Background: Although postural instability is one of the major symptoms of Parkinson's disease (PD), dopaminergic treatment is ineffective for treating postural instability. Recent reports have shown that somatosensory deficit is associated with postural instability, and that somatosensory input improved postural instability. The purpose of this study is to evaluate the effects of lateral wedges for quiet standing postural control in people with PD. Methods: Twenty-two patients who were diagnosed with PD were enrolled in this study. The participants stood on a force plate under two conditions (wedge and no wedge) with or without having their eyes open or closed. The center of pressure (COP) range and velocity were analyzed using a two-way repeated-measures analysis of variance. Results: The range and velocity of COP in the anterioposterior and mediolateral (ML) directions were significantly improved after the patients stood on the lateral wedge with their eyes closed (p < 0.05). The range in ML direction and velocity in both directions of COP were significantly decreased when their eyes were open (p < 0.05). Conclusions: Regardless of vision, standing on lateral wedges improved postural sway in people with PD.

The purpose are to assess the efficacy of a contact lateral-wedged insole with strapping on the femorotibial angle in patients with varus deformity osteoarthritis of the knee after treatment period. 25 outpatients with knee osteoarthritis (OA) were randomized to be treated with a contact lateral-wedge insole with strapping. Standing radiographs were used to analyze the femorotibial angle for each subject. The result of repeated two-way ANOVA's reveled that a contact lateral-wedged insole with strapping produced significantly differences in the femorotibial angle between groups after treatment period (P<0.05). and repeated one-way ANOVA's reveled that it produced significantly differences in the femorotibial angle between experimental groups after treatment period (P<0.05). We suggest that these results may be beneficial for manufacturing foot orthotic devices, such as wedged insoles, to control medial and later compartment forces in the knee varus-valgus deformity.
In the radiation therapy for breast cancer patients, wedge shaped compensators are essentially used to achieve appropriate dose distribution because of thickness difference according to breast shapes. Tangential Irradiation technique has usually been applied to radiation therapy for breast cancer patients treated with breast conservative surgery. When a primary beam is incident on wedge shaped compensators from medial direction In tangential irradiation technique, low energy scattered radiation is generated and gives additional dose to the breast surface. As a method to reduced additional dose to breast surface, the use of virtual wedge shaped compensator is possible. Eclipse radiation treatment planning (RTP) systems Installed at our institution have virtual wedge shaped compensator for radiation therapy treatment planning. The dose distributions of 15, 30, 45, 60 degree physical wedges and virtual wedges were measured and compared. Results showed that there was no significant differences In symmetry of $10{\times}10$ field among various wedge angles. When the transmission factor was compared, transmission factor Increased linearly as the wedge angle Increased. These results Indicates that the appilcation of virtual wedge in clinical use is appropriate.
The purpose of this study is to confirm the effect of reducing the surface dose around the radiation field in breast cancer radiotherapy using the Field-in-Field (FIF) technique. X-ray was exposed from a linear accelerator (Linac) was used for irradiation, and the surface dose was measured with a glass dosimeter. The source-to-surface distance (SSD) was 90 cm, the field size is 10 × 10 cm2, and the X-ray energy was 6 MV and 10 MV, respectively. The surface dose of the FIF was compared with the dose measured in the physical wedge (PW) and dynamic wedge (DW). Wedge angles of 15° and 30° were used in the PW and DW, respectively. Surface dose was measured at 1 cm, 3 cm, and 5 cm from the center of the field size, respectively. According to the results, FIF showed lower surface dose compared to PW and DW regardless of the energy of the X-ray beam, wedge angle, and dose measurement point. Since FIF could reduce the radiation dose in periphery of the field size in breast cancer treatment, it is expected to be able to reduce the secondary damage caused by the radiation beam as well as to obtain a uniform dose distribution on the target.

A complete form of physical optics solution to the E-polarized diffraction by a composite of conducting and dielectric wedges is obtained by sum ov geometrical optics solution and edge-diffracted field. The diffraction coefficients of the edge-diffracted field are expressed in series of cotangent functions. The electric field patterns of the physical optics solution are plotted in figures.

The nonlinear forward diffraction of a modulated wave train by a thin wedge has been studied analytically. Since the physical variables involved in the problem have vastly different scales, the multiple scale expansion method has been used to obtain an approximate solution. To simplify the problem. the wedge is assumed to be thin and the parabolic approximation is utilized. The wave evolution can be described by a kind of the cubic Schrodinger equation. which consists of the linear time evolution. the lateral dispersion and the nonlinearity. Numerical results indicate that the nonlinearity. which it defined by the ratio of the ratio of the incident wave to the wedge angle. governs the amplitude and the stability of diffracted waves. The instability of dirffracted waves becomes more pronounced as the nonlinearity increases and the modulation ratio decreases. It is also found that the stem waves. initially developed along the wedge. can not sustain for a long time.

