Purpose: Differences in scapular kinematics and muscle activity appear in the forward head and rounded shoulder posture (FHRSP). Thus, the aim of this study was to investigate the following effects according to different postures on scapular kinematics and muscle activity around scapular region in individuals with and without FHRSP during overhead reaching task. Methods: Thirty pain-free subjects with/without FHRSP participated in this study. All subjects were positioned into three positions: habitual head posture (HHP), self-perceived ideal head posture (SIHP) and therapist-perceived neutral head posture (TNHP). Muscle activities of upper trapezius (UT), lower trapezius (LT) and serratus anterior (SA) were measured during overhead reaching task. Results: Muscle activity of trapezius muscle (UT and LT) during HHP was significantly higher than SIHP and TNHP in FHRSP group (p<0.05), but there was no difference between SIHP and TNHP. SA also significantly increased muscle activity in HHP more than SIHP and TNHP in FHRSP group (p<0.05), but there was no significant difference between SIHP and TNHP. In Non-FHRSP group, although there was a tendency of different muscle activities among three postures, it was not statistically significant. Conclusion: This result demonstrates that muscle activity associated with overhead reaching task is increased in HHP which affects the scapular kinematics and SIHP contributes changed scapular kinematics and proper recruitment of muscle activity in FHRSP similarly to TNHP.
Community-based group exercise programs for people with chronic stroke are relatively uncommon in Korea. In addition, it is currently not known whether a community-based group exercise program is effective or not. The purpose of this study was to evaluate an 8 week community-based functional exercise program for its effects on balance performance and occupational performance in persons with chronic stroke. Twenty-five community-dwelling individuals with stroke participated in this program. Outcome of the program was assessed by the Berg Balance Scale and the Canadian Occupational Performance Measure (COPM). The functional exercise program lasted for eight weeks, with a 1-hour program twice per week and it consisted of mobility, stability, balance, functional strength, and gait training. The subjects were trained by one physical therapist but were under one-to-one supervision from students. The data of sixteen individuals who scored more than 24 on the Mini-Mental State Examination (MMSE) were analyzed. There was a significant effect both in terms of the COPM Performance Score & the Satisfaction Score (p=.002) and with the Berg Balance Score (p=.001). It was found that a short-term community-based exercise program could improve both performance of activities and balance. Further, all subjects reported that they were satisfied with this program.
Purposes: Despite a lot of prior studies on psychological empowerment on positive outcomes, the objectives of this paper were to examine the role of psychological empowerment on informal learning and the mediation role of informal learning between psychological empowerment and outcome variables focused upon physical therapists working in hospital industry. Methodology: Using survey methods, the data were collected from 198 physical therapists who have worked in Ulsan city and attended in annual meeting. Findings: Results showed that psychological empowerment predicted informal learning positively but differences in mediational mechanism. Specifically, the path between psychological empowerment and proactive behavior partially mediated but no mediating effect between psychological empowerment and helping behavior. This study identified the main role of psychological empowerment on informal learning, and it expands on positive functions of the concept to learning area in organizations. The other results help advanced understanding of differential mechanism through informal learning in the process between psychological empowerment and both outcomes. Practical Implications: The current study contributes to expend the area of prior findings on psychological empowerment to learning activities implemented by individual volunteer effort. For hospitals operating the teams of physical therapy, the significance for considering psychological empowerment is highlighted as for individual growth related to job and for change behavior in the individual level.
Purpose: The purpose of this study was to examine the effects of a day care rehabilitation program managed by nurses on physical and emotional function of patients with job-related injuries. Method: A one group pre-test and post-test quasi-experimental design was used. Thirty patients participated in a day care rehabilitation program and 9% of those were unable to complete the 16 weeks program due to absence. The physiotherapist, occupational therapist, and clinical psychologist offered the day care rehabilitation program, 5 times a week for 16 weeks. Outcome measures included physical and emotional function. Results: The program participants had significantly greater motor function (t=-2.85, p=.008) and activity of daily living (t=-5.34, p<.001), and lower depression (t=5.20, p<.001), state anxiety (t=4.71, p<.001), and trait anxiety (t=4.40, p<.001). Conclusion: The nurse managed day care rehabilitation program significantly improved physical and emotional function in patients with job-related injuries. The program should be further tested in a larger sample to validate the findings.
The term thoracic outlet syndrome (TOS) is used to describe patients with compressed subclavian arteries, veins, and brachial plexuses in the region of the thoracic outlet. The objective of this study was to evaluate a scalenus stretching exercise that aims to restore normal function to patients with TOS. This study consisted of 60 patients with symptoms of TOS, and divided the patients into 3 groups: one that received manual therapy, one that practiced self stretching, and a control group. Each group consisted of 20 patients. This study assessed the efficacy of scalenus stretching exercise by examining the resting pain, tenderness, spherical grip power, and pinch grip power of patients. The data were analyzed using one-way ANOVA, Scheffe post hoc test, and independent t-test. The results showed that resting pain was statistically significant within the manual therapy and self stretching groups (p<.05), and that the resting pain of the manual therapy group was more statistically significant than that of the self stretching group (p<.05). Tenderness, spherical grip power, and pinch grip were statistically significant within the manual therapy and self stretching groups (p<.05), but there was no statistically significant difference between the two groups (p>.05). Finally I could see that there were no statistical differences between manual therapy and self stretching to improve the symptoms of the patients with TOS. These results imply that self stretching by patients is as important as manual therapy by a physical therapist.
