ICF Tool을 적용한 외상성 뇌손상 환자의 보행능력 증진을 위한 점진적 중재전략의 증례 (A Case Report of Progressive Intervention Strategy Applied ICF Tool about Gait for TBI Patient)

  • Objective : The purpose of this study was to describe the Progressive Intervention strategy applied ICF (International Classification of Functioning, Disability and Health) Tool about Gait for TBI(Traumatic Brain Injury) patient. Methods : The data was collected by TBI patient. We applied the progressive Intervention strategy applied ICF Tool to TBI patient. Parameters of result were collected for using the Berg balance scale, TETRAX, Timed up and go test, Sit to stand test and ICF Evaluation Display Results : Significant differences were observed the TBI patient for Berg balance scale, TETRAX, Timed up and go test, Sit to stand test and ICF Evaluation. TBI patient improved all test. Conclusion : Progressive Intervention strategy applied ICF Tool is very useful and effective. It is effective in clinical practice.

  • Of critical importance to the success of any e-commerce site are the two factors: rapid application development and quick response time. A three-tier architecture composed of presentation layer, business layer, and data access layer emerges to allow rapid changes in user interface, business logic, and database structures. Too often, such a logical three-tier architecture is considered as requiring a three-tier physical architecture: Web server, application server, and database server running on separate computers. Contrary to the common belief, a Web stress test reveals that the three-tier logical architecture implemented on a two-tier physical platform guarantees a quicker response time due to the reduction in cross-machine communications. This would lead business firms to economize their spending on e-commerce: increasing the number of physical servers to expedite transaction is not necessarily the best solution. Before selecting a particular hardware configuration, a Web stress test needs to be conducted to compare the relative merits of alternative physical architectures. Together with capacity planning, Web stress test emerges as a powerful tool to build robust, yet economical e-commerce sites.

Relationships Between Cognitive Function and Gait-Related Dual-Task Interference After Stroke

  • Previous studies have reported that decreased cognitive ability has been consistently associated with significant declines in performance of one or both tasks under a dual-task walking condition. This study examined the relationship between specific cognitive abilities and the dual-task costs (DTCs) of spatio-temporal gait parameters in stroke patients. The spatio-temporal gait parameters were measured among 30 stroke patients while walking with and without a cognitive task (Stroop Word-Color Task) at the study participant's preferred walking speed. Cognitive abilities were measured using Computerized Neuropsychological Testing. Pearson's correlation coefficients (r) were calculated to quantify the associations between the neuropsychological measures and the DTCs in the spatio-temporal gait parameters. Moderate to strong correlations were found between the Auditory Continuous Performance test (ACPT) and the DTCs of the Single Support Time of Non-paretic (r=.37), the Trail Making A (TMA) test and the DTCs of Velocity (r=.71), TMA test and the DTCs of the Step Length of Paretic (r=.37), TMA test and the DTCs of the Step Length Non-paretic (r=.36), the Trail Making B (TMB) test and the DTCs of Velocity (r=.70), the Stroop Word-Color test and the DTCs of Velocity (r=-.40), Visual-span Backward (V-span B) test and the DTCs of Velocity (r=-.41), V-span B test and the DTCs of the Double Support Time of Non-paretic (r=.38), Digit-span Forward test and the DTCs of the Step Time of Non-paretic (r=-.39), and Digit-span Backward test and the DTCs of the Single Support Time of Paretic (r=.36). Especially TMA test and TMB test were found to be more strongly correlated to the DTCs of gait velocity than the other correlations. Understanding these cognitive features will provide guidance for identifying dual- task walking ability.

리듬청각자극을 동반한 동작관찰 신체훈련이 만성 뇌졸중 환자의 하지 근활성도와 보행능력에 미치는 영향 (The Effects of Action Observational Physical Training with Rhythmic Auditory Stimulation on Muscle Activity of the Lower Extremity and Gait Ability in Patients with Chronic Stroke)

  • PURPOSE: The purpose of this study was to investigate the effect of action observational physical training with rhythmic auditory stimulation on muscle activity and gait ability in patients with stroke. METHODS: Twenty-six chronic stroke patients participated in this study were assigned into three groups, experimental group 1 (10% faster tempo rhythmic auditory stimulation with action observation training) n=8, experimental group 2 (average tempo rhythmic auditory stimulation with action observation training) n=9, and control group (action observation training) n=9. In this experiment, the corresponding exercise were applied into the subjects of three group for 30 minute a day, 3 time a week during 4 weeks. All participants were measured to muscle activity of lower limb, 10 meter walking test, Figure of 8 walk test, Dynamic gait Index. The collected data were analyzed by using SPSS (version 18.0 for window) and verified that each data was a normal distribution based on Shapiro-Wilk test. Between-group and within-group comparison was analyzed by using One-way ANOVA test, Paired t-test respectively. In all statistical analyses, significance level, ${\alpha}$ was set by .05. RESULTS: The above results revealed that the all experimental group 1 and experimental group 2 and control group were all effective to improve the lower limb muscle activities, gait ability. However more positive effects shown action observational physical training with rhythmic auditory stimulation experimental group. CONCLUSION: This study suggest that action observation physical training with rhythmic auditory stimulation is effective intervention for improvement of muscle activity and walking ability in chronic stroke patients.

