities concerning potential low-value MRI utilization decreased after OOP payments were reduced, even without physician inducement (SII: from 8.15 to 5.37%, RII: from 1.20 to 1.06 for low back pain; SII: from 6.99 to 0.83%, RII: from 1.20 to 1.01 for headache). Conclusions: OOP payment reduction for MRI has the potential to increase low-value care utilization among all income groups while decreasing inequality in low-value care utilization.

Micronutrients as Supportive Care for Gastrointestinal Cancer Patients; Benefits and Concerns

  • Cancer is the first leading cause of death in Korea and the second leading cause of death in the USA. There is extensive research into prevention of cancer and the support of oncology patients with diet or dietary supplements. In vitro and in vivo animal studies have indicated that antioxidants, including beta-carotene, alpha-tocopherol, and ascorbic acid, can yield anti-cancer effects in addition to providing protection against oxidative damage. Although many observational studies have shown that consuming fruits and vegetables can reduce the risk of some cancers, the results of several large-scale human intervention trials testing the benefits of a single or combined higher-dose of individual micronutrients have been inconsistent. Cancer can cause profound metabolic and physiological changes which may affect patients' nutrient requirements. Although the optimal route of nutrient delivery is through diet, cancer patients often suffer symptoms that disrupt their food intake, including anorexia, premature satiety, altered taste and smell, and changes in bowel mobility. In particular, micronutrient deficits can slow postoperative healing, contribute to depression symptoms, and decrease immune competence. Cancer patients are generally motivated to take dietary supplements to improve responses to treatment and quality of life. The Physician's Health Study II (PHS II) randomized controlled trial reported recently that daily multivitamin supplementation significantly, albeit modestly, reduced the risk of total cancer. Although evidence of multivitamin use benefits is limited in cancer patients, taking dietary supplements with constituents in the range of the recommended daily allowance according to the Dietary Reference Intake (DRI) recommendation is generally considered to be safe.

  • 본 논문은 가정에 있는 만성질환자, 퇴원한 환자 및 자신의 건강을 염려하는 정상인 등을 대상으로 매일 측정한 심전도. 혈압. 혈중 산소 포화농도 등과 같은 생리변수와 건강 설문에 대한 응답을 분석하여 건강상태를 지속적으로 파악하고, 비정상적인 상태가 발견될 경우에는 의사가 정확히 확인하여 필요한 조치를 조언하는 재택건강관리서비스에 대해서 기술하고 있다 재택건강관리서비스를 위해서 가입자는 재택건강관리단말기와 인터넷에 연결된 PC를 가정에 구비하여야 한다. 관제센터는 의사와 가입자의 기본정보와 가입자의 건강정보를 저장하기 위한 데이터베이스 시스템, 생체신호와 건강설문을 분석하여 현재상태의 비정상여부를 판단하는 건강상태자동평가시스템, 가입자와 의사들이 웹 브라우저를 사용하여 원하는 건강정보를 데이터베이스에서 검색, 조회하고. 그 내용을 수정. 편집하여 저장할 수 있는 웹 기반 건강정보관리시스템이 필요하다. 또한, 공중전화망 및 무선통신망을 이용한 음성 및 문자 전송과 인터넷을 이용한 전자우편에 의해 의사의 소견을 가입자에게 전달하는 통합 메시징 시스템 (UMS). 종합검진센터에서 의사가 검사결과와 문진 결과를 입력하기 위한 정보입력 PC. 병원에서 의사가 가입자의 정보를 조회하거나 정밀진단결과를 입력하기 위한 인터넷 PC 등이 설치되어야 한다. 일반에게 이러한 서비스를 널리 보급하기 위해서는, 생리변수들의 무구속 및 무자각 측정기술과 지능적인 건강평가 알고리즘의 개발에 대한 연구가 계속 수행되어야 할 것이다.