This study was designed to investigate the effects of an aquatic exercise program on the physical function and depression among the community elderly. The subjects were 41 cases of aged over 65 who went to elderly college in Hawsung-Kun, Kyeongkee province. Seventeen of them were assigned to control group, while twenty four subjects to the experimental group. The period of data collection was from June., 1999 to November., 1999. The aquatic exercise program was developed by the author with the assistance of rehabilitation medicine doctor and physical therapist in the public health center. The program was carried out once a week for 6 months in regular swimming pool. Data were analyzed with t-test, $X^2$ test, ANCOVA. paired t-test using SAS 6.12. Results were obtained as follows: 1. The increments in the range of motion of the shoulder and knee joints in experimental group after exercise was significantly greater than those in the control group. 2. The physical pain score decreases significantly after the exercise in the experimental group only. 3. The experimental group revealed lower level of depression after exercise than the level before the exercise, whereas control group showed no significant changes in this level. These findings may indicate that the aquatic exercise program is effective in increasing the range of motion of shoulder and knee joints and also effective in decreasing pain, the level of depression. Accordingly. the aquatic exercise program can be adopted as an health promotion program for community elderly.
Objectives: The purpose of this study was to evaluate work-related risk factors of musculoskeletal disorders (MSDs) in various departments and tasks at an oriental medicine hospital. Methods: 269 hospital workers were selected for the study. The questionnaire survey included general characteristics, work-related characteristics and MSDs. Cross-tabulation analysis, multiple logistic regression, adjusted for age, work time, job demand and workload, were used to evaluate the effects of work-related factors on MSDs. Results: There were differences in MSDs depending on gender, age, occupation, and work-related factors. Prevalence rates of MSDs were 23.79% shoulder, 14.13% back, 10.41% neck. Risk factors for MSDs was packaging of drugs, transfer, and exercise therapy of physical therapist. Shoulder pain was significantly influenced by using power tools, over-reaching, and job demand while back pain was significantly influenced by weight-lifting operations and keyboard typing. Conclusions: The results of this study suggest that consideration of MSDs characteristics of workers in oriental medicine hospital is necessary to change unsuitable work environments and educate on preventing MSDs.
Muscular dystrophy is a hereditary musculoskeletal disorder caused by a mutation in the dystrophin gene. Duchenne muscular dystrophy (DMD) is one of the most common, and progresses relatively faster than other muscular dystrophies. It is characterized by progressive myofiber degeneration, muscle weakness and ultimately ambulatory loss. Since it is an X-linked recessive inheritance, DMD is mostly expressed in males and rarely expressed or less severe in females. The most effective measurement tool for DMD is magnetic resonance imaging (MRI), which allows non-invasive examination of longitudinal measurement. It can detect progressive decline of skeletal muscle size by measuring a maximal cross-sectional area of skeletal muscle. Additionally, other techniques in MRI, like $T_2$-weighted imaging, assess muscle damage, including inflammation, by detecting changes in $T_2$ relaxation time. Current MRI techniques even allow quantification of metabolic differences between affected and non-affected muscles in DMD. There is no current cure, but physical therapist can improve their quality of life by maintaining muscle strength and function, especially if treatment (and other forms of medical intervention) begins in the early stages of the disease.
Resistive therapeutic exercise prescription goal is to improve functional performance and capabilities through the development of increased muscular strengh endurance or power. Resistance can be applied to either dynamic or static muscle contractions. Resistive therapeutic exorcise can be carried nut concentrically, eccentrically, isometrically, isokinetically. Neurodevelopmental treatment has not resistive therapeutic exercise concept. But proprioceptive neuromuscular facilitate techniques have resistive therapeutic exercise concept with pattens and techniques. It is aid muscle contraction, motor control and increase strength. Manual muscle testing will help the therapist establish a qualitative and quantitative baseline level of strength. Manual resistance maybe applied a against controlled lengthening contraction re static contraction of a muscle. A repetition maximum is not easy to calculate and is not the most accurate method available today to measure strength before of after a resistive therapeutic exercise program. Oddvar Holten Diagram is essy to calculate and is the most accurate method available today to measure strength before of after a resistive therapeutic exercise program. Plyometric training emphasize the development of muscular power and coordination. Quick bursts of force in functional movement patterns are often necessary of a patient is to return to high-demand occupational, recreational or sports related activities.