Reliability of Measured Popliteal Angle by Traditional and Stabilized Active-Knee-Extension Test

  • The active-knee-extension (AKE) test has been used to measure hamstring muscle length. The traditional AKE test measures the popliteal angle to the point of resistance with a 90-degree flexion of the hip fixed by straps, while the stabilized AKE test measures the popliteal angle to the point of resistance with a 90-degree flexion of the hip stabilized using a pressure biofeedback unit providing lumbopelvic stabilization. The purpose of this study was to determine test-retest reliability of the traditional AKE test and stabilized AKE test. Twenty healthy adults participated in the study. The popliteal angles were measured with a digital inclinometer during each test. To assess the test-retest reliability between the 2 test sessions, intraclass correlation coefficients (ICCs) were calculated. The intrasubject coefficient of variation ($CV_{intra}$) was also calculated. To compare the traditional and stabilized AKE tests for changes in pressure, paired t-tests were applied. The results of this study were as follows: 1) ICCs(3,1) value for test-retest reliability was .96 in the traditional AKE test, and was .98 in the stabilized AKE test. 2) The maximal $CV_{intra}$ was 33.7% in the traditional AKE test and 15.7% in the stabilized AKE test. 3) Differences of $6.1{\pm}2.1$ mmHg in pressure were measured in the traditional AKE test, and differences of $1.2{\pm}1.0$ mmHg in pressure were measured in the stabilized AKE test. The results show the traditional and stabilized AKE test to be highly reliable, with test-retest reliability. However, the stabilized AKE test represented less variation and more stabilization than the traditional AKE test. Further study is needed to measure the inter-rater reliability of the stabilized AKE test for generalization and clinical application.

The effect of hip joint exercise using an elastic band on dynamic balance, agility and flexibility in healthy subjects: a randomized controlled trial

  • Objective: The purpose of this study was to examine the effects of hip joint exercise using an elastic band on dynamic balance, agility, and flexibility in healthy subjects. Design: Randomized controlled trial. Methods: Thirty-five subjects (between 19 and 23 years) were randomly allocated to two groups: hip flexion exercise (HFE) group (n=17) and the hip abduction exercise (HAE) group (n=18). The HFE group participated in flexion exercise of the hip joint using an elastic band for 50 minutes a day, three days a week for four weeks, while the HAE group participated in abduction exercises of the hip joint using an elastic band for the same period. Dynamic balance was measured using the timed up and go (TUG) test, agility was measured with the standing long jump, and flexibility was measured using the Schober's test (5 cm, 10 cm). Results: The HFE group showed significant differences in the TUG test, standing long jump, and the Schober's test (10 cm) after training (p<0.05). The HAE group showed significant differences in the TUG test, standing long jump and the Schober's test (5 cm, 10 cm) after training (p<0.05). However, there was no significant difference between the HFE group and the HAE group. Conclusions: Flexion and abduction exercises of the hip joint using and elastic band increased dynamic balance, agility, and flexibility in healthy subjects. Additional research on hip joint exercises using an elastic band for improving dynamic balance, agility and flexibility are necessary.

수동 견갑골 거상 검사에 따라 분류된 경부통 환자의 견갑골 안정화운동 효과 비교 (Effect of Scapular Stabilization Exercise on Patients With Neck Pain Classified According to Passive Scapular Elevation Test)

  • This study aimed to determine the usefulness of classifying patients with neck pain on the basis of the results of passive scapular elevation test. We classified 21 patients with neck pain into positive (n=12) and negative (n=9) groups on the basis of passive scapular elevation test; the 2 groups then equally performed scapular stabilization exercise program for 30 min, 3 times a week, for 4 weeks. Visual analogue scale (VAS), neck disability index (NDI), and range of motion (ROM) were recorded both before and after the intervention for both groups. Paired t-test was used to determine that there were significant changes between before and after the intervention, and independent t-test was used for analyzing changes between two groups of dependent variables. After 4 weeks of training, we observed significant decrease in pain and disability (p<.05) and a significant increase in rotation, flexion, extension, and side-bending ROM (p<.05) in both groups. Further, between pre- and post-intervention evaluations, we observed a significant decrease in pain and disability and a significant increase in rotation and flexion ROM in the positive group than in the negative group (p<.05). These results indicate that passive scapular elevation test may be used to identify mechanical disorders of the cervicoscapular muscle in patients with neck pain. Therefore, we recommend the use of passive scapular elevation test to determine appropriate treatment intervention when treating patients with neck pain.