In order to evaluate the radio-protective advantage of an enhanced dynamic wedge (EDW) over a physical wedge (PW), we measured peripheral doses scattered from both types of wedges using a 2D array of ion-chambers. A 2D array of ion-chambers was used for this purpose. In order to confirm the accuracy of the device we first compared measured profiles of open fields with the profiles calculated by our commissioned treatment planning system. Then, we measured peripheral doses for the wedge angles of $15^{\circ},\;30^{\circ},\;45^{\circ},\;and\;60^{\circ}$ at source to surface distances (SSD) of 80 cm and 90 cm. The measured points were located at 0.5 cm depth from 1 cm to 5 cm outside of the field edge. In addition, the measurements were repeated by using thermoluminescence dosimeters (TLD). The peripheral doses of EDW were (1.4% to 11.9%) lower than those of PW (2.5% to 12.4%). At 15 MV energy, the average peripheral doses of both wedges were 2.9% higher than those at 6MV energy. At a small SSD (80 cm vs. 90 cm), peripheral dose differences were more recognizable. The average peripheral doses to the heel direction were 0.9% lower than those to the toe direction. The results from the TLD measurements confirmed these findings with similar tendency. Dynamic wedges can reduce unnecessary scattered doses to normal tissues outside of the field edge in many clinical situations. Such an advantage is more profound in the treatment of steeper wedge angles, and shorter SSD.
Purpose: The purpose of this study was to investigate the effect of assistance tools such as gym balls, wedges, and straps on lower extremity muscle activity and the increase in the range of motion made possible by the use of these tools. The subjects were divided into two groups: a group capable of deep-squatting (PS) and the second finding it impossible or having difficulty in performing such squats (IS). Methods: Twenty-three subjects participated in this study. Surface electromyography was used to measure the muscle activation of the rectus femoris (RF), vastus medialis (VM), and tibialis anterior (TA) muscles during deep squats, normal squats (NS), gym ball squats (GS), wedge squats (WS), and strap squats (SS). A motion analysis system was used to measure the range of motion of the knee joint during each of these exercises. Results: There was a significant difference in the RF muscle activity between the possible squat (PS) and the impossible squat (IS) groups in the GS, and there were significant differences in the RF and TA muscle activity between the groups in the WS. Both the PS group and the IS group showed a significant difference in the TA muscle activity depending on the tool used. There were also significant differences in the range of motion of the knee joints between the intervention methods using NS and those using the tools. Conclusion: In both groups, the muscle activity of the TA muscles was lower when GS, WS, and SS were performed compared to NS. In addition, compared to NS, the range of motion of the knee joint increased when the three tools were used. This study shows that the activity of the RF, VM, and TA muscles decreased and the range of motion of the knee joint increased during deep squats for both the PS and IS groups when tools were used.

Techniques, using physical wedge filter and using dynamic wedge filter and FIF(Field in Field) and ISCT(Irregular Surface Compensating Technique), have been developed according to progress of radiation therapy of breast cancer. Measurement of dose was done to judge the usefulness of technique using three cases, non tissue loss after breast conserving operating and tissue loss after breast conserving operating and mastectomy. Dose indexes of breast tissue, CI (Conformity Index), HI (Homogeneity Index) and QOC (Quality of Coverage), dose index of skin, or dose indexes of lung, volume of 50 percent dose and 20 percent dose were estimated and compared. Using dynamic wedge filter is useful plan at non tissue loss allowing for high dose of lung. FIF and ISCT are useful plan at tissue loss. ISCT is useful plan at mastectomy. Henceforth, we need to apply to valid plan and body type and thorax size.

For clinical implementation of Enhanced Dynamic Wedge (EDW), it is necessary to adequately analyze and commission its dosimetric properties in comparison to common physical metal wedge (MTW). This study was implemented with the essential measurements of parameters for clinical application, such as percentage depth dose, peripheral dose, surface dose, effective wedge factor, and wedge profile. In addition, through the comparison study of EDW with open and MTW, the analysis was performed to characterize the EDW. We also compared EDW dose profiles of measured values using chamber array 24 (CA24) with calculated values using radiation treatment planning system. PDDs of EDW showed good agreements between $0.2\~0.5\%$ of open beam, but $2\%$ differences with MTW. In the result of the measurements of peripheral dose, it was shown that MTW was about $1\%$ higher than open field and EDW. The surface doses of $60^{\circ}$ MTW showed 10% lower than the others. We found that effective wedge factor of EDW had linear relationships according to Y jaw sizes and was independent of X jaw sizes and was independent of X jaw sizes and asymmetric Y jaw opening. In comparison with measured values and calculate values from Golden-STT based radiation treatment planning system (RTP system), it showed very good agreement within difference of $1\%$. It could be concluded that EDW is a very reliable and useful tool as a beam modification substitute for conventional MTW.