Comparisons of Test-Retest Reliability of Strength Measurement of Gluteus Medius Strength between Break and Make Test in Subjects with Pelvic Drop

  • Purpose: The purpose of this study was to compare the reliability of unilateral hip abductor strength assessment in side-lying with break and make test in subjects with pelvic drop. Hip abduction muscles are very important in the hip joint structures. Therefore, it is essential to evaluate their strength in a reliable way. Methods: Twenty-five subjects participated in this study. Unilateral isometric hip abductor muscle strength was measured in side-lying, with use of a specialized tensiometer using smart KEMA system for make test, of a hand held dynamometer for break test. Coefficients of variation, and intra class correlation coefficients were calculated to determine test-retest reliability of hip abductor strength. Results: In make test, maximal hip abductor strength in the side-lying position was significantly higher compared with break test (p<0.05). Additionally, Test-retest reliability of hip abductor strength measurements in terms of coefficients of variation (3.7% for make test, 16.1% for break test) was better in the side-lying position with make test. All intraclass correlation coefficients with break test were lower than make test (0.90 for make test, 0.73 for break test). Conclusion: The side-lying body position with make test offers more reliable assessment of unilateral hip abductor strength than the same position with break test. Make test in side-lying can be recommended for reliable measurement of hip abductor strength in subjects with pelvic drop.

VR 콘텐츠를 이용한 신체검사 개발 및 사용자 만족도 연구 (A Study on the Development of Physical Examination with VR Content and User Satisfaction)

  • 본 연구는 고령화 및 저출산에 따라 보건의료분야의 만성질환의 증가 및 전문 인력부족 등의 문제를 해결하고 효율적인 헬스케어를 제공하고자 VR 콘텐츠를 이용한 신체검사의 효과성을 연구하였다. 이에 본 연구는 HTC VIVE Pro VR과 스틱 컨트롤러를 착용하여 원스톱 VR 콘텐츠 신체검사 시스템을 구현하였다. 시스템은 1단계부터 5단계까지이며, 색각테스트, 기억력테스트, 청력테스트, 반응속도테스트, 순간인지능력테스트의 5단계를 순차적으로 거쳐 최종 신체 나이를 판별하는 솔루션으로 구성된다.또한 본 연구가 개발한 원스톱 VR 콘텐츠 신체검사 시스템에 대해 대전 소재 A 상급종합병원 건강검진센터 및 VR/AR 임상시험 센터에 내원한 정상인을 대상으로 사용자 만족도를 검증한 결과, 전체적인 만족도와 재이용의사가 높게 나타났고, 성별에 따라 5단계 순간인지테스트는 유의한 차이가 있었으며, 연령에 따라서 4단계 반응속도테스트, 5단계 순간인지테스트에서 유의한 차이가 나타났다.

Relationship Between the Closed Kinetic Chain Upper Extremity Stability Test and Strength of Serratus Anterior and Triceps Brachii Muscles

  • Background: The CKCUES test evaluates the functional performance of the shoulder joint. The CKCUES test scores CKC exercises of the upper limbs to examine shoulder stability. Although the CKCUES test provides quantitative data on functional ability and performance, no study has determined the relationship between CKCUES scores and SA and TB muscle strength. Objects: The objective of this study is to determine the relationship between the CKCUES test scores and the strength of the SA and TB muscles in the CKCUES and unilateral CKCUES tests. Methods: Sixty-six healthy male volunteers participated in the study. A Smart KEMA strength sensor measured SA and TB muscle strength. Two parallel lines on the floor indicated the initial hand placement to start CKCUES tests. For 15 seconds, the subject raises one hand and reaches over to touch the supporting hand, then returns to the starting position. Results: The correlation between the CKCUES test scores and the strength of the SA was strong (r = 0.650, p < 0.001), and the TB was moderate (r = 0.438, p < 0.001). The correlation between the unilateral CKCUES test and the strength of the SA of the supporting side was strong (r = 0.605, p < 0.001), and swing side was strong (r = 0.681, p < 0.001). The correlation between the unilateral CKCUES test and the strength of the TB of the supporting side was moderate (r = 0.409, p < 0.001), and swing side was moderate (r = 0.482, p < 0.001). Conclusion: Our study showed that the CKCUES test had a strong association with isometric strength of SA and moderate association with that of TB. These findings suggest that the CKCUES test can evaluate the function of the SA. Moreover, the unilateral CKCUES test can evaluate unilateral shoulder function.
